About five years ago, as Mesa’s health-care costs continued a doubledigit climb, city administrators began brainstorming more effective ways to protect the health of its workers — and its budget.

An employee committee did the same.

“Out-of-pocket costs were going up not only for the city but also for employees, so they were feeling the same pressure,” Mesa City Manager Chris Brady said. “It was interesting, because we were both working on this separately but kind of came to the same conclusion of, ‘There’s got to be more that we can do to provide more direct services to our employees.’ ” The city after further analysis was surprised to find that, of the more than $40 million it had been paying annually for employee health care, routine or preventive care was not gobbling up the largest chunk. The majority of the cash was going to “back-end” or critical care, such as overnight hospital stays after a heart attack or specialized treatment following the late discovery of a disease.

“Everybody felt as though, if we had some sort of health center upfront, we could detect these kinds of conditions early on,” said Gary Manning, Mesa’s director of human resources. “If it could avoid even one long-term hospitalization that would cost us tens of thousands of dollars, there was a great deal of savings that you could have there.”

The city made the concept a reality in October, when it opened a 4,100-squarefoot wellness center for its roughly 4,000 employees and their dependents. Monday through Friday, the center at 1121 S. Gilbert Road functions as a primarycare clinic and urgent care for adults and children.

The center has six exam rooms, an onsite lab and a pharmacy that stocks commonly prescribed antibiotics and other drugs. Though patients might end up paying for medications, there is no copay or fee for an appointment as long as they have a Mesa insurance plan.

Mesa is not the first Valley city to try its hand at providing health care directly.

smaller-scale clinic for workers at a fire station. A typical doctor’s visit costs the city about $140; an employee visit to the clinic costs Goodyear about $60.

Peoria considered opening a center in 2012 before scrapping the proposal.

Chandler has explored the idea, as has Glendale, according to city spokespeople.

Phoenix, Buckeye and Avondale said they didn’t have anything comparable in

the works.

Mesa’s center has treated about 185 people a week since opening — a striking response given that use of the clinic is optional. Some employees hadn’t seen a doctor in five to 10 years, according to Dr. Cheryl Wathier, the center’s medical director.

“The fact that we’re even getting them in the door is wonderful, because we can get that first set of vitals,” Wathier said. “We’ve found several new diabetic patients, patients with hypertension, a lot with high cholesterol. We’ve had thyroid conditions that had not been discovered. All of those things are now under control with medication.”

The city estimates such early catches could save Mesa up to 5 percent of the $54 million it spends on employee health care annually, or $2.7 million a year. Offi-

cials said that estimate was conservative.

The savings for employees, too, could be substantial.

An emphasis on 30-minute appointments — versus hurried visits limited to a single complaint — allow the center’s medical staff to address several issues at once. At another office, the patient might have to return multiple times, handing over another copay for each visit. The same applies to patients with chronic diseases such as diabetes, who typically need to go in several times a year for tests.

Employees with more straightforward ailments have appreciated the extra time and attention to detail as well.

Mesa will proceed with a contentious move to forgive tens of millions of dollars in loans to Phoenix-Mesa Gateway Airport, despite the reservations of two City Council members.

The council voted 5-2 on Jan. 12 to recharacterize about $75 million in cumulative loans and interest as contributions, after airport and city staff said a failure to do so would obliterate any chance Gateway had to thrive on its own financially. Councilmen Dave Richins and Alex Finter voted against the measure.

“There were definitely questions about my medical history, about past history with other family members,” employee Stacie Peyton said of her first visit to the center. She said she’d returned from a trip with a sinus infection, called the clinic in the morning, and was seen within the hour.

“Before I checked out, I had both prescriptions in my hand and was able to get in my car, drive back to work, and begin my medication,” she said. “I look at (the center) as being my first choice option now.”

Not every experience has been as smooth as Peyton’s. As with any experiment, there have been growing pains.

Employees fretted about privacy in the months leading up to the center’s opening. Workers didn’t want their employer snooping in their medical records; could the city guarantee personal health information would stay private?

from pursuing the financing mechanisms and credit that eventually would help it stand on its own, officials said.

“We’ve gone to extremes to make sure that all of this information stays with their doctor,” Deputy City Manager John Pombier said. “We have aggregate data that allows us to address trends, but on an individual basis, we don’t see anything; we don’t ask anything. It is clearly separate.”

