My name is Vicky Evans and I feel privileged to be the new Regional Director of the Northern Colorado Seniors Blue Book. It has been a truly rewarding and satisfying experience to be able to work with so many wonderful Seniors, Caregivers and Senior Professionals during the past year here in Northern Colorado.

I have lived and raised my family here in the Fort Collins area and am proud to call it my home. During my time in Northern Colorado I have been involved in both the Senior Care arena as well as publishing. The Seniors Blue Book has been a perfect fit for my skill set and I love being able to work in and help the community I love.

I am very excited to get the Fall 2018-19 edition on to the streets as we have added 8 additional pages to

Exploring the Array As we grow older or as we care for of Options an aging loved one, our journey can bring us many surprises that can be confusing to navigate.

As we begin to explore the array of options available in our community to assist with remaining independent and cared for, we often do not know where to start or where to turn for reliable information and support on our journey. That is where the Larimer County Office on Aging (LCOA) can help.

The staff at LCOA is here to provide a roadmap for those seeking information about long term services in Larimer County. The Aging and Disability Resource Center located in the Office on Aging offers a “point of entry” for individuals and families. Staff can assist with creating a roadmap that will help assure that as we age and need services in the home or community for ourselves or a loved one, we know what is out there. Options Counseling is a personalized service that can help individuals and caregivers identify needs and goals and assist in finding available benefits, programs and services to help meet those goals.

In addition to Options Counseling, LCOA offers a variety of programs

to assist individuals remain in their own homes including information and referral; in-home service vouchers; chore voucher and caregiver support through our Family Caregiver Support Program.

The Long-Term Care Ombudsman program, advocates and addresses the needs of individuals in long term care facilities as well as help when looking for a longterm care community.

The office contracts with over 25 service providers in Larimer County to provide such services as transportation, meals, legal services, and health and wellness classes. The staff work with many providers in the community through collaborations and partnerships and keeps abreast of the changing landscape of long term services to help better guide and support individuals on their journey. We’ve become pretty good at addressing our physical, work and social opportunities. We might downsize to a smaller home as our children move out. We may plan for retirement by considering volunteer and travel opportunities. We also connect with a doctor to identify ways to stay healthy. As we are busy planning and enjoying the next phase of our lives, we sometimes forget about formally considering our legal and financial futures if faced with a life changing illness. We’ve paid into Medicare and saved into a 401K, but how do we protect ourselves and our assets in the event of an unforeseen medical crisis? One worthwhile solution is a Life Care Planning approach. Life Care Planning is a holistic, elder-centered approach to the practice of law that helps families respond to many of the challenges caused by chronic illness or disability.

There are several benefits to a Life Care Planning approach. One, Life Care Planning is a prevention based model rather than a crisis-oriented one. In preparation for a possible debilitating health change, an individual can educate himself on

available resources, evaluate current investment/insurance/burial policies, and budget accordingly, avoiding crisis driven decisions.

Second, good Life Care Planning relies on an inter-disciplinary team that evaluates current and future needs and locates and coordinates appropriate resources, helping to ensure quality care and services.

When an individual receives advice from several independent sources, communication and continuity suffer and important information can fall through the cracks. In contrast, Life Care Planning offers a “one stop shop” team of specialists, including a social worker, attorney and paralegal, that provide a more comprehensive, cohesive long term care plan.

Three, Life Care Planning assists with preserving family wealth. When a person requires professional inhome or residential care, families are often shocked at the cost. The average cost of home health care is $23-$37/ hour. If an individual requires residential living, an assisted living can cost between $2,500$8,000 and skilled nursing runs anywhere from $7,500-$11,000 per month. An ombudsman is an advocate for residents of nursing homes, board and care homes, and assisted living facilities who is trained to resolve problems. Under the federal Older Americans Act, every state is required to have an ombudsman program that addresses residents’ complaints and advocates for improvements in the long-term care system.

While ombudsmen do not have direct authority to require action by a facility, they have the responsibility to negotiate on a resident’s behalf and to work with other state agencies for effective enforcement.

Fragility Fractures: What Do BACK PAIN, most people do not auI Need to Know? tomatically think of a fracture when they first experience back pain.

A Fragility Fracture is any fall from a standing height or less, that results in a fracture.

Our bodies, when in good health, should be able to withstand a fall from this height without a fracture. The most common areas involved include the spine, hip, and the wrist.

What causes fragility fractures?

Osteoporosis is the number one cause of a fragility fracture. Osteoporosis is characterized by low bone mass and structural deterioration. This can be caused by many underlying conditions such as; aging, being female, low body weight, low sex hormones or menopause, smoking, and some medications such as cancer medications, SSRIs, and steroids. About 54 million Americans have osteoporosis and low bone mass placing them at increased risk for osteoporosis.

