about us Center for Indonesian Medical Students' Activities (CIMSA) is a non-profit and non-governmental organization independent, nationalist, non-political, and non-partisan with activity-based programs. CIMSA consists of 6 standing committees related to health issues: sexual & reproductive health and rights including HIV/AIDS, public health, medical education, human rights and peace, research exchange, and professional exchange.

Having 30 active locals throughout Indonesia, CIMSA is affiliated with the International Federation of Medical Students' Associations (IFMSA), the most prominent student organization in the world with recognition from the United Nations. With the mission to empower medical students to actively learn and create, plan strategies, and execute movements to improve the nation's health and wellbeing, CIMSA continues its struggle to create a healthier, more secure, and more prosperous Indonesia with equal opportunities in education and health, and also to improve lives and reach prosperity and social justice.

Standing Committee on Professional Exchange (SCOPE) is one of the six (6) standing committees of the Center for Indonesian Medical Students' Activities (CIMSA) that engages in the field of professional exchange and has 17 locals spread in various faculties of medicine throughout Indonesia. Two national officers lead SCOPE CIMSA; National Exchange Officer for Incoming (NEO-In) and National Exchange Officer for Outgoing (NEO-Out), as well as maintained nationally by the National Exchange Committee.

SCOPE is a standing committee focusing on incoming hosting and outgoing sending, exchange promotions, global health awareness, and cultural understanding. SCOPE CIMSA is affiliated with IFMSA and develops good cooperation with various stakeholders and relevant external parties to facilitate student exchange. SCOPE CIMSA also organizes various projects as a form of character development for the members of SCOPE itself and conducts joint activities within CIMSA to improve the nation's health.

Dearest SCOPEople and exchange-enthusiast, Time has passed swiftly, and finally, we can meet face to face at the first national meeting held offline at Yogyakarta, which is National Leadership Summit. It is one of three CIMSA National Meetings that are held to empower all delegates with training, lectures, and encouraging sessions to improve the nation's health.

This year National Leadership Summit has a central theme of "Building Youth's Commitment and Participation to Achieve the 2030 Agenda". We hope that through this meeting, medical students could partake not only in contributing to a better country but also in making this world better as the leading actor.

We are delighted to share the second volume of our twelfth edition, PERISCOPE, a triannual magazine published by SCOPE CIMSA. Through this magazine, we would like to show you how each local committee thrived during the past term. We hope SCOPEople worldwide will be inspired by this edition and keep inspiring us with their unique ideas and creative breakthroughs. If this was the highlight of the past period, we are eager to see what is coming for all of us.

Greetings, SCOPEOPLE and exchange-enthusiast!

Welcome to the twelfth edition of PERISCOPE. We want to send hearty regards to our magazine's readers on behalf of the National Exchange Committee and our diligent editorial team, the Media and Communication Team, and wish them a pleasant reading experience.

In this magazine, we would like to introduce the new face of SCOPE CIMSA with new executives, namely the National Exchange Committee 2022/2023, or conNECt. We hope that conNECt and Local Exchange Officers 2022/2023 will collectively improve the SCOPE CIMSA locals throughout Indonesia.

In this volume of PERISCOPE, readers will get a glimpse of how unique our local activities are. Information, the reader, could read can broaden the reader's perspective as a future professional health provider.

In Search of The Perfect

Health System

by Angelique Nathania Wahyudi CIMSA UNAIR The COVID-19 pandemic has highlighted the daily challenges that hospitals and health systems face: high costs, infection control, and patient satisfaction, to name just a few. Although our countries have made strides to transition to value-based care and better manage population health, Indonesia has spent more on healthcare. Still, it ranks 11th out of 27 in quality compared to other nations in South-East Asian and the Pacific. As we all know, all health systems have strengths and weaknesses, and many reflect their country's history, culture, and values. Healthcare is a complex environment, and dealing with growing demand requires changing perceptions of delivery. Integration of primary and community care is difficult but essential for reducing pressure on the acute sector.

It's hard to imagine that a perfect health system could exist--one that would meet all patients' needs at reasonable costs. So, what must a country do to achieve this? Here are some components we must acquire, and let's take inspiration from other countries.

