How Involved Do You Want to Be? Anthony M. Zacharek, MD MHA

A

s a physician in the Saginaw County Medical Society (SCMS), you have a lot of competing interests for your time. Family, work and outside interests all demand your time. Which of these are most important to you? The answer to that question is different for everyone, but at the heart of any commitment is the question, “How involved do you want to be?”

When it comes to your involvement in the SCMS, one way to meet more physicians and learn more about the issues facing medicine today is through the Michigan State Medical Society (MSMS) and the House of Delegates.

Every year, typically in late April, the MSMS holds its annual meeting bringing physicians across the state together, in

what is called the House of Delegates (HOD). The HOD is an opportunity for physicians to submit resolutions that are important to them, and where those resolutions are debated to determine if they are supported by MSMS. A great example of the role that the SCMS played in the MSMS and HOD, and even the American Medical Association (AMA), is the resolution from 2018 regarding suicide awareness training for health care providers. Originally submitted to the HOD by the SCMS, this resolution was passed by MSMS in April 2018, and went on to be passed by the AMA in June 2018. Because of this resolution, the AMA now has a policy to increase awareness and physician training on suicide. The resolution was the idea of Joan Cramer, Executive

Director of the SCMS, was written by CMU medical students, and presented by a CMU medical student at the HOD. This example shows how our county medical society has the opportunity to affect healthcare at a state and national level.

So, it goes back to my original question of “How involved do you want to be?” Any physician in our SCMS can get involved in the MSMS and HOD, and our medical society is always looking for new members and board members.

Beyond the ability to affect healthcare policy, belonging to the SCMS hopefully will give you a sense of camaraderie and meaning as a physician in Saginaw County.

“The HOD is an opportunity for physicians to submit resolutions that are important to them, and where those resolutions are debated to determine if they are supported by MSMS."

Mirroring

W

hen, last month, I quoted Yogi Berra’s famous comment about “observing a lot, just by watching,” I was, technically, describing a process by which the image of an object from the outside world is duplicated on your retina, then your optical cortex…then analyzed by the frontal cortex of your brain.

It is important to break down this sequence of events because, as I promised last time, I would today tackle another thorny problem, that of “going beyond the history of Present Illness” to get at the root of the patient’s complaint, which is sometimes more mental, financial, educational, social, familial, or attitudinal than purely “biological/ medical.”

Consider that there is abundant evidence from recent neuroscientific studies that more than just images get duplicated onto our brains when we interact with the outside world.

Pianists, for instance, when they hear a familiar piano piece, fire the appropriate neurons in their “motor” cortices, as well as, their visual cortices. They duplicate the keystrokes in their heads. Likewise, dancers, when watching other dancers, trigger their appropriate motor cortex neurons for the dance moves they are observing. They “dance” in their heads.

In many ways, therefore, when we “see” the outside world, we also experience the outside world in a very real way.

Software companies have stolen this idea. Apple has “Mirroring;” and Android “Miracast,” both of which cast anything you can get on your computer onto a television screen. Programs, music, movies. Anything your computer can come up with can be “experienced” on the big screen of your TV. Outside reality, as we have seen, has always been “mirrored” in our brains. When we absorb a patient’s history by asking them questions, their experience becomes “mirrored “in our brains. When that outside experience of our patients is of a sensory nature (pain, for instance), we (depending on our experience as physicians) may learn to, in some way actually experience the same sensations.

With all that in mind, let me rephrase Yogi Berra, and make a quote of my own, perhaps a thesis: “You’ll know your patient’s pain when you feel your patient’s pain.” The sequence here: Your patient describes their symptoms (pain)…the information goes into your ears…into your auditory cortex…which sends it to your frontal cortex, which doesn’t understand it because pain is not a higher-order function. It is a primitive feeling, a sensation, an emotion. So your frontal cortex asks for help from more primitive brain areas involved with feelings, the so-called “lizard-brain” centers (thalamus and amygdala). The final understanding of the patient’s pain is a complex amalgam of intellectual and emotional knowing.

My thesis is two-fold.

First. It seems obvious that you cannot understand a sensation such as discomfort (pain) unless you in some way “feel” that sensation. Thus: “You’ll know your patient’s pain when you feel your patient’s pain.”