Studies suggest that approximately one in two women and up to one in four men, age 50 and older, will break a bone due to osteoporosis.

Osteoporosis is often called a silent

disease because the body can’t feel bones weakening.

Breaking a bone is often the first sign of osteoporosis but patients may notice they are losing height or their upper back is curving forward. If you are experiencing height loss or your spine is curving, be sure to consult your doctor or healthcare professional immediately.

Other causes of fragility fractures include traumatic injuries as well as metastatic tumors.

How do I know if I am at risk?

You can find out if you are at risk by consulting your doctor. With a thorough medical exam and history, you and your doctor can determine what kinds of tests will be needed.

What are the treatment options?

There are many preventative options that may include: n n n

Transitions An employee shared his first child will be starting kindergarten in a few weeks. Such an important milestone for a child.

Shiny new clothes, haircut, and new superhero backpack. Often a tear-filled morning for families. I remember the kindergarten teacher giving all of us a cotton ball with a poem expressing eloquently the feelings of the day. I carried around the cotton ball in my bag for a week…. or maybe I still have it.

I smiled at our employee and disclosed to him that my husband and I would be driving to take our daughter to college….her freshman year. Another important milestone for a child – ahem –young adult. I have been holding back crying all summer. Tears well up when I think about her being on her own and not at home. Not knowing everything about her scares me. But I trust her. I think we have raised her well Moving into a patio home or an independent or assisted living apart-

ment is another transition. As with all changes, sometimes they occur naturally – like going to kindergarten. Sometimes they are a conscious choice – like electing to go to college.

Either way, this change is full of emotions. What can I expect? Did I bring the right stuff, the right clothes? What will change? Will I make friends? Will others like me?

Seniors have lived through so many transitions - they are often very resilient. They rely on their wisdom and experience to adapt once more to change. I find the families are the ones who struggle more.

Why? Being near is not enough.

They need to remember the times when near was enough. Maybe we should provide them with a cotton ball? INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided.

CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

VA Benefit Aid Look for “the horse of a differ& Attendance ent color” when filing for the VA Benefit Aid & Attendance, and ONLY through a veteran-dedicated home care agency.

If you are as old as I am, you will remember “the horse of a different color” from The Wizard of Oz. It stood out from all the rest and was truly different than any other horse.

If you are looking for assistance with a complex VA benefit such as Aid and Attendance, don’t just hope you find the right agency to help. Instead, review the below information then let your search begin.

THE PROBLEM? At last count, there were over two-hundred organizations that purport to help veterans obtain the Aide and Attendance benefit for those that qualify. However, many of these organizations are considered as poachers seeking to get all or a portion of the veteran’s funds. Others are looking to sell the veteran services they may or may not need. Still, others are seeking to get paid for assisting a veteran in applying for benefits. So, how do you pick the right one to help you out of this “herd of horses?”

First, eliminate anyone who wants to charge a fee for assisting you to file for benefits. It is illegal to charge for helping a veteran in the application process for the Aid and Attendance benefit.

Second, eliminate anyone who wants to charge you to re-arrange your assets to qualify. This usually means a lawyer setting up a trust and moving assets to that trust. You don’t have to use anyone to help you if you don’t want to. Any veteran or surviving spouse can file the claim for benefits on their own. However, the problem is the application process is complicated and time-consuming. The process entails a lot of paperwork and veterans tend to give up after realizing so much ‘red tape’ is involved. Then, and to make it more complex, if you did qualify, you’d need to find a home care agency or an individual caregiver to provide services. This takes time, drug tests, background checks, etc.

Even the Internal Revenue Service (IRS) considers individual Caregivers as employees, even if they are a family member. This means withholding taxes and filing federal and Continued on next page 79

24 HOUR ON CALL – Refers to someone being available 24 hours a day to speak with you.

CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health.

Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s.

ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.

INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

MED-ALERT – Company offers a pendant or home alert system that notifies staff that user is in distress and needs immediate assistance.

NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other non-medical in home care. These services are available for hire after, or in conjunction with, Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.

PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness.

Services may include counseling or administering of medications.

SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family.

Services provided may be preventive, developmental, or remedial in nature.

THERAPY SERVICES (ST, PT, OT, RT) – Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.

Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.

What Can Home Health Care Do For Me? Home Health Care (HHC) comes in a variety of different services and agencies such as; Nursing services, Physical and Occupational Therapies, Homemaking services and sometimes Social Work.