1. Universal healthcare The NHS in the United Kingdom offers the best universal health care.

The NHS was the first in the world to create a universal healthcare system that is available to all citizens, 11

regardless of whether they can pay for it.

2. Primary care Indeed, Israel has one of the highest life expectancy rates in the world and one of the lowest shares of gross domestic product spent on health.

Primary care is supported by four health maintenance organizations that act as both purchasers and providers of preventive, immediate, and community services. Out-ofhours care is provided around the clock and integrated with evening care centers, urgent care centers, and home visit services.

3. Community services For inspiration, we can look at Brazil, where community teams made up of doctors, nurses, nurse auxiliaries, and community health workers visit households every month, whether or not they demand or need it. These teams offer immunizations, chronic disease management, and screenings.

4. Mental health and well-being Australia has made the most progress in this area; they offer public funding to invest in crisis and home treatment, early intervention, and assertive outreach.

5. Health Promotion The Nordic countries best address the social determinants of health, encouraging individual responsibility and fostering collective action. The five countries have public health and illness prevention strategies contributing to low smoking, alcohol consumption, and obesity rates.

6. Patient and community empowerment The world can learn about patient empowerment from Africa, which blends community activism, patient education, social marketing, and behavior change to promote better health. One example is the maternity care program in Kenya, which encourages mothers and women to share experiences to help cut maternal and infant death.

7. Research and development Research and development are where the U.S. shines, noting the number of high-impact drugs and medical devices they have developed.

8. Innovation, flair, and speed In India, several organizations have created a hub-and-spoke model that focuses on cost-effectiveness rather than cost-cutting. Examples included standardized care pathways, making it easier to shift tasks and do more with fewer staff; referral networks that channel patients to the correct settings and even hospitals manufacturing their own devices or implants when suppliers refused to reduce prices.

9. Information, communications, and technology Singapore offers excellent examples of the sharing of patient data via a national electronic health record that allows access to all hospitals, community facilities, practitioners, and long-term care homes. This allows the country to comprehensively analyze clinical, financial, and operational data to assess healthcare costs and outcomes better.

10. Choice There are no out-of-network providers in France. Patients can go to any doctor or hospital they wish.

Patient satisfaction is exceptionally high, as are the country's quality.

11. Funding No country does it better than Switzerland in funding, which spends 11.5 percent of its GDP on health, according to Britnell.

The government also boasts high patient satisfaction, good clinical outcomes, and a life expectancy of 82.7 years.

But Britnell notes that the country can afford to spend so much because its economy is globally competitive and dynamic.

12. Aged care Japan offers its citizens compulsory long-term care insurance, which provides social care for all those older than 65 based solely on need.

The ability to pay is separate from the assessment process. The plan offers home help, community-based services, and residential and nursing care. The country has also created dementia homes where groups of people live together in a supportive, home-like environment.

Wouldn't it be wonderful to live in a country that offers this healthcare system? Given the continued fight in the country to provide affordable care to all, it does seem impossible.

But we can dream.

Exploring the Street of

SURABAYA

by David Surabaya, Indonesia Incoming of CIMSA UNAIR This exchange started in November last year when I was selected for our National Language test. Since then, I have worked hard as a nurse in a hospital to save enough money to travel to the other side of the world to experience something different than I have ever tried. I had no idea what city to choose, so my choice was random. I got Surabaya, my number one choice, although I didn’t know anything about this place. When I looked at the internet, everything seemed so interesting to me. Traveling to Indonesia was lovely, and my arrival to Surabaya was without problems.

I was supposed to be in the Internal Medicine – Cardiology department, but in the end, I was placed in the Endocrinology department, where I met nice doctors, namely dr. Deasy Ardiyny and dr.

Hermawan Susanto. They provided me with plenty of knowledge about endocrinology. I got the chance to enter the endocrinology ward, the endocrine ICU, and endocrine polyclinics. I must mention that I am thankful for all the students and residents in this department. They held their morning report in English for me to understand, which was terrific. They are also very communicative and very

curious as they wanted to know a lot about my country and told me reciprocally some exciting things about theirs. I had their numbers in case of any emergency, and they were always helpful when I didn’t know the place where I was supposed to be.

