The Value of Our County Medical Society Anthony M. Zacharek, MD MHA

I

frequently ask myself what is the value of our Saginaw County Medical Society (SCMS)? While the answer to this question may be different for each member, I believe the value of our SCMS is that it represents the coming together of different physicians seeking a sense of unity. In our current healthcare environment with changes occurring rapidly and seemingly every day, it helps to have an organization of physicians who believe in sharing ideas and seeking solutions to the challenges that face us.

Whether you are a physician that is hospital employed, in independent or group practice, affiliated with CMU College of Medicine or retired, the SCMS welcomes you. The value of the SCMS lies in each physician who

belongs to it, as we all bring various backgrounds and interests to our meetings.

As I step into the role of President, I envision a future for the physicians of the SCMS and our area that includes a healthy connectivity and exchange of ideas. Having a resource such as the SCMS allows physicians the opportunity to socialize, listen to each other, and perhaps contribute to finding solutions to the ongoing issues of 21st century medicine.

As President of the SCMS, I would also like to encourage all physicians to take care of themselves and their families. I believe in having balance in your life, with your work and family both deserving attention. My hope is that you will consider the SCMS more as an extension of your family,

since we are all ultimately a family of physicians supporting each other in our goal of providing high quality care to the community that we serve.

What Can You Say?

W

hat do you say to one of your colleagues, perhaps a partner, an 80-year-old fellow who has no interest in retiring? He tells you that, because of ‘unusual circumstances,’ he is justified in remaining active. In this case, the colleague is not just an old doc who doesn’t want to hang up his stethoscope; he’s truly justified in staying on the job. His name is Anthony Fauci, MD, the head of the National Institutes of Allergy and Infectious Disease. He is the only 80-year-old doctor that I know who is still working. And what a worker he is!

This is the guy who steps in front of a TV camera several times a day, seven days a week, to deliver a prognosis to a patient load of 330 million! And he knows that those patients do not like to hear his bad news; and he knows that they will get angry at the messenger; and he knows he will get even more death threats. But, does he even bat an eye?

What do you say to another one of your colleagues who dons his protective gear, walks into a hospital room where a COVID patient struggles to breathe, a room coated with virus, the room’s air saturated by virus; the patient coughing, choking and spewing the virus everywhere.

He leans over with an endotracheal tube in hand, his own respiratory tract centimeters from that massively infected one, and deftly does the intubation, saving a life. Then he walks nonchalantly

out of the room without looking back; in his mind nothing remarkable happened, just ‘doing his job.’ And then he goes on, doing the same thing, to the next patient and the next patient, and the next patient…day in and day out, for months.

What do you say to any doctor1 who sees any patient in a live setting during a pandemic of this magnitude? Exam rooms were never designed to maintain six feet of distance between doctor and patient; and, even if a doctor were to do so, he could not deliver proper medical care, which often requires intimate contact between doctor and patient. So those doctors just do what they must do to correctly take care of their patients, accepting the risk, doing what they always have done.

Right now, the population is applauding doctors (and nurses) and blubbering about what great heroes they are. We know that such adulation will not last past the success of the immunization campaign. The American public will all-too-soon go back to treating doctors as they have become accustomed to treating them: Suing at the least sign of a bad outcome, blaming them for the high cost of healthcare, and griping about not getting enough face-to-face time.

In other words, my friend, accept it while you can. It won’t last long. What is more important, I will argue here - is sincere appreciation from those who actually know what you are going

through, your colleagues. A nonphysician could never understand the blood sweat and tears Anthony Fauci put in during his half century of devoted service to the American Patient. A nonphysician could never fathom what it is like to be in a hospital room with a sick or dying patient, to experience the fear of personally getting an infection, to dig deep into a lifetime of dogged determination so as not to surrender to that fear, to tap into a three-millenniumtradition of selfless service.

Therefore, when you get a chance to speak to one of those doctors, don’t waste it. Say something. But, what do you say? Some version of “thank you” would be appropriate. The words are not so important as is the fact that they are coming from a colleague who understands what they are going through. Nowadays letters, Christmas cards, texts, emails, twitter, Instagram’s, Facebook messages…all are acceptable.

Oh, if anybody actually knows Dr. Fauci, do extend your thanks to him for not retiring, and for demonstrating by his personal sacrifice all that is best in American Medicine. He richly deserves it.

when there's a concern and how to respond in a life-saving way.

Then the aftercare is for people who have been impacted by a suicide, whether it's through outreach, meeting with the family immediately after a death or through our ongoing support group and helping those who care for the bereaved to learn how to be better support and an understanding of suicide grief, which can be complicated.

When someone is having thoughts of suicide, they start to feel very alone in their thoughts. They don't feel understood.

And many times, people overreact or overrespond to those people in a way that makes them feel ashamed or embarrassed for having those thoughts. In our Network, we're very comfortable with those conversations, so we can help them connect to resources in a way that can feel safe and not shaming.

The sooner we can get people help, the better chance we have at preventing suicide. In the aftermath of a suicide, it's critical to give the families an immediate connection to let them know they aren't alone. I think it's an honor to be able to help people in a moment that is so private and personal and offer them help, guidance and care."

BARB'S INTERVIEW WITH THE CITY OF SAGINAW "My brother took his life when I was 18. This was many years ago when there weren't programs around to help and we didn't talk about suicide as openly as we do now. It was a hush-hush topic, taboo to talk about and there wasn't any help out there.

I was determined that while my brother's suicide was going to change me, it was not going to destroy me. I ended up going to a lot of national conferences about suicide and then brought that information back to our community. It became my purpose. We started small and focused on aftercare for survivors, but today our mission is really to provide both prevention and aftercare services.