Then, the city found the push for comprehensive visits was leading to lengthy wait times for walk-in patients. The city said it is emphasizing calling for appointments as it works out scheduling kinks.

There’s also the fact that the city’s much-touted savings are likely a couple of years down the road. Mesa had to fork over about $1.5 million to ready the former pediatric center for use, and it will pay about $1.2 million in annual operational costs going forward.

Still, “even if the center just breaks even and pays for itself, I think we’re doing wonders,” Brady said.

“If there are some challenges in your health, well, that’s going to affect your work,” he said. “If employees can improve their health, then I think we get happier, healthier, more productive employees, the value of which is hard to measure.”

nance & Enterprise subcommittee for further study. That day, Councilman Dennis Kavanaugh had worried aloud that wiping Gateway’s slate clean would be a betrayal of the public’s trust.

“I get the concept of where they’re going and applaud the airport for the success they’ve had over the last 10 years,” Richins said at the Jan. 12 council meeting. “I just don’t quite feel comfortable with the way that we’re recharacterizing this at this time.”

Finter said he’d spent “hours and hours ... researching and talking with staff” and had “seen a few red flags” that gave him pause, though he didn’t elaborate.

Both councilmen said they wished they’d been provided additional alternatives. City Manager Chris Brady had said in December that if any feasible alternative existed, it would’ve already

been identified and provided.

Mesa was the last of Gateway’s six member agencies to approve the measure, after Gilbert, Queen Creek, Phoenix, Apache Junction and the Gila River Indian Community. No other airport-authority member has contributed even a third as much as the city, and the figures in question don’t include the tens of millions Mesa has funneled into infrastructure improvements in the Gateway area.

Yet, as council members and key city staffers have pointed out, Mesa likely has the most to gain.

“Today, the airport is responsible for over $1.2 billion in economic activity and ... 10,000 jobs,” said outgoing Councilman Scott Somers, who represented the Gateway area for more than eight years. “It is one of the most important pieces of in-

frastructure that is going to be developed by this city and by this region over the coming decades. To saddle it with debt would only mean that it does not have the opportunity to grow and to flourish.”

Despite his earlier doubts, Kavanaugh said keeping the airport “in a very competitive position as the economy keeps improving and opportunities keep coming in the way of the airport ... is the wisest course of action to take.”

Mayor John Giles marveled at the airport’s growth since his first council term in the 1990s, when “it took a whole lot of faith to imagine, to see (Gateway’s) potential.”

“It literally brought a tear to my eye when I went out there a few months ago, to see the progress that is there and what

a jewel it is for our city now,” he said. “In my mind, what’s best for the city of Mesa is for the Gateway Airport to thrive and to be strong.”

Councilman David Luna also voted for the proposal, describing Gateway as “an economic engine ... we need to support as a community,” as did Vice Mayor Chris Glover.

Finter and Richins never disagreed with their colleagues about Gateway’s value. Their primary concern seemed to revolve around future financial commitments to the airport, given the hefty investments Mesa already has made. “(Mesa) is a big city with lots of challenges, lots of needs all over,” Finter said at a Jan. 6 subcommittee meeting.

“These are all good things that are happening at Gateway, but it would be nice to know what the expectation is and have a really good public debate about how much more we’re going to put in to get what effect.

“When do we say, ‘Enough’s enough.

You’ve got to fly on your own’?” he said.

“I think those questions ought to be asked.”

Mesa Historical Museum hosts annual Home Tour JAY MARK SPECIAL FOR THE REPUBLIC

Given Mesaâ&#x20AC;&#x2122;s remarkable growth over the past half-century, it should come as no surprise that most of the more than 150,000 homes in the cityâ&#x20AC;&#x2122;s 133 square miles were constructed in the past 50 years.

In and around Mesaâ&#x20AC;&#x2122;s original square mile are seven neighborhoods and individual properties that represent an historic and architectural timeline of the cityâ&#x20AC;&#x2122;s transformation from village to urban powerhouse.

Generally these private properties are inaccessible to the public.

But one day each year, for the past 15, the Mesa Historical Museum has hosted as many as a thousand visitors who have been treated to open-house tours of

The houses cover the gamut of historic styles: late Victorian, Bungalow and Mid-Century Modern Ranch. Additionally, six â&#x20AC;&#x153;Special Stopsâ&#x20AC;? are included as a bonus. Three are homes that have been adapted for commercial use.