On the last week of my exchange, I asked the doctors if they could arrange for me to work some hours in the Internal Medicine – Cardiology department since Internal Medicine – Cardiology is my future residency.

They then introduce me to dr. Yusuf. He was a fantastic mentor to me. He stayed in touch even after the hospital, asking me if everything was fine and if he could do anything for me. He is a massive inspiration to me. He also did a morning report held online via ZOOM in English so that he could show me some interesting cases. I hope to see him again one day at some international congress of intervention cardiology.

I also mention something about my experience with the city of Surabaya. As I come from a small village and have never lived in a town bigger than 300.000 people, this is something different for me. Traffic here is unimaginable, and after three weeks here, I can thoroughly cross the streets without fear. I spend some excellent time in lovely places and some time in the suburb of the city. There is a massive difference between them.

When I heard France's name, I only thought about a beautiful and captivating country full of art, culture, and essential historical background. I am Kemal (21) from Yogyakarta, Indonesia. I became one of the participants in the IFMSA-SCOPE Professional Exchange Program.

Participating in an Exchange Program has been my dream since I was a kid.

When I was little, I always imagined being able to live abroad in thousand miles from Indonesia. When I discovered that my university could offer the SCOPE Professional Exchange Program, which cooperates with CIMSA and IFMSA, it boosted my spirits to always believe in and always dream about joining an Exchange Program. This exchange will be an enormous addition to my studies and an excellent once-in-a-lifetime opportunity that will contribute to my advancement in many ways.

I was placed in a not-too-small city, but an excellent one called Tours. I live in an apartment provided by the local committee. The condo was very comfortable and clean. Then the apartment is also close to the bus station and supermarket, which makes it very easy for me to live my days there. I started my 17

journey in Le Centre Hospitalier Régional Universitaire (CHRU) de Tours, Trousseau Hospital, a big hospital in a small town with extraordinary facilities. I was given a pass for public transportation for a month, where I could freely choose to take the bus or the tram. I usually woke up at 6.30 am to catch the bus to the hospital. The journey from my apartment to the hospital took about 45 minutes.

I was placed in Surgery-Orthopedics Department. In that department, I met many great doctors. One of them is the head of the department, Prof. Luc Favard. He is an orthopedic consultant for the elbow and shoulder. He taught me much about orthopedics, and I witnessed some of his major surgeries. I am also often accompanied by residents who are very friendly and kind. They helped me with my days in t h e hospital.

I have witnessed countless orthopedic surgeries from top to bottom during my time there. I'm estimating that maybe I've seen 30-40 surgeries in a month. In addition, I was also allowed to be an operating assistant five times. One of the best surgeries I will ever forget was when I had surgery with doctor Ramy Samargandi.

The surgery was oncological resection of the bone tumor (osteo-liposarcoma) and the insertion of a prosthesis. Apart from that, I also found various other cases, such as osteosarcoma, osteoarthritis, rotator cuff tendon rupture, hallux valgus, anterior cruciate ligament rupture, and broken cartilage of the shoulder.

The social programs that I had were also excellent. Incidentally, Tours is a small city surrounded by many beautiful castles. I've never visited a castle at all.

So, that experience became one of my best moments when in Tours. I have seen Chambord castle, Chenonceau castle, Villandry castle, and The Royal de Blois castle. The court was huge and magnificent, which shows the heyday of a kingdom at that time.

Besides, I do various other fun activities such as bowling, pizza party, and playing laser games. I also had time to visit some m a g n i fi c e n t cathedrals and the old city center, which is very exotic. It made me very happy, and I will never forget it.

Studying abroad is an opportunity that only comes once in life. In addition, working with different groups of people with different cultures has improved my abilities as a person, and being an independent person is also one of the recognitions I have gained through this program. In addition, this program has been a massive milestone for me by providing an overview of the diversity in foreign healthcare. I also gain deep insight into other cultures and systems that can develop me and give me a different perspective. It's a process to get and expand as much education as possible from other countries with tremendous advances in medical science, especially in orthopedic health sciences. These results have provided a new perception of advancing Indonesian health services. On the other hand, studying abroad has improved my skills and education in medical science, primarily orthopedic health sciences.

