The Future of Medicine in Saginaw

A

s my year as President comes to a close, I would like to thank each and every one of you for your membership and support. Whether you are in solo or group practice, employed, a surgeon, family physician, internist, Ob/Gyn or any of the many other specialties that embody SCMS members, thank you. We are stronger as a group than as individual physicians practicing medicine in 2017.

I find it appropriate for my last article to share an email received from Dean Kikano of the CMU College of Medicine. The SCMS congratulates the 2017 Inaugural Class, and encourages the new doctors of medicine to return to Saginaw to practice, and continue to grow the relationships made in our community during your four years of medical

school. Special congratulations to Swapna Gudipati, MD, CMU CoM Class of 2017, and daughter of long-time SCMS members and past presidents, Drs. Rao and Suhasini Gudipati.

The pride we are all feeling today is well-deserved. The Central Michigan University College of Medicine’s inaugural commencement ceremony yesterday was the successful culmination of many years of hard work, vision in action and a dedicated focus on our mission to care for the under-served.

Yesterday’s ceremony means there are now 62 passionate individuals who now have “M.D.” after their names, and will be going out into the world as excellent physicians, bringing our mission to life. We are so proud each one of our graduates has also been matched for residency training, many of them right here in Michigan.

I invite you to experience the celebration and joy of our faculty, staff and students by viewing these photographs from the event. Local and regional media have also shared the news of our success, as is appropriate; we are impacting the critical future of health care in a pro-active, positive way – by training these much-needed physicians who we know will care deeply for their patients. continued on page 5

I have no doubt that this unique Inaugural Class of the CMU CoM group of individuals, who took a chance on a new medical school and believed in our mission from the start, will do just that.

It will be exciting to see the impact these new and ambitious doctors have on our region, on the state of Michigan, and in the communities they serve throughout their careers.

Thank you to all of the staff, employed and volunteer faculty, our supporters, the CMU administration, and our community and clinical partners for the hard work and the support each of you has given to help make this historic moment a reality. Five Things We Know 1 For Sure

S

o many problems in the world. How can we ever keep up? Do we even need to keep up? When the next patient2 who walks in the door presents with a problem caused by climate change, we know that the time has come to pay attention to this problem. The patient who had to move from the polluted city of her birth because the pollution there hurt her COPD. The patient who can’t afford to eat vegetables because of the price increase caused by the drought in California. The patient who is a refugee from a stricken country.

There does seem to be some skepticism out there, though. Many are pushing back on climate change, in spite of what we hear is the overwhelming scientific evidence.

How can this be? We now know one of the reasons: Corporate greed. Exxon Corporation recently admitted that it knew about Global Warming in 1981, seven years before most in the scientific community, and did it’s very best

to hide what it knew and discredit others who were warning about the catastrophe to come. For 27 long years, Exxon gave many millions of dollars to pseudo-scientific groups and individuals whose sole purpose was to deny the reality of climate change. Corporations like Exxon have enriched themselves by causing harm to the people of the world, and to our patients.

For the sake of thoroughness, let’s review some of the ways global warming causes harm to our patients: • Drought. Dehydration contributes to renal failure, renal stones, poor mental and physical development in children. Water is essential to life. You die without it.

• Water shortages lead to food shortages. Malnutrition makes patients vulnerable to disease and epidemics. Food is essential to health. You die without it.

• Pollution leads to thermal inversions over big cities, trapping particulates and

noxious gasses, making it hard to breathe especially for those with asthma and COPD. Documented tens of thousands have died from this cause.

Clean air is essential to health.

You die without it.

• War. Competition for scarce water and food has led to wars in Africa and Syria. Hundreds of thousands have been affected. Peace is essential to health. You die without it.

• Joblessness. Economic activity is hurt when companies lose customers and employees to illness caused by global warming. Some companies fail because they cannot obtain materials made scarce by climate change. Without jobs, people can’t afford necessary medical care, marriages fail, crime goes up, suicides abound.

continued from page 6 It is hard to reverse 27 years of negative publicity on climate change. For most people, as I mentioned in a previous column, one prominent person’s denial is capable of overriding much good scientific evidence.