The prevention part is to help bring awareness and education to teach people the tools they need to recognize

Drs. Veverka and Willy Elected to MSMS Leadership Roles Dr. Veverka to Serve as MSMS President-Elect Thomas J. Veverka, MD, was voted president-elect of MSMS during the 156th annual meeting of the House of Delegates held virtually on May 1, 2021.

Dr. Veverka is a board certified General Surgeon, a fellow of the American College of Surgeons, and has received added certification in Surgical Critical Care. He served as the Associate Director of Trauma at North Memorial Medical Center in Robbinsdale, Minnesota before starting in private practice as a general surgeon and surgical intensivist in Saginaw in 1994.

While in Saginaw, Dr. Veverka served in several physician leadership roles, primarily at St. Mary’s of Michigan Hospital, as well as, on the Steering Committee for the General Surgery Residency program as the MSU surgical coordinator for the medical students. Dr. Veverka has served the SCMS as Director, Alternate Delegate and President over the past 15 years, most recently as Region 7 Representative on the MSMS Board of Directors. Dr. Veverka is a board member of the Michigan Health Improvement Alliance (MiHIA).

Dr. Veverka has a special interest in trauma care and trauma systems development, and served as the initial medical director of the trauma program at St. Mary’s of Michigan Hospital which eventually was designated as a Level II Trauma Center by the American College of Surgeons Committee on Trauma. He subsequently became the initial Trauma Medical Director for MidMichigan Medical Center in Midland, which also successfully underwent verification as a Level II Trauma Center. He serves on the Michigan Committee of Trauma and performs Trauma Center Level III and IV verification visits for MDHHS. He also serves on the State Committee of Trauma in Michigan.

Dr. Veverka is very passionate about addressing the opioid crisis through volunteer activities at MidMichigan Medical Center, MiHIA and through his position on the Board of Directors of MSMS. He also serves on multiple other subcommittees.

Dr. Veverka received his medical degree from the University of Minnesota in Minneapolis. He completed his surgical residency at Saginaw Cooperative Hospitals, Inc., and returned to the University of Minnesota for his Surgical Critical Care

Fellowship training. He maintains a passion for general surgery, trauma care and critical care, as well as, patient safety issues.

Dr. Veverka has held a clinical faculty appointment with MSU College of Human Medicine Department of Surgery for over 25 years, as well as, holding a current clinical appointment with CMU College of Medicine. He is married to Cathleen and is the father of five.

MSMS’s presidents have greatly impacted MSMS’ programs, position on policy issues and its prominence in Michigan. The MSMS president typically serves a term of one year and works with the Board of Directors to set the strategic agenda for the Society. Past MSMS Presidents from Saginaw are: • James H. Jerome, MD (1867 and 1881) • Lyman W. Bliss, MD (1890) • Vernon V. Bass, MD (1977) • Jack L. Barry, MD (1994) Dr. Willy to Serve as Region 7 Representative on the MSMS Board Mildred J. Willy, MD, was voted Region 7 Representative of MSMS during the 156th annual meeting of the House of Delegates held virtually on May 1, 2021. A graduate of the Indiana University School of Medicine, Dr. Willy will serve a three-year term on the MSMS Board representing physicians from Arenac, Bay, Clare, Gladwin, Gratiot, Isabella, Midland and Saginaw Counties.

Dr. Willy is the Associate Dean of Student Affairs at CMU College of Medicine. Appointed in 2019, Dr. Willy oversees student services, career advising, wellness and professionalism. She has practiced emergency medicine for 22 years in the Great Lakes Bay Region at multiple affiliated hospitals including Ascension St. Mary’s, MidMichigan Medical Center, McLaren Bay Region Hospital and Bronson Lakeview Hospital. She continues to practice clinically part-time. Dr. Willy recently completed a two-year term as President of the SCMS, and has served the SCMS as an Alternate Delegate, Delegate, Director and President over the past six years.

The SCMS congratulates Drs. Veverka and Willy and thanks them for their service to SCMS members and physicians across Michigan!

Welcome Home Taylor Boehler Gaudard, MD! The SCMS would like to welcome home Taylor Boehler Gaudard, MD who completed her Family Medicine residency at Mercy Health Grand Rapids in June. Dr. Gaudard was a 2018 graduate of CMU College of Medicine, and a member of the SCMS during that time.

She returns to Saginaw as an attending faculty physician with the CMU Health Family Medicine Residency program in Saginaw this fall!

IMPLICIT BIAS TRAINING REQUIRED FOR LICENSURE The Department of Licensing and Regulatory Affairs (LARA) announced the Public Health Code – General Rules have been revised to require implicit bias training for physicians and licensees in 24 other health care professions, effective June 1, 2022. This action is the result of an Executive Directive issued in July of 2020.

The requirements apply to both new applicants, as well as, those renewing their existing licenses or registrations.

An applicant for license renewal shall have completed a minimum of one (1) hour of implicit bias training for each year of the applicant’s license or registration cycle. An applicant for new licensure shall have completed a minimum of two (2) hours of implicit bias training within the five (5) years immediately preceding issuance of the license or registration.

The term “implicit bias” is defined as: An attitude or internalized stereotype that affects an individual’s perception, action or decision-making in an unconscious manner, and often contributes to unequal treatment of people based on race, ethnicity, nationality, gender, gender identity, sexual orientation, religion, socioeconomic status, age, disability or other characteristic. (R 338.7001 (c)) The rules outline that content must include: • Information on implicit bias, equitable access to health care, serving a diverse population, diversity and inclusion initiatives, and cultural sensitivity.