I want to thank all who have allowed me to participate in this program. Last but not least, this program is one of the best exchange programs ever. Thank you.

PREBIOTHICS 3.0 SCOPExSCORE Bioethics Day -by CIMSA BEM-KM FK Unand

A fundamental part of being a physician or researcher is the concept of bioethics. It is the first thing to consider as we, medical students, will work on the human body with their single lives. In preparation for doing so, medical students must have enough information and common ground in bioethics.

PREBIOTHICS 3.0 (SCOPExSCORE BIOETHICS DAY) was held, commemorating World Bioethics Day. This year, we encouraged medical students to actively contribute to our activities: social media campaign, public discussion, and public poster competition. We kicked off this year’s PREBIOTHICS 3.0 by opening registration for college students of any background with an interest in bioethics to record a video with a concept called “Middle Ground.” The campaign set out a space to talk and listen to volunteers’ arguments on the given prompt without judgment. So that the discussion became interactive, we selected volunteers with not-so-exact perspectives each other. On October 16th, 2022, the video was taken offline, with four volunteers signing a written informed consent statement before.

In our public discussion, we divided participants into several breakout rooms for a focus group discussion. In each room, there were moderators to lead the participants to sharing their opinions on eight questions. After 60 minutes, we returned to the main room and listened to three spectacular speakers, Nabila Puspakesuma, S.Psi, M.Sc of CBMH FK-KMK UGM, Prof. Dr. Rianto Setiabudy, Sp.FK(K), President of the Governing Board of MKEK IDI, and Prof. Eryati Darwin, PA(K)., Lecturer of Faculty of Medicine FK Unand, Member of MKEK IDI Wilayah Sumatera Barat.

As professionals in their field, we got much more knowledge than we thought we would work throughout their presentations. We learned about The Impact of Technology Advancement on Bioethics and Humanities, Research Ethics of Vulnerable People, and Future Innovations in Technology and Healthcare and Its’ Bioethical Issues, respectively. All participants actively listened and discussed with our experts through the presentation and discussion session.

To close PREBIOTHICS 3.0, we held a public poster competition with nationwide participants judged by linguists and professionals on aesthetics and bioethics. We hope that our PREBIOTHICS 3.0 will positively impact our audiences, and through future improvements to be done, we can reach a wider audience.

TERMINAL International Medical Students’ Talk -by MSCIA UB

Stem cell research, genetic testing, cloning: so much progress has been made in medicine and health sciences, thus allowing humanity to improve our health and control the development processes of all living species. However, concerns arise from social, cultural, legal, and ethical points of view of such progress, finally leading to one of the most significant debates of the past century. A new word has been coined to encompass these concerns: bioethics.

And so, the Universal Declaration of Bioethics and Human Rights was agreed upon during the General Conference on 19th October 2005.

World Bioethics Day is a celebration of the declaration’s adoption during the General Conference, which the Center for Indonesian Medical Students’ Activities annually celebrates. This year’s World Bioethics Day theme was ‘Social Responsibility and Health.’ And so, Medical Students’ Committee for International Affairs Universitas Brawijaya (MSCIA UB) has also celebrated World Bioethics Day to increase awareness of the ethical and legal issues relating to medicine and health sciences.

International Medical Students’ Talk or TERMINAL is one of MSCIA UB’s many activities which aims to promote bioethics awareness. This year, TERMINAL was held on 23rd October 2022, where two different sessions were held on the same day. First was a webinar with Dr. Dra Retna Siwi

Padmawati, M.A as a representative of the Center for Bioethics and Medical Humanities (CBMH) UGM, where she gave a thoroughly enjoyable lecture on “Euthanasia from a Bioethical Viewpoint.” Then, a second webinar was held by Dr. Fajar Ari Nugroho, S.Gz, M.Kes as a representative of Komisi Etik Penelitian Kesehatan UB to discuss bioethics from a research viewpoint.

