First Call is an RN-staffed and clinically supported service designed to connect you quickly to the physicians and staff of SCL Health, eliminating the need to keep track of multiple numbers or make multiple calls. Among other services, First Call includes: • Referrals to medical cardiology and specialized heart and vascular services. • Call ahead processing for ER referrals with patient details tracked prior to arrival – avoiding miscommunication and unexpected arrivals. • Coordination of in- and out-of-system high level care transfers. • Facilitating physician-to-physician communication and care conferencing. • Arranging all forms of medical transport including air. • Managing logistics for a smooth patient experience in and out of a hospital setting. • Practice-to-practice referrals and transfers.

Program Highlights u Working closely with clinical partners at National Jewish Health, ranked as the nation’s #1 respiratory hospital in the country by U.S. News and World Report, Saint Joseph Hospital is one of the most capable heart programs in the Rocky Mountain region with some of the best patient outcomes. u Saint Joseph Hospital is developing a cardio-oncology program to treat heart conditions of patients undergoing cancer treatment as well as cancer survivors. As chemotherapy and radiation can adversely affect heart function, this highly-specialized program can help patients live longer and better lives. u One of the newest additions to the comprehensive heart program at Saint Joseph Hospital is the Advanced Heart Failure Program at SCL Health Heart & Vascular Institute – Denver. Focusing on education for self-care, medication management, careful testing and monitoring and fast access to adjunct therapies when needed, the program provides a holistic and personalized approach to support patients and their families. u Cardiothoracic surgery at Saint Joseph Hospital continues to be one of the highest performing surgical programs in the United States. As measured by the Society of Thoracic Surgeons, Saint Joseph Hospital has some of the best outcomes for adult heart surgeries, earning better than expected ratings in overall CABG and mitral valve replacement and repair. u Extracorporeal Membrane Oxygenation (ECMO) is a highly specialized treatment for patients with severe and life-threatening illness. Saint Joseph Hospital is one

of just a handful of hospitals in the Rocky Mountain region that offers the necessary technology and unique multidisciplinary expertise to provide this advanced cardiopulmonary support for patients needing ECMO. u Nationwide, 50 percent of patients in cardiogenic shock do not survive, making it one of the most lethal cardiovascular diseases.1 To combat this, Saint Joseph Hospital has created a cardiogenic shock team to institute protocols proven to reduce mortality and improve patient outcomes.

u Due to the collaboration and clinical partnership with National Jewish Health, Saint Joseph Hospital is one of the few places in the world able to diagnose and treat pulmonary hypertension (PH) in a comprehensive and patient-focused way. National Jewish Health and Saint Joseph Hospital are one of only four programs in Colorado accredited to treat PH having demonstrated an ability to properly diagnose the disease and have the capability to manage complex patients. u Structural Heart has been a hallmark and a major focus area for Saint Joseph Hospital. The hospital offers minimally invasive procedures that include TAVR for aortic stenosis, MitraClip for mitral valve regurgitation, ASD/PFO closures and Watchman for stroke risk reduction in atrial fibrillation (AFib) patients. Structural

heart procedures combine the talents of cardiothoracic surgeons and specially trained interventional cardiologists to address structural defects in the heart and valves in patients for whom open-heart surgical procedures are not recommended. u Surgical intervention for ruptured Abdominal Aortic Aneurysm (rAAA) can present elevated risks an d requires a team with considerable experience and expertise. The Saint Joseph Hospital vascular surgery team is one of the most experienced teams in the Rocky Mountain region in the surgical treatment of AAA and specializes in advanced open and endovascular repair. This type of surgery has been highly successful at Saint Joseph Hospital, despite it being a major surgical intervention for patients who require it.

