THE IMPORTANCE OF SCREENING FOR ABDOMINAL AORTIC ANEURYSM (AAA) www.mayoclinic.org/

An Abdominal Aortic Aneurysm (AAA) occurs when a lower portion of the body's main artery (aorta) becomes weakened and bulges. An AAA is an enlarged area in the lower part of the aorta, the major blood vessel that supplies blood to the body. The aorta, about the thickness of a garden hose, runs from the heart through the center of the chest and abdomen.

Because the aorta is the body's main supplier of blood, a ruptured AAA can cause life-threatening bleeding.

Depending on the size and the rate at which an AAA is growing, treatment may vary from watchful waiting to emergency surgery. Once an AAA is found, doctors will closely monitor it so that surgery can be planned if it's necessary.

AAA’s often grow slowly and usually without symptoms, making them difficult to detect. Some aneurysms will never rupture. Many start small and stay small, although many expand over time. Others expand quickly. Predicting how fast an AAA may enlarge is difficult. As an AAA enlarges, some people may notice: • A pulsating feeling near the navel • Deep, constant pain in the abdomen or on the side of the abdomen • Back pain If you have any of these signs and symptoms, such as sudden severe back or abdominal pain, get immediate emergency help.

AAA risk factors include: • Age - occur most often in people age 65 and older • Tobacco use • Being male • Being white • Family history • High blood pressure Signs and symptoms that your aortic aneurysm has ruptured may include: • Sudden, intense and persistent abdominal or back pain, which can be described as a tearing sensation

• Pain that radiates to your back or legs • Sweatiness • Clamminess • Dizziness • Nausea • Vomiting • Low blood pressure • Fast pulse Talk with your family physician for a referral to a specialist who can perform a screening.

How should you prepare for surgery?

Your doctor or the hospital’s pre-surgery team will examine you and review your medical history.

• If they order any tests, ask why.

• Ask your doctor to check your test records for the past four to six months. Usually you don’t need to repeat a recent test if your condition hasn’t changed.

• Bring a list of the names and doses for all your supplements, medicines, and vitamins.

• Report any new symptoms—even if they occur after your exam.

These steps can help make your surgery safer: Quit smoking, at least for the surgery. It is important not to smoke on the day of your surgery. The sooner you quit, the lower your risk of complications. If you need help quitting, ask your doctor.

Ask about banking your own blood. You can have some of your blood drawn and stored before surgery. That way, if you need a blood transfusion, you will get your own blood. This reduces the risk of infection or a bad reaction.

Ask about pain relief. Ask your doctor if you should stop aspirin or other blood thinners. You may want to use acetaminophen (Tylenol and generic) for pain relief. Avoid ibuprofen (Advil, Motrin IB, and generic) and naproxen (Aleve and generic) because they can cause bleeding.

Ask for help. Ask someone to drive you to and from the hospital and stay overnight with you.

Ask about nursing or rehab care, too.

• Storage containers for dentures, contact lenses, and eyeglasses.

• A few items for comfort, such as a music player and headphones, photos, and a robe.

Antibiotics for your skin When you need them—and when you don’t Skin problems can sometimes look like infections, especially if they’re red, swollen, or tender. So it might seem like treating them with antibiotics is a good idea. But some skin problems don’t stem from infections at all. So treating them with antibiotics can do more harm than good.

Learn about situations when you don’t need antibiotics for your skin. Also learn how to talk to your doctor about when antibiotics may be needed.

Antibiotics don’t help if your skin is not infected Eczema causes red, itchy, and scaly skin. People with eczema often have high amounts of bacteria on their skin. But that doesn’t mean that the germs are causing infection. Even so, some doctors treat eczema with antibiotics that you take by mouth (in pill or liquid form) to kill the germs.

Antibiotics also don’t help your itching or redness. And they don’t make your eczema less severe. Plus, your skin bacteria usually come back in a month or two, if not sooner.

