Who we are We work closely with our partners, extending your care plan into the home. Comfort Keepers™ of Northern Michigan is a privately owned company with over 250 employees, offices in both Traverse City and Petoskey, and deep roots in this region's health care and senior communities. Over our twenty years of serving the region’s seniors and their families, we have become champions of independent living for seniors, and expert solution finders. Too many seniors and their families make uninformed decisions about home care. Unaware of our many services that can solve what may seem unsolvable problems, they see seniors’ lives upended. We see solutions rather than accepting limitations. Partnerships with physicians, discharge planners, case managers, home health professionals, hospice teams, and community partners bridge this gap. A Message From Our Owners To Our Partners in Care: At Comfort Keepers of Northern Michigan, we believe the best care happens when everyone involved is working toward the same goal: helping each client remain safe, supported, and as independent as possible at home. We created this Partnership Guide for providers to make it easier to understand how we can support the patients and clients you serve. Our services range from everyday caregiver assistance to Life Care Coordination, safety solutions, and specialized, nurse supervised support for more complex needs. That support can include oxygen assistance, insulin injections, catheter care, wound care, feeding tube care, and other private duty needs, depending on the care plan. Trained Comfort Keepers caregivers provide hands-on support, with assessments and ongoing health monitoring by an RN on our staff. We work alongside physicians, discharge nurses, home health providers, therapists, hospice teams, and other professionals – complementing, not replacing, their clinical role. Our goal is simple: to be a trusted, top-of-mind resource whenever someone you serve needs additional support at home. We look forward to working alongside you as one care team.

What Happens After You Refer Your referral begins an ongoing process of assessment, planning, communication, and follow-through. Comfort Keepers works with your patient, family members, skilled home health, therapists, hospice, physicians, discharge teams, and other involved providers to help carry the plan into daily life at home.
Referral Received Referrals are received by phone, fax, email, website inquiry, or directly from a provider and routed to the appropriate Care Coordinator. Nursing or clinical concerns are identified early so an RN can be involved when needed.
Your Patient Is Contacted We contact your patient or their family the same business day whenever possible. Urgent referrals are prioritized, and next steps are explained clearly.
Care Consultation A Director or Care Coordinator conducts the initial consultation in the home, by phone, or virtually. An RN also participates when there are medication concerns, wounds, medical equipment, changing health conditions, or needs requiring nursing oversight.
The Care Plan Is Built The plan is developed around your patient’s daily routine, safety needs, family concerns, preferences, and goals. Clinical or delegated-care portions are reviewed by the RN, and the final plan is approved by the patient or responsible family member.
Identify Available Resources During the planning process, Comfort Keepers can also work with the client’s long-term care insurance provider to help initiate a claim for benefits and coordinate the documentation needed to move care forward. Coordination With the Care Team With appropriate authorization, Comfort Keepers can coordinate with skilled home health, PT, OT, physicians, hospice, and other providers involved in the patient’s care. Our role is to help reinforce the established plan between visits—for example, helping a patient follow a PT or OT home exercise routine that has already been prescribed or taught. Comfort Keepers caregivers can reinforce PT and OT home exercise plans between skilled visits by providing reminders, cueing, encouragement, and standby support.
Services and Caregivers Are Matched The team recommends the appropriate combination of Caregiver Support, Specialized Caregiver Support, Life Care Coordination, Health Care Coordination, Wellness Hub, or emergency response services. Caregivers are matched according to experience, training, availability, location, personality, and the patient’s specific needs and preferences.
Care Begins and Is Monitored Caregivers follow the established care plan and document each visit. Care Coordinators monitor the plan and family feedback, while the RN monitors delegated care, medication concerns, wounds, vital signs, or changing health conditions when applicable.
We Reassess, Communicate, and Adjust Every Comfort Keepers client is formally reassessed at least every six months and sooner after a change in condition, hospitalization, fall, new diagnosis, medication change, or other concern. Families are kept informed, and providers are contacted when changes require medical guidance, revised orders, or a higher level of care.