It is a tragedy that Capitalism, the backbone of our economic system, can lead to bad health for the common citizen/patient. The corporate CEOs responsible for this mess may feel that because of their great wealth (the CEO of Exxon made $24 million last year), they and their families are immune to the harm they have caused to the environment. They are wrong! We only have one planet, and we all must live here.

As our country moves into a new political era, in which the very billionaires that have refused to accept their responsibility for global warming gain increased political power, it is more important than ever that they change their obstructionist ways and start contributing their great wealth to a solution to Global Warming.

What can you and I do? First, we must educate ourselves

on the facts, realizing that what we hear on the “News” may be inaccurate. Second, we must be straight with our patients.

They need to understand the environmental determinants of their illness. Third, we must contribute to the struggle to curb Global Warming by supporting those groups who are doing the hard work for this cause. Locally, we have the Saginaw Basin Land Conservancy, which preserves thousands of acres of natural environment. Nationally we have “Friends of the Earth,” “Environmental Defense Fund,” “the Citizen’s Climate Lobby,” and many more. Pick one, or two, or three, and let your voice be heard. Do it for your patients…and for yourself

CASA VOLUNTEERS NEEDED We are currently working to recruit potential volunteers to serve as Court Appointed Special Advocates (CASA) for our children in the system due to child abuse and/or neglect. This is such a fabulous way to make a difference in the life of a child.

CASA volunteers come from all walks of life,with a variety of professional, educational and ethnic backgrounds. Social work or legal background is not required. CASA volunteers are screened and receive more than 40 hours of training on Michigan law, the child protection system, the dynamics of abuse and neglect, child development and the juvenile justice system.

The following information was made available to those attending: • The 8th Annual SCMS Foundation Golf Outing will be held on Saturday, June 10, 2017, at the Saginaw Country Club. Proceeds from the event are used for low interest loans to medical students with ties to Saginaw. Physicians who return to Saginaw upon completion of their residency have their loan interest forgiven, and may be eligible for principal forgiveness as well. The Foundation also funds research grants to medical students and residents, in addition to financial awards and mentorship to high school juniors and seniors who are interested in becoming a physician in Saginaw.

Dr. Dedicatoria welcomed Thomas J. Veverka, MD to the podium. Dr. Veverka introduced Rick Wilkening, Director of Business Development of MiHIN, who gave a CME accredited overview of MiHIN. The purpose of MiHIN is to develop the capacity for a statewide electronic health record exchange.

The next Membership Meeting will be held on Tuesday, May 16, 2017, at Horizons Conference Center.

IN MEMORY It is with deep sadness the Saginaw County Medical Society acknowledges the death of Ramesh Kumar MD, the beloved son of Dr. Narenda and Minni Kumar, and brother of Sarada Das. Ramesh passed away unexpectedly on May 4, 2017, at the age of 32 years.

Ramesh grew up in Saginaw, and attended Saginaw Township schools including Hemmeter Elementary, White Pine Middle School and Heritage High School for one year before heading off to boarding school in Bloomfield Hills at Cranbrook-Kingswood. Each year of schooling got Ramesh 10 The Bulletin | April/May 2017

closer to his goal of becoming a urologist. He attended Boston University and continued his studies at Amrita Institute of Medical Sciences in Kerala, India. At the time of his death, he was a second-year urology resident at Henry Ford Hospital in Detroit.

His family and friends have created a nonprofit foundation in his honor to continue his legacy of kindness, generosity and unselfish giving. Donations are being collected through youcaring.com. A week after his death, the charitable foundation has raised over $30,000 of its $100,000 goal.

Funeral services were held on Saturday, May 6, at the Snow Funeral Home in Saginaw.

Perfect score: 100% of CMU Med Students Secure Residencies All 62 students from first class placed in residencies; half to stay in Michigan

Every graduating medical student in the nation had March 17th marked as Match Day. It takes place on the same day, at the same time, at every medical school. It is the day future doctors learn if and where they will do their residency training.

