Helen Resides at Hillcrest Rehabilitation Center, which is considered a “SNF” What exactly is a SNF? As recent as ten years ago, when people referenced SNFs, they were generally talking about a traditional long term care nursing home. In recent years, the SNF model has evolved to include specialized short term rehabilitation centers, geared towards helping people return home after a serious illness, injury or operation. These centers are staffed with a team of licensed therapists, compassionate nurses and doctors.

Admission eligibility depends on a few factors. First, are you in need of long term care or short term rehabilitation services? If you are in need of short term rehabilitation and you have Medicare or private insurance, both will help pay for these services. Prior to entering a SNF, Medicare beneficiaries must have been hospitalized for a minimum of three consecutive nights. Medicare covers the first 20 SNF days paid in full. Days 21-100, the Medicare beneficiary either pays a daily co-insurance rate or may have opted for a secondary insurance that picks up these co-payments. Medicaid traditionally pays for long term care services.

What should you do if you’re discharged home but don’t feel strong

enough to be home? Medicare allows a 30-day window for people who return home to be admitted into a SNF, as long as the patient was in the hospital for at least three consecutive midnights.

There is no need to readmit to the hospital, as you can be directly admitted to a SNF. There are multiple skilled nursing facilities in your area, how should you choose where to go? Understanding the 5-star rating system is crucial to ensuring that you receive exceptional clinical care from a reputable facility. New SNFs will not obtain their 5-star rating until after one year of operation. As with rated facilities, it is incredibly beneficial to schedule a tour for yourself or a loved one to gain understanding about your SNF of choice.

While on a tour of the SNF be sure to observe the staff friendliness, building cleanliness, amenities and overall feel of the facility. It’s our privilege to guide you through this process and be your personal resource on the path to optimal wellness. ~

This prototype guide is just a glimpse of the information that will be available in the first full publication in January, 2019. This edition will be upwards of 128 pages and will list over 70 categories of information with over 2000 providers. We will be publishing this free guide semi-annually with 60,000 copies in each distribution. The guide will be available in your local senior centers, libraries, medical clinics, hospital case management offices, senior housing facilities and many other high traffic senior locations.

What is Care Management?

Geriatric Care Managers are professional resource specialists and advocates for older adults and their families.

They can be social workers, nurses, gerontologists, psychologists or other health and human services specialists. They are dedicated to assisting elders and their loved ones navigate the health care system.

They assist with current day to day care issues, crisis management, and future care planning. They are skilled at securing appropriate living arrangements and coordinating services for complicated medical/ psychiatric needs. Geriatric Care Managers are professional problem solvers and can be your guide to the complicated and sometimes confusing array of services that are available in our community.

Benefits for older adults or people with chronic illness and their families: n

C are Managers draw on their training and experience in the senior care industry to provide professional care planning and high quality, cost effective services.

C are Managers conduct a comprehensive needs assessment addressing medical, social, emotional, legal, financial, and housing needs. Options for care are based on the individual situation and client/family needs.

n

Care Managers assist in navigating a variety of services as the client’s condition changes.

They provide continuity and coordination with doctors, home health care agencies and other providers. This includes monitoring care and advocating on the senior’s behalf as often as necessary to ensure quality care.

Care Managers are a fee-for-service entity. They do not receive payment from Medicare or Medicaid and they do not take referral fees from third party providers, insurance companies or equipment manufacturers.

However, some Long Term Care Insurance policies now recognize the importance of these services and cover the cost.

Care Managers can be the ‘eyes and ears’ for out-of-town family members; not only to assist with in-home services, but, also to objectively assess care in a facility and function as a liaison with far away family.

Care Managers facilitate family communication and aid in the decision-making process.

When to call a Geriatric Care Manager: n When you are overwhelmed with options or don’t know where to start researching options. n When you are out of town and unable to attend to the needs of your loved one.

Estate Planning: Not Just A well-executed estate plan is for Boomers! critical for everyone. A will or trust, and powers of attorney are the best way to ensure your wishes are honored after your death, and during any period of incapacity.

In past columns we have discussed the importance of estate planning for seniors. Here, we’ll look at these issues for the younger generation.

Planning for these events seems natural as we age and families grow.

It is especially important for younger adults and for unmarried couples to develop or update a plan that accurately reflects their wishes. People without a plan in place face the very-real possibilities of passing their assets to someone for whom they never intended to provide, leaving someone out altogether, or having someone they do not trust (or even know!) make decisions on their behalf.

Once we turn 18, we have the right to make our own financial and medical decisions. While we might still rely on our parents’ advice and direction, the law does not give parents the power to make decisions for their adult child, even if the child is incapacitated. In a situation such as this, a parent would likely have to

go to court to be appointed as their child’s guardian. Executing powers of attorney documents can prevent this. Powers of attorney designate someone (an “agent”) to act on your behalf should you become unable to do so. Anyone 18 years of age or older can, and should execute medical and financial powers of attorney, to avoid unnecessary delay, expense, and to ensure the decisions made on their behalf come from a trusted person.

Unmarried couples face similar risks. Without a document designating an agent, an unmarried partner is not treated the same as a spouse. In fact, the partner may be left out of any medical decision-making, entirely. Further, if an unmarried couple does not designate each other as beneficiaries of their assets, the property passes according to law at their death, rather than to each other. So, a loved one could be left out of their partner’s estate altogether. Therefore, planning for these events is crucial.~ Editor’s Note: This article was submitted by Richard E. Those who plan in advance will have more options regarding location, amenities, etc. Those who move in crisis mode generally have fewer options as they must settle for what is available at the moment.

