Lawmakers weigh whether to legalize ‘medical aid in dying’

CHICAGO — Lawmakers are considering legalizing a controversial medical practice that proponents say could ease suffering for the terminally ill.

It’s sometimes called “assisted suicide,” although physicians and advocates for the practice prefer the term “medical aid in dying,” or MAID.

While Compassion & Choices — a group that advocates for medical aid in dying policies — found a majority of Illinois voters supported legalizing MAID in a 2023 poll, some critics call the process “barbaric.”

The measure, contained in Senate Bill 9, is being backed by Sen. Linda Holmes, D-Aurora, who told her Senate colleagues at a hearing Friday that she supports the proposal because of her parents’ deaths. Both her mother and father died after extended battles with cancer.

“You think the toughest thing you go through is watching somebody die, and you know what? It’s not,” Holmes said. “It’s not as tough as watching somebody you love suffer and there’s nothing you can do to ease that suffering. That is the

hardest thing I’ve ever gone through.”

Holmes’ proposal would legalize MAID — a process where a doctor prescribes but does not administer a lethal combination of drugs — for patients whose doctors determine have less than six months to live due to a terminal illness. The patient then administers the drugs on their own at a time of their choosing.

The bill contains several safeguards to prevent abuse, according to its proponents, including a waiting period to receive a prescription, a requirement that the patient receive a terminal diagnosis from two doctors, a requirement that patients prescribed lethal medication have sufficient “mental capacity.”

Friday’s meeting of the powerful Senate Executive Committee was a “subject matter” hearing, meaning no vote was taken. The bill will need more committee hearings, a vote in both legislative chambers and approval by the governor before becoming law.

Ten other states and Washington, D.C., have all legalized some form of medical aid in dying. Oregon was the first state to legalize MAID in 1994.

Advocates for the proposal include patients with terminal illnesses, people

whose loved ones used the procedure in other states and doctors who specialize in end-of-life care.

In 2022, Deb Robertson of Lombard was diagnosed with neuroendocrine carcinoma — a rare and aggressive form of liver cancer. She asked lawmakers to give her “permission” to take her own life.

“It would give me the option to die peacefully and on my own terms,” Robertson said. “There’s a level of comfort in that.”

Diana Barnard, a doctor in Vermont who offers MAID prescriptions, said most patients have “a very clear understanding” of what’s an acceptable quality of life as they approach death.

“We have now 27 years of national experience with the practice that really shows these laws are working well,” Barnard said.

But the medical practice is controversial among doctors and disability activists.

Benjamin German, a doctor on the West Side of Chicago, said the “problem” with the bill was its safeguards.

“For some of my patients, these safeguards will be just tight enough for lawmakers to assume things will be okay and amply generous to allow abuse to happen,” German said. “People and organizations looking for ways to exploit this law, I fear, will find a way.”

Disability advocates, meanwhile, say they worry about medical professionals mischaracterizing illnesses as terminal, misdiagnosing people or pushing vulnerable or marginalized people to consider ending their own life.

“As someone with a disability myself – I use a wheelchair – I can say firsthand that my life is often viewed as something to pity and not something to cherish,”

Riley Spreadbury, an independent living advocate from Joliet, said. “It’s sentiments like these that make me incredibly concerned regarding Senate Bill 9.”

MAID is also opposed by groups that express a “consistent life ethic,” meaning they object to abortion, capital punishment, assisted suicide, and euthanasia. Those groups include the Catholic Church and non-religious groups such as Illinois Right to Life.

Western leaders visit Kyiv and pledge military support against Russia on the war’s 3rd anniversary

KYIV, Ukraine (AP) — Ukraine on

Monday marked the bleakest anniversary yet of its war against the Russia invasion, with the country’s forces under severe pressure on the battlefield and the Trump administration apparently embracing the Kremlin in a reversal of U.S. policy.

The three-year milestone drew more than a dozen Western leaders to Kyiv for commemorative events in a conspicuous show of support. They warned of the war’s wider implications for global security and vowed to keep providing billions of dollars in support for Ukraine as uncertainty deepens over the U.S. commitment to help. Washington did not send any senior official to the occasion.

The fourth year of fighting could be pivotal as Trump uses his return to office to press for a peace deal.

World security is at stake in talks over how the war ends, European Commission President Ursula von der Leyen warned.

“The autocrats around the world are watching very carefully whether there’s any impunity if you violate international borders or invade your neighbor, or if there is true deterrence,” she said.

Canadian Prime Minister Justin Trudeau echoed that sentiment. Canadians, he said, “believe deeply that it’s not just about Ukraine. It’s about the rules and the values and the principles of sovereignty, of independence, territorial integrity that protects every country in the world. All of us rely on those rules to be able to build peace and security.”