• Strategies to remedy the negative impact of implicit bias by recognizing and understanding how it impacts perception, judgment, and actions that may result in inequitable decision making, failure to effectively communicate, and result in barriers and disparities in the access to and delivery of health care services.

• The historical basis and present consequences of implicit biases based on an individual’s characteristics.

Training must include strategies to reduce disparities in access to and delivery of health care services, and the administration of pre- and post-test implicit bias assessments.

Acceptable sponsors of this training include nationallyrecognized or state-recognized health-related organizations, a state or federal agency, accredited college or university or an organization specializing in diversity, equity and inclusion issues.

Acceptable modalities of training are a teleconference or webinar that permits live synchronous interaction, a live presentation or an interactive online instruction.

While the rule becomes effective on June 1, 2022, the implicit bias training requirement must be fulfilled prior to the 10 The Bulletin | Summer 2021

next renewal date. Anyone who renews prior to June 1, 2022, will not have to complete the training requirements until their next renewal. Any renewals after June 1, 2022, will need to complete the training requirements. Any training completed after June 1, 2021, will count toward the new requirement, as long as all the criteria above is met.

LARA is still finalizing an FAQ document with additional information. The SCMS will share as soon as that information is released.

Implicit Bias Training is not yet available, but will be shared as soon as finalized.

n 2020, HealthSource Saginaw opened Saginaw’s first and only Inpatient Aquatic Therapy Pool which is also used for outpatient services.

The state of the art Aquatic Therapy Pool does not require steps or a lift to get in as the floor rises to the surface so patients can simply walk on or wheel on in a wheel chair. The floor then lowers to the depth the therapist wishes to work with the patient. Jets can be added to give more resistance like walking against a river current. The new therapy is available for a broad range of diagnoses and is covered by most insurances.

Our inpatient and outpatient services have remained open throughout the COVID-19 pandemic and our mitigation strategies have been effective at minimizing exposure to illness.

Coaching, Mentoring and Sponsorship By: John E. Lowry, Ph.D. Director of Faculty Development CMU College of Medicine In service-oriented organizations like healthcare and medical education, the people in the organization are the greatest asset. It can be challenging for companies to develop each of their employees to reach their full potential. Coaching and mentoring programs are gaining popularity as more evidence shows their effectiveness. Sponsors can greatly accelerate professional growth and accomplishments.

Coaching Coaching programs have been used in many professional settings. A coach can be someone with expertise in a particular skill. High performers in almost every field have coaches, including athletes, musicians and public speakers.

Some coaches help with a very specific skill, such as golf swing coaches or baseball hitting coaches. Others provide mental coaching services to help people handle the pressure of competition. These coaches can give very detailed feedback on their performance, which is critical for making improvements. The coach does not need to be better at the skill than the performer, but rather is someone who can observe the performer and provide the right feedback and opportunities for further practice. Other examples include life coaches and career coaches, who help individuals with specific situations or challenges they face.

Many physicians have benefitted from working with a coach. Some coaches help physicians develop new clinical skills or learn new medical procedures. Other coaches help physicians grow in their practices as managers and leaders.

Career coaches can help physicians find ways to adapt their careers to match their evolving professional interests.

Another widely-used coaching program is peer coaching.

In these programs, professionals are paired with a peer in their field, and they provide mutual support for each other. A peer coach can be a listening ear and a sounding board for creative ideas. Working with a peer coach can provide opportunities for collaboration on meaningful projects. Faculty and staff at the CMU College of Medicine participate in a peer coaching program with all of the other medical schools in the state of Michigan. Being able to work with a peer from another institution provides new opportunities and perspectives.

Mentoring In Greek mythology, Mentor was charged by Odysseus to teach his son Telemachus. A mentor is defined as a trusted counselor or guide. A mentor is usually a seasoned

professional who can teach and advise someone because of the mentor’s knowledge and experience. The primary difference between a mentor and a coach is that mentoring involves a longer and more meaningful relationship. A mentor is truly invested in the person she or he mentors.

There are many benefits to mentoring. Working with a mentor can be a powerful professional development tool.

Junior colleagues receiving mentoring can see rapid growth early in their careers. Leaders and high-performers can be grown from within an organization. Organizations see many benefits, including lower employee turnover, and improved employee engagement and company culture. Many mentors report positive experiences as they impart their wisdom to help their colleagues grow. Mentoring can be a great experience for all who are involved.

The CMU College of Medicine’s Academy of Medical Educators recently launched a mentoring program for medical faculty. Senior faculty members serve as mentors to junior colleagues in a dyad mentoring structure. In some cases, faculty are paired with someone who has similar interests and training. In other cases, faculty are paired with someone who has complementary experience, such as a physician with a particular research interest paired with a foundational science professor with research experience.

While the program is just getting started, there is potential for many good experiences to come from it.

Medicine and medical education provide many opportunities for mentoring. Medical faculty have the opportunity to teach and mentor medical students and residents. Experienced clinicians can mentor their junior colleagues. Mentors show support for their mentees by teaching and guiding. They help people avoid mistakes and learn from their experiences. However, good mentors also provide corrective feedback when needed, and can engage in crucial conversations which are in the best interest of the mentee. Because mentoring relationships are built on trust, mentors have a unique opportunity to positively influence the lives and careers of those with whom they work.

Sponsorship Very few people make successful careers completely on their own. Almost everyone makes connections with others who help them along the way. Coaches and mentors can be very valuable, but sponsoring others can have even more powerful effects on others. Sponsorship is a way of accelerating the connections and accomplishments of employees by actively steering people toward opportunities and connections that will be good for their careers. continued on page 14

Employees who do good work can go unseen by others, and may not get the same chances for advancement as others. Sponsors can help by showing support for colleagues both behind the scenes, as well as, showing allyship in the important meetings where ideas are presented.