You can control eczema better with lotions and other steps. To ease itching and swelling, ask your doctor about other treatments, such as creams and ointments that contain medicine. You can get them with or without a prescription.

Inflamed cysts usually don’t require antibiotics

Most surgical wounds don’t need antibiotics

Swollen, red, and tender lumps under the skin are usually either inflamed cysts or small boils.

You usually don’t need antibiotics for either of these problems.

Some doctors prescribe antibiotic creams or ointments to keep wounds from getting infected after surgery.

Although infections still happen at hospitals and ambulatory surgery centers, the risk of an infection is fairly low. And topical antibiotics for your skin don’t lower your risk of infection.

Other measures, such as good handwashing by staff, work better to prevent infection.

Petroleum jelly (Vaseline and generic) can help wounds heal by keeping them moist. Plus, it’s cheaper and less likely to make the wound sore.

Inflamed cysts sometimes get better on their own. If they keep getting inflamed, or if they are large or painful, the doctor can open and drain the cyst by making a small incision.

Some cysts can be removed surgically. Both are simple procedures that can be done in a doctor’s office. After that, your cyst will likely heal on its own without antibiotics.

Dangers of superbugs Each year, at least 2 million Americans get sick from superbugs.

Some 23,000 of them die as a result.

About 14,000 Americans die from Clostridium difficile (C. diff) bacteria every year. This infection is triggered by antibiotics.

It causes bad diarrhea and often high fever.

Swelling and redness in your lower legs may not require treatment with antibiotics If one or both of your lower legs are swollen and red, visit your doctor to find out why. In most cases, if both of your lower legs are swollen and red at the same time, it’s not because of an infection. That means you don’t need antibiotics.

There are many other reasons why your lower legs could be swollen and red. For instance, you could have varicose veins or a blood clot in your leg. You could have an allergy to something you touched, such as a detergent or soap. Leg swelling could even be a sign of heart disease.

Before prescribing an antibiotic, your doctor should talk to you and do any tests needed to rule out these problems.

Even then, you should take antibiotics only if there’s a clear sign of an infection like cellulitis. That’s a common skin infection that causes redness and swelling.

Who needs antibiotics for skin problems?

You need antibiotics only if you have signs of a skin infection. These may include: Bumps filled with pus Cracks and sores that ooze pus Wound that oozes pus or has yellow crusts Feeling very hot or cold Fever

High white blood cell count Crusts the color of honey Very red or warm skin with other signs of infection Wound that is red, painful, swollen, or warm

Side effects from antibiotics cause nearly 1 in 5 trips to the emergency department.

Antibiotics can be harmful If you have an infection, antibiotics can save your life. But antibiotics can also be harmful if you take them when you don’t need them. The more antibiotics you use, the less likely they are to work when you need them.

Antibiotics can cause side effects Antibiotics that you take by mouth can cause upset stomach, vomiting, diarrhea, and vaginal yeast infections.

They can also cause allergic reactions.

These include rashes, swelling, itching, and trouble breathing.

Antibiotic creams and ointments can slow the healing of wounds. And they can cause redness, swelling, blistering, draining, and itching.

Antibiotics can breed superbugs More than half of all antibiotics used in the U.S. are not needed. That’s according to the Centers for Disease Control and Prevention. Taking antibiotics when you don’t need them can breed “superbugs.” These are bacteria that are hard to kill. They can make it harder to get well and cause health problems. You can spread superbugs to other people.

IMMUNIZATION RECORDS If your Immunization Records are unavailable, your physician may run an antibody titer (please note the tests may not be covered by insurance). An antibody titer is a laboratory test that measures the level of antibodies in a blood sample.

Why the Test is Performed The antibody level in the blood tells your doctor whether or not you have been exposed to an antigen or something that the body thinks is foreign. The body uses antibodies to attack and remove foreign substances. In some situations, your health care provider may check your antibody titer to see if you had an infection in the past (for example, chickenpox) or to decide which immunizations you need.