Second, physicians, who spend much of their lives processing information from patients about their discomfort (pains) actually become pretty good at doing just that. Practice makes perfect.

This, perhaps, is where that physicianintuition I mentioned last month comes in. Physicians, masterful observers that they are, sometimes seem to come up with a diagnosis…and cannot explain how they did it. Intuition? Or clues they did not realize they were using? Could physician’s intuition be a result of the emotional mirroring effect described above? A profound understanding of the patient’s situation that transcends traditional data-gathering and rises to a level of symbiosis between frontal lobes and primitive lizard-brain centers.

The doctor understands the patient.

He understands what is wrong with the patient. He has the knowledge required to treat and hopefully cure him. The perfect resolution.

We live in perilous times for our profession. The trust of the American patient in the American doctor is in decline. There are, of course, many reasons for this, many out of our control. continued on page 7

But, perhaps, the exercise of the skill described here is one thing we can control. It is a skill we should be proud of. It is a skill possessed by all doctors, generalists, as well as, specialists.

It is a skill not possessed by most other “practitioners.” Google has data, but cannot read patients. Computer diagnostic programs have algorithms, but they cannot do mind-to-mind mirroring as we can. Mid-Levels have “protocols,” but do not understand why they are important. We doctors, and we doctors alone, are the complete package, knowledge, skill, the ability to get into the mind of the patient and understand in a visceral way what he is going through. An accurate diagnosis is essential to successful treatment. We should embrace this skill, celebrate it, be proud of it, and USE IT. If you find it difficult to communicate with your doctor; if you find it hard to get personal attention from an 'impersonal healthcare industry'; if you don't understand all those insurance-company rules; if you don't know how to change your bad health habits; if you think you may be on unnecessary medications; if you are perplexed by those annoying health-product advertisements; and if you'd like to know which are your greatest health risks - you'll appreciate this Family Doctor's advice, gleaned from 44 years of practice.

Feist said one particularly important piece of the Breen legislation is that it also provides for a comprehensive study of the prevalence and causes of burnout and mental health struggles among health care workers, which Feist said he hopes will illuminate more state and federal policy needs. “When we created [the Lorna Breen legislation], it was the beginning of the pandemic,” Feist said. “The mental health impact of the pandemic on the workforce has now magnified significantly, which means we are literally just getting started here from a policy perspective in supporting the well-being of the workforce.”

MSMS ADVOCACY LEGISLATIVE ALERT Urge Your Senator to Support the Medicaid Primary Care Rate Increase The Senate Appropriations Subcommittee on Community Health/Human Services recently adopted an MSMS-initiated budget proposal that would provide a Medicaid rate increase for primary care services. The budget bill which contains the funding, Senate Bill 828, appropriates roughly $95 million toward the initiative. Senate Bill 828 successfully passed the Senate Appropriations Subcommittee on Community Health/ Human Services on April 20, 2022, and now resides in the Senate Appropriations Committee for consideration.

Lower payment rates in Medicaid have historically created substantial barriers for Medicaid beneficiaries in accessing health care, and these low reimbursement rates combined with increasing Medicaid enrollment exacerbates the financial instability of physician practices that provide essential care to Medicaid patients. The Medicaid uplift, which applies to both adult and pediatric primary care services, will expand access to care for Medicaid beneficiaries, support primary care and physician practice sustainability, and improve patient health.

Now, more than ever, patients need timely access to primary care, and this important initiative will ensure that physician practices stay open to continue caring for patients who trust them.

In Response: What U.S. Health Care Will Look Like in 2032 Submitted by Leah C. Davis, DO Radiologist in Traverse City I read the ten predictions for the future of health care in the article written by The Doctors Company/TDC Group in the March/April 2022 edition of Michigan Medicine®. The fifth prediction perpetuated the myth that nurse practitioners and physician assistants (collectively nonphysician practitioners, or NPPs) are poised to “become the primary care providers for many Americans, reserving MDs and DOs for complex cases.”

This was surprisingly out of touch with the current political landscape and in direct opposition to recommendations of the American Medical Association (AMA), the Michigan State Medical Society (MSMS), and other physician organizations.1-4 I felt extremely disappointed to see this published in Michigan

Medicine as it sends the wrong message to physicians and health care organizations in our state.