Dr. Ferguson was born and raised in South Africa. He earned a bachelor’s degree and medical degree from University of Cape Town in South Africa. He then completed his residency in internal medicine and fellowship in cardiology and electrophysiology at Oxford Radcliffe Hospitals in Oxford, England. While in Oxford, Dr. Ferguson also did a two-year medical doctorate research degree investigating cardiac pacing. Following this, he moved to the United States and completed an additional electrophysiology fellowship at the University of Virginia. During his time at UVA he was promoted to Professor of Medicine, with an endowed chair and was Director of Clinical Electrophysiology for five years.

Dr. Yarlagadda completed her fellowship in advanced heart failure and cardiac transplant at University of California, San Francisco. She specializes in treatment of patients with heart failure. Dr. Yarlagadda received her medical degree at Albany Medical College. She completed her cardiovascular disease fellowship at the University of Texas Health Science Center in Houston and her internal medicine residency at Rush University Medical Center in Chicago. Additionally, Dr. Yarlagadda has participated in research projects and co-authored numerous publications. Program Highlights u Lutheran Medical Center is home to one of the most advanced electrophysiology programs in the Denver area. With a specialization in laser lead extraction for patients with implanted devices who develop systemic infections and/or have implant leads that need to be removed and replaced, the program is equipped to care for patients with a wide range of EP needs. u In 2019, Lutheran Medical Center added a new, multidisciplinary intervention called Convergence. Convergence is a procedure for intractable arrhythmia that combines cardiothoracic surgery and electrophysiology modalities to perform cardiac ablations for patients who do not respond to traditional AFib ablations. u Since Fall 2019, Lutheran Medical Center has been certified as a Comprehensive Stroke Center, including full interventional neuro-radiology capabilities for advanced interventions for stroke. Lutheran is now one of only six facilities in Colorado to hold either a DNV GL certification or a Joint Commission advanced comprehensive stroke designation. u As a recipient of the 2019 NCDR Chest Pain – MI Registry™ Platinum Performance Achievement Award, Lutheran Medical Center has demonstrated sustained, top level performance in quality of care and adherence to NCDR guidelines and recommendations. The hospital has engaged in robust quality improvement processes, using data to drive improvements and positively impact patient outcomes for heart attack patients. u Lutheran Medical Center has one of the fastest door-to-balloon times in the Denver area. At 54 minutes, Lutheran is faster than the national average of 60 minutes when treating heart attack patients.

Born in Montreal, Quebec, Dr. Diamandopoulos grew up in a suburb of Athens, Greece before moving back to Montreal for high school. She received her undergraduate degree in physiology from McGill University in Montreal. Dr. Diamandopoulos earned her medical degree from the University of Toronto in Ontario. She completed her residency in internal medicine at the Mayo Graduate School of Medicine in Rochester, Minnesota and her fellowship in cardiovascular disease from Emory University School of Medicine. Dr. Diamandopoulos is passionate about cardiac imaging, particularly echocardiography and how it can be used to help her patients.

Program Highlights u Good Samaritan Medical Center doubled its cardiovascular care and treatment capacity in 2019 by expanded staffing of medical cardiologists and interventionalists. The additions of Aparna M. Bhagavat, MD, John D. Ferguson, MD and Sucheta Gosavi, MD to the existing team in Lafayette allows Good Samaritan to keep up with the growing needs of the Broomfield and Boulder County communities. In addition, two new interventional cardiologists, Dr. John Frank and Dr. Wojciech Nowak, will be joining SCL Health Heart & Vascular Institute in 2020. u Good Samaritan Medical Center offers easy-access, flat-fee calcium scoring tests to better assist patients in the community with the early detection of coronary artery disease. All tests are evaluated by specially trained cardiac specialists who then work with a patient’s existing care team to address risks and treatments. u As a recipient of the 2019 NCDR Chest Pain – MI Registry™ Platinum Performance Achievement Award, Good Samaritan Medical Center has demonstrated sustained, top level performance in quality of care and adherence to NCDR guidelines and recommendations. Raised in a family of doctors, Dr. Bhagavat was born and raised in India. She earned her medical degree from Kolkata Medical College in India. She completed her residency in internal medicine at the University of Missouri and a fellowship in cardiology at Tufts Medical Center in Boston. Dr. Bhagavat is especially interested in preventive cardiology, metabolic syndrome and diabetes. Sucheta Gosavi, MD Medical Cardiology Good Samaritan Medical Center, SCL Health Heart & Vascular Institute – Lafayette