The antibody titer is also used to determine: • The strength of an immune response to the body's own tissue in diseases such as systemic lupus erythematosus (SLE) and other autoimmune disorders • If you need a booster immunization • Whether a previous vaccine helped your immune system protect you against the specific disease • If you have had a recent or past infection such as mononucleosis or viral hepatitis

MEDICAL STUDENT LOAN APPLICATIONS Do you know of a medical student (with ties to the Saginaw area) in need of a loan?

The SCMS Foundation may be able to help. The Foundation was formed in 1968 and originally funded through physician donation of earnings from educational and charity work. The Annual Golf Outing and donations now fund the Foundation which makes low interest loans to medical students with ties to the Saginaw area. The intent is to encourage physicians to return to Saginaw County to practice medicine.

The terms of these loans are generous. No interest is charged while the student is in medical school, simple interest is charged at a rate of four percent per annum during a residency program, and interest is charged at a rate of eight percent per annum upon the completion of a residency program.

As of 2012, the Foundation Board voted to forgive all interest if the student returns to Saginaw upon completion of a residency to practice. Additionally, the Board voted in May 2016 to start a loan forgiveness program. If the loan recipient returns to Saginaw to practice upon completion of their residency and they are a dues paying member of the SCMS/MSMS, 25 percent of the principal balance will be forgiven at the end of each year they are practicing in Saginaw County, with a maximum of $5,000 per year forgiven.

The Foundation Board generally considers students who are past their first year of medical school, and among other things, according to: • Strength of connection to Saginaw • Financial need • Scholastic performance • Community service/extracurricular activities The intent of the Foundation loans are to assist and encourage students to return to Saginaw to practice medicine.

NURSING SCHOLARSHIP APPLICATIONS The SCMS Alliance and SCMS Foundation provide $500 nursing scholarships to Saginaw County residents. Over the years, we have awarded many scholarships to help students continue their nursing education.

Requirements for consideration: • Must be a permanent resident of Saginaw County; AND • Currently enrolled in a nursing program or beginning nursing clinical core courses for award year; AND • Overall college GPA no lower than 2.79.

Application packet MUST be complete for consideration. Incomplete applications will be denied. All applications must include a one page essay describing your nursing career goals and how this scholarship would help you financially in completing your nursing degree.

Two letters of recommendation from past or current professors must be included.

NOTE: Prior award recipients must complete a new application packet with new letters of recommendation.

This scholarship is not for graduating high school seniors.

Treating heartburn and GERD

H

eartburn is a feeling of burning pain in the pit of your stomach or your lower chest. It comes from acid backing up from your stomach into your throat. You may have seen ads for heartburn drugs, such as Nexium, Prilosec, or Prevacid. These drugs are called PPIs (proton pump inhibitors). They keep the stomach from making too much acid. They have been shown to heal irritation of the tube between the throat and the stomach (the esophagus).

In most cases, you don’t need a PPI for heartburn. You can get relief from a less powerful drug. And when you do need a PPI, you should take the lowest dose for as short a time as possible.

Here’s why: You may not need a PPI.

PPIs have risks.

More than half of the people who take PPIs probably do not need them. Simple heartburn can be treated with antacids or other drugs, plus diet and lifestyle changes.

If you need a PPI, taking a low dose for less than a year is probably safe. PPIs are expensive and have been linked to higher risk of some problems. Talk with your doctor before taking them for longer than two weeks. Be sure you have a good reason to take the PPI, and take it for the shortest time possible.

You may only have heartburn every now and then—such as after a big, spicy meal. This may be uncomfortable, but it is not serious. You can usually get relief from an antacid, like Rolaids or Tums, or an H2 blocker, such as Pepcid AC or Zantac.

MEDICAL RECORDS

The SCMS does not, and never has, had possession of any patient medical records.

If a physician was a member of the SCMS and is either retired or no longer practicing in Saginaw, we may know where the records are located. Generally, if a patient is looking for records from a specialist, we advise them to check with their primary care physician if the specialist is no longer practicing.