Physician education and regulation is unmatched.

The clinical authority of physicians is found in our education and regulation. The Flexner Report, published in 1910 when medical schools were unregulated and prolific, resulted in one out of every three medical schools being rated as “defective” and forced to close.5,6 Today, all 142 accredited medical schools are required to meet specific standards in order to receive accreditation through the Liaison Committee on Medical Education (LCME) or the Commission on Osteopathic College Accreditation (COCA).7-9 This accreditation ensures that 100% of medical school graduates are prepared for the practice of medicine before the MD or DO degree is awarded and their 3+ year internship and residency begin. Upon graduation, MDs and DOs are overseen by the Board of Medicine (BOM) and held to the highest standard of medical care in the courtroom.

In contrast, and similar to where medical schools were in the early 20th century, proprietary NP (and increasingly PA) schools are popping up everywhere, with nonspecific accreditation and curricular standards, resulting in over 400 different NP curricula across the nation.10-13 The absence of educational standards, 100% acceptance rates at 20 NP schools, and 100% online preclinical courses at more than 45 NP schools results in a wide variability in the knowledge and skills of graduating NPs.14,15 As a recent study noted, “until this variability [in educational programs] is resolved, we conclude that NPs should not perform independent, unsupervised care in the ED regardless of state law or hospital regulations in order to protect patient safety.”16 Upon graduation, NPs are overseen by the Board of Nursing (BON) which does not have the authority to judge any NP by medical standards, creating a regulatory loophole that limits the accountability of NPs in the courtroom.17 Quality of patient care and cost-effectiveness of NPPs has not been proven.

Advocates of “independent” or “unsupervised” practice

by NPPs often refer to the most complete meta-analysis of nonphysicians in primary care, the Cochrane Collaborative, which found only 18 out of 4,847 screened studies were of adequate quality to analyze and noted that “in many of the studies, nurses could get additional support or advice from a doctor.”18 Clearly, this data does not represent a direct comparison of unsupervised NPP-led versus physician-led care, although it is almost always presented as such.

Earlier this year, a 10-year retrospective review of physicianled and NPP-led primary care teams concluded that physicians performed better on 9/10 quality metrics for quality patient care (physicians scored less than 1% lower on quality metric continued on page 13

of “average of HTN pts BP <140/90”).19 In this study, patients managed by NPPs had an 8% higher referral rate per disease to specialists and were 1.8% more likely to visit the Emergency Room, even when compared to patients with no medical care at all, supporting the conclusion that poor-quality health care is worse than no health care at all.20 Additionally, patients managed by NPPs paid $43 more out of pocket per month (translating to an additional $10.3M in spending per year across the attributed population) than patients managed by a physician, making NPPs less cost-effective for patients. To be clear, statements of “costeffectiveness” must be qualified in terms of their benefit to patients versus corporations because depending on the financial structure of the health care organization, more tests, specialty referrals and visits to the ER may result in less money in the pocket of the patient, but more revenue for the corporation.

Access to care has not been solved by increasing the number of nonphysician practitioners.

The originally published prediction also claimed that NPPs can improve access to care. This argument is commonly presented and offers hope for rural and underserved urban areas but does not materialize. Demographic assessments from many areas reveal the same finding: NPPs set up practice in the same general locations (urban > rural) as physicians and they “choose to work in adequately served urban areas and to subspecialize to earn higher compensation. This creates shortages of providers in inner city urban, poor and rural populations.”21-23 It is imperative that we refocus on safety, truth and transparency.

Patient safety and the delivery of quality patient care are our highest priorities. Scope of practice discussions tend to devolve into individual battles, but we must deliberately shift the focus of these discussions beyond individual (physician or NPP) ideas, opinions, preferences, or predictions and back to the primary focus – our patients.

Replacing primary care physicians with NPPs is not good for patients: • It places inadequately trained health care workers at the forefront of medical care.16 • It costs patients more money in the form of unnecessary referrals and lab work/tests.19

• It is confusing for patients, who often assume an introduction of "Dr. X" or the presence of a white coat implies "physician."

