This Folder Contains

Discharge Summary - This information is an overview of why you were in the hospital, which healthcare providers saw you, which procedures were done and any medications that were prescribed.

Medications - This is a listing of medications that you were given during your hospital stay, medications that you will continue after discharge and new medications that were prescribed.

Resources - This is information on who to call and where to go for follow-up care now or in the future.

Follow-Up Instructions - We will provide you with information on any tests and follow-up visits that you may need and when to schedule them. This portion will also contain phone numbers in case you have questions about your after-hospital care.

Medications

This packet includes information about medications that you received in the hospital and new medications that were prescribed. Please read the printed information about your prescriptions, and talk to your nurse or doctor if you have questions about your new prescriptions, supplements, or any medications that you may need to stop.

Gauze and wound care items may be purchased at retail pharmacies or at medical supply stores.

MEDS-TO-BEDS

Do you want to receive your medications at your bedside upon discharge? Do you want to have a personalized consultation with a pharmacist on your medication regimen?

If the answer is YES, talk to your healthcare provider about sending your prescriptions to the RUHS-MC-Retail Pharmacy as part of your discharge plan!

If you have Medicare and limited income and resources, you may qualify for extra help to pay for your prescription drug coverage.

Follow-Up Instructions

Recovery and Support

• Ask if you'll need any medical equipment (like a walker). Who will arrange for this?

• Ask if you're ready to do the activities below. If you need help with some or all of the activities, tell the staff:

◦ Bathing, dressing, using the bathroom and climbing stairs;

◦ Cooking, food shopping, house cleaning and paying bills;

◦ Getting to medical appointments and picking up prescription drugs.

• Make sure you have the support of family or friends who can help you.

• Ask the staff to show you and your caregiver any other tasks that require special skills (like changing a bandage or giving a shot)

• Ask to speak to a social worker if you're concerned about how you and your family are coping with your illness
