Do That Tomorrow

Time goes by quickly — make some memories.

I

was recently able to go on a vacation with my two children — Taylor (age 17) and Nikki (age 14). While on vacation, it occurred to me that we think we have so much time in life. The idea of “I’ll wait and do that tomorrow” is a common thought pattern in many of our lives.

As I spent time with my children, it became very apparent that life as a family together was drawing to a close. My son is ready to spread his wings and go off to college, and my busy daughter has a high school schedule, soccer and social calendar. Time

is flying out the window. Finding those precious moments with family and friends

Tomorrow will come all too soon, leaving us wondering where it all went. is something that we need to commit to.

As I look back on my dental career, I recall that 26 years ago I came to Sacramento and started my dental practice. That too seems like yesterday.

My advice is to savor the moment, live for the day and not put off meeting with friends. Spend some extra time with the family and create some memories, because the memories in life that you create and the time spent with people will be your legacy.

In our busy world of dentistry, don’t forget to make those memories. 

Infection Control — Resources for Patient Communication March 29, 2013 As you are probably aware, there is widespread news coverage involving the investigation of an Oklahoma oral surgeon for allegedly poor infection control practices and delegation of dental procedures. The ADA is monitoring this developing story closely.

As a result of this story, the ADA fielded numerous media inquiries about infection control and health practices in US dental offices. Below are links to resources that may be helpful to you: Policy Statement on Bloodborne Pathogens, Infection Control and the Practice of Dentistry www.ada.org/1851.aspx Statement on Infection Control in Dentistry www.ada.org/1857.aspx CDC Guidelines for Infection Control in Dental Health-Care Settings (2003) www.ada.org/sections/professionalResources/ pdfs/guidelines_cdc_infection.pdf Should your patients express concern, here are some suggested talking points: • I’m glad you asked me about this because I want to assure you that in my dental practice, we follow stringent infection control procedures. Also, as your doctor, I would only delegate procedures to my staff that they are licensed or qualified to perform per state regulations.

• Before you enter the examining room, all surfaces, such as the dental chair, instrument tray, dental light, drawer handles and countertops, have been cleaned and decontaminated.

• Non-disposable dental instruments are cleaned and sterilized between patients.

In my office we sterilize instruments using [DESCRIBE WHETHER YOU USE AN AUTOCLAVE WHICH INVOLVES STEAM UNDER PRESSURE, DRY HEAT OR CHEMICAL STERILIZATION. IF YOU WISH, YOU COULD OFFER TO SHOW YOUR PATIENTS YOUR STERILIZATION OR SET-UP AREA] • Disposable items like needles or gauze are placed in special bags or containers for special, monitored disposal.

• Your well-being is important to me and my staff, which is why we follow stringent infection control procedures and comply with all state regulations for the protection of patients.

• You can visit the American Dental Association’s website at www.mouthhealthy.org and use the search term “infection control” to see a video on this topic. The website has a lot of information to help you take care of your oral health, too.

Your patients may also ask if your office is regularly inspected. Inspection requirements vary from state to state, so if you are unsure about your state’s requirement, we encourage you to contact your state dental board.

CURES — DEA Drug Monitoring Program SDDS has received several calls about drug seekers. We called the DEA and received the CURES information below. Doctors with DEA licenses should know the following:

• I would never delegate procedures to make sure only duly trained or licensed concerning who performs and we make sure that. I care about my patients and your health and safety are my foremost priorities.

CURES (Controlled Substance Utilization Review and Evaluation System) is a prescription drug monitoring program, run by the State of California. Dentists can sign up for free. It is a database in which the doctor can enter the patient’s name and date of birth, and are then given the patient’s prescription history. There is typically a 2–3 week delay on data entry, but doctor’s can at least get an idea of the individual’s history.

• Studies show that following proper infection control procedures greatly reduces risk to patients to the point of an extremely remote possibility.

BEFORE PRESCRIBING ANY MEDICATION, ALL DOCTORS SHOULD GET THE PATIENT’S DRIVER’S LICENSE NUMBER, DATE OF BIRTH AND NAME.

• The Centers for Disease Control and Prevention has developed special recommendations for use in dental offices.

Let me describe just a few of the things that we do in my practice in terms of infection control: • All dental staff involved in patient care scrub their hands before each and every patient and use appropriate protective garb such as gloves, masks, gowns and eyewear.