• It places patients at higher risk for medical mismanagement while removing the ability to hold the NPP accountable for medical mistakes.17 That is not to say NPPs are not effective, valuable members of the health care team - they are! The Hattiesburg study concluded that patients who are “co-managed” by NPPs on physician-led teams have the best quality and cost outcomes of all.19 Based on their study conclusions, the Hattiesburg clinic enacted system-wide changes that require physicianled health care teams that alternate visits with a physician and an NPP for all patients and physician-only appointments for all new specialty care consults (except in emergency situations or with referring physician approval). As physicians, we must advocate for our patients, especially in the midst of profit-driven discussions at the corporate level and politically charged discussions on the legislative floor.

Physician-led medical care is best for patients, as supported by recent studies yet the same old (now refuted) arguments continue to be promoted by lobbyists, nonphysician advocacy groups and even our fellow physicians. To counter this, I propose that The Doctors Company and similar organizations support the following which is based on recently published research and aligns with the goals of physicians, MSMS and the AMA: Updated Prediction #5: Physicians will be positioned at the head of the health care team, which may include nonphysician practitioners as valued health care team members, to provide the highest quality, safest, and most cost-effective medical care to patients.

Patricia “Pat” Ann Koenig Patricia “Pat” Ann Koenig, age 79, passed away at Hoyt Nursing & Rehab Centre on November 14, 2020. The daughter of Salih Kalel and Nazetta Farrah, Pat was born on April 22, 1941, in St. Louis, Missouri. She married Dr. Nick Koenig on June 12, 1965, and they shared 44 years of marriage. He preceded her in death in 2009.

Over the years, Pat had two careers. She received her Bachelor of Science in Nursing degree from DePaul Hospital Nursing School in 1961. Pat was a pediatric nurse at Cardinal Glennon Children’s Hospital in St. Louis, Missouri. It was in St. Louis she met and married the love of her life, Nick. They moved to Saginaw, Nick’s hometown, where she met and made many lifelong friends. She began her second career as a Real Estate Broker/Owner and retired after 25 years in the business.

Pat loved her granddaughters, shopping, and socializing with her many friends. Over the years, Pat was active in the Saginaw County Medical Society Alliance, Boys and Girls Club, Saginaw Board of Realtors and St. Raymond Maronite Church in her hometown of St. Louis, Missouri.

Pat is survived by her loving children, Joseph “Joe” (Jean) Koenig, Mimi (Matt) Shier, and Mike (Amy) Koenig; beloved grandchildren, Jordyn Koenig, Kaitlyn Koenig, Heather Shier, and Ruby Koenig. She leaves behind many beloved nieces and nephews. She appreciated and adored her nephew, Jeff Koenig, who always loved and protected her. She was preceded in death by her husband, Dr. Nick Koenig, her parents, and her 12 siblings.

Cremation has taken place. Due to the ongoing pandemic, Pat’s family plans to hold a memorial mass at a future date when it is safe to gather again.

Sean Hoban, MD Sean Hoban, MD was born in Westport, County Mayo, Ireland on August 22, 1932, and died in Milwaukee, Wisconsin on March 25, 2022. Dr. Hoban was educated, pre-med and Medical School, at University College Dublin, graduating in 1959. He came to Saginaw for his Internship, setting up a Family Practice on Michigan Avenue.

He married Carmel Cassidy in 1962; she predeceased him in 2011. They had two children, Michelle and Dermot, who survive them.

Dr. Hoban believed that Saginaw was a great place to practice medicine, and convinced over 100 Irish medical students to take Externships here; over 60 Irish doctors to

take Internships here; and 14 doctors to practice here. He left a large footprint in Saginaw. He was also a citizen of the world, doing medical missionary work in Africa, and practicing medicine in Saudi Arabia, California, as well as, other states in the U.S. Other interests, from his first day in medical school, included theology (he actually studied to be a Cistercian Monk), political advocacy for the poor and promoting musical artists.