Advocates for opportunities

Conclusions Coaching, mentoring and sponsorship can do many great things for individuals and organizations. Many of the top businesses and universities are using these to make the most of their people and the company. They find happier, more productive employees who are engaged at work.

Diverse employee groups are supported, and employees find overall more professional satisfaction. Improved morale and low turnover can lead to significant cost savings for an organization.

Professionals should consider working with a coach or finding a mentor. If you do, lean in to the experience and make the most of it. If you are an experienced professional, consider being a sponsor for a junior colleague. You can help accelerate your colleague’s growth, and your organization will benefit tremendously!

2021 CMU College of Medicine and CMU Medical Education Partners Research Symposium The 2021 CMU College of Medicine and CMU Medical Education Partners Research Symposium was a success.

Students and their mentors submitted 85 abstracts from diverse disciplines. A judging panel selected eight abstracts for oral and poster presentations, with the remaining presented as posters only. Inaugural Recipient of Student Research Award Announced at Research Symposium The Louis and Yvette Saravolatz Research Scholar Award was presented for the first time at the College of Medicine and CMU Medical Education Partners spring symposium.

Maher Megaly, M1, received the award for a project titled “Thrombosis and d-dimer formation as risk assessment for COVID-19 patients’ mortality at CMU Health and Covenant HealthCare,” which was the top-rated proposal submitted for the College of Medicine Medical Student Summer Research Scholars program.

Nicholas Haddad, MD and Neli Ragina, PhD are the mentors for this award. Made possible through the generosity of Louis and Yvette Saravolatz, the award provides a $3,000 summer research project stipend to be done by an M1 medical student in cooperation with a College of Medicine faculty member mentor. In addition, the award provides up to $1,500 for the student, while studying at the College of Medicine, to present the results of the research at a regional or national meeting, or to pay for costs associated with publication of the research.

CMU Graduate Dr. Akbar Husain ’21 is Fired Up and Focused!

There’s no single reason S. Akbar Husain, MD wants to dedicate his future to helping others. But the son of Pakistani immigrants has a special empathy for the challenges people face in difficult circumstances. Husain's father worked his way through medical school after being displaced from his homeland, moving to Canada and then Michigan.

Following his example, Husain graduated May 7 from CMU College of Medicine. In June, he began his three-year internal medicine residency at Brown University in Providence, Rhode Island. After that, his commitment to the underserved via his National Health Services Corps Scholarship could lead him anywhere in the country. Wherever he goes, he will be needed.

The highly competitive NHSC program is a rare full-tuition scholarship awarded to about 60 medical students in primary care fields each year. Awardees commit to serving in a highneed area of the U.S. after residency. continued on page 19

he Annual Membership Meeting of the Saginaw County Medical Society was held via Zoom on Tuesday, June 15, 2021, due to the COVID-19 pandemic. Anthony M. Zacharek MD, PresidentElect, called the meeting to order at 7:02 p.m.

Dr. Zacharek noted he becomes President at tonight’s Annual Meeting, however, the official ceremony will be held at the inperson Membership Meeting on September 21, 2021.

Dr. Veverka is only the fifth president from Saginaw County. Past presidents were James H. Jerome, MD who served in 1867 and 1881; Lyman W. Bliss, MD who served in 1890; Vernon V. Bass, MD who served in 1977; and Jack L. Barry, MD who served in 1994.

• Congratulations to Dr. Millie Willy who was elected Region 7 Representative serving Arenac, Bay, Gladwin, Gratiot, Isabella-Clare, Midland and Saginaw Counties.

• Dr. Steve Vance expressed a heartfelt thank you to the many retired physicians who supervised CMU medical students administering the COVID vaccine. He especially thanked Drs. Caroline Scott and George Gugino for their countless hours spent volunteering.

Implicit Bias Training Now Required for Licensure

• The Bureau of Professional Licensing and Regulatory Affairs (LARA) will require implicit bias training effective June 1, 2022, for health care providers. The SCMS will work to schedule a training for members as soon as it becomes available.

Report of the Nominating Committee and Election of Officers • Dr. Zacharek, as a member of the Nominating Committee, presented the Slate of Nominees for 2021-22. The Slate was approved by the SCMS Board at their meeting on April 20, 2021, and published in the Spring issue of The Bulletin.

Online voting was made available to members who couldn’t join the Zoom meeting tonight. o A call was made for nominations from the floor. o There being no nominations from the floor, a motion, second and vote was requested to approve the Slate. SCMS Foundation • Generally, the SCMS Foundation holds their Annual Membership Meeting concurrent with the SCMS Annual Meeting, however, the meeting will be held at the September 21, 2021, Membership Meeting.

• The SCMS Foundation Bylaws were originally written in 1968. In 2019, the Bylaws were revised to bring them upto-date, and to add additional Trustees to increase the size of the Board. Before the Foundation Board can add Trustees, the revised Bylaws need to be approved. A request was made for a motion, second and vote to

approve the Foundation Bylaws as presented. Online voting was made available to members who couldn’t join the Zoom meeting tonight. MOTION: Approve the SCMS Foundation Bylaws as presented. MOTION APPROVED. The Foundation Board will meet this summer, and the Slate of Nominees for Officers and Trustees will be presented to the membership for vote at the September meeting.

• The Foundation did sponsor their annual Essay Contest, “Why I Want to Be a Physician in Saginaw County” again this year. Winners were notified by the SCMS. Joan thanked Dr. Yahya Khan and his family for sponsoring an additional first place award in honor of Zubeda S. Khan, MD who passed in January. Dr. Zubie Khan served on the SCMS Board for several years.