I have two favorite months of the year, October and March. Why March? March brings the promise of longer, sunny days, the end of the dismal Central Valley fog and the spring season. New beginnings are a symbol of spring, which officially began March 20th. On March 1st CDA leapt into action on behalf of our members with Delta Dental of California and a different attitude.

Why am I discussing March when you are reading this in April? CDA staff and volunteers have to write their columns and articles well in advance of the upcoming CDA Update issues. So while you are reading this in April, I’m writing this in March (March 4th actually) just as CDA members are opening their email to learn that CDA is asking Delta Dental tough questions regarding Delta’s plans to reduce Premier provider reimbursements later this year. Day 1 is almost over and so far the response from CDA members has been primarily positive.

SURPRISED? I am not surprised by the responses from CDA members. In fact, I’m encouraged about the future and how our members will fare as Delta Dental takes its actions. Why? CDA members are smart, they are savvy and, for the most part, they stay in the know about what’s happening in the dental benefits market. They have to possess these attributes in order to ensure their practice’s viability. Even those members who read for the first time today about the future of their Delta Dental Premier fees weren’t angry, they asked thoughtful questions and are looking ahead to mitigate the actions of Delta Dental so that it doesn’t impact their practices, and more importantly, their patients.

What happened in other states to the profession as a result of these fee decreases?

The good news is that dentistry continues to thrive in those states, albeit changed.

Washington experienced several effects, some positive and negative. These included changes to the cost of hygiene services, consolidation of practices, more competitive lab fees, some reduced practice values, and reduced income for some dentists. However, many dental offices benefited from reevaluating their methods and implementing policies and procedures to increase office efficiencies and ensure that the quality of care delivered to their patients remained high.

When speaking with consultants who worked with Washington dentists during their fee reduction, the common thread for those dentists who successfully managed their practices through that process was flexibility, the willingness to do things differently than they had previously. To some dentists, that meant negotiating with or reconsidering labs, suppliers, etc. For other dentists it meant completely restructuring how they ran their office, both procedurally and physically.

Being Social

Social Media Marketing for Dentists: Managing Your Online Identity

can establish yourself as an authority in the minds of your audience. You show that you are knowledgeable, capable and respected. For many people, this can go a long way toward easing anxiety.

Who do you want to be online? Are you projecting an image of a successful dental practice, or does your lack of online activity say “Sorry, We’re Closed?” Managing your online identity should be an important part of your marketing strategy. But, what constitutes an online identity, anyway?

BE REAL: No relationship is made up entirely of academic, professional or marketing dialogue. Occasionally make sure to post about trivialities like birthdays, favorite songs, or even cats doing funny things. Don’t be afraid to have opinions and share them. Consider sharing your own experiences as a dental patient, or even anecdotes from your personal life.

Your professional website can connect users to your presence on social media outlets.

Customer reviews on Yelp, Google, or Angie’s List can join with an active Facebook profile, a busy Twitter feed and a photo stream on Instagram to create a clear overall picture of your personality and business. All of these interactions create your online identity.

When managing your online identity, it’s important to focus on how you want your audience to perceive you or your practice. Let’s assume you have the basics: an email account, and a functioning professional website to link to. These are the first building blocks of an online identity. The next piece should be social media.

What is social media? The Wikipedia entry for the term states: “Social media refers to the means of interactions among people in which they create, share and exchange information and ideas in virtual communities and networks.” Some examples include Facebook, Twitter, blogs and YouTube.

Remember that social media is like a huge dinner party. Would you want your dinner party interrupted by aggressive salespeople?

BE CONSISTENT: Don’t post only once in a while. Post at least three times per week. Some dentists even post several times per day! 74% of marketers saw an increase in website traffic after investing as little as six hours per week in social media.

Despite this, 70% of Facebook brand pages are updated less than twice per month.

Commit to spending time and resources on your social media campaign. Treat it like a valuable part of your overall marketing effort — because it can be!

BE ACCESSIBLE: Write in terms that people can understand, and avoid delving into procedural detail unless someone specifically asks for it. Also, make sure to post pictures or video, when applicable. After all, a picture says a thousand words in a language everyone understands!

First, decide whether using social media is right for you. It isn’t right for every dentist.

Some dentists actually choose to minimize their online presence because they’re already too busy with patients gained by traditional means.

BE RESPONSIVE: Respond quickly to questions, comments or friend requests. If someone takes the time to make contact with you online, treat it like a golden opportunity. People hate to be ignored, and will quickly turn hostile if they feel you aren’t interested in them.

