NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other nonmedical in home care. These services are available for hire after, or in conjunction with Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.
PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications.
SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature.

Hope for Caregivers Today’s family caregiving looks different from that of our great grandparents or great, great grandparents. Today’s caregivers are facing a perfect storm of challenges that make caregiving more stressful than ever before. Modern medicine has extended the aging process. While this means we are living longer, it means the need for care is longer too. On average, family caregivers are caring for their loved ones 4.5 years. That length of time doubles if they are caring for a family member suffering from dementia. Research has shown that the longer a caregiver has been providing care, the more likely she or he is to report their own declining health. This added caregiving time also means greater financial strain. Those receiving care are dipping into their life savings to pay medical co-pays for expensive medical treatments. As a result, many do not have the money to pay for basic services that would make it possible for them to live at home. 25% of caregivers report it is difficult to get affordable services in the community to help them with their caregiving. Because the care commitment is exponentially longer now than gen-
erations ago, caregivers are under extreme emotional distress. 40 to 70% of caregivers have clinically significant symptoms of depression. Many report strong feelings of frustration, anger, guilt and helplessness in their role as a caregiver. Even with all of these challenges, there is hope and support. The Family Caregiver Support Center at the Pikes Peak Area Council of Governments Area Agency on Aging (PPACG AAA) helps family caregivers. The center provides free consultations for caregivers, to help them identify needed resources, how to access these resources and then make a plan for continued caregiving. The Center provides free respite vouchers to give the caregiver a much needed break from the strains of caregiving. And the Center provides classes and support groups through community partners to help caregivers understand the unique challenges of today’s caregiving. Managing the Effects Care Management Can Help! of Aging Mrs. Morris, a 77 year old woman living at home, used to spend time with her daughter going to craft shows and working in the garden. A few months ago, Mrs. Morris began struggling with feeling unsteady, having difficulty walking and feeling some depression. As a result, much of the time spent with her daughter focused on filling her medication planners, going to doctors’ appointments, or running errands. Her daughter, busy managing her two boys and career, did not have extra time to spend doing the things the two of them enjoyed. This strained their relationship and Mrs. Morris wondered what she could do to relieve her daughter’s stress. Sound familiar? This is a common scenario for families: adult children are trying to manage their own children and careers while supporting their aging parents’ increasing needs. Fortunately, there is help! Care Managers provide support to individuals and families so that they can focus their energy on spending quality time together. Care Managers are highly educated, resourceful and unbiased advocates who can do many things a home care aide does not. They perform comprehensive assessments to create customized recommendations and care plans. They follow a client’s
16 COLORADO SPRINGS - NOVEMBER 2015-2016
goals, preferences and medical history and help them manage their appointments, services, medications and therapies. Some Care Managers are even on-call 24 hours a day to respond to emergencies. Ultimately, Care Managers can enable individuals to stay at home longer, to spend less time in the hospital and, most importantly, relieve stress for families. Last month, Mrs. Morris hired a Care Manager who did everything possible to understand her individual situation. The Care Manager coordinated physical therapy and a medication review to address her unsteadiness and risk of falls. The Care Manager assisted with her medication planners and medical appointments. And the Care Manager communicates frequently with Mrs. Morris and her daughter to keep everyone updated. Now Mrs. Morris feels relieved that her daughter can focus on her family and there is time again for the dearly missed crafting and gardening. Taking Dental Care Sometime ago Dr. Couchman To The World realized that there are people in the world, who are suffering & even dying due to dental disease.
Dr. Couchman has traveled on mission trips to numerous countries, including Argentina, Honduras, Guatemala, Dominican Republic & most recently to Uganda. Future trips are being planned for Ecuador & Honduras. Many people do not realize that dental & periodontal infection effects every cell & organ in the body by the continuous circulation of bacteria & toxins throughout the body by the blood system. Thus it effects every aspect of a person’s life. It effects their physical health & susceptibility to other diseases. It also affects their longevity, economic and social outlook. In children it affects their education. In Uganda 50% of the population is 15 years of age or younger. Dental equipment, such as motors, chairs & supplies can be packed up in 50 pound cases & be transported in checked luggage. For example 2 dental chairs that can hold up to a 400 pound person each pack up in a 42 pound box. People with many different interests & skills are extremely helpful in the care of people. For example Dr. Couchman’s wife, Cathy, who is an artist, therapeutic horse