Not long after, TERMINAL also held a speech and essay competition revolving around the different aspects of bioethics, with the final held on 29th October 2022. Many competed for the last prize money, and we are glad many people were interested in participating in our competition. TERMINAL was a big success from how active participants were in both events. We genuinely hope TERMINAL next year will be a much larger event with a much more significant impact on those who participated.

EXPECTO Explore English with SCOPE Today -by CIMSA UNAIR

Universal health coverage (UHC) means everyone has access to the health care they need, whenever and wherever they need it, without financial hardship. This includes various essential health services, from health promotion to prevention, treatment, rehabilitation, and palliative care. Countries experiencing rapid population growth have young populations that can promote economic growth and reduce poverty.

However, to benefit from the demographic bonus, governments must invest in the health and well-being of their people to build human capital and promote inclusive growth.

Improving reproductive health, mothers, newborns, children, and adolescents, and overcoming mental health disorders, are essential to achieve UHC. In realizing UHC, one of the significant steps that Indonesia has taken is to use the National Health Insurance, an integration of various forms of health insurance that have been previously implemented.

EXPECTO, or Explore English With SCOPE Today, is an annual event regularly held by SCOPE CIMSA UNAIR. The theme that is presented every year is also different, of course.

Last December, December 12 to be precise, WHO celebrated Universal Coverage Day. And that is one of the reasons UNAIR CIMSA SCOPE chose to raise the theme of Universal Health Coverage or UHC, which in Indonesia is usually called JKN this year.

There are three series of events from EXPECTO: UHC, pre-event - the main event - post-event.

The pre-event is the opening of a whole series of events. Therefore we chose the Upgrading and Social Experiment at Taman Bungkul Surabaya as one of our 'appetizers' before entering the 'main course.' For the upgrading itself, we invited Renata Evania as a CIMSA Certified trainer to provide provisions for all SCOPE CIMSA UNAIR members and newbies before going directly to the community. And for a social experiment at Bungkul Park, we brought posters and provided brief education regarding UHC to the community, and we managed to get positive feedback from the wider community.

Entering the main event, we have two events: offline talk shows and educational videos. First, there was an Offline Talkshow, where we got speakers from the Head of Health Services, Dinas Kesehatan Jawa Timur, and moderated by the Finalis Duta FK Unair 2022. The topic discussed was "Universal Health Coverage from the public point of view." As well as educational videos are videos filled with talk show participants about definitions, opinions, and hopes for UHC in the future.

In the post-event, we chose infographics and after the movie because we want people to remember the material we've discussed and see and appreciate some of the flashbacks of our events for nearly three weeks.

Bioethics In Research & Medical Practice -by CIMSA UPH

The question is, a 14-year-old girl was hospitalized for an abortion. She was two months pregnant from what she claimed was caused by rape. The family was desperate and demanded that doctors and medical personnel initiate an abortion for the girl immediately. But the girl’s condition did not allow for an abortion to be executed because of medical reasons.

What will you do as medical personnel?

As medical personnel, we would be asked for opinions and questions related to our patient’s conditions in the near future. Those answers, responses, and solutions must be based on bioethics principles.

SCOPE and SCORE of CIMSA UPH wanted to introduce and expose knowledge and education about bioethics, health practice, research fields, and bioethics in cultural differences to your future doctors, nurses, and pharmacists in this event.

All of the participants from this event were also given a pre-event challenge, where participants shared posters about the importance of bioethics through Instagram Story in how to solve moral conflicts so that we can make wise decisions and how it could increase the quality of healthcare, hoping that people in our surrounding could also understand about the purpose of bioethics. It can also be helpful and applied in the future as the participants are aspiring professional medical personnel.

We also invited Dr. dr. Theo Audi Yanto, Sp.PD, FINASIM as our speaker for this event with many exciting materials. We were given an interactive lecture and comprehensive insights about bioethics, what we should do, what to expect, and how to express it to patients. That’s not just the exciting part. Through this event, enjoyable activities were done, such as group c h a l l e n g e s .