If you aren’t ready or able to commit to a fully realized social media strategy, then don’t even open a Facebook account. You don’t want a poorly managed online identity to show up in the search results. It’s better to have no identity than a sloppy, haphazard one. A Facebook account with few posts or a YouTube channel with only one or two videos can do more damage to your online identity than having nothing at all.

Managing your online identity is becoming a vital aspect of marketing. Even if you haven’t created any social media accounts, or even a web page, you may still have an online identity that you don’t know about. User reviews on sites such as Yelp, or even Google can destroy your online reputation if they go unchecked.

Once you’ve decided to incorporate social media into your online identity, it becomes even more important to manage users’ perceptions. If you don’t take an active role in deciding who you want to be online, your users will decide who you are for themselves. Mastering the following qualities will help turn your online identity into a powerful marketing tool:

Try searching for your practice, or even your own name, on any of the major search engines. The top three listings will determine what most people see when they look for you, but the rest of the listings may provide further insight. Once you discover what people are saying about you, you can take steps to respond.

BE AN EXPERT: Dentists are faced with a unique challenge. As many as 20% of Americans avoid seeing a dentist because they are afraid. How can dentists overcome this anxiety in their target audience?

Dentists face unique challenges in shaping public opinion; from mitigating fear and anxiety to simply finding prospective customers. This makes it even more important for dentists to be aware of and actively engaged in shaping their online identities. Take charge of your online identity now, and make sure your customers see who you want to be online. 

The best way is to demonstrate expertise in your industry. Social media is the perfect venue. By writing and/or sharing relevant articles, videos or photos, you

The January 2013 issue of The Nugget on mid-level providers caused quite a stir.

This issue presents additional information on that topic from guest editor, Dr. Deb Finney.

H

ey, how about that January 2013 issue of the SDDS Nugget, huh?! The editor of that issue, Dr. Jim McNerney, set out to express some of the membership's feelings about the failure of the legislature to pass SB694 in particular, and about the entire concept of "mid-level providers" (MLP) in general. This editor even contributed a humorous (he thought) article on the matter, alluding to a camel's desire to enter a tent.

Well, it seems our issue caught the attention of people far beyond our local Society, leading to a long series of telephone conversations between your editors and various people who

felt the "other" side of the MLP question was given short shrift. It was determined that, in fairness, a future issue of the Nugget would be given to present the arguments supporting the entire concept.

Fortunately, we have in the membership of our Society a former president of CDA and a former dental hygienist, who is also imminently qualified to comment on the MLP question.

Dr. Debra Finney has gathered additional articles she felt would help the membership understand the many facets of this complicated issue since, as the articles make abundantly clear, it isn't going to go away any time soon.

On a much sadder note, the passing of two of our most stalwart members, Drs. Marty Rosa and Gordon Harris, within a month of each other must be noted and shared with the readers. They were such good friends and mentors to so many, for so long. They will be sorely missed. 

he January 2013 issue of The Nugget featured several opinions about SB694, cheering its death in the Assembly and questioning the legitimacy of an actual lack of access to care which led to a perceived need for such legislation. The Nugget fell under criticism for presenting a unilateral assessment of the access to care issue and the proposed solutions, specifically, mid-level providers. I was asked to guest edit

I would like to share my perspective which is based on my background and experiences . . .

There are many factors responsible for the barriers to receiving care; it is not just the poor and uneducated who are underserved.

I was trained as an RDHEF and worked in a dental office in Wasilla, Alaska where I did everything but the restorative preparations.

I know the quality of the restorations I placed was better than many I see in patients today. I also taught restorative dentistry at the Western Kentucky University Dental Hygiene Program. I have first-hand familiarity with the educational standards and the ability to train hygiene students in expanded functions as proposed by Dr. David Nash at the University of Kentucky. I know that it is a model that can increase the delivery of care and can make a difference in some situations. I have followed the development and implementation of the Alaska Dental Health Aide Therapist and I understand why it is a model that works in remote areas of Alaska.