riding instructor & business manager has been extremely helpful on the mission field. At home she has ministered to many people with special needs, such as physical disabilities, cognitive issues, emotional struggles, autism, alzheimer’s, cerebral palsy, multiple sclerosis & other syndromes. Recently, in Uganda she painted pictures to music to help calm people as they awaited dental care. At the same time she sterilized instruments over a charcoal fired sterilizing unit. Another aspect of Dr. Couchman’s trips is the training & mentoring of national dentists & staff members. On the recent trip to Uganda Dr. Couchman mentored 3 local dentists, who take care to the rural, underserved area. In Argentina he mentored a recent dental graduate who was caring for prisoners & others in need with a mobile dental unit, that Dr. Couchman, his wife & others helped to procure. As Dr. & Mrs. Couchman travel they share love, compassion & the The Good News to those in need. However, they feel as if they receive more than they give. Members of a discount dental plan are given special reduced fees from a network of excellent dentists who will perform all dental procedures at 20-60% savings. And you do get a tax deduction for your dental care costs. Dental insurance has a low annual limit on benefits-- typically $1200. AARP’s basic plan costs $478 per year, which gives you a maximum of $1000 for dental work. Or they have a plan with a $1350 maximum for $664 per year. This is typical of most stand alone dental insurance coverage. Though you usually
have twice a year cleanings and exams that do not count against the maximum, are they really “free” when you are paying those types of premiums? Today there are more options for seniors to help save money on dental costs. Consider what you spent on dental care last year, and find a discount dental plan so you can compare those costs. An estate plan includes the way your estate is passed on to your loved ones at the time of your death. It can be accomplished through a variety of methods, including Revocable Living Trusts, Wills, Joint Tenancy, and Beneficiary Designations. Problems happen when people don’t coordinate all of these methods of

passing on their estate. An estate plan ensures that whatever you own goes to whom you want, when you want, and is carried out in the way you want. With the right estate plan, you can even keep your family out of probate court. Estate planning may also include planning in case you become incapacitated, including health care directives. One of the biggest concerns today is that we will end up in a nursing home and lose all of our assets paying for it. While long-term care insurance (LTCi) is the best way to cover these costs, a good attorney can also help you plan to maximize your money in the event you don’t have sufficient LTCi to cover all of your needs. Medicaid Planning and Veterans’ Benefits Planning can help stretch your dollars. The most important element of a good estate plan is information! Make sure you work with a qualified estate planning attorney. A good attorney can help preserve the value of your assets and that they pass the way you want. Medicaid Planning for Finding a Pathway to Approval Long Term Care Webster defines planning as a map, or a way of doing in pursuit of a desired outcome. That’s why we make plans when it comes to things we value. From the more weighty matters of our education, career, taxes and retirement to the more light topics of vacation, workouts or even yard sales, we invest time and energy to build a pathway to what we want. As we continue to age and look beyond our retirement years we are confronted with yet another challenge and the need for a plan. Many of us have loved ones or will ourselves eventually need long term care (LTC). Whether in need of nursing home, assisted living or in home care, the cost of these services far exceeds what most seniors can carry on their own. For those who cannot afford or qualify for private LTC insurance, Medicaid remains the most common source of funding such services. However, Medicaid approval comes with eligibility requirements that most applicants lack clear understanding of and are not prepared to meet. That is where the value of Medicaid planning begins. Medicaid planning starts with a financial assessment that provides clarity through accurate information which enables better decisions with greater confidence. Planning identifies potential problems with eligibility and works to find solutions

that best match the client needs and priorities. Ultimately, Medicaid planning takes an individual from a point of ineligibility to a position of eligibility before the application is filed. The result is a smoother application process with less stress and worry. Medicaid planning can also identify legitimate means of preserving income and assets. To the surprise of many, Medicaid guidelines enable planning methods that protect income and assets for a spouse, disabled family members or even the applicant themselves. Such preservation can provide a means of supplementing future needs. Remember, proper planning works within the guidelines and never calls one to compromise his/her integrity. Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a "Medigap Plan." Medicare Part A HOSPITAL INSURANCE