TONIGHT’S PROGRAM Dr. Zacharek introduced speakers Sasha P. Savage MD, Family Physician and Chief Health Officer of THRIVE; Mark G. Greenwell MD, Family Physician; Claudia C. Zacharek MD, Nephrologist; and Jisselly Salcedo MD, PGY-3 Psychiatry Resident who each gave TED Talks on “Overcoming Obstacles in Health Care.” Each presentation was unique and heartfelt, and coincidentally followed a common theme of emotional intelligence as referenced in Dr. Zacharek’s Winter 2022 article in The Bulletin. According to Bradberry and Greaves, “emotional intelligence is your ability to recognize and understand emotions in yourself and others, and your ability to use this awareness to manage your behavior and relationships.”

Dr. Zacharek thanked Drs. Savage, Greenwell, Zacharek and Salcedo for sharing their very personal stories of overcoming obstacles in health care.

The SCMS Annual Membership Meeting and SCMS Foundation Annual Membership Meeting will be held on Tuesday, May 17, 2022, at Horizons. Featured during the Annual Meetings will be an address from Thomas J. Veverka MD who will be installed as MSMS President on April 30 at the House of Delegates in Kalamazoo. Dr. Veverka is only the fifth MSMS President from Saginaw. Dean George E. Kikano MD will also give an update on the CMU College of Medicine.

MEMBER NEWS Dr. Waheed Akbar Honored with Drum Major Award at MLK Regional Celebration Three champions for diversity, understanding and support of underserved people in the Great Lakes Bay Region were selected to receive the Martin Luther King, Jr. Drum Major for Community Service Award.

The award honors people in the Great Lakes Bay Region who perform everyday acts of service with reliability and commitment, but who seldom receive recognition and whose work exemplifies one or more of Dr. King’s values or beliefs.

The three leaders, representing Bay, Midland and Saginaw counties, were recognized during the Martin Luther King, Jr. Regional Celebration, which was held virtually on Wednesday, January 19. Dr. Condoleezza Rice, 66th Secretary of State, was the featured guest in a moderated fireside chat.

The 2022 Drum Major Award honorees are Waheed Akbar, MD (Saginaw County), Darold Newton (Bay County) and Erin Patrice (Midland County). They received a $1,000 award for a charitable organization of their choice.

Dr. Akbar is an orthopedic surgeon with Ascension Medical Group and has been a dedicated community volunteer, serving on boards of directors for a number of organizations, including Ascension St. Mary’s, the Saginaw Valley State University Foundation and the Saginaw Branch of the NAACP. He established the Dr. Raana Akbar Memorial Lecture on Islam and Culture at SVSU in memory of his wife, and he is active in Interfaith Saginaw.

“I have strongly felt that it is important to bring diversity, to bring love, to bring understanding and to bring forgiveness in everything we do in our day-to-day life,” Akbar said. “I have worked relentlessly on interfaith dialogue and discussion and to bring understanding and diversity and to bring people representing minority groups together and on to the same platform.”

Dr. Delicia Pruitt Receives Governor’s Service Award Congratulations to CMU Health's family medicine physician, Delicia J. Pruitt, MD, for receiving the Governor's Service Award for going above and beyond during the pandemic!

Just as COVID-19 was making its way into the U.S. in January

of 2020, Dr. Delicia Pruitt began working as the Medical Director of the Saginaw County Health Department. “When Saginaw County had its first case in March 2020, I immediately gathered the medical community to discuss our approach to handling COVID-19,” said Dr. Pruitt.

With local healthcare professionals, Dr. Pruitt - who has worked as a doctor in Saginaw for 19 years - organized a task force to provide care for the community and met weekly to decide how to distribute medical supplies and other resources when the demand was high, but supply was low.

When vaccine rollout began, Dr. Pruitt could sense that many individuals were hesitant and concerned about receiving their COVID-19 vaccines. She, along with her colleague, Health Officer Chris Harrington, live-streamed receiving their first doses of the COVID-19 vaccine. The pair aired 65 episodes on Facebook Live with psychiatrists, pediatricians, infectious disease experts, and other physicians as guests to inform the public about important COVID-19 information. Dr. Pruitt hoped this would increase vaccine confidence, especially among people of color. “In African American culture, it’s a mistrust that’s been there for years, and there are definitely disparities when it comes to health care access,” she said.

Understanding this, Dr. Pruitt wrote for a grant to give underserved communities access to vaccinations. She gathered resources and organized mass vaccination clinics at four churches located in underserved areas with the help of multiple health systems. In just two days, over 2,500 vaccines were administered at these clinics, thanks to volunteers from the churches and health systems, including Dr. Pruitt herself.