I was born in Sitka, Alaska and lived until age 12 in logging camps on Prince of Wales Island. The closest health care providers were 40 miles away by boat or air. Our groceries were delivered once a month by boat — we are still trying to convince my youngest

My goal with the opinions expressed here is to take the emotion out of the situation — lofty goal, I know. But it will be difficult for dentistry to agree on potential solutions — or even if there is a need, unless we can view the situation through the eyes of our colleagues.

challenges to providing oral health care to a significant portion of our population. We have tried to make a dent in the local need through our SDDF programs and CDA Cares. We have provided LOTS of care but at the end of the day, there are still people standing in line, waiting and hoping that they will be seen. One solution will not fit all; we need a variety of solutions. I believe we must partner with legislators or be forced to accept the consequences of what they decide without dentistry’s input. I don’t know which solutions will work for which populations in California and I don’t believe anyone does. The only way we will know is to research the options in a qualified, well designed study. As a CDA Past President, I trust the direction set by our leaders and I wish I could instill the same confidence in all dentists.

And lastly, we are not the only health care providers dealing with this issue. Medicine will be faced with a shortage of primary care providers when the Affordable Care Act takes effect. Senator Hernandez has introduced legislation that would allow optometrists, nurse practitioners and pharmacists to provide expanded care.

What about including dentists? What an opportunity we would have to educate the public about our training and our knowledge of systemic health. Oh wait, that would make us mid-level providers. Hmmm, wonder how medicine would feel about that.

My openness to looking at different solutions to the access/barriers issues comes originally from having been trained on the job as a dental assistant at a U.S. Army Base outside of Stuttgart, Germany. I was seventeen years old. There were two general dentists responsible for 4,000 patients, with one dental assistant.

When approaching a complex issue, it is important for me to understand the big picture perspective of what I am dealing with and how to create a win-win situation. Whether it is a blessing or a curse, I have been afforded the opportunity to wear many different hats in my various professional roles. I have a unique perspective in that I have and manage my own private practice, along with being the Chief Dental Officer for a Federally Qualified Health Center. I served as the Chair of Legislation for the Dental Board of California and later on the CDA Government Affairs Council. I understand the regulatory and sausage making process more than I would like. The lesson I learned from these experiences is that being in the decision-makers shoes is very different than the more comfortable role of playing armchair quarterback.

Every time I hear the term mid-level provider, I cringe, as I know what is coming next. The visceral response is very clear on faces. At

the recent CDA Leadership Conference, one of the speakers gave a great example of their definition of fear: False Evidence Appearing Real. Communication ends. Panic and mistrust set in. This is what we have lived through the past two years. I believe that we are slowly and cautiously moving forward.

What is the definition of a mid-level provider? If you ask five dentists you will get five different definitions and opinions.

All “mid-levels / dental therapists” are not alike in education or scope. These do and will vary tremendously state by state.

The Pew Foundation invited representatives from each of the 50 states to Minnesota to see their dental therapist models and visit the delivery sites. I was one of the invited guests. I can tell you first-hand that they have very good educational programs and the care we observed was provided in an appropriate fashion.

The dental therapists (DT) in Minnesota, with the exception of a few, are all hygienists.

The dental hygiene and dental assisting scope of practice in Minnesota allows for them to place and finish amalgams and Class I and Class V composites. They are also allowed to place and remove IV lines. They already have a great deal of experience in delivering care to patients. The DT curriculum fills in the missing pieces of their education. They are taking and passing the same CRDTS licensure examinations as the dental graduates. The grading is blind and the examiners do not know who they are grading.

Why is the dental therapist model working (for now) in Minnesota? It is all about the math.

he clinic manager was the noncommissioned officer-in-charge. He was a hygienist but was running the clinic so he taught the dental assistants to do hygiene procedures. I was a Red Cross volunteer and they put me to work. I placed all of the sealants on the kids, learned how to take x-rays and assisted chairside. There were staff shortages. We had a daily sick call of 15–30 patients in need of services to be able to do their jobs to defend our country.

Necessity is the mother of invention. I was well trained and provided quality care to the patients I served. This volunteer experience led to my decision to become a dentist.

• Why does this seem to be working in Minnesota?

• Is there an access issue?

• How do we fix our broken system?

Minnesota has a shortage of dentists and an abundance of under-employed hygienists.

Starting salaries for dentists are between $130- 140K/ year. Dental Hygiene salaries are $60- $70K/year. California starting salaries for dentists are approximately $100K/year.

Hygienists working full time are making the same amount and in some cases, even more.

In Minnesota it is less expensive to have the dental therapists provide the dental care.

