2018 Impact Summary Connecting with the Homeless Saban Community Clinic is addressing the unprecedented homeless crisis in Los Angeles with several new initiatives. Last year, the Clinic provided more than 6,700 homeless patient visits.
The E6 Outreach Team administers care to the chronically homeless, on the streets. In just six months, the collaborative team provided care to 600 people and placed 75 into housing.
The Continuing Your Care program coordinates care for frequent ER visitors who have been discharged from Cedars-Sinai Medical Center. In just one month, 16 individuals have been linked to services at the Clinic.
Our new Homeless Connect Kit is a vital resource to someone on the streets. It provides hope, restores dignity and provides a gateway into the Clinic for a warm shower and health care. More than 1,500 kits have been distributed.
Increasing Technology: Saban App
Expanding Pediatrics: Saban Kids
The Saban App connects patients to everything they need in one central location. The app is also linked to MyChart, allowing patients to make appointments, refill their prescriptions and receive their health information in real time, providing better communication with their medical home.
The expansion of our pediatric care team including a Pediatric Dentist and Practitioner of Family Medicine allowed us to see more children at our Melrose Health Center. In the last year, pediatric visits have increased by 30%.