Within Mesaâ&#x20AC;&#x2122;s city center, farmland has long given way to housing, with one notable exception, the Fitch Farm, which has tenaciously hung on to its agrarian roots.

The Fitch farmhouse, constructed in 1934 by Larkin and Mildred Dobson Fitch, was once the centerpiece of a 650-acre farm running from Center Street to Stapley Drive. Although much smaller today, the farm still grows corn, watermelon and other crops that are sold fresh from the field directly to consumers.

The tour of the property includes the historic farmhouse and a recently added â&#x20AC;&#x153;carriage houseâ&#x20AC;? in which is housed a horse-drawn buggy that once belonged to the famous Mellon banking

A tour highlight is the diminutive Queen Anne style Joel E. Sirrine House, built in 1896. The three-room brick home was saved from demolition in 1980.

Six years later, the carefully restored relic from Mesaâ&#x20AC;&#x2122;s first two decades debuted as a house museum. Since 2008, because of budget cuts, the home has only been opened once a year for the Home Tour.

First time on the tour is the 1920s Rosalea Court, one of Mesaâ&#x20AC;&#x2122;s earliest â&#x20AC;&#x153;planned developments.â&#x20AC;? Its intimate design features six 800-square-foot modest bungalows built on two sides of a common courtyard.

The aging property was sensitively brought to life in 1997 by local resident Lori Osiecki, who at the time was concerned about the potential loss of the unique property.

Tickets for the Jan. 24 self-guided tour are $20 and help fund the non-profit museumâ&#x20AC;&#x2122;s programs. They can be purchased online at http://www.mesa museum.org/plan/events/hometour. Two Sossaman Middle School students earned perfect scores on the ACT Explore exam given this fall. Layla Headley, 13, and Caleb Gooch, 13, topped their peers locally and nationally.

“We are very excited to have two perfect ACT Explore scores again this school year, Caleb Gooch and Layla Headley,” said Assistant Principal Renee Kory. “Not only are Caleb and Layla outstanding students, they are humble leaders and model citizens. A perfect Explore score is rare, and we are extremely proud of Caleb and Layla. They work hard, diligently striving for excellence, and they definitely deserve this honor.”

Caleb and Layla are both taking advantage of accelerated classes at Sossaman. Both are enrolled in physics and accelerated language arts, along with taking a second-level foreign language class and their other courses.

The Explore is one of two “pre-ACT” tests given to students. Explore covers math, English, science and reading. It is given to all Higley District eighth-graders. PLAN, the other pre-ACT exam, is given in 10th grade. Results of the “pretests” are used to help students prepare for the ACT, which colleges and universities use in admission and scholarship decisions.

The Explore test also provides information to students about what classes they need to better align their academic plans with their career goals.

Higley Education Expo on Jan. 24 On Jan. 24, all Higley schools will be featured at San Tan Village in the courtyard area in front of Harkins Theaters during the Higley Education Expo. The

event runs from 10 a.m. to 1 p.m. with demonstrations from schools, including robotics, television production, musicians, ROTC, engineering, art displays and more. This will be a great opportunity to talk to an engineering student, watch the Rubik’s Cube team in action and visit with principals and teachers.

Higley families and the Gilbert community are invited to discover all the district has to offer. Parents of preschoolage children may also pick up registration packets for next school year.

Preschool details, registration The Higley Unified School District is offering quality early learning programs at two state-of-the-art Early Childhood Development Centers on the campuses of Cooley and Sossaman Middle Schools. Both centers provide three early childhood programs to fit the interests and schedules of all families. Brilliant Beginnings is available for children ages 3-4. Kindergarten Prep is available for children ages 4-5. STARS is offered for children with special needs. Classes are taught by certified early childhood educators with small student-to-teacher ratios.

Open houses and information nights for the 2015-16 school year will be held in January. Dates and times are: 6 to 7 p.m.

Question: My dad lives in a skilled nursing facility, and he has spent nearly all of his savings on his care.

Three months ago, we applied for the Arizona Long Term Care System, believing that he satisfied the eligibility requirements. We reviewed his bank statements, which revealed that he had $700 in his account and $1,900 of monthly income depositing each month. To our shock, however, his application was denied because he has too much income. How can this be?