The “death” of SB 694 reminds me of Mark Twain’s famous quote about the reports of his death being greatly exaggerated. The bill number may be gone for the time being but the issues that brought it forward are still unresolved.

The California State Legislature has to answer to its constituents. The calculation of the number of 30% of the population that has not been able to access care was prior to loss

of adult dental services through the Denti-Cal program. To those who still don’t think we have an access issue, I invite you to spend a day with me at one of our clinics or volunteer at a CDA Cares event. We are seeing a much greater demand for services at our clinics from patients that were previously fully employed and benefitted. Now, after being out of work or out of benefits for several years, patients are trying to maintain their dental health by seeking lower cost care at our sites. I am having patients from my private practice, who are struggling financially due to the economy, ask if they can receive services at the clinic as they can no longer afford to come to see me in private practice.

Our system is broken and, until we fix the system, no matter how many dental providers we throw at the problem, we are not going to resolve it. That is the bottom line. That being said, it is up to dentists to find viable solutions and to present them to the legislature. We have started this process and the legislature did not move forward.

We as members of CDA have very clearly demonstrated that there isn’t one silver bullet to solve the access/barriers to care issue in California, but that we are willing to address the issues. 

What I am writing here is my own personal perspective, not that of any organization or group. I feel that I can offer a unique perspective, having not only lived through the many divisive debates we have had, but also having had the opportunity to speak at length with all parties to this issue.

This has been an emotional issue for many of us. In the process of the debate, however, we have often lost our sense of propriety, honesty and even civility. As a dental professional, I found this of far greater consequence than the issue itself.

By the same token, in the Alaska and Minnesota MLP programs, we haven’t seen evidence of post-operative disasters by MLPs providing irreversible dental procedures (admittedly the sample size is quite small). I had occasion to meet with some of the Alaska MLPs and discuss in detail how they handle unexpected difficulties when extracting teeth.

Their knowledge was quite impressive; although it should be pointed out that they were always in telecommunication with a dentist while performing extractions. Even these conversations, however, did not convince me that MLPs could perform surgical procedures safely on a large scale, given their level of training. A well designed study might be helpful in making that determination.

Senator Padilla communicated clearly to us his commitment to the policy contained in SB 694. He has already introduced a placeholder bill in the new legislative session that will basically pick up from where SB 694 left off. It would be worthwhile to discuss some general thoughts on the issue of MLPs and how we might be better served relating to the issue this time around.

Much has been made of the “urban legend” of an access to care problem in dentistry.

Claiming to have empty chair time in one’s office does not mean that there is no access

irst, let me make a disclaimer. I am not currently in favor of establishing a new member of the dental team who would be able to do irreversible procedures, generally referred to as a mid-level provider (MLP). The model does not seem to make economic sense to me, especially when the provider would have to receive nearly as much training as a dentist, as we see in Minnesota. It is worth pointing out that the vast majority of trainees in Minnesota are hygienists, who already have background, training and experience in the dental field. It also seems that because of the high cost of training and the basic expenses involved in providing services, the possible cost savings to the consumer of the dental service provided by MLPs would be minimal

problem. The question is whether the dentist with that extra chair time is willing and able to see those who have the access problem.

That may involve helping to provide transportation, employing a multi-lingual staff and accepting significantly lower fees.

Anyone who has volunteered at CDA Cares events has no doubt that there are many Californians who, for a variety of reasons, cannot access dental care. Our system is broken. There are too many expensive ER visits for basic dental concerns. Even those with Medicaid insurance (Denti-Cal) cannot find practitioners who will accept it. Reimbursement rates in these programs are ridiculously low. On top of all this, it is projected that there will be close to 1,000,000 more children in California who will be insured under Denti-Cal, because of the Medicaid expansion under the Affordable Health Care Act. Who will treat these additional kids if we aren’t able to treat the ones already in the program?

Again, I don’t think that MLPs are the answer, but let’s not turn a blind eye to the problem.

I had an opportunity to have lunch with Wendy Lazarus, founder and Co-president of the Children’s Partnership (the organization involved in advocating for MLP’s in California). The fact that her organization has identified the lack of dental care for California’s children as a critical agenda issue is quite laudable, even if we don’t agree on all of their solutions. I asked her why the Children’s Partnership was so focused on MLPs and not on other proposals that may be more effective. She responded that they wanted to pursue all possible solutions.

They are focusing on enrolling more children in Denti-Cal, ensuring that there are an adequate number of dental providers, making Denti-Cal more user friendly, and yes, increasing reimbursement to the dentist.

