Our Impact: Redefining Community Clinic Care Quality Improvement
Health Coverage
The Clinic achieved Patient Centered Medical Home Level 3 Recognition, the gold standard, at all three sites! This model improves the quality of care and patient experience by streamlining the entire operation. Patient access has increased and wait times have reduced by 30%.
Clinic staff enrolled more than 5,000 individuals in a health plan that was right for them. The Clinic participated in 34 enrollment events.
Behavioral Health Our Behavioral Health department flourished with the addition of new groups including: Weight Loss, Skills Building Workshop, Smoking Cessation and an additional Women’s Trauma group.
Pharmacy The dispensary at the Beverly Health Center was converted into a fully licensed pharmacy. This convenient one stop shop allows patients to receive their prescriptions during their visit. More than 30,000 prescriptions were dispensed.

New Technology SCC launched the Saban MyChart App, allowing patients to make appointments, see their lab results, send their provider a message and refill prescriptions all from their phone or computer.
Pediatrics A Director of Pediatrics was hired to head the expansion of pediatric services. Last year the Clinic provided 2,790 pediatric visits.
Serving Those Most in Need 7% of our patients are homeless and 73% live below the Federal Poverty Level. Over $6,000,000 in free services were provided to uninsured patients in 2017.
