HEALTHY LIVING

WHO WE ARE The Riverside Community Health Foundation (RCHF) is the lead agency of the Riverside Community Diabetes Collaborative (RCDC). RCDC is a groups of over 40 diabetes focused organizations and business that work together to strengthen community programs and services for the prevention and management of diabetes in Riverside and surrounding areas.

RCDC is your local resources for diabetes and chronic disease prevention information, events and more.

DIABETES TIPS Barbeques, picnics and trips to the beach may be on your calendar this summer. But how can you enjoy all the fun while staying within your healthy eating goals? You can still enjoy all aspects of summer gatherings by eating a bit less of each item, and including other healthy lifestyle habits such as exercising, and eating in moderation, taking medication, and monitoring blood sugar levels. If you need help setting or sticking with your goals, register and attend a class.

ROAD TO HEALTH Road to Health is a product of the National Diabetes Education Program (NDEP). Designed for African Americans and Hispanics/Latinos at risk for type 2 diabetes. The class is designed to reinforce the message that type 2 diabetes can be delayed or prevented. This program offers lifestyle change education that focuses on healthy choices and increased physical activity.

Together We Can! DIABETES WELLNESS PROGRAM Pg. 5

What is the Together We Can! Diabetes Wellness Program?

The “Together We Can!” Diabetes Wellness Program is designed for people with diabetes and caregivers of people with diabetes to learn more how to manage the condition and lead a healthy, happy life! The program is free. Registration is required for these classes and seminars. Please RSVP by calling 951-788-3471.

CONVERSATION MAP CLASSES The US Diabetes Conversation Map ® education program is an interactive learning tool that helps patients better manage their diabetes. It was created by Healthy Interactions, Inc., with the American Diabetes Association. It covers the following topics: “Overview of Diabetes”; “Healthy Eating”; “Monitoring Your Blood Glucose and Using the Results”; and “Continuing Your Journey”. These 2-hour classes are interactive, small group sessions taught by a licensed healthcare professional.

DIABETES COOKING CLASSES Our diabetes cooking classed are led by a registered dietitian, these cooking classes focus on how to cook healthy meals for someone with diabetes. Topics include nutrition label reading, healthy substitutions, and serving portions.

Learn how to manage your chronic condition (diabetes, heart disease, depression, etc.) or that of a loved one in this highly interactive, 6-week evidence-based, Stanford University curriculum course. Obtain strategies to communicate better with your healthcare provider, set goals, eat healthy, be physically active and maintain your independence.

QUIENES SOMOS La Fundación Comunitaria de Salud de Riverside (RCHF) es la agencia líder de Riverside Community Diabetes Collaborative (RCDC). RCDC es un grupo de más de cuarenta organizaciones y negocios enfocados en la diabetes que trabajan juntos para fortalecer programas comunitarios y servicios para la prevención y el manejo de la diabetes en Riverside y sus alrededores. RCDC es su recurso local para diabetes y prevención de enfermedades crónicas, información, eventos y más.

CONSEJOS DE DIABETES Asados, picnics y paseos a la playa pudieran estar en su calendario este verano. Pero ¿como puede disfrutar de toda la diversión mientras se mantiene dentro de sus metas de comer saludable? Usted aun puede disfrutar todos los aspectos de las reuniones del verano al comer menos de cada platillo e incluir otros hábitos de estilo de vida saludable, tales como ejercitarse y comer con moderación, tomar sus medicamentos y monitorear sus niveles de azúcar en la sangre. Si necesita ayuda en forjarse metas o mantearlas, regístrese y asista a las clases.

¿SABÍA USTED QUE LA DIABETES TIPO DOS PUEDE PREVENIRSE? DIABETES 101 Diabetes 101 es una presentación de 60 minutos diseñada para proveer conocimiento básico en la prevención y el manejo de la diabetes. La presentación cubre lo siguiente: estadísticas de diabetes, que es la diabetes, los tipos de diabetes, factores de riesgo de la diabetes, señales y síntomas de la diabetes, complicaciones de la diabetes, prevención de la diabetes, manejo de la diabetes, y recursos sobre diabetes.

CAMINO HACIA LA SALUD

CAMINO HACIA LA SALUD Camino Hacia la Salud es un producto del Programa Nacional de Educación de la Diabetes (NDEP). Diseñado para Afroamericanos e Hispanos/Latinos en riesgo a la diabetes tipo dos. La clase está diseñada a reforzar el mensaje de que la diabetes tipo dos puede atrasarse o prevenirse. Este programa ofrece educación de cambio de estilo de vida que se enfoca en opciones saludables e incrementa la actividad física.

¿Que es el Programa de Bienestar de Diabetes ¡Juntos Podemos!?

¡El Programa de Bienestar de Diabetes “Juntos Podemos”! Es diseñado para personas con diabetes y cuidadores de personas con diabetes para aprender mas como manejar la condición y guiar hacia una vida mas saludable y feliz! El programa es gratuito. Se requiere registración previa para estas clases y seminarios. Favor de reservar llamando al 951-788-3471.

CLASES DE MAPA DE CONVERSACIÓN El programa educativo de Mapa de Conversación de Diabetes de US es una herramienta interactiva de aprendizaje que ayuda al paciente a manejar mejor su diabetes. Fue creada por Healthy Interactions. Inc., con la Asociación Americana de la Diabetes.

Cubre los siguientes temas. “Descripción de la diabetes”, “Alimentación saludable”, “Monitoreo de azúcar en la sangre y uso de los resultados”, y “Continuando con su travesía”. Estas clases de dos horas son sesiones interactivas de grupos pequeños impartidas por un profesional de la salud certificado.

CLASES DE COCINA DE DIABETES Nuestras clases de cocina de diabetes son lideradas por una dietista registrada, estas clases de cocina se enfocan en como preparar comidas para alguien con diabetes.

Los temas incluyen, lectura de etiqueta de nutrición, sustituciones saludables y porciones.

MINI SERIES DE DIABETES

*Se require registración previa para asistir a estas clases.

Nuestro Programa de Bienestar ofrece seminarios trimestrales para aquellos con diabetes y sus cuidadores. Los seminarios se llevan a cabo en Febrero, Mayo, y Agosto con profesionales de la salud desacreditando mitos y discutiendo la conexión entre la nutrición, la actividad física y la salud emocional. Se provee pruebas de salud y cena en cada seminario.

Fecha del Seminario: Jueves, 25 de Agosto 6:00 - 8:30pm en Riverside Community Hospital Health Education Center *Se require registración previa para asistir al seminario.

CLASES DE SOLUCIONES Aprenda como manejar su condición crónica (diabetes, enfermedades del corazón, depresión, etc.) o la de algún ser querido en este curso interactivo de seis semanas basado en evidencia de la Universidad de Stanford. Obtenga estrategias en como comunicarse mejor con su proveedor de salud, fijarse metas, comer saludable, ser físicamente activo y mantener su independencia.
