SCHEDULE Participants will commit to specific days each month in advance and will pay for that month in advance via bank draft or credit card on the 24th of the previous month.

HOURS OF OPERATION 7:30 am – 6:00 pm Monday – Friday. Participants should arrive no earlier than 7:30 am and must be picked up by 6:00 pm (see late pick up fees).

BILLING All participants in the Daytime Center by Runk & Pratt must set up automatic payments via credit card or ACH from a checking or savings account. Those choosing to use a credit card will have a 3% processing fee added to their total. An email address is required to receive your monthly invoices and statements.

The completed automatic payment form must accompany the application fee and enrollment forms in order for the participant to be accepted into the program.

The billing for the upcoming month’s schedule will be drafted on the 24th of the current month. (For example, charges for April scheduled days will be drafted on March 24th.) Participants who enroll late will have to pay their first month’s scheduled days via credit card, but subsequent months can be drafted from a bank account.

A returned payment fee of $30 will be charged for any account unable to be drafted successfully.

If the account is not paid in advance by the 1st of the month of service, the participant will not be allowed to attend until the account has been paid.

MISSED DAYS/VACATION/APPOINTMENTS • Snow days, as deemed by the Daytime Adult Day Center by Runk & Pratt, will be credited $75 per day on next month’s bill.

• If a participant misses a day for any reason, payment will still be expected to be made for those scheduled days, per the scheduled agreement.

• A 14-day notice must be given to the corporate office in advance of any doctor’s appointments or scheduled vacations. If such a notice is given, then a partial credit will be added to the account the missed days

ADDITIONAL DAYS

Additional days may be added to the schedule during the month if a spot is available on the requested date.

Payment of $120 for each additional day will be due at the time of scheduling to secure your spot. This can be paid via credit card (3% processing fee), cash or check.

LATE PICK UP FEES

The Center closes promptly at 6:00 pm. The fee is $10.00 per every 5 minutes (or portion of 5 minutes) until the participant has been picked up. These will be added to their account and drafted with the incidental and activity fee charges.

INCIDENTAL CHARGES AND ACTIVITY FEES: Our participants will have the opportunity to participate in day trips, activities, salon days and other exciting activities during their scheduled days. Participants can pay via credit card (plus 3%fee), check, or cash at the time of service or add them to their account. Any unpaid incidental charges will be drafted from the payment method on file on the 5th or 24th of the month.

WITHDRAWAL POLICY Participants who wish to withdraw from the program must give at least a 30-day, written notice to the corporate office. Participants will be expected to pay for the services during the 30-day notice. The 30-day notice will be waived in the event of extended hospitalization or death.

REFUND POLICY Refund will not be issued for days not attended or for those who do not submit a 30-day written notice to withdraw from the program. Refunds must be requested in writing to the corporate office and must be submitted within 4 weeks of withdrawal from the program or the refund is forfeited.

FORMS OF PAYMENT All payments for scheduled services must be paid via automatic draft from a checking, savings or credit card. Application fees, incidental fees and activity fees can be paid via cash, check, credit card at the time of service or automatic payment. Please note that if you choose to pay using a credit card that a 3% processing fee will be added to your total. We are private pay only but will be glad to provide invoices for you to submit for reimbursement to your insurance.