Central Michigan University’s College of Medicine (CMU CoM) experienced its first-ever Match Day on March 17, 2017, and each of its first class of doctors-to-be — all 62 of them — obtained placements. Nearly half — 47 percent — were matched to residency programs in Michigan. This 100 percent placement is especially notable because the number of medical students far exceeds the number of U.S. residencies. In 2016 for example, more than 10,000 students went unmatched.

Match Day is the medical school version of March Madness. A computerized mathematical algorithm is used by the National Resident Matching Program to align the specialty and location preferences of medical students with the preferences of program directors to fill positions at U.S. teaching hospitals. It’s a dramatic event, with embargoed match information released to students in sealed envelopes opened simultaneously across the nation at 11:59 a.m. EST. “Today is a momentous event in the history of our university and our college. This is our inaugural class, and the very first Match Day experienced by our faculty, staff and students,” CMU CoM Dean Dr. George E. Kikano said. “It also is an exciting day for the state, because of our unique mission focused on preparing physicians to serve in rural and medically underserved regions of Michigan.

“CMU is changing the face of health care in the state, in this moment.”

The 62 students in CMU CoM’s inaugural class matched in the following areas: • Emergency Medicine – 19 percent; • Internal Medicine – 18 percent; • Family Medicine – 11 percent; • Psychiatry – 8 percent; • Pediatrics, Anesthesiology, Obstetrics/Gynecology and General Surgery – 24 percent; and • The remaining 20 percent in various other specialties.

Match Day with CMU’s Clinical Partners CMU Medical Education Partners (CMEP), in partnership with the CMU CoM, focused on the opposite end of the matching process, securing residents for its five residency programs to perform specialty training in Saginaw for the next three to four years. CMEP (formerly Synergy Medical Education Alliance) was formed in 1968 to provide undergraduate, resident and physician clinical training. It partnered with CMU CoM in 2012.

“We have a total of 32 applicants matching in our programs this year,” said Dr. Mary Jo Wagner, CMEP’s Director of Graduate Medical Education. “We’ll have four new residents in the Psychiatry program, four in OB/GYN, six in Family Medicine, eight in Internal Medicine, and 10 in Emergency Medicine. One of our goals for these programs is that many of these new doctors choose to stay in our community after they finish their training.”

Key points: • The proposal includes core requirements, which are aligned with current MA and ACCME requirements-and do not represent any new rules fpr accredited providers.

• The AMA Council on Medical Education is proposing simplified and reduced format requirements to provide more flexibility for CME providers.

• Accredited CME providers can introduce and blend new instructional practices and formats appropriate to their learners and setting, as long as they abide by the core requirements.

• CME providers may designate an activity format as “other” if it does not fall into one of the established format categories, without asking permission from the AMA.

13th Annual Free Health Fair “The Doctor Is In!” The 13th Annual FREE Saginaw County Medical Society Health Fair “The Doctor Is In,” held on Saturday, March 18, 2017, at Horizons Conference Center in Saginaw was another success! The SCMS planned this great event in cooperation with CMU Health, Covenant HealthCare, Great Lakes Bay Health Centers, Michigan CardioVascular Institute, St. Mary’s of Michigan/Ascension and the Saginaw County Osteopathic Society. Over 1,000 people attended this year’s event and visited a record 82 vendor booths. Numerous SCMS physicians, residents and medical students participated in the event as well.

During the Health Fair, vendors performed screenings for balance, blood pressure, body mass index, bone density, cholesterol, depression, diabetic foot exams, glucose, hearing, lung function, posture, skin cancer and sleep apnea.

The SCMS had very positive comments from vendors and participants on everything from location to the numerous screenings that were available. Many SCMS members attended the event and either answered questions at the SCMS booth, or just made themselves available for questions during the Health Fair. Both the vendors and participants commented how nice it was to have doctors available for questions.