WellnessHub Supporting the Whole Person – Blended Care Technology is not a replacement for personal care. It is another way to strengthen it. Comfort360 supports a blended-care approach in which in-person caregiving and remote connection work together. A caregiver may help establish healthy routines during a visit, while the Wellness Hub helps reinforce those routines throughout the week. Family members can stay connected from a distance. Additional information about activity and patterns in the home can help the care team recognize changes and adjust support as needs evolve. Caregiver Support Skilled Nursing = Comfort Keepers When to make a referral Bring us in while prescribed Skilled Care is still active– especially when the client needs more support between visits or family cannot carry the plan alone. The skilled nursing plan becomes part of the client's day– not just the skilled visit.
How we support the plan... Our Comfort Keepers Coordinator sets up a Care Plan to support and build on the Recovery Plan to build on the Recovery Plan as defined by the skilled team. We then reinforce it during daily routines, monitor the client's progress between visits, and communicate any barriers or changes back to the skilled team so the plan can be adjusted as needed. • Monitor and promote carryover between PT, OT, nursing, and provider visits. • Relieve pressure on family members by supervising follow-through and reinforcing the plan. • Provide early visibility when safety, tolerance, function, or follow-through changes. • With client authorization, we coordinate with PT/OT, home health, physicians, hospice, and other providers.

Specialized Care Our Specialized Care services provide expanded Skilled Care to clients beyond caregiver support.
Medication & Diabetes Support Many clients need additional assistance managing complex medication routines and diabetesrelated care at home. We reinforce established care plans, provides appropriate support under RN oversight, and communicates changes or concerns to the appropriate members of the care team. Specialized Care Wound & Skin Support Our on-staff wound-care-certified registered nurse supervises wound-care routines and ongoing monitoring of skin integrity and changes. She is highly trained to assess, treat, and monitor complex or chronic wounds, including pressure injuries, surgical wounds, diabetic ulcers, skin tears, and other hard-to-heal conditions. She can develop wound-care plans, perform dressing changes and other prescribed treatments, monitor healing and signs of infection, educate patients and caregivers, and coordinate with physicians and other healthcare professionals when a wound requires additional medical attention.
Feeding & Nutrition Support Our specially trained team members provide assistance for clients who require additional support with feeding routines and nutrition-related and feeding tube care at home. We help promote consistency, safety, and comfort while following the established care plan.

Adaptable Care In-Facility Care, continued Our custom care plans are designed to work with a client’s existing care team and crafted to consider physical, mental, social, and emotional well-being. Comfort Keepers routinely collaborates with families and care teams at senior facilities, rehabilitation facilities, and many other healthcare facilities to implement this individualized plan of care. This teamoriented approach creates a more efficient process and better outcomes for the patient.
Post-Hospital Care An At-home Alternative to Rehab Comfort Keepers provides a range of in-home senior care and custom care plans, including support for hospital discharge plans and rehabilitation for your patient after a hospital stay, all in the comfort and privacy of home. Our team supports the discharge-to-home plan, including occupational/physical therapy exercises and dietary guidance. Trained in infectious disease control and senior care, our caregivers help those in recovery stay safely at home and reduce the risk of re-injury or hospital readmission by following discharge care plans. We can also provide respite care, offering a short period of relief for family members and other family caregivers. Adaptable Care End-of-Life Care For patients receiving hospice care at home, Comfort Keepers can provide an added layer of day-to-day support alongside the hospice team. Hospice physicians, nurses, and other clinicians provide the medical care associated with end-of-life needs, while Comfort Keepers caregivers can help with the personal care, household tasks and companionship that make it easier for a person to remain comfortable at home. Our specially trained end-of-life caregivers and Private Duty nurses support the care plan and family wishes. FAQ Is there a minimum level of care? No. Comfort Keepers assists as needed, even if that means a couple of hours a week.
Are services available 24 hours, 7 days a week? Yes. Services are available for as little as a few hours, or up to 24 hours, 7 days a week, 365 days a year.
What if a Comfort Keeper is sick or on vacation? We employ a team of caregivers to ensure there is no interruption in care if someone is sick or on vacation.