In addition to these important efforts during the COVID-19 pandemic, Dr. Pruitt has also been appointed by Governor Whitmer to the Protect Michigan Commission (PMC) and the Food Security Council. Dr. Pruitt felt drawn to assist the Food Security Council due to growing up food insecure herself. “It was important for me to have a voice in how we can help identify and help those who are food insecure,” she said. “I felt that food insecurity should be a medical diagnosis so that insurance and other medical professionals, like social workers, could easily identify these patients and make it easier to give them help.”

Ascension St. Mary’s Orthopedic Surgeon Authors Publications Congratulations to Tarek A. Taha, MD, PhD, an Orthopedic surgeon and Section Chief at Ascension St. Mary’s Hospital in Saginaw, who authored the following robotic and bone transport publications: • Elsevier's Arthroplasty Today, with a case report that discusses robotic arm-assisted total hip arthroplasty to correct leg length discrepancy in a patient with spinopelvic obliquity. This approach helped with the patient’s symptoms and gait, as well as, radiographic pelvic alignment.

• Journal of Orthopedic Surgery and Techniques, with a case report on robotic arm-assisted total knee arthroplasty in patients with flexible versus fixed deformities. A “skim cut protocol” was used and effectively restored alignment of rigid knees while minimizing soft tissue releases.

• The Journal of Bone and Joint Surgery, Inc., with a case report on the salvage of bone transport with the induced membrane technique in an open tibia fracture with a segmental defect.

Economic Development Award Presented to Ascension St. Mary’s Hospital Ascension St. Mary's Hospital received an Economic Development Award from Saginaw Future at their annual meeting. The award recognizes Ascension's multi-million dollar investment in operational capital towards their northern Michigan health ministries in Saginaw, Standish and Tawas City. In November 2021, Ascension St. Mary's Hospital completed an $18 million emergency care and trauma center expansion. Stephanie J. Trauma Awareness Night a Success at the Saginaw Spirit Hockey Game As part of Traumatic Brain Injury month in March, Ascension St. Mary's Hospital teamed up with the Saginaw Spirit Hockey Club to host a trauma awareness event. Hospital leaders 20 The Bulletin | Spring 2022

and staff from trauma, emergency, stroke, neurosciences, orthopedics and rehabilitation participated in pre-game activities that included education and preventative tips on how to avoid injuries, especially those to the head and brain.

Field Neurosciences Institute provided 100 free bike helmets and fitting for kids ages 2-15. A recent trauma patient, Denise Wolfe, who was transported by FlightCare to Ascension St. Mary’s Hospital, shared her story and expressed thanks and appreciation for all caregivers who saved her life. Ascension St. Mary’s trauma surgeons, nurses, clinical staff and FlightCare crew members participated in a ceremonial puck drop. The inaugural event was well attended and a huge success.

Welcome Dr. Nishtha Sareen Nishtha Sareen, MD, MPH, will join Ascension Medical Group Riverfront Cardiology on May 1, 2022. She is a highly experienced board-certified interventional cardiologist who joins us from the Detroit market, most recently having worked at Ascension St. John Providence Hospital in Southfield.

Dr. Sareen has been a practicing Interventional Cardiologist, specializing in cardiac and vascular disease, in Michigan since 2012. She is board certified by the American Board of Internal Medicine in Interventional Cardiology, Nuclear Cardiology, Echocardiography, Coronary CTA, Endovascular medicine, General Cardiology and Internal Medicine.

Proud to have completed her undergraduate and medical school training at the Rabindranath Tagore University in India, medical school clinics with High Honors at Harvard Medical School; she went on to complete her Internal Medicine residency training at Hofstra North Shore Hospital in New York.

ten training programs in the country. Dr. Sareen furthered her education by receiving a Master in Public Health from the University of Nevada.

Dr. Sareen has been appointed faculty at various hospitals throughout the state. She is very passionate about education and is involved in many committees and task forces. She is co-chair for the Educational Webinars, WIN with the Society for Coronary Angiography and Interventions. She is an editor for clinician well-being web pages with the American College of Cardiology and part of the Hypertension Working Committee Preventive Work Force with the American College of Cardiology.