Some observers say Russian success in Ukraine could embolden China’s ambitions. Just as Moscow claims that Ukraine is rightfully Russian territory, China claims the self-governing island of Taiwan as its own. North Korea and Iran have also aided Russia’s war effort.

In a cascade of unwelcome developments for Kyiv, Trump has in recent days called Ukrainian President Volodymyr Zelenskyy a dictator, suggested Ukraine is to blame for the war and ended Putin’s three-year diplomatic isolation by the United States. U.S. officials have also indicated to Ukraine that its hopes of joining NATO are unlikely to be realized and that it probably won’t get back the land that Russia’s army occupies, which amounts to nearly 20% of the country.

Meanwhile, Putin’s troops are making steady progress on the battlefield while Ukraine is grappling with shortages of troops and weapons.

The guests in Kyiv and the leaders appearing by video had similar messages: Ukraine and its European partners must be consulted in any peace negotiations, Putin’s ambitions must be thwarted, and Europe must take on more of the burden for its own defense.

Alarm bells sound in Europe as Washington changes course

The shift in Washington’s policy has set off alarm bells in Europe, where governments fear being sidelined by the U.S. in efforts to secure a peace deal. They are mulling how they might pick up the slack of any cut in U.S. aid for Ukraine. The changes have also placed strain on transatlantic relations.

European Council President Antonio Costa on Sunday announced that he would convene an emergency summit of the 27 EU leaders in Brussels on March 6, with Ukraine at the top of the agenda.

British Prime Minister Keir Starmer and French President Emmanuel Macron are both visiting Washington this week.

EU foreign ministers on Monday approved a new raft of sanctions against Russia. The measures target Russia’s so-called “shadow fleet” of ships that it uses to skirt restrictions on transporting oil and gas, or to carry stolen Ukrainian grain. The EU said 74 vessels were added to its shadow fleet list.

Asset freezes and travel bans were imposed on 83 officials and “entities” — usually government agencies, banks or companies.

Britain, too, imposed new sanctions, targeting 107 businesses and individuals in what it says is its biggest package targeting Russia’s war machine since the early days of the conflict in 2022.

The measures take aim at Russia’s military supply chains, including companies in several countries — notably China — that Britain says are supplying machine tools, electronics and dual-use goods for Russia’s military.

Spanish Prime Minister Pedro Sánchez said his country would provide a 1 billion-euro ($1.04 billion) military systems package to Ukraine this year.

Starmer said Ukrainians’ voices “must be at the heart of the drive for peace,” while Trump’s intervention had “changed the global conversation” and “created an opportunity.”

“Russia does not hold all the cards in this war,” he said.

Coming off a victory in Sunday’s German elections, conservative leader Friedrich Merz — also a staunch backer of Ukraine —

posted on X Monday: “More than ever, we must put Ukraine in a position of strength.”

“For a fair peace, the country that is under attack must be part of peace negotiations,” he wrote.

Diplomacy ramps up after record Russian drone attack

On Sunday, Russia launched its biggest single drone attack of the war, pounding Ukraine with 267 drones.

The EU’s top diplomat, Kaja Kallas, insisted that the U.S. cannot seal any peace deal to end the war without Ukraine or Europe being involved. She highlighted what she claimed were pro-Russian positions being taken up by the Trump administration.

“You can discuss whatever you want with Putin. But if it comes to Europe or Ukraine, then Ukraine and Europe also have to agree to this deal,” Kallas told reporters in Brussels, where she was leading a meeting of EU foreign ministers.

Kallas is scheduled to travel to Washington on Tuesday for talks with U.S. Secretary of State Marco Rubio.

U.N. Secretary-General António Guterres said the third anniversary was “a grim milestone.”

“More than 12,600 civilians killed, with many more injured. Entire communities reduced to rubble. Hospitals and schools destroyed,” he said in Geneva.

In a win for Ukraine, the United States on Monday failed to get the U.N. General Assembly to approve its resolution seeking to end the war without mentioning Russian aggression. The U.S. draft resolution was amended by the assembly to add language making clear that Russia invaded its smaller neighbor in violation of the U.N. Charter.

Washington and Moscow draw closer Russia’s foreign ministry said Saturday that preparations for a face-to-face meeting between Trump and Putin were underway, and U.S. officials have said that they agreed with Moscow to reestablish diplomatic ties and restart economic cooperation.

At a virtual meeting with leaders of the Group of Seven economies also held Monday, Zelenskyy said Ukraine and the U.S. are “working productively” on an economic agreement that would help lock them together. Trump attended the meeting.

“And, President Trump, we would really like to hear from you because all our people, all our families, are very worried. Will there be support from America? Will America be the leader of the free world?” Zelenskyy said.

Patients struggle with lack of consistent coverage for popular weight-loss drugs

Supplies of high-demand obesity treatments are improving, but that doesn’t mean it’s easier to get them.