REHABILITATION HOSPITALS Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. THE XLIF® PROCEDURE: TURNING SPINE SURGERY ON ITS SIDE BY PAUL STANTON, D.O.
In most traditional spine surgeries, surgeons approach the spine from the patient’s front or back. These approaches are necessary in many cases, but they can also be very disruptive to muscles, nerves, and blood vessels. This surgical disruption can cause many complications and require a long recovery period. That’s why, when a patient with chronic back or leg pain is a good candidate for it, I usually recommend the minimally invasive eXtreme Lateral Interbody Fusion (XLIF) procedure. How does XLIF benefit patients? With the innovative XLIF technique, I approach the spine from the side of the patient’s body, rather than the front or back, thus avoiding most muscles, nerves, and blood vessels. Every individual is different, of course, but most of my XLIF patients experience a shorter operative time, less blood loss, shorter hospital stay, and fewer complications than traditional spine surgery patients. Many XLIF patients are up and walking the same day as the surgery and return home the next day.

How does XLIF work? First, I make a small incision on the patient’s side. Then I use dilators to direct my path to the affected disc in the spine, while monitoring the local nerves. Once the safest path has been determined, I use a retractor to provide access and visibility to the affected area. Next, I remove the diseased or damaged disc to reduce pressure on the patient’s spinal cord or nerves (that’s what causes the patient’s back or leg pain). Then, still working from the side, I place an implant into the empty disc space to restore proper disc height and provide support while bone grows between the two vertebrae during the fusion (bone healing) process. Sometimes, I also affix a combination of screws and plates to the adjacent vertebrae, to help hold everything in place while fusion occurs. Once the vertebrae have naturally fused together, the spine is stabilized. Is XILF right for you? The XLIF procedure might be a good option for you if you require an intervertebral fusion at any lumbar level between vertebrae L1 and L5, if you do not require direct nerve decompression through the same approach, and you would benefit from a less disruptive approach. Not everyone may be a good candidate for the XLIF procedure. As with any surgery, each patient should be treated individually, and a discussion of suitability should take place between patient and physician.

These agencies are licensed by the state. HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service.
INSURED – This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance. LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided. CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services. Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. Services Provided: N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations. HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement. Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are ade-

quate in maintaining the individual’s symptom management. Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility. RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Longterm care facility. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control,

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays. AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being. This is not reimbursed by Medicare.
IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.
PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare.
PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which
VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care.

“You matter because you are you, and you matter to the end of your life”. –Dame Cicely Saunders, Founder of the Hospice Movement. While the number of Americans receiving hospice care grows each year, the National Hospice and Palliative Care Organization (NHPCO) estimates that for every patient who receives hospice, there is at least one more that could have benefitted from the comprehensive services for the patient and family, but did not receive this specialized care. Hospice workers hear over and over from those they serve, “I wished we’d signed up sooner”. Studies show that patients under Hospice care actually live, on average, 27 days longer than patients with similar ailments who are not under Hospice care. Hospice is a philosophy of care, not a place. At the center of hospice care, is the belief that we all have the right to die with comfort and dignity, and that our loved ones will receive the necessary support to allow us to do so. Hospice services are available to patients facing a life limiting illness. Services are fully covered under Medicare, Medicaid,
most private insurance plans, HMOs and other managed care organizations. Hospice involves a team-oriented approach to expert medical care, pain and symptom management, and emotional and spiritual support tailored to the wishes of the patient and family. Hospice care is delivered where the patient is. Most often, that is the patient’s home, but may also be provided in facilities. The hospice benefit is being underutilized. There is an understandable reluctance to contemplate end of life. For some, they view it as giving up hope. Perhaps, there is no longer hope for the condition, but the patient might still hope to accomplish a meaningful event or activity. The team works with the patient and family to achieve these goals. When facing end of life, we encourage you to look at the “H” word not just as hospice, but also for the help that can be provided to the patient and their family through comfort, care, and choice.