• It was noted several Foundation loan recipients would be returning to Saginaw to practice this summer after completion of their residencies. The Foundation offers a loan forgiveness program to those who return to Saginaw to practice and join the SCMS. A maximum of $5,000 or 25 percent per year of the loan principal is forgiven for every year they are a member. Joan noted loan recipient and SCMS Board member, Dr. Chris Allen, just completed the loan forgiveness program. Dr. Allen joined the SCMS upon completion of his residency, and has served on the SCMS Board since that time. Dr. Elizabeth Marshall, also a loan recipient in the loan forgiveness program, was elected as a new Alternate Delegate at tonight’s meeting.

• It was mentioned the Foundation is looking for members to serve as Trustees, and anyone interested should contact Joan. Dr. Allen asked to be considered for a position on the Foundation Board.

Membership Meetings • The next Membership Meeting will be held in-person on Tuesday, September 21, 2021, at a location to be determined. o Molly E. Gabriel-Champine, PhD, LP will present Physician Compassion Fatigue (program originally scheduled for April 2020).

2021 Essay Contest Winners “Why I Want To Be a Physician in Saginaw County” First Place Winner of $1,000 Lydia Back, 2021 Graduating Senior from Freeland High School attending Alma College A poster hangs in my laundry room from 1st grade. In a box on it, we had to draw what we wanted to be when we were older - I drew a doctor. Ever since I found out my grandpa was a cardiologist, I wanted to be a doctor. My mom could tell you, I was a curious kid and my favorite word was “why.”

My love only grew for medicine as I shadowed my grandpa one day at work. At lunch, he asked me if I wanted to go home and I replied “No way! The day is just getting started.” Later in 8th grade, on a road trip to West Branch, my dad told me an inspiring story about my grandpa. My aunt’s classmate had passed away during the mile run because of a heart condition.

The boy’s family had visited my grandpa’s house the next night and asked my grandpa, who was a cardiologist, to explain what happened to them. My grandpa showed strength and composure while explaining the sudden loss of a loved one.

My dad said it was a sad and tough moment, but he was never so proud to call my grandpa his dad as he was in that moment.

I knew that I wanted to be that person for someone one day, I wanted to offer a family clarity in a time of confusion with medicine. I have always admired the knowledge and tenacity of doctors and it was my dream to be like them one day.

In 10th grade, my uncle was diagnosed with stage IV colon cancer. This was a devastating discovery for him and my family. I’ve watched him for the last two years go through pain and procedure after procedure. I listened to his complaints and experiences with doctors and the hospital. I knew I wanted to be better for people like him. People dealing with a life-threatening condition should not have to worry about being comfortable or heard. I wanted to change that experience for other patients by becoming a doctor.

My sophomore year, I fractured my arm during a soccer game. This caused me to have a procedure screwing a plate into it. I was so fascinated by my own surgery that I wanted them to film it. They said no of course, but this caused me to want to do my senior project on fractures.

Through my lifelong experiences with my grandpa, uncle, coach and a broken arm, I knew that being a surgeon is my dream and I would do whatever it takes to achieve it. I owe everything to Saginaw County and the people in it like my teachers, my doctors, and my senior project mentor for never giving up on me and that’s why I want to give back. My dreams are big, but my determination is bigger. I can’t wait to help people and change lives, even if it is only one person, I want to make a difference.

Winner of the Zubeda S. Khan, MD Special Scholarship of $1,000 Emily Irish, 2021 Graduating Senior from Freeland High School attending the University of Michigan Home is where the heart is. For the last 18 years, I have called Saginaw County my heart and home. It is time for me to recognize its strengths and work to improve its shortcomings.

Two powerhouse hospitals, Ascension St. Mary’s and Covenant Healthcare, both staffed by outstanding, but overworked nurses and physicians, provide support to the county’s residents.

However, many of the inhabitants of Saginaw County are underserved, especially in rural communities such as Reese, Merrill and St. Charles. Citizens use the emergency rooms of these essential hospitals for basic care. For some residents, this method is their only access to any type of healthcare. For example, the majority of the geriatric population in the Buena Vista Senior Community Apartments can’t afford to see a specialist or even go into the hospital.

I've seen this first hand. I participated in an Emergency Medical Technician class during the winter of 2021 in order to

become a licensed EMT before college. During my first EMT clinical, my crew and I helped an older woman who had fallen and should've been examined by a medical professional.

However, she refused transport because she could not afford the medical bill that would come along with the ride to the emergency room. This was just one call during one 12 hour shift, but this woman is not alone in her experience.

Our residents who also call Saginaw County their home and heart deserve better. For our neighbors, families and friends, this is not good enough care. I want to be a part of the movement for change. This desire leads me to pursue an education to become an emergency medical physician.

Abraham Lincoln once made the comment, “Next to creating a life, the finest thing a man can do is save one.” While firmly believing in this quote, I have already started to put it into practice. I have stepped on the first rung of the ladder as an emergency medical technician working in Saginaw County and plan to make even more of an impact as a physician.

The county continues to grow and progress. Saginaw Valley State University and Delta strive to serve educational needs.

Local school systems work to bring young minds into the 21st century. The county will thrive. Physicians will be needed to care for those who live and work in Saginaw County. I plan to be one of those physicians.

An educational journey Growing up in the Detroit suburb of West Bloomfield, Husain first considered teaching as he pursued his undergraduate education. He spent four years teaching career and college readiness to at-risk youth in Detroit public schools. As he delivered lessons in STEM and anatomy, he also saw medical needs and struggles for proper care in his students and their families. "I got such an insight into what a lot of families have to deal with," he said. "It became clear that so much of medicine is education. I realized I could still be an educator and practice medicine."

