What Is the Wayfinder Program?
A pro-bono post-discharge care management program offered by Bluebird Health. It bridges levels of care by educating patients and families, as well as connecting providers and coordinating resources. Nurse Navigators communicate with hospitals, health plans, facilities, and/ or other providers involved to ensure patient centered care is through an interdisciplinary approach.
Wayfinder Goals:
n Ensure patients have the education and resources needed to avoid trips to the Emergency Department (reducing unnecessary and often unwanted hospitalizations) n Maximize the in-home benefits that patients qualify for under their insurance plan (coordinate care) n Provide the highest level of customer service (Improve patient satisfaction) n Provide the highest level of communication fostering continuity of patient care across the healthcare system n Reduce stress/waste in the community and the healthcare system (lower overall cost of delivery)
Wayfinder Team:
n Nurse Navigator – Nurse oversight to assist with medication and home safety, promotion of self-management, transitional support bridging needs between levels of care and transfers such as from
hospital to home, and streamlining communication among providers. n Social Worker – Licensed Social
Worker who provides counseling, resource connection, insurance and financial questions. n Bluebird Team – Access to our on-phone nursing staff 24/7. Joint staff care plan meetings and full agency support in providing a bridge to high quality care.
Who qualifies for the Wayfinder
Program? – Anyone who is transitioning between levels of care. Patients with a recent hospital stay, recent inpatient rehabilitation stay, or recent home health certification. Transitional care is intended to bridge providers and support those who need the extra oversight through the process.
Who Pays for the Wayfinder Program?
– No bills are issued to patients or their families, ever. Bluebird has identified a gap between providers, and as part of our mission Joy Through Service, we hope to provide value to the community and help.
Can I receive Wayfinder support and be on service with another provider? – Yes. Because there are no billed services, the team can supplement Home Health, Personal Care Services, or other in-home care. ~