We may yet find the Children’s Partnership to be an important ally in improving access to dental care in our State.

Some have complained that MLPs would create two levels of care. Anyone involved in serving the poor can attest to the fact that different levels of care already exist. In fact, there are at least three levels of dental care in California today. Some receive good care, some receive only basic or episodic care, and some receive no care at all. The “levels of care” argument no longer resonates with the public or the legislators. Our goal should be to at least provide basic care to everyone which is better than no care to some.

Others have expressed concern about even doing a study as if studies are just manipulated to achieve the desired result. Having been privy to a lot of the negotiation around SB694, I feel very confident that CDA would only agree to a study that is university based and independent of political pressure. We have to realize that we don’t look very professional when we oppose science and scientific study, especially when we regularly invoke that same science when advocating for water fluoridation or the use of amalgam.

I have also tried to give some thought to how we should prioritize issues around MLPs. Without demeaning the strong concerns that have been expressed on this issue, I don’t think it should be our first priority.

I recall vividly during our debates about

RDHAPs and licensure by credential, the ominous warnings that these initiatives would destroy our practices. They both became law and neither has had any effect on dental practices to date. Of the 500 or so licensed RDAHPs in California, those who practice tend to do so in nursing homes and other non-traditional settings and only a small number of dentists from other states have taken advantage of the licensure by credential pathway. I suspect that we would see a similar scenario with MLPs. We have to learn to trust each other again.

CDA staff and volunteer dentists put in a great amount of time to study issues before recommendations are made. It should be appreciated even if one thinks their conclusions are wrong. We are all colleagues, and share the same basic interest of enhancing our practices and the oral health of the public.

Finally, we have invested much to create a positive image of dentists and dentistry both in the halls of government and among the public. That positive image has served us well as a profession, but more importantly, has served the public well by making oral health a priority. We invented the concept of preventive care. We have successfully undertaken the largest public health project in history, water fluoridation. There are those in the media who want to portray us as only interested in financial gain. We cannot afford to contribute to that negative perception.

Our public positions should be based on the enhancement of oral health in our society.

Damage to our image will do us far more harm than any mid-level provider ever could. 

How should we approach the new Padilla Bill as it makes its way through the Legislature?

We should all commit to healthy respectful debate based on facts and refrain from personal attacks.

It is fair to bring all of the scientific literature into the discussion, whether it is from New Zealand or from a CDA commissioned study. We should take the time to read and understand the material before discussing it.

Where gaps in knowledge exist, we should embrace the possibility of scientific study to answer outstanding questions. We should also be vigilant to ensure that proposed studies are well designed and properly done.

We should remember that the legislative process is complicated and often messy. As bills progress they are rewritten and amended. While it is important to follow the changes as the bill moves through the legislature, it is the final product that counts.

Once decisions are made by our House of Delegates, let’s all get behind them and move on.

What To Do

About Doctor Shortages?

any areas in our state already deal with doctor shortages. From inner city Los Angeles to rural counties, doctor shortages have become the norm.

According to a report commissioned by the California Health Care Foundation, the number of primary care physicians actively practicing in California is at the very bottom range of, or below, the state’s need based on Council on Graduate Medical Education estimates. The distribution of these physicians is also poor. In 2008, there were 69,460 actively practicing physicians in California (this includes Doctors of Medicine and Doctors of Osteopathic Medicine), and only 35 percent of these physicians reported practicing primary care. This equates to 63 active primary care physicians in patient

care per 100,000 persons. According to the Council on Graduate Medical Education, a range of 60 to 80 primary care physicians are needed per 100,000 in order to adequately meet the needs of the population. When the same metric is applied regionally, only 16 of California’s 58 counties fall within the needed supply range for primary care physicians. In other words, less than one third of Californians live in a community where they have access to the health care services they need.

A recent New York Times editorial titled “When the Doctor is Not Needed” noted “there is plenty of evidence that well-trained health workers can provide routine service that is every bit as good or even better than what the patients would receive from a

doctor. And because they are paid less than doctors, they can save the patient and the health care system money.” As a practicing optometrist, I know this is true. I have helped patients become aware of general health issues related to eyesight, like diabetes or hypertension. Other trained professionals are also appropriately positioned to use their considerable training and knowledge to assist patients as well.