Caring for the long haul CMU's medical mission to reach underserved areas of Michigan caught Husain's attention as he considered medical schools. Learning about CMU's signature Comprehensive Clinical Clerkship program clinched his decision.

At most colleges of medicine, clinical rotations are four to eight weeks long. The CMU College of Medicine's unique curriculum includes a 24-week longitudinal clerkship in primary care. Embedding students in rural or urban underserved communities provides a comprehensive experience that emphasizes caring for a community over a longer term. Husain served his clerkship at Ascension St. John Hospital in Detroit.

Third-Year Students Publish Peer-Reviewed Research on COVID-19 Behaviors and Perceptions This spring, the Journal of Primary Care and Community Health published “Revelations From the Clinic: Protective Behaviors and Perceptions Among People at High Risk for Severe Illness From COVID-19.” Third year students, Frances Greathouse, Sally Nagia and Morsi Rayyan, along with Beth Bailey, PhD, Director of Health Services, conducted the research during the students’ Comprehensive Clinical Clerkship experience during the fall of 2020. The writers say the goal of the research is to “characterize the risk perceptions and protective behaviors among residents [patients] of a rural central Michigan region.” The researchers concluded being at high risk for severe illness from COVID-19 was not a significant factor in predicting the behavioral decisions of mask-wearing and social distancing.

From Farm Boy to Family Physician – Michael J. Slavin, MD

By Michael Slavin, Jr. As a young boy, my dad never dreamed of becoming a doctor. He spent time in the field picking crops, playing baseball, climbing trees, swimming in the ditch that ran by the family farm, helping his father at the family gas station, listening to the Tigers on the radio, living the idyllic farm boy life.

The exuberance and innocence of youth are sometimes tempered by tragedy, however, and such was the case for Michael Slavin and his eight siblings. At age 47, Sergeant Stanley Slavin finally succumbed to wounds sustained fighting in World War II and left a 10-year-old Michael fatherless.

Michael considered joining the priesthood, completing his studies at the seminary in Saginaw, but life changed again when Eleanor Slavin passed away at the age of 49, and a now 18-year-old Michael and his eight siblings were parentless. The older siblings took turns caring for the younger, and as tragedy befell another branch of the family, they also took turns caring for a quadriplegic cousin who had lost both of his parents and all his siblings.

The experience of caring for this cousin inspired Michael to care for others, so much so that he abandoned a desire to teach mathematics and changed his college career path to medicine. Without parents or scholarship, Michael paid for college and medical school by taking out loans and by working any job he could find: Jarring pickles at a pickle factory, landscaping at a cemetery, working demolitions for the county road commission and joining the U.S. Air Force. Things had not gotten easier by then, but after finding his lovely young wife, Mary, he now had a partner with which to share his life’s joys and burdens.

All physicians take up the responsibility of caring for their fellow man, but few have done so with the level of dedication and compassion that have defined my Dad’s career. Anyone who has met him at any point between the early days of his private practice to his role as Medical Director would have met the same person. He has always been the man who put everyone else’s needs before his own. He has always been the man who focused on his family, his patients and his colleagues before himself. Early in his career if it meant staying up all night delivering a baby and then going directly into a 12 hour day at the office, he would do it without a complaint. Later in his career, if it meant staying up until 3 a.m. to prep for a 7 a.m. meeting and then head to another 12 hour day at the office, he would do that without a complaint too.

Though he retired last year, I believe I can say without hyperbole that he has had a significant and lasting impact on the quality of medical care in Saginaw. Because he has spent his life doing what he loves, caring for people, he would never in a million years ask for gratitude or even think it was due.

Ascension St. Mary's Presents a New TAVR Approach at National American College of Cardiology Meeting Catheter based aortic valve replacement (TAVR) procedure has become the gold standard for managing aortic valve stenosis (a/k/a narrowed heart valve), a disease that can lead to shortness of breath and heart failure. Medical procedures like TAVR can be associated with its own complications such as needing a pacemaker after the procedure or renal (kidney) dysfunction because of the contrast dye used during the procedure. The structural heart team at Ascension St. Mary’s Hospital in Saginaw has developed a new approach to implanting the balloon expandable valve to minimize the potential TAVR associated complications.

The new approach was presented as an abstract at the American College of Cardiology meeting. Safwan Kassas, MD, Director of the Structural Heart Program at Ascension St. Mary’s, has shown the new approach has led to the lowest published TAVR related pacemaker rate of three percent compared to the national average of 8.5-10 percent. This also has led to a low procedural contrast amount between 8-14 CCs per procedure as compared to the national average of more than 60 CCs, thus reducing renal dysfunction adverse events.

Ascension St. Mary’s Welcomes CMU Health Residency Program to Their Riverfront Building Ascension St. Mary’s welcomed CMU Health and their internal medicine residency program to Ascension St. Mary’s Riverfront building at 1015 S. Washington Avenue in Saginaw.

On Monday, July 19, CMU Health internal medicine and infectious disease patients began going to a new and larger office area on the third floor of Ascension St. Mary’s Riverfront building. The care site was newly renovated to improve patient comfort and includes a spacious waiting area, 14 exam rooms, access to an onsite laboratory and other medical specialties.

“Ascension St. Mary’s has been a part of what is now CMU Health and its graduate medical education programs for more than 50 years,” says Stephanie J. Duggan, MD, President and CEO, Ascension Michigan Northern Ministries. “Our clinical collaboration continues with the 2020 signing of a 25-year commitment with CMU and Covenant for medical education in the Northern Michigan region. The relocation of CMU Health’s internal medicine residency program and infectious disease practice to Ascension St. Mary’s Riverfront building will

further our respective missions of enhancing access to patientcentered, compassionate care.”