Many employers and insurers are scaling back coverage of Wegovy and Zepbound and a key government program, Medicare, doesn’t cover the drugs for obesity. Meanwhile, some big employers are adding coverage, but their commitment isn’t guaranteed.

Treatment prices that can top hundreds of dollars monthly even after discounts make it hard for many people to afford these drugs on their own. That can make the life-changing weight loss that patients seek dependent on the coverage they have and how long it lasts.

Coverage complications are not unusual in the U.S. health care system. But the challenge is magnified for these obesity treatments because a wide swath of the population could be eligible to take them, and patients have to stay on the drugs to keep the weight off.

“There are a lot of people right now who want access to the medication and can’t get it,” said Katherine Hempstead, a Robert Wood Johnson Foundation senior policy adviser.

Coverage varies depending on who pays the bill

Paul Mack dropped about 70 pounds after he started taking Wegovy. The Redwood City, California, resident said food noise — constant thoughts of eating — faded, and he was able to have a heart procedure.

The treatment was covered by California’s Medicaid program, Medi-Cal. Then the 50-year-old security guard got a raise. He no longer qualified for Medi-Cal and lost coverage of the drug for several months starting last summer.

He regained two pants sizes.

“I couldn’t control the eating,” he said. “All the noise came back.”

Coverage of these drugs remains patchy more than a year after Zepbound entered the market to challenge Wegovy.

The benefits consultant Mercer says 44% of U.S. companies with 500 or more employees covered obesity drugs last year. It’s even more common with bigger employers.

More than a dozen government-funded Medicaid programs for people with low incomes also cover obesity treatments.

But few insurers cover the drugs on individual insurance marketplaces. And some plans restrict their coverage with things like requests for prior authoriza-

tion or pre-approval.

The lack of Medicare coverage remains a concern as well, especially for people who retire and move to the government-funded program from employer-sponsored coverage.

“Patients come to us terrified about switching to Medicare and losing coverage,” said Dr. Katherine Saunders, an obesity expert at Weill Cornell Medicine and cofounder of the obesity treatment company FlyteHealth. “We start talking about backup plans a year before they transition.”

Cost and uncertain payoff loom as concerns payer concerns Philadelphia-area insurer

Independence Blue Cross dropped coverage of the drugs solely for weight loss for some customers starting this year. Company officials say the insurer worried about premium hikes it would have to impose on all customers if it continued.

Cost also was a factor in decisions by West Virginia and North Carolina officials to end similar programs that provided coverage for state employees.

These concerns make Vanderbilt University researcher Stacie Dusetzina wonder how long employers who have added coverage will keep it, now that the drugs are no longer in short supply.

“That’s probably going to spike spending,” said Dusetzina, a health policy professor who studies drug costs.

Drugmakers tout the savings these drugs can provide by improving patient health and warding off future serious medical conditions like heart attacks or strokes.

But health care experts note that there are no guarantees that the employer or insurer who covers the drug will eventually reap those benefits because people may change jobs or insurers.

Will coverage ever become consistent?

There’s no clear path toward widespread coverage of these drugs for obesity, even as polls show Americans favor having Medicaid and Medicare cover the costs.

Leaders at Zepbound maker Eli Lilly have seen coverage grow steadily for their drug, and they’re optimistic that will continue.

Former President Joe Biden’s administration proposed a rule that would allow for Medicare and broader Medicaid coverage. Its fate remains uncertain in President Donald Trump’s administration.

A bill calling for Medicare coverage has

been floating around Congress for years. But it isn’t scheduled for a vote.

Drugmakers are currently testing several additional obesity treatments. Such potential competition could reduce prices and prompt more coverage.

Patchy coverage complicates treatment plans

Dr. Amy Rothberg says the lack of consistent coverage leaves her conflicted about writing prescriptions because she’s not sure how long patients will be able to take the drug.

“We know from the studies that people go off these medications, they regain their weight,” said Rothberg, director of the

University of Michigan’s weight-management program. “I don’t want to do harm.”

Some insurers require diet and exercise changes for the patient before they will cover a weight-loss medication. Those changes should happen in conjunction with starting the medicine, said Dr. Lydia Alexander, president of the Obesity Medicine Association.

She’s also seen requirements for a body mass index of 40 or more, which equates to severe obesity, before coverage can start.

“We’re saying that obesity is a disease, but were not treating it like a disease,” she said.

On a small scale, cider apples could be an option in the backyard though they have unique challenges.

ored, and very astringent, were grown specifically to be pressed and made into hard cider. By the late 1800s, hard cider was on the decline due to a number of factors. But there is growing interest in revisiting and growing these trees on a commercial scale at cideries or in your own backyard. In fact, CiderCon, hosted by the American Cider Association, is a yearly convention where you can learn to grow cider trees, make hard cider, and develop tasting skills with growers, cidermakers, and industry professionals.