This month I introduced three bills (Senate Bills 491, 492, and 493), which are designed to bridge the “provider gap.” This legislation will allow nurse practitioners, pharmacists, and optometrists to provide medical advice and assistance to the full extent of their education and training, unrestrained by artificial barriers to practice.

This legislation will improve Californians’ access to high quality healthcare from our state’s range of safe, efficient, and regulated providers. Over the past decade, testing and certification programs have significantly raised their standards, standards which must be met by physician assistants, nurse practitioners, pharmacists, and optometrists.

Moreover, practitioners in these areas have received enhanced clinical training, moved

to graduate or advanced degrees, and met the requirements included in upgraded accreditation processes.

Dental professionals are also in short supply. According to the California Dental Association, an estimated 10 million Californians experience barriers to dental care. The need for dental services continues to increase, and I applaud CDA’s efforts to increase access to these crucial services. Just one example to highlight is the California Dental Foundation’s CDA Cares event, which provides free dental clinics to provide oral health services and education for more than 2,000 patients at each event. These patients are low-income and have the greatest needs — and this may be the first time seeing a dentist.

Other states have recognized the advances in these medical practitioner programs and have expanded the roles of these more highly qualified professionals. California has not kept pace. It’s time to fall in line with what makes sense for the people of our state.

The people of California deserve to receive affordable, quality healthcare. Please join me in supporting this legislation. 

Foundation President…

I want to personally thank everyone who participated in the Sacramento District Dental Foundation events in 2012. I humbly ask you to continue that participation and multiply it in the years to come. Our Foundation is extremely successful because of the support of our dentists and the community at large. $70,000 of community grants were awarded to our Foundation in 2012. We recently surpassed $1 million in total assets which will be used to educate and treat the underserved in our five county area.

Community leaders are extremely impressed with the generosity of organized dentistry in the Sacramento region. Not only do we have our own programs like Smiles For Kids and Smiles For Big Kids, we helped recruit volunteers for RAM and CDA Cares, both held at Cal Expo. They became the two most successful charitable clinics in California history.

Smiles For Kids (SFK) continues to be the signature project of the Foundation. In 2012 approximately 35,000 kids were screened in 112 schools. The percentage of children with observable caries or emergency situations has constantly decreased over the years.

That statistic is probably the best measurement of the success of the SFK program.

Smiles For Big Kids will be highlighted again this year. In 2013 SFBK received a $50,000 grant from Dignity Health (Catholic Healthcare West) to partner with three other organizations. The coordinated services of the partnership will increase the number of women and children who transition out of homelessness and into healthier and more productive lives. We need about 50 additional doctors to adopt one of the women for treatment. Please volunteer and make this ambitious project a success.

As the Foundation has grown and prospered, it has become more complex. We cannot “fly by the seat of our pants” like we did years ago. In 2012 we completed a 2013– 2015 Strategic Plan and updated all of our policy documents. Thank you Cathy for your superhuman effort in helping the Board make these documents more relevant for the future. We are now ready to put the $1 million to work in a thoughtful and responsible way.

Please become a Foundation member by paying the $75 dues and renew your membership each year. In 2012 there were 357 members, the most in our history. Also remember Crowns For Kids where you contribute scrap gold and precious metals to help fund SFK. Last year we collected nearly $36,000, our best year yet. Please consider contributing through the Spirit of Giving program which made a significant comeback in 2012. Enjoy the Broadway Series and the Swing For Smiles Golf Tourney on May 3. Come to the SDDS holiday party and participate in the silent auction. Enjoy the fun and help the Foundation at the same time.

We are an amazing organization. It truly changes the lives of patients and volunteers alike.

Remember to post OSHA Form 300A, which lists a summary of the total number of job-related injuries and illnesses that occurred during 2012. The form must be posted from February 1 through April 30, 2013.

he summary must include the total number of job-related injuries and illnesses that occurred in 2012 and were logged on OSHA Form 300, Log of Work-Related Injuries and Illnesses. To assist in calculating incidence rates, information about the annual average number of employees and total hours worked during the calendar year is also required. If a company recorded no injuries or illnesses in 2012, the employer must enter "zero" on the total line.