The internal medicine practice at CMU Health is a patientcentered medical home and has some of the region’s foremost specialists in adult medicine and the treatment and prevention of complex diseases and illnesses. CMU Health internal medicine residents and faculty physicians provide care for adult patients, 18 years of age and older. Providers focus on disease prevention and health management and treatment of conditions, including diabetes and its complications, gastrointestinal conditions (constipation, diarrhea, abdominal complaints), heart disease, high blood pressure, lung disease, non-obstetric needs of pregnant patients, preventive health care, primary care of patients with HIV and Hepatitis C, screening and prevention of cancers, sports medicine and travel medicine and immunizations.

Ascension St. Mary’s Loosens Visitor Guidelines Due to the decreasing community positivity rates and lower number of COVID-19 hospitalizations, Ascension St. Mary’s Hospital recently moved to a Tier I visitation policy, which allows one visitor at a time, multiple persons allowed per day in an acute care setting. Visitors should be at least 18 years old (unless they are the parent of the patient or special circumstances) and not in a high-risk health category such as older adults and those with underlying medical conditions.

• The term visitor is inclusive of one guardian or support person who is medically necessary to facilitate patient care • One support visitor is allowed in Labor and Delivery in addition to a midwife/doula • One visitor is allowed for outpatient surgery patients must stay in waiting room • Two parents/guardians are allowed for all pediatric patients (21 years and younger) • One visitor allowed in Ascension Medical Group clinics, outpatient radiology and lab if necessary for patient assistance

All visitors must stay in the patient room or designated area, and limit visiting other areas in the hospital. They must also sanitize their hands before and after the visit.

Ascension St. Mary's Introduces My Dining On June 16, Ascension St. Mary's introduced a new inpatient dining program. My Dining allows inpatients to call Food and Nutritional Services (FNS) anytime - 45 minutes before they would like to eat. They can choose from expansive menu options based on their therapeutic diet. In addition, the majority of the menu is cardiac and renal compliant. Patients that are unable to choose will have the daily chef’s special for each meal. For diabetic trays, FNS will continue to call the nursing unit ahead of delivery.

Health Can’t Wait is a coalition of patients, physicians and health care providers dedicated to putting Michigan patients first and ending delays in patients’ access to health care.

This past spring, the Senate unanimously passed SB 247 prior authorization reform that reduces the red tape patients and providers are forced to navigate.

The bill has now moved on to the House of Representatives for consideration. To be as proactive and productive as possible in the lead-up to this critical fall legislative session, we are asking for your help in reaching out to these lawmakers on SB 247.

Making direct contact with Representatives on this issue will go a long way towards helping us duplicate the success we experienced with this legislation in the Senate. We have also produced two new action alerts - one for patients and one for providers - that make it easier for an individual to quickly contact their Representative and communicate the importance of the reforms outlined in SB 247.

SafeHaven™ Now Offering Michigan Physicians & Health Care Providers Resources to Manage Stress and Burnout New program offers physicians and health care providers with the resources they need to meet the demands of their personal and professional lives.

In an effort to better serve and support clinicians struggling with stress, burnout and the effects of COVID-19, MSMS has launched SafeHaven™, a comprehensive and confidential physician and health care provider wellbeing program offering clinicians with the resources and support they need to address career fatigue and behavioral health concerns.

The Medical Society of Virginia (MSV) conceived and manages SafeHaven™, which is implemented in partnership with VITAL WorkLife. SafeHaven™ was created by the passage of the Virginia legislation, which is the first of its kind in the nation, signed in March 2020. During the 2021 Virginia General Assembly Session, legislation was passed to include additional provider groups to the SafeHaven™ protections including nurses, nurse practitioners, pharmacists and medical, PA, nursing and pharmacy students. The law allows healthcare providers to seek professional support to address career fatigue, burnout and behavioral health concerns with confidentiality and civil protections.

SafeHaven™ provides a host of discreet and confidential set of tools and resources that physicians and health care providers can access to stay well, avoid burnout, and connect to their purpose without the fear of undue repercussions to their medical license.

What To Tell Patients About Myocarditis After COVID-19 Vaccination As the country continues to push for more people to get vaccinated against COVID-19, some remain concerned over rare cases of heart inflammation - myocarditis and pericarditis - linked to the Pfizer-BioNTech and Moderna mRNA vaccines. While some parents may be thinking twice about teen vaccination, medical experts reassure that the risk of myocarditis and pericarditis are far lower than the risks of serious illness or death from contracting COVID-19.

Since April, there have been more than 1,000 reports to the Vaccine Adverse Event Reporting System (VAERS) of cases of myocarditis and pericarditis occurring after mRNA COVID-19 vaccination in the U.S. with more than 300 cases confirmed. Myocarditis is inflammation of the heart muscle and pericarditis is inflammation of the lining outside the heart.

In both cases, according to the Centers for Disease Control and Prevention (CDC), the body’s immune system is causing inflammation in response to an infection or other trigger.

“As physicians, nurses, pharmacists, public health and health care professionals and, for many of us, parents, we understand the significant interest many Americans have in the safety of the COVID-19 vaccines, especially for younger people,” the nation’s leading doctors, nurses, pharmacists and public health leaders, including the AMA, said in a joint release.

Here’s what physicians should share with patients about incidents of myocarditis and pericarditis after Pfizer or Moderna mRNA vaccination.

These are rare cases Confirmed cases have occurred mostly in male adolescents and young adults aged 16 years or older. But given the hundreds of millions of vaccine doses administered, says the CDC, reports of myocarditis and pericarditis are rare. “The CDC’s Advisory Committee on Immunization Practices, or ACIP, met recently to discuss this issue,” AMA Chief Health and Science Officer Mira Irons, MD, said during an episode of “AMA COVID-19 Update” on vaccination challenges and masking guidance. “While the CDC did determine the mRNA COVID-19 vaccines can, in rare instances, be linked to myocarditis or pericarditis, the majority of patients have recovered.”

