Boulder County Area Agency on Aging Delivers an Array of Services

The Boulder County Area Agency on Aging (BCAAA) funds and delivers an array of services that promote and encourage aging well across Boulder County.

BCAAA helps people of all means, backgrounds and abilities connect and engage with their communities, learn how to navigate and access available services, make more informed choices, and better manage the process of getting older or caring for a loved one. Many of BCAAA’s services are for people age 60 and older, adults with disabilities, family and friends caring for older adults, veterans and their families, and professionals who work with older adults.

BCAAA understands that health happens at home and in the community and recognizes that a little help can go a long way toward fostering healthy, informed, active, and connected lives. The Aging and Disability Resource Center

(ADRC) is the BCAAA’s front door, where anyone can speak with or email staff with questions related to aging services and resources, caring for an aging friend or relative, or resources for people with disabilities. Resource options counselors are on hand to help anyone navigate resources, understand options, and make informed decisions.

The Boulder County Area Agency on Aging offers an array of direct services to individuals, including Medicare basics classes and counseling sessions, family caregiver support, chronic disease management and other wellness opportunities, and ongoing support for older adults who may need an additional level of assistance and “wrap-around” services. BCAAA advocates for older adults through programs and services that support residents of long-term care facilities through the Long-Term Care Ombudsmen Program, LGBTQ+ older adult responsiveness trainings for professionals, and a significant amount of community and political advocacy for Continued on page 28

Medicaid penalty period, which would make you ineligible for Medicaid for a period of time.

However, there are circumstances in which transferring a house will not result in a penalty period.

One of those circumstances is if the Medicaid applicant transfers the house to a “caretaker child.” This is defined as a child of the applicant who lived in the house for at least two years prior to the applicant’s entering a nursing home and who during that period provided care that allowed the applicant to avoid a nursing home stay. In such cases, the Medicaid applicant may freely transfer a home to the child without triggering a transfer penalty. Note that the exception applies only to a child, not a grandchild or other relative.

Each state Medicaid agency has its own rules for proof that the child has lived with the parent and provided the necessary level of care, making it doubly important to consult with your attorney before making this (or any other) kind of transfer.

Others to whom a home may be transferred without Medicaid’s usual penalty are: n Your spouse n A child who is under age 21 or who is blind or disabled n Into a trust for the sole benefit of a disabled individual under age 65 (even if the trust is for the benefit of the Medicaid applicant, under certain circumstances) A sibling who has lived in the home during the year preceding the applicant’s institutionalization and who already holds an equity interest in the home. ~

The 3 P’s to Better Hearing TV commercials or online ads may imply something different, but there are 3 critical factors to successful hearing treatment. A lack of any of the 3 could negatively impact your success to better hearing.

The first and most important “P” is the Professional. Finding the right professional is essential! In fact, 7 of the top 10 factors in patient satisfaction are directly related to the relationship and best practices performed by the professional!

The second “P” is the Process. The majority of hearing-impaired people did not lose their hearing overnight. Patients only realize they are ‘missing’ things when it affects conversations that are valuable, but by then there are many sounds that have disappeared.

Know Where to Go for Emergency Care There was a time when the local hospital emergency room was a person’s only choice for an illness or injury that wouldn’t wait for normal business hours.

Today, it’s one of a menu of options for such care, from urgent cares to neighborhood micro-hospitals and stand-alone ERs. Defining an emergency It starts with knowing what a health emergency is. Generally speaking, it’s a serious or life-threatening condition that requires immediate attention or advanced care such as heart attack, major broken bones and large wounds. Emergency rooms, whether they are attached to a hospital or are stand-alone, are open 24 hours a day, seven days a week, and offer extensive testing options to care for life-threatening emergencies.

Have a Plan In the event you are sick or have an emergency, you don’t want to be scrambling for where to go or what to do. Calling 911 is the obvious first step in the most serious of emergencies, but you should know what services are close to you for needs that don’t rise to such a level. n

Find out where the nearest accredited Chest Pain and Certified Stroke Centers are. These emergency departments provide the best care for these specific critical issues.