Non-perishable food items for the East Side Soup Kitchen were also collected from those attending. The Saginaw County Health Department and Sheriff ’s Department collected old, unused or unwanted medications. Gun locks were distributed thanks to the efforts of Dr. Meena Ramani.

Over 1,000 people attended this year’s event and visited a record 82 vendor booths.

Newest Neurosurgical Technology Now at St. Mary’s of Michigan The newest neurosurgical microscope on the market has arrived at St. Mary’s of Michigan. This new microscopic system allows surgeons to achieve even better patient outcomes because of its image quality, outstanding visualization and ergonomic-friendly maneuverability.

During this time, a mobile MRI coach will be located behind St. Mary’s hospital, near the Emerson Hospitality House, and will remain there until the completion of the project. this upgrade, patients that are slated for the open MRI will still go to the Imaging Center for their scan. Quiet Time is Healing Time Studies show that noise is a stressor and detrimental in the healing process for patients. It is also known the quiet time is important for patient satisfaction, as well as, for healing purposes. St. Mary’s of Michigan began quiet time hours in late February from 2-4 p.m. and 11 p.m.-4 a.m. every day.

Nationally Known Cardiologist Visits St. Mary’s of Michigan Seth Worley, MD, FHRS, FACC, nationally known cardiologist who invented the Worley catheter for electrophysiology procedures, met with cardiologists and electrophysiologists from St. Mary’s of Michigan last month to talk about a new technique he uses to place AICD leads – a snare technique that will provide improved contractibility to the heart.

Dr. Worley practices with the Heart Group of Lancaster General Health in Pennsylvania. He was invited to St. Mary’s of Michigan to meet members of the heart team and observe a few cases in the Electrophysiology (EP) lab. He observed a cardiac resynchronization therapy case in the cardiac EP lab at St. Mary’s, and also verbally assisted physicians on performing the AICD lead snare technique.

Gerard Bishop Receives Ninan Award Gerard Bishop wasn’t sure what his colleagues were clapping for when he walked into the conference room at the Covenant HealthCare Mackinaw campus. The applause was all for him. Bishop, an Oncology Social Worker at the Covenant Cancer Care Center, is the first recipient of the Dr. Jacob Ninan Educational Oncology Award established through Covenant HealthCare Foundation.

“Gerry has provided tremendous support for cancer patients, as well as, their families,” said Covenant Foundation Director, Carol Cottrell. “His gentle, caring manner

resonates with patients, providing them comfort and hope as they deal with the diagnosis of cancer. Gerry’s empathetic approach exemplifies the approach demonstrated by Dr. Ninan as he engaged with and treated patients.” In honor of Dr. Ninan’s years of service and commitment to his patients, the hospital and his staff, this annual award is given to a staff member who demonstrates outstanding dedication and commitment to cancer patients and their families. MSMS Releases New Legal Alerts as a Member Benefit (Member Log-In Required) New Physician’s Assistant (PA) Law Now in Effect In mid-March, a new law impacting how Physician’s Assistants (PAs) are regulated under Michigan’s Public Health Code took effect. It is important that physicians who currently supervise PAs understand these changes. Two key changes include the need to have a practice agreement in place, and the requirement that PAs have their own Michigan Controlled Substance license to prescribe drugs listed in schedules 2-5. MSMS, as a new member benefit, has created several resources addressing these changes and others.

Legal Issues for Physicians Treating Hearing Impaired or Limited English Proficiency Patients Many physicians have hearing impaired patients and/or patients with limited English proficiency (“LEP Patients”).

Communicating effectively with these patients is essential to ensure proper diagnosis and treatment, as well as, compliance with legal obligations.

• The AMA Council on Medical Education is proposing simplified and reduced format requirements to provide more flexibility for CME providers.

• Accredited CME providers can introduce and blend new instructional practices and formats appropriate to their learners and setting, as long as they abide by the core requirements.

• CME providers may designate an activity format as “other” if it does not fall into one of the established format categories, without asking permission from the AMA.