Born and raised in India, Dr. Gosavi has always been attracted to medicine. She received her medical degree from R.G. Kar Medical College in Kolkata, India. Program Highlights u By the end of 2020, Platte Valley is anticipating 24/7 cath lab coverage thanks to doubling the size of its cardiology practice as well as leveraging physician coverage from across SCL Health’s entire Denverarea heart and vascular team. Expanded services will greatly benefit patients in Adams and Weld Counties who no longer have to travel out of the community for vital care. u As part of the cardiovascular offerings at Platte Valley Medical Center, peripheral vascular services have been expanded to support their robust advanced wound care and hyperbarics program. As the only hospital with the capabilities and technology to treat chronic wounds north of I-70, Platte Valley can better treat patients suffering from non-healing wounds and other peripheral vascular conditions caused by diabetes or heart and vascular disease including peripheral artery disease (PAD), chronic limb ischemia (CLI), aortic aneurysms, deep vein thrombosis, pulmonary embolism, phlebitis and varicose veins. u Thanks to increased investments, the addition of new providers and being able to offer new treatments and specialties, Platte Valley Medical Center is now the only advanced heart center in the Brighton area and the only major hospital along the I-76 corridor in northeast Colorado.

Program Highlights u St. Mary’s Medical Center is the only comprehensive structural heart program within 200 miles of Grand Junction. For patients in Western Colorado or Eastern Utah, St. Mary’s is the only facility able to provide minimally invasive procedures such as TAVR or Watchman implants outside of Salt Lake City or Denver. u With the state-of-the-art Heart & Vascular Institute expansion, St. Mary’s has been able to expand their structural heart offerings as well as add advanced EP procedures. u In 2019, the vascular and cardiothoracic surgery programs were greatly expanded with the addition of new surgeons and providers. St. Mary’s is now able to provide advanced surgical care better than any other program in Western Colorado.

Dr. Univers is specially trained in general surgery and vascular surgery. The West Palm Beach, Florida native is happy to call Grand Junction home. Prior to relocating to western Colorado, Dr. Univers lived and worked in Knoxville, Tennessee. He received his bachelor’s degree from the University of Florida in 2005 and obtained his medical degree in 2012 from Creighton University School of Medicine in Omaha, Nebraska. A former aerospace engineer, Dr. Walpole refocused his career to become a surgeon in the hopes of helping people and making them better. Dr. Walpole holds several degrees, inducing a degree in chemical engineering, but received his medical degree from Ross University. Program Highlights u St. Vincent Healthcare stands out as one of the few hospitals in the Western United States to have earned a prestigious CMS 5-star rating. St. Vincent was the only hospital in Billings to earn this rating and one of only three in the state of Montana. u St. Vincent has been at the forefront of population health by creating a preventative health clinic that specializes in heart failure. Focused on reducing hospital readmission rates and improving patient outcomes, the clinic helps patients manage their symptoms but also aids in improving their quality of life through dedicated patient education.

u Structural heart programs have been a recognized area of expertise for SCL Health; and St. Vincent Healthcare has been able to expand on this specialty. The structural heart program now is performing minimally invasive MitraClip procedures in addition to TAVR. u Due to the large geographic expanse of Montana and pockets of limited healthcare access, St. Vincent Healthcare has invested in expanding cardiology services to support outlying communities. This includes expanding vascular surgery follow-ups and consults via an outreach clinic in Bozeman. Additionally, thanks to grant support, St. Vincent has also been able to expand cardiac rehab services to remote patients so that patients in rural areas have the monitoring and care afforded to those in larger metropolitan areas.