Keep vital information with you, Keep a list in your wallet of such information as medications, allergies, primary care doctor and advanced directives.

Know your insurance, such as if you have differing co-pays for urgent care, emergency care and doctor visits. When you have a true emergency, don’t worry about insurance and call 911 immediately.

As we grow older, good nutrition is important but may cause a challenge to aging adults. Getting to the grocery store alone can be an obstacle, as well as having the mobility to actually prepare nutritious meals on a daily basis.

Although some delivery services like Meals on Wheels are convenient, they are not customized to the individuals tastes and likes. Hello Fresh and other meal box type services can be expensive, and the meals still have to be prepared. Some home care services provide meal prep, but only on a limited basis and the care provider is probably not a trained Chef.

Chefs For Seniors provides meals prepared in the home by a professional Chef. Meals can be customized to individual tastes and dietary needs. The meals are packaged with care for the week in microwave safe containers.

How to Preserve Assets When Faced With Long Term Care Costs Meet Charlie and Fran. They have three children, 9 grandchildren and 5 great grandchildren… so far!

They have hopes and dreams for their future generations and have worked hard to ensure their legacy is passed on. As they age, they realize that if either one of them needs Long Term Care, that their property and other assets could be at risk. They need accurate information they can trust so that if Long Term Care needs come on suddenly they are prepared and able to protect their assets.

It is estimated that over 70% of people over 65 will need some type of Long Term Care(LTC) according to the US Department of Health & Human Services. Colorado has many resources for financial help like Health First Colorado, Colorado’s Medicaid Program and Veterans Aid and Attendance Pension that will help pay for in home care, assisted living and nursing facility care.

Most Colorado elderly who need help paying for LTC feel that they will not qualify for these programs because they have too much income or too many assets. In reality, many could qualify if they had the information from a knowledgeable professional, holding a VA Accreditation or a Certified Medicaid Planner (CMP) designation. The problem is, that often plans for one program will conflict with good planning for the other. With uncertain care costs it is valuable to have one person or firm

who understands multiple programs for assistance to ensure that in planning for one solution, an applicant isn’t disqualifying themselves for future aid from other programs. Aid and Attendance is available only to veterans who have served during wartime. It has a cap to benefits and if costs of care exceed the VA maximum benefit, then other programs will offer a better solution.

The rules of eligibility are readily available online. However, incomplete or inaccurate advice often causes people to think they must “spend down” to qualify for these programs. This simply is not true and can lead to an unnecessary liquidation of the assets people have worked a lifetime to obtain.

There are legitimate means of preserving valuable or sentimental assets and income. Here in Colorado, to the surprise of many, following guidelines carefully afford numerous opportunities to protect income and assets. What is PACE? Program of All-inclusive Care for the Elderly (PACE) are programs that provide comprehensive health services for individuals age 55 and over who are sufficiently frail to be categorized as “nursing home eligible” by their state’s Medicaid program.

Eligibility for PACE requires that individuals be 55 years old or older, certified by the state to need nursing home-level care, reside near a PACE program, and be able to live safely in the community.

Services include primary and specialty medical care, nursing, social services, therapies (occupational, physical, speech, recreation, etc.), pharmaceuticals, day health center services, home care, health-related transportation, minor modification to the home to accommodate disabilities, and anything else the program determines is medically necessary to maximize a member’s health.

How PACE works: PACE programs become the insurance and the health care provider and are directly responsible for providing all health services and manage all aspects of their participant’s healthcare. PACE is paid

through Medicaid and Medicare combined (dual eligible) or private pay on a monthly basis.

Most PACE patients have multiple diagnoses, with an average of over 7 diagnoses per member. Among the most common are cardiac problems, diabetes, hypertension, and vascular disease. PACE programs tend to provide high levels of preventive services, such as very frequent check-ups, exercise programs, dietary monitoring, programs to increase strength and balance, etc.

