Advantages to Utilizing a Revocable Trust in A critical piece in one’s estate plan is the disposition of assets Your Estate Plan upon death. This can be done through a last will and testament or a revocable trust, which is referred to as a “will substitute.”

When a revocable trust is utilized, it is the governing instrument as to where assets go, and it does have some advantages over a will.

A revocable trust provides more privacy than a will. When someone dies, his or her will is lodged with the court and becomes public record. A trust does not become public record, and there is no need to show any public parties or entities the dispositive provisions of the trust. This protects the privacy of the creator of the trust and the beneficiaries.

Furthermore, a trust is a good vehicle to manage assets in the event of incapacity. While a will does not come into effect until death, a trust is in force as soon as it is signed.

If the trust is properly funded, meaning that the person’s assets are transferred into the name of the trust, then the successor trustee can easily step in if the creator of the trust (also known as the “settlor” or “grantor”) becomes incapacitated and can manage the trust assets and make distributions for the benefit of the settlor. It is also typically easier, faster, and cheaper for the successor

trustee to administer the trust upon the death of the settlor, rather than going through the probate process to administer an estate.

It is especially beneficial to use a revocable trust when you have out-ofstate property. This is because, typically, one must go through the court process of probate to administer your estate in any state where you own real property in your name individually. Therefore, if you have real property in two states other than Colorado, your loved ones could be forced to open probate in three states, resulting in greater costs and a much longer process. If the real properties are transferred to a revocable trust, then this is avoided.

While there are advantages to using a revocable trust, it may not be the best estate planning tool for everyone. Good Samaritan Medical Center (GSMC) provides community members with convenient access to resources and specialized care without having to transfer to another facility.

The leading cause of trauma at GSMC and in Boulder County is older adult falls. In 2016, the hospital saw over 300 patients admitted due to a fall, with approximately 65 percent of these patients aged 65 years and older. As a Level II Trauma Center, the hospital is charged with addressing the most common mechanisms of injury by empowering community members with injury prevention strategies and education.

One such resource available to all Boulder County residents is the Matter of Balance class. Taught in conjunction with the Boulder County Area Agency on Aging, the Matter of Balance class is an eight-week course held in the fall and spring, and runs for two hours once a week.

Focusing on strategies for remaining active and independent, the course teaches exercises to improve balance skills, flexibility, strength, coordination and overall health condition. In addition to exploring and discussing their own beliefs about falling, participants learn about nutrition, how medications can increase the risk for falls, ways to talk with a doctor about falls, and methods for improving home safety.”

Motor vehicle accidents are the second leading cause of traumatic injuries treated at GSMC. To help facilitate improved driving safety, the hospital hosts a CarFit event once a quarter. Developed by AAA, AARP and the American Occupational Therapy Association, CarFit utilizes professional technicians to properly fit a driver’s seat, safety belt, mirrors and more to ensure the driver is as safe as possible. Occupational Therapists are also involved to help recommend adaptive features to address unique physical needs.

What is PACE? “PACE programs help the most vulnerable seniors in the community improve their quality of life.”

PACE stands for Program of All-inclusive Care for the Elderly.

PACE programs coordinate and provide all needed preventive, primary, acute and long-term care services so that their participants can continue to live in the community. At the core of the PACE model is the Interdisciplinary Team (IDT): the IDT is comprised of physicians, nurse practitioners, home-health nurses, social workers, physical and occupational therapists, transportation coordinators, aides and others.

They meet regularly to exchange information and solve problems as the conditions and needs of PACE participants change.

PACE Centers PACE participants attend the PACE center an average of three days per week. The center includes a health clinic with an on-site physician and nurse practitioner, physical and occupational therapy facilities and at least one common room for social and recreational activities.

Because PACE participants have regular contact with primary care professionals who know them well, slight changes in their health status or mood can be addressed immediately.

How do you qualify for PACE? A person must be age 55 or older, live in a PACE service area, be certified by the state to need nursing home-level care and assessed to be able to live safely in the community with PACE services at the time of enrollment. PACE programs tend to attract participants who are older and have very high care needs.

Who pays for PACE? PACE is a dual eligible program through Medicare and Medicaid; private pay may also be arranged.

PACE receives a monthly lump sum payment, which is used to pay for a variety of comprehensive services and care the participants need. ~ Editor’s Note: This article was submitted by Leslie Mader. It is the only one of the ten top causes of death in America with no treatment, prevention or cure.