Participants were divided into groups, and each group was given cases and presented their discussion in a 10-minute drama role plays.

Each group was very enthusiastic and could express the values of bioethics very well.

Hello, Kak Nadya! How are you?

What have you been busy with recently?

Greetings, SCOPE! It's been a while since I've echoed the iconic chant that brings back memories of lively meetings and a unifying spirit. "We say the SCO to the PE (6x), SCO to the people!". Though time flies, the memories of SCOPE members joyfully shouting the anthem remain fresh in my mind. How have you all been? I'm thrilled to reconnect with you once again! The past two years have presented unprecedented challenges, particularly for SCOPE's exchange programs. I extend my heartfelt admiration to all NEOs, NEC teams, LEOs, and SCOPE members for their unwavering dedication to continuing the agenda despite the difficulties of balancing medical school and organization responsibilities during the COVID-19 pandemic. I adore you guys! Thank you for your earnestness towards SCOPE! The pandemic has undoubtedly disrupted our plans, and as a medical student, my clinical rotation years were not immune to its effects. However, I focused on the silver lining and used the occasion to delve deeper into discovering my passion for medical science. This led me to pursue a postgraduate degree before returning to clinical practice. Long story short, this determination has brought me to the United Kingdom to learn Precision Medicine at the University of Manchester.

Can you tell us about your experience in CIMSA and IFMSA? My three-year involvement with CIMSA has significantly impacted my current position. As a Local Exchange Officer for Universitas Gadjah Mada in 2016 and later as a NEO for Outgoing and a member of the SCOPE Advisory Team, I honed my soft skills through various responsibilities, such as handling paperwork, fostering

partnerships, and engaging with international students and colleagues.

While it was a challenging and, at times, physically demanding experience, it was also advantageous and allowed me to gain valuable international exposure.

I became active in CIMSA because I saw the vast opportunities it offered for my personal and professional growth. As a future medical doctor, I understand the importance of being knowledgeable and social, confident, and selfsufficient. CIMSA provides a platform for me to develop these 5-star doctor qualities. Don't you agree?

Can you tell us about your exchange experience with SCOPE? In the Summer of 2018, I was fortunate to participate in the SCOPE exchange program as an outgoing student at Wroclaw Medical University in Poland. There, I completed a clinical clerkship in the Surgery Department, doing rounds and assisting medical procedures in the operating room while learning to comprehend the Polish language and customs. I got the chance to meet other incoming students from Greece, Hungary, Taiwan, Brazil, Mexico, Jordania, Kazakhstan, Korea, Iraq, and Portugal, creating a diverse and enriching international experience.

Every day was a new adventure, from the clinical setting to exploring the city with my exchange mates and even traveling to nearby countries like Germany during the weekends. This experience will forever hold a special place in my memories, and I yearn for the opportunity to relive those moments.

Is having an international exchange beneficial? If yes, why do you think it is?

International experience is precious and offers a wealth of growth opportunities, both academically and personally. Through my exchange program in Europe, I could compare

and contrast the medical education systems and appreciate the need for global health improvement, an area where Indonesia still has much room for improvement. One of my most cherished memories was making lifelong friends from around the world, and even today, we keep in touch through group chats though infrequently. I remember we did share how each of our countries survived COVID-19 and how my friends in Iraq and Jordan were involved in the medical team during their crisis. It was heartening to hear about their experiences and resilience. This international network of friends also gave me the confidence to pursue my Master's degree abroad, where I learned to live independently, manage my finances, and become involved in the local community in Manchester.

Any words of advice to SCOPEOPLE? I am frankly not an expert to give advice, so I instead count this as a message for my fellow SCOPEOPLE :) Embrace the endless opportunities offered by SCOPE and CIMSA to expand your capacity and potential.

Consider taking on roles such as a contact person for incoming students, trainer, project participant, active national team member, or any other activity that aligns with your interests. SCOPE has personally opened my eyes to the benefits of studying abroad, and I hope it will do the same for you. While we all have different aspirations for our future careers as medical doctors, I hope that through your time in SCOPE, you will discover your passions and continue to explore the world and all it has to offer.