Senior Housing Definitions AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community. ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Specifically trained staff usually serves the special needs of this population. Amenities vary for each community.
DEPARTMENT OF HOUSING & URBAN DEVELOPMENT (HUD) – helps apartment owners offer reduced rents to low-income tenants. HOUSING CHOICE VOUCHER PROGRAM – (Sec 8) - find your own place and use the voucher to pay for all or part of the rent. Participating households are required to pay the greater of 10% of their gross annual income or 30% of their adjusted income towards rent. To qualify, a household’s annual income must not exceed the applicable income limit for the area as adjusted by family size.

Happier, Healthier Caregiving, You may be a spouse, or a sibling. A It Is Possible! grandchild, a member of the same church, or even a neighbor. You are a caregiver. To the person you are providing care, you are a lifeline. You are ensuring they get the emotional and physical support they need, but what about you? Is the stress of caregiving leaving you vulnerable to a wide range of physical and emotional problems? The answer is a resounding YES. As a caregiver, you probably have been told that by doing so you put your own health at risk, and this can certainly affect your ability to provide care. Most importantly, this can ultimately impact both you and the person you’re caring for. But have you really considered the consequences? You can’t help anyone if you yourself become ill or injured. So what strategies can you employ to help prevent or at least minimize the chances of that happening? Here are a few helpful tips to consider. First and foremost - please don’t be afraid to ask for help, and by all means say yes when offered help or a break. Take care of yourself - don’t cancel or fail to make important appointments in lieu of caring for another person. Get plenty of sleep, eat well, exercise, continue to pursue passions

or better yet find a new one. Reach out - informal support in the form of sharing with family and friends, or formal support such as counseling with you minister, pastor or priest, or caregiver support groups. Inspiration and Community Caring for someone with Alzheimer’s for Alzheimer’s or dementia can be one of the most challenging roles any of us Caregivers will ever face. Alzheimer’s can be a very long course of illness.
It is the only one of the ten top causes of death in America with no treatment, prevention or cure. It robs the person living with the disease of their memories and basic social skills, which can leave the caregiver feeling isolated even from the person they care for daily. In addition, studies have shown that caring for someone with Alzheimer’s can impact the caregiver’s health and cognitive function. To help address the unique challenges of Alzheimer’s and dementia caregivers, the nonprofit ClergyAgainstAlzheimer’s has produced a book of meditations, Seasons of Caring – available at www.seasonsofcaring.org where you can read excerpts of the book free of charge. Meet Our Cover Girl, Margo was born in New Margo Lucey Orleans in July 1936.
She grew up oversees spending time in both Cairo and Rome, as her father was a navigator with Trans World Airlines.
The family moved back to the states just in time for Margo’s senior year in high school. She was picked as the first drum majorette for the high school band. It was with this selection that Margo’s life would turn in an exciting direction. In working with the band photographer, he asked Margo if she had ever considered entering any beauty pageants. Margo had not. Margo’s parents were hesitant to allow her to enter these beauty pageants, but with discussions and training on the “dos and don’ts” of pageant life, they allowed her to take part. In the summer of 1956, Margo went on to win four consecutive pageants: Miss Drum Point Beach, Miss Old Dominion Speedway, Miss Tacoma Park, and Miss District of Columbia. With her successes that summer, in the fall of 1956, Margo flew to Atlantic City to enter the Miss America Pageant, representing the District of Columbia. Back then the pageant took several days to complete 134 COLORADO’S WESTERN SLOPE - JULY 2015-2016
and competition categories were Evening Gown, Talent and Bathing Suit. Personality was judged at breakfast. On the final evening, 10 semifinalist were chosen and then again competed in the three aforementioned categories. Five finalist were selected, with Margo being the first runner up! It was later learned that Margo lost the 1956 Miss America Pageant by four one hundredths of a point to Miss South Carolina. Margo treasures her experience from those pageant years and knows it shaped who she became in later years. Directly, her winnings helped pay for her remaining two years of college, finishing with a BA in Liberal Arts. Margo also married the escort she was assigned during the Miss America Pageant. Together they raised their children and lived a wonderful life together. Eventually one of Margo’s children encouraged her to move to Colorado. Margo now spends her time in Colorado Springs at Crestview Assisted Living where her meals, medications and even entertainment are taken care of. Because of this, Margo can read, knit, watch wildlife out her window, reminisce about her pageant days, and best of all spend quality time with her daughter.