Answer: What you are describing sounds like a situation in which your father has more gross income than what you are aware of.

ALTCS has a hard and fast income requirement, under which applicants with more than $2,163 of gross monthly income cannot qualify for the benefit unless they route some or all of their income through an income-only trust.

Assuming your father collects Social

Security, there is a fairly good chance that his Medicare premium of $104.90 is automatically deducted from his award.

The amounts withheld for Medicare, taxes, and insurance premiums are all factored into gross income. Given that your fatherâ&#x20AC;&#x2122;s net income is so close to the limit, there is a very real possibility that his gross income exceeds the threshold.

Just because your father is presently income ineligible does not mean that he is without options. There is a solution for these types of scenarios, referred to as an income-only trust.

By establishing an income-only trust and routing his income through an account titled to this trust, it is quite possible for your father to satisfy the income requirement and go on to receive benefits.

Gilbert is hoping to outfit all of its patrol officers with body cameras this spring after a pilot program last fall was well-received by the department.

Gilbert Police Chief Tim Dorn, in a recent blog post, wrote that, “It is essential that we move forward with the program to not only hold ourselves accountable for our actions and maintain transparency, but also to protect our officers and our department.”

The Gilbert Council was slated to vote on purchasing the additional cameras from Scottsdale-based Taser International last December, but “software issues” officers are having with the initial

batch of 32 cameras caused the vote to be delayed. The $231,000 purchase would have covered an additional 139 AXON Body cameras and 21 docking stations where officers can upload videos of incidents they encounter. Once the software issues are solved, Gilbert police hope to move forward with the purchase.

“While we are still working to resolve some issues (such as slow download times), it is my desire to complete the roll-out to officers in patrol functions during calendar year 2015,” Dorn wrote in the Dec. 29 blog post. “The benefits of the body worn cameras far outweigh the administrative burden.”

As of Jan. 7, Gilbert police spokesman Jesse Sanger said police are “still working out a few small kinks” and hope to present the purchase to council in March.

Sanger added that despite the issues, the reception from officers has been mostly positive.

“We move forward with the program to not only hold ourselves accountable for our actions and maintain transparency, but also to protect our officers.”

Tensions between police and communities nationwide have fueled the debate over whether officers should wear body cameras to record their actions. In December, President Obama proposed spending $263 million to purchase 50,000 body cameras for police departments nationwide. In addition to Gilbert, other police departments, including Mesa, Peoria and Flagstaff, have purchased body cameras from Taser.

Dorn cautioned that the body cameras are not an “all-encompassing solution to maintain trust and transparency.”

He stressed that officers “must be prepared to explain what is seen on the videos and must clearly explain and justify their actions in their departmental reports.”

Like his father before him, Hanson got into the construction industry in hometown of Des Moines. After serving in the Army, where he finished up in the 77th Armored Division and received an honorable discharge, he found himself

working as an assistant manager in a payroll department.

While it wasnâ&#x20AC;&#x2122;t a terrible fit â&#x20AC;&#x201C; after all, â&#x20AC;&#x153;when the other kids wanted to be firefighter, I wanted to be a CPA,â&#x20AC;? he quipped â&#x20AC;&#x201C; he knew he couldnâ&#x20AC;&#x2122;t sit behind a desk for a couple of decades.

So he got into real estate in 1973, and three years later, in 1976, he managed to sell 54 properties with an average sales price of $33,000. â&#x20AC;&#x153;I made the Million Dollar Club for the first time,â&#x20AC;? he recalled, adding that

he sometimes wonders how he sold that many by himself. â&#x20AC;&#x153;I sold over 50 homes recently, but Iâ&#x20AC;&#x2122;ve had help. And then my wife points out that we didnâ&#x20AC;&#x2122;t have all the paperwork then that we do today.â&#x20AC;?

While Hanson said the industry has had its rough moments, such as the recent market crash that sent many real estate agents fleeing, heâ&#x20AC;&#x2122;s been able to weather the storm thanks to good clients and colleagues. â&#x20AC;&#x153;I was fortunate enough to be able to rely on people Iâ&#x20AC;&#x2122;ve worked with in the past, who felt my service was exemplary, and they referred people to me,â&#x20AC;? said Hanson, whoâ&#x20AC;&#x2122;s planning to finish this year his memoir-like book of growing up in Des Moinesâ&#x20AC;&#x2122; Little Italy.