It robs the person living with the disease of their memories and basic social skills, which can leave the caregiver feeling isolated even from the person they care for daily. In addition, studies have shown that caring for someone with Alzheimer’s can impact the caregiver’s health and cognitive function.

To help address the unique challenges of Alzheimer’s and dementia caregivers, the nonprofit ClergyAgainstAlzheimer’s has produced a book of meditations, Seasons of Caring – available at www.seasonsofcaring.org where you can read excerpts of the book free of charge. Memory Care: When Is It Time to Seek Help?

When you or your loved one is having difficulty with memory, it can feel overwhelming and trigger fear about what will happen and how will we cope. Here is a quick guide to match what support you need during the changes that occur over time with memory care.

Getting the Diagnosis The primary care MD can provide initial testing, such as a Mini-Mental Status Test, and may refer to a neurologist or gerontologist. The goal of the evaluation is to rule out any medical concern that affects memory. We are

Aging Gracefully: Reducing The Risks It is no secret that we are living Of Polypharmacy longer. Unfortunately, we are not necessarily living healthier.

An ever-growing percentage of the population is managing one or more medical conditions, which are managed by one or more medicines.

While medicines can help maintain health and prevent further illness, taking combinations of drugs means that you could be at risk for drug interactions. Polypharmacy is defined as the concurrent use of many different medicines by the same person, and presents a growing risk among seniors in the US. This can result in a gradual accumulation of side effects. Problems may be increased by taking: n Dosages that are too high n Medicines that are incorrectly prescribed or filled n Medicines that interact with or duplicate the actions of other medicines n Herbal supplements that interact with prescription medicines Polypharmacy can result in adverse drug events, complicating therapy, increasing cost, and presenting a challenge for healthcare agencies. Unfortunately, the symptoms caused by polypharmacy can be confused with the normal aging process.

The good news is there are things you can do to help reduce your risk of polypharmacy.

Carry your medicine list with you everywhere. Bring it and your pill bottles along when you see the doctor.

Ask your doctor or pharmacist to check your list for possible interactions.

If you have more than one doctor, make sure each one knows what the other is prescribing.

Ask your primary doctor if you need to take all the medicines on the list, or if you can reduce the dosages.

Always read labels. They may help you avoid a possible drug interaction.

Always get your prescriptions filled at the same pharmacy.

Learn the names of your medicines and why you take them.

Avoid combination products like cold remedies. Ask for help buying only the specific medicine that treats the symptoms you are experiencing.

Never take a new drug without discussing side effects and interactions with your doctor/pharmacist.~

Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions.

Why Water Exercise? Time to Aquatic exercise is one of the best Take the Plunge! all-around workouts for seniors.

Here are a few reasons to take the plunge and try aqua fitness.

Less impact on your joints While in the water, your body is buoyant and the impact to your joints is significantly less than exercising on land.

More resistance Water provides substantially more resistance, making each movement in the pool more challenging. Additionally, muscles typically work in pairs; i.e. biceps & triceps or quadriceps & hamstrings.

Water exercise works both muscle pairs because you are always encountering resistance.

Gets your blood moving You can workout at a higher intensity in the water than on land. Hydrostatic pressure, the pressure that the water exerts on the body while submerged, assists in blood flow and improves blood oxygenation.

DIABETES – Diabetes, like hypertension, responds well to increased physical activity. A regular exercise program is a cornerstone to diabetic care.

You will be able to find a variety of water exercise classes; such as Aqua Strength, Aqua Circuit, Aqua Kickboxing, and Aqua Zumba just to name a few. If you’re not ready for an exercise class, just walking in the water or swimming - both have great fitness benefits. ALZHEIMER’S/DEMENTIA CARE – Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change.

BONDED – Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company.

HOMEMAKER/COMPANION – Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state.

HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service.

INSURED – This refers to what happens if someone gets hurt on the job.

If the company isn’t insured, then the claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided.

CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

Six Signs It’s Time for More Help One of the most difficult decisions adult children can face is discerning when to step in and support their aging parents with their basic daily living tasks.

The decision to help an aging parent is complex, both emotionally and practically. Admitting the need for help and accepting assistance is not easy for older adults. Although every situation is different, here are six signs to consider when discerning if your aging parents need more assistance to maintain their independence at home.