How to move also has options: you can pack and move yourself, hire a moving company to move your things, or hire a move management company to plan your move, pack and move your things, and unpack everything for you. Whether or not you have planned your move or are moving in crisis mode, hiring a move management expert will greatly reduce the stress of the situation and get you settled quickly.

There are wide spread misconceptions, untruths, surrounding HECMs. They are bad, they are scams, they are only for poor people, the bank will own the house.

HECM’s are home equity loans with options not available on traditional home equity loans or HELOC’s as they are called.

General statements from critics have deafening effects to the HECM program. When misinformed people

make general assumptions or statements about the HECM program, at times, that has a stronger influence on people than the professionals who could actually show you how the program really works. Maybe I should ask my physician which car would be best suited for me or better yet ask my auto mechanic their opinion as to why my shoulder hurts?

The point is, ask professionals in their field of expertise to explain HECM’s and HECM options, if this is a path that you are wishing to explore. They have been around for almost 30 years helping seniors in their retirement. Ask an expert.

A New Lease on Life When Mike Cullen began having trouble playing just three or four holes of golf in 2013, he knew something was wrong.

For 34 years, he enjoyed putting in long hours as a beloved teacher, principal and superintendent at local South Dakota schools while he and his wife, Betty, raised their two boys. His love of working with students meant he took on many additional roles over the years too: athletic director, football game announcer, substitute bus driver and even temporary cheerleading coach.

After a series of doctor’s visits and tests, Mike learned that kidney disease was to blame for his poor golfing and the host of symptoms he was experiencing. He began dialysis sessions three times a week and a course of prescribed medications, but knew a transplant was the solution he really needed to feel better.

Mike decided to retire in July 2015 and hoped that his name would soon come up on the long kidney donor list. Then something extraordinary arrived in the mail — a box from a former student filled with kidney beans and a message that she wanted to donate her kidney to thank him for giving her “a new lease on life” years before. This former student, nurse Elizabeth Blankenfeld, learned about Mike’s need for a transplant via a local TV story and decided to reach out.

The following month, Mike gratefully received the kidney transplant and gradually started to feel more like his old self. He and his wife moved to Minnesota to be closer to their grandson, and Mike began morning swims to build his strength.

But he wanted a meaningful way to fill his free time now that his career had ended.

“I wanted to do something to make a difference, and my son mentioned that the new LSS Neighbor to Neighbor service needed volunteers to support older adults,” he said.

The service, launched in September 2016, offers older adults a little extra support at home and in the community. Mike dove into the opportunity. He completed training and began supporting Bill Wherland, a Minneapolis resident. “I was surprised how open he was,” Mike said.

“You get close and become friends almost immediately.”

Bill signed up for the Neighbor to Neighbor service following a stroke, and Mike visits him three to four hours each week. Mike drives him to speech therapy appointments aimed at improving his pronunciation. They catch up over lunch, collect groceries and walk outdoors to improve Bill’s balance. The visits also give Bill’s wife a much-needed break from caregiving. It is a time she can run her own errands, visit with friends and family, or just catch-up on housework.

HOSPICE (See definition in home health grid definitions) Services Provided: N-Nurse – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed.

SSW-Social Worker – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs.

Chaplain – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.

Home Health Aide – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. Volunteer – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement.

Levels of Care: RO-Routine Care – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is provided at the individual’s residence, this can their private home, a retirement community, an assisted living, or a skilled nursing facility.

RE-Respite Care – Provided when

the family needs time to rest or go out of town for a short time.

Ideally, this service is provided for five consecutive days. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility, at no cost to the patient or the family.

GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-Continuous Care – Provided to prevent the individual from going to the hospital when a crisis occurs.

Continuous care also is given when more helps is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be The criteria for continuous care may include: Severe pain or symptoms which are out of control Psychotic episodes Breakdown of caregiver Imminent death and family

AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community.

ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Specifically trained staff usually serves the special needs of this population.

Amenities vary for each community.

ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations.

Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. Rates vary from an all-inclusive to base rates with levels of care. Amenities vary for each community.

DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote

these services without a specific license.

DEPARTMENT OF HOUSING & URBAN DEVELOPMENT (HUD) – helps apartment owners offer reduced rents to low-income tenants.

HOUSING CHOICE VOUCHER PROGRAM – (Sec 8) - find your own place and use the voucher to pay for all or part of the rent. Participating households are required to pay the greater of 10% of their gross annual income or 30% of their adjusted income towards rent. To qualify, a household’s annual income must not exceed the applicable income limit for the area as adjusted by family size.

SENIOR HOUSING DEFINITIONS

Senior Housing Definitions

(Sec 236) Property owners in this program make mortgage payments that are based on a 1% mortgage interest rate. HUD then provides a subsidy to their lender to cover the difference between 1% and the market interest rate on the property’s loan. Participating households are required to pay rent equal to the greater of 30% of their adjusted income (not to exceed the market rent) or the basic rent amount set by HUD for that particular property. Any amount paid by the household that is more than basic rent is considered excess rent, which the owner pays back to HUD in repayment of the subsidy.