Additionally, Dr. Sareen is one of the elected advocacy representatives for the American College of Cardiology in the state of Michigan for 2022. She also serves on the committee for racial disparity by the American College of Cardiology.

Dr. Sareen serves as the medical director for the Ascension Providence Heart Institute Women’s Heart Clinic in Southfield and will be bringing this role to Ascension St. Mary’s Hospital in Saginaw. Intraoperative neuromonitoring is used to assist in preventing injury and preserving functionality of the brain, spinal cord and nerves during high-risk surgeries. Continuous EEG monitoring is used for early detection of abnormal brain patterns in critically ill patients who have suffered a stroke, traumatic brain injury or undergone brain surgery.

CMU College of Medicine Celebrates Successful Match Day

By Kate Worster Central Michigan University College of Medicine fourthyear students gathered at Dow Diamond in Midland at noon on Friday, March 18, 2022, to open envelopes containing the names of the residency programs in which they will continue their physician training for the next three to seven years.

At that exact moment, thousands of medical students around the United States and the world were opening similar envelopes or emails. The annual event is known as Match Day - and it is one of the most significant milestones in the life of a medical student.

“Match Day confirmed that residency programs across the country know the CMU College of Medicine recruits and

educates fabulous students,” said Tina Thompson, Senior Associate Dean of Academic Affairs at CMU College of Medicine.

“They are an amazing group of individuals - bright and dedicated, socially responsive, and eager to positively impact their patients and the communities in which they serve. We are very proud!”

One hundred and five students from the College of Medicine participated in the match process this year. In fulfillment of the College’s mission to train culturally competent physicians to provide comprehensive health care and services to underserved populations in Michigan and beyond, 67 percent of CMU medical students matched to primary care residencies - internal medicine, family medicine, pediatrics, combined internal medicine and pediatrics, psychology, obstetrics and general surgery. CMU medical students also matched to programs including orthopedic surgery, pathology and diagnostic radiology.

Approximately half the class, 52 percent, will remain in Michigan. Four students matched with the CMU Medical Education Partners residency programs in Saginaw, and four students matched to CMU affiliated hospitals. Two students matched with the United States armed services.

Hamadeh Named Chair of Medical Disciplines

By Kate Worster The College of Medicine has named Ghassan Hamadeh, MD the Chair of Medical Disciplines. In his new role, Dr. Hamadeh will lead the academic development for faculty physicians and community educators in the medical discipline, provide mentorship, and support a positive and productive culture.

For more than thirty years, Dr. Hamadeh has served as a physician, educator and health services leader. Passionate about patient-centered primary care, Dr. Hamadeh earned his medical degree at the American University of Beirut (AUB) where he was inducted into the prestigious Alpha Omega Alpha Honor Medical Society in 1985. He completed his family medicine residency at the Medical University of South Carolina, and faculty development fellowship at the University of North Carolina at Chapel Hill. He is currently board certified in family medicine and clinical informatics. Dr. Hamadeh served as the chair of the Department of Family Medicine at AUB from 2005-2021. He came to CMU in early 2021 as a professor of family medicine and the Chief Medical Information Officer for CMU Medical Education Partners. Dr. Hamadeh has extensive experience as primary care and medical education expert consultant with the World Health Organization, the World Bank and Middle Eastern governments.

Dr. Hamadeh began his role as Chair of the Medical Disciplines on March 1, 2022.

Controlled Substance Administrative Rules Promulgated The Bureau of Professional Licensing (BPL) within the Department of Licensing and Regulatory Affairs (LARA) would like to inform you that the new Controlled Substance Rules were effective as of January 6, 2022. A list of the notable changes is below. This is not intended to provide a comprehensive listing of all changes or current requirements.

Licensees should review the rules and statute for a complete understanding of all applicable requirements.

• Except for a few exceptions, adopt the complete list of drugs and substances that are controlled substances under the federal Controlled Substances Act. • Clarify who must obtain a controlled substance license and reorganize the licensing rule.

• Require physician’s assistants to meet the opioid training requirements that apply to controlled substances licensees.

• Expand the individuals who must take the opioid/ controlled substances training.