Changes in Personal Hygiene The first sign that it might be time for more help is an obvious change in appearance. Take note of unpleasant body odor, strong smells of urine on clothing or in the house, and infrequent bathing. Scan mom or dad looking for disheveled grooming habits, such as unkempt hair, untrimmed nails, unshaven face, or insufficient oral care.

Medication Errors Next, take a look at mom or dad’s medication, both prescription and over the counter. Perhaps they use a pillbox and they have noticeably missed doses. Look for incorrect dosages as well by counting pills or reviewing their pillbox set up. Ask to

go with them to their next doctor appointment to review their medication and verify their prescriptions.

Neglecting Household Responsibilities Another sign that it might be time for more help can be very apparent when you visit mom or dad in their home. Look for dirty laundry piling up, dust buildup, unswept floors, unclean bathroom, extreme clutter, stains on carpet or furniture, and/ or stacks of unopened mail. Look around for dirty dishes laying around or placed back in cabinets. Peek in the refrigerator and pantry to check for spoiled or expired food.

Checkbook Activity A quick glance at your parents’ checkbook can be another indicator it might be time for more help. Look for mathematical errors, transposed numbers, checks that were written but not recorded, or checks that were written but never left the checkbook. Also look for unpaid bills or checks placed in envelopes but never sent.

Withdrawal from Social Engagements Learn your parents’ typical weekly social engagements and take note of how often they cancel. Do they get out on a regular basis, or have they been staying home more than usual?

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions.

WOUND CARE – Refers to services for providing diagnosis and treatment for ulcers, pressure sores and other wounds. Treatments may include but is not limited to irrigations, dressing, and wound vacs. Some agencies may employ specialty nurses to provide and instruct in care specifically for patients with more complicated wounds. ~

Active Minds Create Healthy Brains You’ve may have heard the phrase “use it or lose it” in reference to certain types of learning—perhaps speaking a foreign language—or maybe regarding physical fitness.

But did you ever stop to think this might be relevant for your brain as well?

The human brain is the largest muscle in our bodies.

And research indicates that the more we work it, the better it works!

Mentally stimulating activities are associated with a variety of brain health benefits including lower risk of Alzheimer’s disease and other forms of dementia, higher levels of memory and recall, and increased ability to perform all sorts of thinking tasks.

This is great news! We can have a positive impact on our brain as we age! In addition, the harder we work our brain, the more it bene-

fits. If you wish to create your own brain workout program, here are some guidelines to keep in mind: n

Do new things. That which is new is typically more difficult and thus a better workout for the brain.

Take a new route home, learn to play a musical instrument, or learn a foreign language. n Do things that are mentally difficult.

The more mentally challenging the activity, the better.

Play chess rather than tic-tac-toe.

Do things with other people.

Social stimulation is important for mental health. Attend lectures with friends or join a book group.

Make it fun. If it is drudgery, it won’t be sustainable. If you find something you like, that continues to be mentally challenging, stick with it!

What to Look For When Visiting a Facility Finding the best place for mom or dad is one of the most important decisions you can make. Listed below are some helpful hints on what to look for when you tour a senior housing or skilled nursing community.

CLEANLINESS – Look past the furnishings and into corners, baseboards, and windows.

Ask how often housekeeping is provided. hat services does maintenance provide? What is their response time?

WHAT DO YOU SMELL? If you find odors in a concentrated area of the tour this may indicate a single incident. Odors throughout the community are likely to indicate a problem.

WHAT ARE THE RESIDENTS DOING? Ask to watch activities. Are they well attended? Does the staff seem to be enjoying the activity as well? View the community event calendar. Look for small and large group activities, trips or outings.

WHAT IS THE STAFF’S ATTITUDE? Staff attitude and friendliness are of the utmost importance. Observe the staff interacting with current residents.

Do they listen and make eye contact? Make sure to get a good

understanding of the staffing pattern. How many people are truly involved in care?

ARE THERE OUTDOOR AREAS? Does it feel safe and secure? Do they host activities such as gardening in these areas?

Discuss entree choices, dining hours and procedures. Bring your loved one in for a meal to taste the food and meet some of the residents. Discuss what happens if a resident cannot make it to the dining room for a meal.

ARE THE SURROUNDINGS SAFE? Are the bathrooms accessible and do they have grab bars in convenient locations? How does a resident contact staff if they have an emergency in their room/ apartment? How may licensed nurses or staff members are available? What are the staffing patterns at night versus daytime?

How are medications managed?