Joan Zimmerman, an associate broker with HomeSmart in Phoenix, achieved Realtor Emeritus Status last year. The octogenarian, who said she felt â&#x20AC;&#x153;blessedâ&#x20AC;? to get the award and has the designation printed on her business card, got into real estate in the early 1970s after a friend told her, â&#x20AC;&#x153;I hear ladies are getting into real estate.â&#x20AC;? â&#x20AC;&#x153;I said, â&#x20AC;&#x2DC;Well, that sounds like something Iâ&#x20AC;&#x2122;d like to do,â&#x20AC;&#x2122; â&#x20AC;? said Zimmerman, adding that she thrives on being with, and helping people, especially since buying a house is often the biggest purchase a person will make. â&#x20AC;&#x153;People appreciate it when you give them a leg up, you help them with something,â&#x20AC;? she said. â&#x20AC;&#x153;There are so many opportunities to help other people, and if youâ&#x20AC;&#x2122;re an older person, you perhaps have more information and more knowledge.â&#x20AC;?

Pancreatic cancer often seen too late

What makes a vehicle â&#x20AC;&#x2DC;historicâ&#x20AC;&#x2122;?

Question: Why is pancreatic cancer often diagnosed in late stages?

Answer: The pancreas is an organ located just below the stomach and its function is to release hormones like insulin to regulate blood glucose level and digestive enzymes.

Even if cancer is developing in the pancreas, the organ can typically continue functioning before symptoms of the cancer appear.

Unfortunately, these symptoms can be very vague and may be associated with other conditions. The symptoms include unexplained or unintentional weight loss, a lack of appetite, a yellowing of the skin and whites of the eyes, and upper abdominal pain.

Pancreatic cancer is usually very advanced by the time a person notices any significant health changes. Because there are no effective screening tools for pancreatic cancer (like the colonoscopy for colon cancer and

the mammogram for breast cancer), most patients are still diagnosed too late.

Pancreatic cancer is extremely aggressive, and the rapid growth of a cancerous pancreatic tumor can make surgical removal difficult or even impossible.

In 50 percent of all diagnosed cases of pancreatic cancer, the cancer has spread beyond the pancreas.

Though pancreatic cancer is rarely diagnosed early, several risk factors exist that can be monitored. Having family members with certain genetic syndromes or who have been diagnosed with pancreatic cancer can increase a personâ&#x20AC;&#x2122;s risk.

Individuals who are overweight or use tobacco have higher risk, as do people who suffer from chronic pancreatitis or new-onset diabetes.

In 2014, a 25-yearold vehicle would be a 1989 model. So, any 1989 model or older qualifies,â&#x20AC;? he said.

Take a pass on drive-train warranty Question: Some of the dealerships where I live are offering â&#x20AC;&#x153;lifetimeâ&#x20AC;? warranties on new car purchases. This is above and beyond the manufacturers warranty. The catch is that you must have all your service work done at the dealership where you purchased your new vehicle. This sounds to me to be in violation of the Magnuson-Moss law.

What is your opinion?

Answer: There are three kinds of warranties. The most common one covers what the original warranty covered, everything except wear items like tires, brakes, hoses, belts and fluids and filters.

The second type only covers â&#x20AC;&#x153;thisâ&#x20AC;? list of parts, and it is called an inclusive warranty or â&#x20AC;&#x153; only these parts.â&#x20AC;?

The third covers everything except this list of parts, which makes it an exclusive warranty, otherwise â&#x20AC;&#x153; everything but these parts.â&#x20AC;?

If the warranty you refer to is a drivetrain warranty, then I would pass. If you maintain your new car as the manual dictates, you will get 95K to 150K miles on the transmission or transaxle and get 175K to 205K on the engine. Thatâ&#x20AC;&#x2122;s assuming you never overheat the engine or run low on oil.

You describe an agreement that appears to be saying, you let me do all your service and then and only then, I will warranty something in or on your car.

Magnuson-Moss says if you insist your customer only use your vacuum bag in their new vacuum, you must provide the vacuum bag for free. Another way to say the same thing is this, if a carmaker says you must use our oil filter, they must provide it for free.
