2019 ANNUAL REPORT

OUR MISSION To serve as a medical home to the most vulnerable members of our community by providing affordable quality health care in a caring environment.

OUR VISION To be a recognized leader in improving the health and wellbeing of Los Angeles’ underserved communities by delivering quality medical and social services through innovation, creativity, partnerships, and public policy advocacy.

Some highlights from 2019 include:

• Our OB-GYN program relaunched in June and has welcomed four new infants into our pediatric care family

• We successfully piloted a telehealth psychiatry clinic

Saban Community Clinic has been on the front lines of community care since we opened our doors in 1967. We have weathered the ups and downs within an uncertain and ever-changing health care landscape, and continue to honor our founding principle that healthcare is a right, not a privilege.

Muriel Nouwezem

• A complete renovation of our Beverly Clinic and construction on the Virgil Family Health Center, opening in April

• We expanded our volunteer services to include Homeless Connect Kits and Pediatric Care Kits and assembled more than 5,000 kits

MESSAGE FROM THE CEO SABAN COMMUNITY CLINIC | 2019 ANNUAL REPORT | 1

This year we focused our efforts on updating our infrastructure, expanding our community engagement opportunities and continuing to implement cutting-edge technology to increase access to quality care.

We take pride in the successes highlighted in this report. We are grateful to our dedicated staff, Board Members, community partners, generous donors and the Friends Board for the role they all play in the continued success of the Clinic. Without your steadfast dedication to our mission and patients, none of this would be possible. Thank you for your continued support,

• We received the most HRSA Quality Improvement Awards in the Clinic’s history

Interim Chief Executive Officer

• 240 individuals were successfully routed from the ER at Cedars-Sinai into our care

Dear Friends, Colleagues and Partners,

The Clinic’s goal is to continue expanding to meet the needs of the community we serve.

COMMUNITY CLINIC | 2019 ANNUAL REPORT | 5 STAFF STORY

Shower to Care

“I can relate to it all,” says Jaime Gregory, Shower Program Supervisor. “I’ve experienced abuse, homelessness, crime, gang life, going in and out of jail…. I know why they are on the streets.”

Jaime spent his early childhood in Boston. At 7 years old, Jaime’s mom left his abusive father. They lived in cars and on people’s couches for a couple years until they moved to Watts. “My childhood was taken from me in that environment,” says Jaime. “My mom was always working, and my sisters and I raised ourselves. It was the height of the gang epidemic in that area, and I couldn’t escape it.” After years of gang involvement, and a decade of going in and out of jail, Jaime turned his life around. He dedicated himself to giving back to his community. Today, Jaime wears many hats. He calls himself a community leader, peace advocate and activist. He’s received extensive training in conflict resolution, managing trauma and grief and non-violent conflict response. He brings all this and more to his role at the Clinic. For the past two years Jaime has supervised the Clinic’s shower program, which provides more than 7,000 showers each year at two sites. His tough love approach motivates patients to take the steps to change their lives. “I tell them that this isn’t a retirement shower. We will always be here when you need us, but the situation you are in is temporary,” says Jaime. “I help them set short and long term goals. We need to be realistic, these big changes take time.”

The biggest point of entry into the Clinic for people experiencing homelessness is the shower program. Jaime and the case managers work to establish relationships in order to link patients to care. “Mental health issues are prevalent amongst this population,” says Jaime. “The linkages to the medical side of the Clinic are integral to their success.” The Clinic’s “warm handoffs” and “one-stop shop” model allow for patients to receive all of their care in one place. Last year, 40% of the Clinic’s shower participants were successfully linked to comprehensive medical, dental and behavioral health care.

MAVEN is a volunteer-based non-profit focused on connecting specialists from around the country with primary care clinicians serving low-income populations. Their unique platform allows clinicians to consult with specialty providers about their patient’s cases remotely, saving valuable time for both patients and staff. The Plunum Clinics (SCC is a member) proudly collaborate on this project.

GROWTH 6 | SABAN COMMUNITY CLINIC | 2019 ANNUAL REPORT

The Enrollment Department saw more than a 20% increase this year. Through increased outreach efforts and participation in health fairs, the Clinic enrolled more than 6,000 individuals into a health plan.

Pharmacy

The number of prescriptions filled almost doubled in 2019. New technology to help fill prescriptions and a partnership with West Coast University provided Clinic staff the time to work on refills and more importantly focus on medication adherence with patients. A new, free prescription delivery program also allowed patients to receive their medications at their front door through a secure delivery program.

The Clinic is excited to open our newest site this April in Rampart Village. Clinic staff and construction crews have been hard at work designing and building a site with top of the line amenities that will provide a safe and comfortable space for kids and their families to receive medical, dental and behavioral health care. The site will provide an additional 15,000 patient visits each year. Follow our progress at www.sabancommunityclinic.org/virgil

Paramount Assembly Day

8 | SABAN COMMUNITY CLINIC | 2019 ANNUAL REPORT

Bins filled with basic essentials lined the Paseo at Paramount Pictures where hundreds of employees put together Homeless Connect Kits.

COMMUNITY IN ACTION

Homeless Connect and Pediatric Care Kits have allowed hundreds of individuals, schools and local businesses to get involved in a hands-on way.

Last year, more than 5,000 kits were assembled by over 20 groups. The Pediatric kits provide children with basic essentials, including school supplies and a vaccine. The Connect kits provide individuals experiencing homelessness with basic essentials and a warm shower.

We extend a huge thank you to Paramount Pictures for sponsoring and hosting the largest Homeless Connect Kit assembly to date. As a part of the company-wide give back event, employees put together 1,000 kits on the lot. Interested in sponsoring an assembly day? Please contact Audrey Rufe at (323) 330-1653. “I didn’t know where to go or what to do,” says Daniella* .

“I found out I was pregnant when I was visiting my family and boyfriend in my native Italy, but when I came back, I was at a loss.” At her first visit, Daniella broke down upon answering a routine question. She told Glenda, a care coordinator, that she had been molested just a few weeks prior. Glenda immediately referred Daniella to the Clinic’s Behavioral Health department and she was seen that Throughday. the Clinic’s new Telehealth program, Daniella met with a licensed social worker via video chat and was subsequently referred to Dr. Michelle Kuranishi at the Hollywood site. “The first half of my pregnancy, I was so depressed. I’m 42 years old and the dreams I had about becoming pregnant were so far from my reality. I was previously in an abusive relationship and now I am in L.A. alone and pregnant — a lot of stuff was bubbling up. If I didn’t have the luck to find this place and Dr. Kuranishi, I’m not sure what I would have done,” says Daniella. Today, at 32 weeks pregnant, Daniella feels nervous but excited. She looks forward to her weekly therapy and OB-GYN appointments. “The Clinic has been a blessing. Being an immigrant doesn’t mean you are ignorant, it means you don’t fit in. The Clinic makes me feel safe. I feel welcome here.”

Our prenatal and pediatric program provides expectant and new mothers the complete care they need in one location. In the last few months, four prenatal patients had their babies and are continuing their care at the Clinic. Daniella is looking forward to doing the same once her little boy arrives.