These services are often needed when discharging from a hospital or rehab or in the home when there is a decline in condition after a doctor visit.

These types of short term services help to ensure you are progressing in a positive direction.

Other services and agencies may be in place along with HHC in a supportive manner. You may need a full time Certified Nursing Assistant or a Registered Nurse to help you while retaining services through another agency. This is allowed when you are paying private or through a long-term care insurance or trust, however, two Medicare agencies getting paid from Medicare cannot have service overlap.

Medicare agencies often cover medication fills, injections, wound management and therapies. After a period they may not be able to cover some of these items. With HHC you have the option of privately paying for an agency to pick up where the Medicare agency may have to left off.

A common misconception is that Medicare will cover the cost of a Caregiver in your home who would do housekeeping, go shopping, cook

meals or sit with you for extended periods of time. Medicare does not cover the cost to these performed tasks. When private pay is involved, two agencies can often be found in one home at the same time.

Over the years I have been asked, “If hospice is already in place and I need help all the time what happens?” The hospice agency cannot be with you twenty-four hours a day or for long periods of time. If you wish for your loved one to remain in the home, it is recommended that you call another agency to help you fill the hours needed and support the hospice agency already in place.

It is an easy decision to keep you and your family well cared for while honoring their wishes. The time and labor caring for them following that decision is the challenge.

With so many agencies in our area providing different levels of care and supportive services it is becoming easier to keep our loved ones in place longer while honoring their wishes. Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system.

It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions.

WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds. ~

Hospice Care Definitions continued VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement.

Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.

RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility.

GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration.

This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs.

It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope.

Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays.

AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being.

This is not reimbursed by Medicare.

GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience

INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients.

PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump.

If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency.

IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building.

The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting.

LICENSED MASSAGE THERAPY – Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare.

MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing

grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting.

PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

The Myths of Hospice Care: When many hear the term “hospice What You care,” they are understandably apprehensive. The final months of a Need to Know loved one’s life can be very difficult, but hospice care is a valuable resource that assuages some of these challenges and brings comfort to patients and their families.

Here are a few myths (and facts) surrounding hospice care, so you can make the right choice for your loved one when the time comes: MYTH: Hospice Care Is Limited To Six Months A patient is eligible for hospice care when they receive a life expectancy prognosis of six months or fewer, but their time in hospice care is not limited to six months. Hospice care is available for as long as a patient meets the criteria, even if they outlive their sixmonth diagnosis.

MYTH: Hospice Care Hastens Death Hospice care does not speed up the dying process, nor does it prolong it. The purpose of hospice is to keep a patient comfortable so they may die naturally and with dignity on their own terms.

designed to support families of patients to relieve caregiving stress and allow them to live out their time with loved ones to the fullest.

MYTH: Hospice Care Is Only Necessary In Final Days Of Life Many families who utilize hospice care say they wish they had brought in hospice sooner. Hospice professionals offer medical, counseling and spiritual support for patients and their families to provide ease during the end-of-life journey. This comfort care can improve the quality of life for patients in their own homes, assisted care facilities or in a hospice care center.

What Hospice Truly Is Imagine a team of professionals whose greatest concern is your complete comfort. Imagine being empowered and supported to have the conversations you’ve always wanted to have. we live. We breathe in and out. We experience sorrow and joy. We have things we ache to do. Hospice is about those moments. About respecting the process as much as we do the process of birth.

The idea of taking control of our last moments appeals to us all. In viewing death as a process that grants us the ability to do this, we are able to see the service of hospice as the tremendous benefit it is. Available to all, it’s covered by Medicare, Medicaid, VA benefits, and most insurance plans.

So often hospice isn’t utilized until the last possible moment, or sometimes not at all. The association with death makes it hard to talk about before it’s absolutely necessary. Though when we understand what hospice truly is, an individualized plan of care provided by a team of experts, we remove the fear of it. We can see it for the remarkable service it really is: thorough comfort care.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness.

This service is not reimbursed by Medicare.

VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

Senior Housing Definitions ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/patio home, or Condo/ Townhome style living. There are often community gatherings and many offer enhanced amenities like concierge, golf courses and recreation centers. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property.

DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants.

INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...)

INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible.

INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner.

LONG-TERM CARE – Chronic, custodial care provided on an on-going basis MEDICAID WAIVER – An assisted living waiver may be used to transition individuals out of nursing facilities or to provide persons at risk

of institutionalization with options for remaining in their communities.

SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders.

SKILLED NURSING FACILITY – Facilities provide 24/7 care for residents and offer full assistance with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service,

cable television and all personal/ nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources.

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea).