PACE programs organize their services in a “PACE Center”. These Centers have a Day Center, medical clinic, social services and rehabilitation services, along with administrative staff, all in one site. Members attend centers from rarely to 5 days a week, depending on their care plans. Alongside cost of care these two questions are commonly asked by Medicare beneficiaries when considering whether to change their health plan. Unfortunately, opportunities to look beyond costs and coverage are sometimes overlooked.

There are many other factors to investigate when reviewing plans and considering change.

Of most importance is quality of care. The level and quality of care associated with a health plan can play a significant role in the healthcare experience as well as overall health of the member.

Find out: what are the quality ratings associated with plans being considered? What does the plan offer that would be valuable to me?

Do they simply provide insurance, or do they plan an active role in helping members to obtain or maintain good health standing? Health plans are easy when healthy… what about when sick? What resources are provided to assist in that time of need?

Also consider time and communication are the two most crucial elements to success in healthcare yet not all organizations recognize

Can CBD Help You? CBD (cannabidiol) is one of hundreds of chemical compounds, including TCH (tetrahydrocannabinol), found in the hemp or cannabis plant.

While THC is known for producing a “high,” CBD is non-psychoactive.

Because of this, its appeal as a potential therapeutic option for a myriad of conditions has grown. Extensive preclinical research—much of it sponsored by the U.S. government—indicates that CBD has potent anti-tumoral, antioxidant, anti-spasmodic, anti-psychotic, anti-convulsive, and neuroprotective properties. CBD directly activates serotonin receptors, causing an anti-anxiety effect. It works by modulating the endocannibanoid system, a hitherto unknown and complex neurochemical system encompassing most of the human body, including the brain, central and peripheral nervous systems, immune system, and peripheral organs.

Recently there has been an onslaught of CBD products hitting the market amid claims that it is a cureall. While certainly not a panacea, a solid body of research shows CBD’s numerous potential benefits.

Non Medical Home Care Definitions 24 HOUR ON CALL – Refers to ‘around the clock treatment’ in a home environment. The care may be shared by multiple caregivers or by a single caregiver.

ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change.

BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company.

HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state.

HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service.

INSURED – This refers to what happens if someone gets hurt on the job.

If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided.

CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button.

MEDICATION REMINDER – Assistance with reminding an individual about medications but not dispensing medications.

TRANSPORTATION – Rides to shopping, appointments and other outings.

WORKER’S COMP – Insurance that covers medical and rehabilitation costs and lost wages for employees injured at work. ~

Cardiac care may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s.

ENTERAL FEEDING – Enteral feeding, or tube feeding, is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facil-

ity. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment.

HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.

INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

Facing the Challenges The challenges faced by individuals of Parkinson’s with Parkinson’s disease (PD), even within their own homes, can be Disease many and varied. PD is a disease that develops and worsens slowly over time.

Symptoms range widely from motor symptoms (like tremors, coordination, balance, walking deficits, lack of facial expression, decreased vocal volume and inflection) to non-motor symptoms (like depression, anxiety, constipation, lethargy). Symptoms can vary greatly from person to person, depending on how long they have had the disease, what sorts of treatments they employ, as well as other disease processes and other factors which may impact them. Treatments which can be used to successfully manage the symptoms and may assist with slowing the disease progression include medication management, evidence-based, PD-specific exercise programs and possibly surgery.

Skilled home care agencies working with these individuals are in the unique position of seeing exactly what challenges patients with PD and their care partners face in their homes. Symptoms can fluctuate, sometimes in an unpredictable manner. Skilled home health staff can help to improve safety and function within the home, as well as help with the transition back to the community.

Within the home, individuals with

Parkinson’s disease may struggle with slow, small movements which can affect their ability to dress, bathe, move in bed and prepare food efficiently and safely. Deficits in walking caused by PD may make it difficult to negotiate changes in flooring, small spaces and doorways, and may also increase an individual’s risk for falls.