Dental Practice Management in the Cloud — Part II

he last article discussed the general concepts about the emerging trend called “cloud computing” where computers share information across local and remote network locations. A dental practice utilizing the “cloud computing” model would usually be structured so its server computer is maintained off-site, and its database information synchronized with the local office computers as well as any device capable of communicating over the internet. The cloud-based computing model has its primary usage benefit in environments where data acquisition and manipulation are performed in numerous locations. This does not reflect how a dental practice operates: most practices are single location entities. Our patients are treated in the chair located in the dental office and our patient’s treatment data is primarily altered chair-side in the back office or at the reception desk. There are some infrequent instances where remotely changing data may be helpful like updating charts from home or accessing a patient’s chart when an after-hours phone call is received. But the primary data manipulation is performed locally in a dental office so changing to a completely different model of information access should provide some significant benefits without many drawbacks to be worth the structural change.

One of the main structural differences with storing all information on a server outside of the local dental office walls in a cloud-based system means that your practice management software is continually in a “remote access” mode. This means data gets continuously transmitted back and forth from the practice where it has a higher security risk of having data intercepted or corrupted. Having the cloud server always accessible outside of a local network makes the server itself more visible to attacks from hackers, decreasing its potential reliability from a security standpoint. A noncloud system would usually have a local server computer that is not accessible or visible to

the outside world via the internet, making it unavailable to lurking hackers. Considering how the internet connects the entire world, having continuous open network access to a server theoretically permits potential hacking from any location of the globe. This security risk is likened to having a large window at the front of your practice which is located on a busy city street; when leaving for the day you would not leave your lights on in the office so passing traffic can peer inside and potentially burglarize the facility. This security risk needs to be weighed if you are considering moving towards a cloud-based practice management system.

The reliability of the system needs to also be evaluated, and based on the structure of a cloud-based computing platform, there are far more places for a cloud-based system to fail compared to a local server platform structure.

Utilizing a cloud-based platform requires information to travel between two remote computers using the internet as the backbone for information travel. The transmission of information across the internet is a far more complex process compared to information travel across a local dental office computer network. In the local dental office, a workstation is connected to a wire which is connected to a switch or hub, which is then

connected to the server by another wire; and that is all that represents the simple local server platform model. On the other hand, the cloud-based system can be represented by the following: the office workstation is connected to a wire which is connected to a switch, connected to a router, connected to a modem, which is connected to the outside internet provider which itself is made up of countless wire junctions, computers, switches and routers, which then connect to various domain name server computers which then connect to the server computer which itself has its own set of wires, switch, modem and router over its own internet provider. Since network hardware and the interconnecting structure is far from faultless, a cloud-based system has a significantly higher potential for communication failure than the simpler local server platform model.

When treating patients we have enough stress that we can’t afford to worry about the reliability of our computing systems. Given the security and reliability issues surrounding cloud-based computing systems, there needs to be some significant benefits to warrant its use. In the next article the potential benefits of a cloud-based system will be evaluated to help determine if the dental practice fits the cloud computing model. 

GORDON HARRIS, DDS

In memoriam

Dr. Gordon Harris passed away suddenly on April 2, at the age of 81.

Dr. Harris grew up in Watsonville, and spent many summers with family in Canada. He attended University of California Berkeley, earning his undergraduate degree there before going on to receive his DDS from the UCSF School of Dentistry. Upon graduation in 1955, he was drafted by the U.S. Army, serving as a dental officer until 1957. He then began a dental practice in Roseville.

Dr. Harris married Mary Ann Dreher, the love of his life, on June 28, 1959, and the couple went on to have three children.

Dr. Harris was actively involved in his community at large and in the SDDS Foundation specifically. He served as President of the Roseville Chamber of Commerce, on the Council at St. John’s Lutheran Church and was a long time member of the Lion’s Club. He supported Del Oro High School, Child Advocates of Placer County, the Boy Scouts of America and numerous other charities. For our Foundation’s Smiles for Kids program, he screened school children EVERY YEAR. In the last two years, he screened nearly 9,000 kids himself!

Dr. Harris was a member of SDDS for 57 years, serving as President in 1970. He was one of the founders of the SDDS Foundation and was still serving on the Foundation Board at the time of his passing. He was also serving on the SacPAC committee, the Foundation Finance Committee and the Education and Speakers Committee (with Dr. Wai Chan) for dental health information for moms and their kids. Just last month, he was awarded the Harry Wong Community Service Award for his outstanding contributions to better the oral health of the community. His enthusiasm for service and his love of our Dental Society and its Foundation will be remembered fondly and will be deeply missed.

A little more than a decade later, that number has grown 20 times over.