The apples we grow now, like ‘Honeycrisp’, ‘Fuji’, ‘Gala’ and ‘Granny Smith’, are considered dessert apples with a sugar and acid content that are more ideal for eating fresh or baking

than bitter/astringent cider apples. You can make sweet/hard cider with dessert apples but it may be missing a certain mouthfeel characteristic that is imparted by cider apples.

Cider apples are classified in 4 broad categories: bittersweets, bittersharps, sharps, and sweets. Each category is based on a percent tannin and malic acid of the juice quality when these apples are pressed. Most of the apples we grow are in the sharps and sweets categories. ‘Golden Delicious’ and ‘Gala’ are examples of sweets with ‘Granny Smith’ and ‘McIntosh’ as sharps. Many orchards are combining different types of sweets and sharps to make their cider. It’s very rare for orchards in our area to have

In the 1930s Bob’s family bought a lake lot in Wisconsin and had local farmers build a small cottage. The farmer needed to sell 10 lots because a drought ruined his corn crop and he need money to pay off a new car loan. Two other couples, including Bob’s mother’s brother, felt if the men lost their jobs their families could survive the depression with fish, gardens and fruit available to them from their land. People at that time had a difficult time affording homes in the city.

There was no water wells available on the lots and they hauled water from a nearby well with coaster wagons and milk cans. One neighbor tired of fetching water and installed a sand pipe well. After a few years Bob’s parents decided to have a similar well established and soon after that the neighbor’s well went dry. The experience was an early introduction to water competition.

Years later we had two brothers in our classes who were raised on a farm in the Dakotas who wanted to continue operating the family farm. However the family well went dry as large scale irrigation systems were drawing down water levels forcing other farm owners to drill replacement wells to secure water. One is left to wonder what the impact of

fracking for natural gas is on existing well water supplies.

We were both raised in Milwaukee and municipal water drawn from Lake Michigan was considered safe for human consumption. Years later citizens experienced illness when the water became contaminated. The contamination issue was resolved when necessary corrective action was taken but what was the financial impact on the water bills of citizens.

When we moved to Illinois to begin our teaching careers we were frequent visitors to the museums and aquarium in Chicago. Our undergrad work including courses in ecology and we were determined to understand the ecology of the Chicago Region. An article in the Chicago Tribune raised alarm over the deteriorating ecological conditions in Lake Michigan including the Great Lakes. Around that time Bob met and spoke with Dr. Stanley Cain, who headed the Department of Interior in Washington, D.C.. They discussed the potential of developing a public educational program focused on the Great Lakes. At that same meeting, during a press conference, questions were being asked of government officials as to the ecological impacts of large scale herbicide spraying occurring in Vietnam during the war. After briefly commenting on the situation the government official told the reporters they were

not to use his comments in articles. If they did they would never be allowed to attend any future press briefings. Control of information remains a contentious issue in our democracy and around the globe.

During our doctoral studies at the University of Michigan we worked with professors developing articles on water quality issues. We had limited success as our supervisor of the projects newsletter had to approve of them. Sonia continued to take water quality classes, worked on the Sea Grant Project and developed a curriculum on the Great Lakes, including a component on the ecological conditions in them in contrast to smaller bodies of water, available for student studies on the water ecology.

The competition over water supplies remains in the headlines regarding California water supplies. As climate change increases the demand for water and deteriorating ground water supplies expands competition for water intensifies. Michael Harris published an article in the Tyee on Trump’s Obsession with Canada’s Water in which he provides an overview of how Canadian water could be redirected to supply water to Southern California if political support within Canada can be found.

A similar lust for water from the Great Lakes has been expressed over the years.

The main challenge with cider apples is that they are difficult to grow. Some of the common issues includes producing small crops every other year, heavy disease pressure like fireblight and scab, rotting on the tree, uneven ripening, and delayed blooming. Since the main goal is juice quality for your cider, having small fruit isn’t an issue. The other issues listed here can be and be difficult for you to overcome. Next week, I’ll continue my discussion on varieties and some solutions for some of these challenges.

It is asking citizens to communicate with their congressional leaders telling them to reauthorize The Great Lakes Restoration Initiative and appropriate the needed funds to support the effort. The Initiative is a by- partisan program that creates a coordinate system. between federal, state and local partners to protect the Great Lakes. There are multiple environmental issues addressed regarding protecting the Lakes addressed on their website.

After completing our doctorates we returned to Illinois and, along with others, formed the nonprofit the Friends of the Rock River to encourage citizen efforts to protect the Rock River. Later on we encouraged Frank Schier former owner of the Rock River Times to pursue his dream of creating the Rock River Trail Initiative to encourage people to enjoy the river and develop a greater appreciation of its scenic and ecological benefits. The effort continues under the leadership of co-coordinators: Greg Farnham and Therese Oldenburg.