Individuals with PD and their care partners may require community resources and/or may need to find additional caregiver help in the home and information regarding finances and planning for the future. Lack of facial expression, deficits in speech and decreased breath support may impact communication and individuals may have cognitive deficits. Correct and timely medication regimens are also integral to safe and efficient management of symptoms.

Home health clinicians with specialized training coordinate closely with all team members to target the deficits associated with Parkinson’s disease. Antioxidant vitamins and minerals (AREDS2 multivitamins) are an effective treatment for dry AMD that has been diagnosed by your eye doctor. Smoking is a large risk factor for macular degeneration, especially wet macular degeneration. Treatment of exudative (or wet) AMD includes injections of medications into the eye, or less commonly a

laser treatment to the retina.

Macular Degeneration is an area of intense research by scientists and physicians, and there are many ongoing clinical research trials and new treatments in development.

Easing the Journey With Palliative (pal-lee-uhtiv) care is Supportive Care a medical specialty for people of any age living with a chronic or serious illness.

It is sometimes referred to as “supportive care.” The focus of palliative medicine is relieving pain and distressing symptoms, providing resources and helping patients and families plan for the time ahead.

Palliative care starts with a question: What’s important to you? Some patients want to feel well enough to attend a wedding or family reunion.

Others want to continue working, or to enjoy simple pleasures, such as a meal with friends.

Patients may desire help planning around treatment, to mitigate side effects and have more energy for everyday life.

The palliative team creates a plan to meet the patient’s goals. Care typically is provided by a palliative nurse practitioner (NP), who works with a patient’s existing physicians (including specialists), providing an added layer of care that considers a patient’s whole picture.

The NP is skilled at managing complex symptoms and staying on top of them as conditions change.

This can include treating nausea, shortness of breath, neuropathic 98 BOULDER COUNTY - OCTOBER 2019-2020

pain, sleeplessness, constipation and other distressing conditions.

The goal is helping people feel well enough to participate in their lives to the fullest extent.

Living with illness can be a hard road. Palliative care serves to ease the journey for patients and their families.

When is the right time for palliative care? For someone living with a chronic or serious illness, such as cancer, COPD, or diabetes, the answer is: Today. Studies have shown that those receiving early palliative care not only have improved quality of life, but less depression and in some cases, longer overall life expectancy.

Palliative care is typically covered by private insurance, Medicare and Medicaid and provided wherever a patient resides. Senior Housing Definitions ACTIVE ADULT COMMUNITIES are traditionally geared towards the physically active senior, these communities can be apartment style, cottage/ patio home, or Condo/Townhome style living. There are often community gatherings and many offer enhanced amenities like concierge, golf courses and recreation centers. They usually do not offer meal or meal option community dining (but may have a restaurant on site). May be a buy-in or a rental property.

AGE RESTRICTIONS – Some communities have a restriction on the youngest age they are allowed to accept.

These restrictions vary by community.

ALZHEIMER’S / DEMENTIA ASSISTED LIVING COMMUNITY – Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety.

ASSISTED LIVING COMMUNITY – Typically apartment-style with kitchenettes and private baths.

Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation.

DAY CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for

the day. Licensed communities are prohibited by law to promote these services without a specific license.

DEPARTMENT OF HOUSING & URBAN DEVELOPMENT - (HUD) helps apartment owners offer reduced rents to low-income tenants.

INDEPENDENT HOUSING – Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...)

INDEPENDENT LIVING is usually apartment style communities for adults 55+ with little or no senior support services. Many but not all are wheelchair accessible.

INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner.

LONG-TERM CARE – Chronic, custodial care provided on an on-going basis.

MEDICAID WAIVER – An assisted living waiver may be used to transition individuals out of nursing facilities or to provide persons at risk of institutionalization with options for remaining in their communities.