Minimum Required Retention Period Unless a longer retention period is otherwise required under federal or state laws or regulations or by generally accepted standards of medical practice, a licensee (physician/health care provider) is required to keep and retain each record for a minimum of seven (7) years from the date of service to which the record pertains.

Destruction of Medical Records Less than Seven (7) Years Old A licensee may destroy a record that is less than seven (7) years old only if both of the following are satisfied: a. The licensee sends a written notice to the patient at the last known address of that patient informing the patient that the record is about to be destroyed, offering the patient the opportunity to request a copy of that record, and requesting the patient's written authorization to destroy the record. b. The licensee receives written authorization from the patient or his or her authorized representative agreeing to the destruction of the record.

Fees Pursuant to the Michigan Medical Records Access Act, licensees may charge a fee for the records.

Palliative Care

Support at any time during a serious illness

P

alliative care is a new idea in health care.

When you get palliative care, a trained team helps you and your loved ones live with a serious illness. With palliative care, you can get physical, emotional, and spiritual support. You can get help to relieve pain and symptoms—such as fatigue, anxiety, shortness of breath, nausea, and depression. And you can get help in making a treatment plan.

Palliative care can help you at any stage of a serious illness, including congestive heart failure, kidney disease, multiple sclerosis, or cancer. But many patients who are seriously ill miss the benefits of palliative care. Their doctors often wait too long to order it. Or they simply don’t refer patients for palliative care. Palliative care is a big help when you are seriously ill. Here’s why: Palliative care improves your quality of life and may help you live longer.

In a study of people with advanced cancer, those who got palliative care early reported better control of pain and other symptoms, compared to people who didn’t get palliative care. People who got palliative care had a better quality of life and less depression. And they spent less time in the hospital.

These same patients who received early palliative care, along with standard cancer treatments, also lived longer than those receiving only standard treatments.

Studies suggest that there are similar benefits for people with other serious illnesses, such as congestive heart failure and multiple sclerosis.

You can get help with difficult decisions.

Your palliative care team can work with you and your family to: • Think about the pros and cons of various treatments.

• Talk to your doctor about invasive life-support treatments, such as breathing machines or feeding tubes.

• Make your wishes clear to your family or other caregivers.

Start palliative care early for best results.

Don’t wait for your doctor to talk about palliative care. You or your family can request it. Palliative care is most helpful if you start it early during a serious illness. It will have more effect on your quality of care and treatment decisions.

Palliative care is not “end-of-life” care or hospice.

If you are getting palliative care, you don’t have to give up any other treatments for your illness—such as medicines and surgery. Palliative care can be useful, no matter how long you are expected to live. continued on page 68

Tips to help you get the best medical care during a serious illness

Keep copies of your medical records.

Make sure your coordinating doctor also gets your records. Keep a list of your prescriptions and overthe-counter medicines.

Prepare for transitions.

Problems often happen when patients move between hospital, home, a rehab center, or a nursing home.

• Fill out forms to make sure your doctor gets records of your stay.

• Ask for “medication reconciliation” when you transfer.

• Make sure you have a follow-up care plan when you leave the hospital.

Choose a coordinating doctor.

This may be your primary care doctor or a specialist who oversees your treatment and works with your palliative care team and other specialists.

Ask your doctor and team about potential tests and treatments: • Will the test change my treatment? If not, why have the test?

• Is a treatment likely to help me live longer?

If so, how much longer?

What is hyperbaric oxygen therapy?

Hyperbaric oxygen therapy (HBOT) is the administration of 100% oxygen in a pressurized environment. Diffusing oxygen throughout the body promotes angiogenesis, allowing a chronic wound to get the nutrient and oxygen rich blood it needs to heal.

Specialized wound care - including HBOT - is often necessary for optimal treatment of chronic wounds. HBOT is an effective adjunctive therapy used in conjunction with advanced wound care.