“This is an extremely rare side effect, and only an exceedingly small number of people will experience it after vaccination,” the joint release states. “Importantly, for the young people who do, most cases are mild, and individuals recover often on their own or with minimal treatment. “In addition, we know that myocarditis and pericarditis are much more common if you get COVID-19,” the release adds, emphasizing that “the risks to the heart from COVID-19 infection can be more severe.”

Symptoms appear after second dose Severity of myocarditis and pericarditis cases can vary, but reports have increased since April, mostly in young males 16 and older, several days after vaccination, and more often after the second vaccine dose. Symptoms include chest pain, shortness of breath and palpitations. While these cases are rare, ACIP agreed that a warning about the potential risks should be added to the FDA's official fact sheets on the vaccine so that people would not ignore symptoms.

If a parent or their child has any of these symptoms within a week after COVID-19 vaccination, it is important to seek medical care. For instances of myocarditis and pericarditis after mRNA COVID-19 vaccination, most who received medical care have responded well to medications and rest.

New Suicide Prevention Resources A new clinical update on suicide among medical students, residents and practicing physicians from the Society of General Internal Medicine's recent meeting is now available. It provides a summary about what is known on rates of physician suicide, as well as, recommended organizational approaches for prevention, along with recommended modifications to state medical licensing applications to reduce the risk of physicians not seeking help for mental illness. A recent change in New Mexico's licensing application is highlighted.

2022 Medicare Physician Fee Schedule On July 13, the Centers for Medicare & Medicaid Services (CMS) released the proposed rule for the 2022 Medicare physician fee schedule. While AMA staff will analyze and develop a summary of the 1,700+ page proposal, we wanted to make you aware of a key issue. Notably, the 2022 Medicare conversion factor would be reduced by approximately 3.89 percent from $34.8931 to $33.5848. This is largely a result of the expiration of a 3.75 percent increase to the conversion factor at the end of calendar year 2021, as averted for 2021

by Congressional action. The AMA will strongly advocate that Congress avert this significant cut and extend the 3.75 percent increase for 2022. Please note that the impact table in the proposed rule does not seem to include the 3.75 percent reduction in the conversion factor.

IN MEMORY James A. Letson Jr. MD, beloved son, father and grandfather passed away peacefully surrounded by his loving family on Monday, April 19, 2021, at home in Saginaw. He is survived by his children James A. Letson III (wife Tammy), Lisa A. Conlee, Kristin M. Snyder (husband James) and Andrew T. Letson, as well as, his two grandchildren, Elise G. Letson and Wolfgang V. Snyder.

Jim was born March 29, 1940, to James and Gerda Letson in Huntington, New York. He was raised in Philadelphia, Pennsylvania where he attended Monsignor Bonner Catholic School for Boys. Growing up, Jim enjoyed the companionship of his beloved cousins Gene, Susan, Bonnie and Percy with whom he loved dearly and shared many beautiful childhood memories.

Graduating at the top of his class, he studied Biology and Pre-Med at Saint Joseph University in Philadelphia. He went on to attend Jefferson Medical College where he earned a Doctor of Medicine degree.

In 1965, Jim married Ilse E. Letson of Hamburg, Germany and moved to Columbus, Ohio to do his residency at The Ohio State University and complete his internship in Otolaryngology.

Jim served in the United States Army as a Flight Surgeon during the Vietnam War earning the prestigious Bronze Star. Serving on The S.S. Hope Ship, he worked to achieve sustainable advances in health care around the world by implementing health education programs and providing humanitarian assistance in areas of need. Jim was a proud United States Veteran.

Dr. Letson lovingly treated patients with Ear, Nose and Throat diseases, as well as, those in need of Head and Neck surgeries for 44 years at Valley ENT. He was known for his affable charm, wisdom and astute medical acumen among both his patients and his colleagues. Dr. Letson was also an adjunct Professor and guest Lecturer at Michigan State

University and Central Michigan University while authoring a textbook on Middle Ear Disease.

Jim loved the game of golf. For many years, he was a member of the Bay City Country Club. He participated in various tournaments with his dear friends and had many great stories to share. He was known for his remarkable sense of humor, and was always armed with a joke or three. He had a wonderful way to make everyone laugh.

During his retirement, he went on many fishing trips to Canada, traveled the world and enjoyed the Fine Arts. As an avid art collector, he opened The A & J Art Gallery in Bay City, Michigan. He curated art and was a supporter of many fine local artists.

Jim’s biggest passion was Notre Dame football. He lived for football season. Andrew and his Dad enjoyed gathering with friends to cheer The Irish onto victory every Saturday. He relished when they were a championship team.

Jim is preceded in death by his parents, James and Gerda Letson, Ilse Letson and his two grandchildren, Connor and Hunter Letson.

Jim was a devout Catholic and a parishioner of St. Dominic’s Church in Saginaw where a private mass in his honor was held. Those planning an expression of sympathy may wish to consider memorials to the charity of their choice.

Memorial donations are graciously accepted by the Saginaw County Medical Society Foundation. Donations are used to provide low interest loans to medical students with strong ties to Saginaw, encouraging students to return to Saginaw to practice after completion of their residency at which time their student loans may be forgiven. The Foundation provides awards and scholarships to high school juniors and seniors interested in a career in medicine, as well as, medical students and residents for research through the CMU College of Medicine. The Foundation also assists the SCMS Alliance in funding nursing scholarships.