Substance Use Treatment and Actions in the Great Lakes Bay Region THRIVE is helping to advance substance use education, prevention and treatment occurring throughout the region including multiple efforts around training and community outreach.

Connecting Treatment Courts and Health Professionals Regional Cross Training The Great Lakes Bay Region was recently recognized as a leader for their work in the treatment of and programs supporting substance use disorders (SUD).

Recent training and education efforts included a program titled Connecting Treatment Courts and Health Professionals Regional Cross Training, that took place last year in three areas around the state, including the Great Lakes Bay Region, as well as, Grand Traverse and surrounding areas and the Upper Peninsula.

The feedback from these sessions was overwhelmingly favorable, with several inquiries about what next steps and actions could be taken. In response, additional trainings will be held, building upon this work, focusing on how stakeholders can implement and sustain change in their communities. Survey work is currently being completed by the Wayne State University Center for Behavioral Health and Justice (CBHJ), focused on initiating best practices and coordination of care for individuals who participate in the treatment court system. The outcome will help to shape future training and education efforts in the region and beyond.

Drug Courts as a Bridge to Help Another current effort is the work being done with drug treatment courts locally to reach individuals who need help with SUD. Drug treatment court is designed to be non-adversarial and is implemented via a team approach to address underlying substance use, mental health and trauma issues. Drug courts employ evidence-based practices as a sentencing alternative that provides life-saving treatment to people living with serious addiction and mental health conditions.

Some of the partners in this work include the Michigan Department of Corrections, County Sheriff’s and prosecutor’s offices in the Great Lakes Bay Region, Great Lakes Bay Health Centers, Ten16 Recovery Network, THRIVE and the Underground Railroad.

THRIVE is working with Judge Andre Borrello from the Michigan 4th District Court of Appeals to strengthen the ties previously set between MiHIA, THRIVE and drug courts locally, and within Michigan State Medical Society’s training pursuits with drug court personnel.

Treatment courts are part of a multi-pronged strategy to offer MOUD (medication for opioid use disorder) to those in both the medical system (ED, hospitalization, or EMS) and the criminal justice system.

In 2021, the CAREring Quick Response Team (QRT), comprised of partnerships between THRIVE, Ten16, Mobile Medical Response, Great Lakes Bay Health Centers and Peer360, served 280 people with assistance and support after an overdose. Another 260 people participated in an alternative pain management pilot with Great Lakes Bay Health Centers, a model that could be implemented at scale.

Currently, THRIVE is working on distributing 1,300 fentanyl strips throughout the region in an effort to provide the community with safe, effective testing resources.

Fentanyl testing strips, also known as FTS, are a simple, inexpensive, and evidence-based method for averting an overdose. FTS detect the presence of fentanyl in any form, either pills, powder, or injectables and is a lifesaving tool for users that enable educated decision-making about a user’s safety.

BENEFITS OF SCMS/MSMS

MEMBERSHIP T he Saginaw County Medical Society (SCMS) is the professional association of physicians in Saginaw County, and a component of the Michigan State Medical Society (MSMS). In order to be a member of one, you must be a member of both.

I have been asked many times by both members and nonmembers “Why should I belong to the SCMS and MSMS?” “What do you do for me?” Some have responded they “don’t use their membership because they don’t attend SCMS meetings.” SCMS and MSMS membership is so much more than membership meetings. We are not only support for you, but for your office staff as well. I’m hoping the information below will help members who are on the fence about paying their dues decide to remain members, and also convince nonmembers why they should belong. We represent physicians in all specialties and in all types of practices – solo, group, employed, etc. Active membership is open to MDs and DOs. • Active physician dues are $795 per year ($495/MSMS and $300/SCMS). One hundred percent of SCMS dues and 87 percent of MSMS dues are tax deductible as a business expense.

• The SCMS offers a discount for first year physicians, and MSMS offers discounts for first, second and third year physicians, as well as, physician spouses when both are members.

• Resident and medical student memberships are also available.

What We Offer SCMS Members and Their Staff: • Five Membership Meetings per year where our members have an opportunity to network and enjoy the camaraderie of fellow physicians who love the practice of medicine and want to see progress made in the Saginaw medical community. In addition, our meetings include interesting speakers and an opportunity to earn FREE CME credit.
