Mead Assisted Living has two floors- one of which is dedicated for our memory support residents- for a total of 82 apartments. Village Assisted Living has 20 apartments. All our Assisted Living neighborhoods are staffed with nurses and caregivers 24/7, allowing them to provide full nursing services.
Home Health
Our Home Health department is staffed with nurses and caregivers. They provide a variety of services, including companionship, 1:1 resident support, household chores, pet care, private wellness visits, and errands on campus. Home Health holds a foot clinic weekly (by appointment) and monthly dermatology and audiology clinics (by appointment). Please call Home Health to set up appointments.
Nurse Practitioners
RiverMead has two options for Nurse Practitioners weekly. One works for Monadnock Community Hospital (MCH) and is happy to help residents that have practitioners at MCH. Another is a provider with AlignMed and no affiliation is required. Please call Home Health to set up an appointment with either NP.
Rehab/ Therapy
Our skilled therapy team meets with new residents and performs baseline assessments of self-care abilities. They offer a vast array of fitness and wellness classes as well as 1:1 therapy sessions to help residents meet their goals. Please call Marcia Orr to make an appointment or get a schedule of classes.
Life Enrichment
Life Enrichment (also known as Activities) holds many daily, weekly, and monthly programs. Each neighborhood has a monthly resident meeting that is open for all Assisted Living residents to attend.

Health Center Frequently Asked Questions
Q: What is Care Coordination?
A: The RiverMead Care Coordination team consists of RiverMead professionals that strive to keep all residents in the setting that best promotes maximum independence and safety. The team consults with residents and/or families when a change in a resident’s health status or self-sufficiency necessitates a move to a different level of care. The move may be temporary or permanent depending on the expected outcomes of agreed upon goals.
Temporary stays in the Health Center are usually around 45 days or less.
Once a move is permanent, arrangements need to be made to remove items from the Independent Living apartment within 30 days. For permanent moves to the Health Center, the RiverMead Facilities department will move the large pieces of furniture and resident belongings. Family members or the resident are responsible for packing and moving small items, dishes, clothing, breakables, etc. and unpacking items in the new apartment.
Monthly service fees continue until the residence is vacated.
Q: What is the difference between a respite stay and becoming a permanent resident at the Health Center?
A: A respite stay is short-term. It is generally between 1 week and 6 weeks in length. Respite stay residents come to the Health Center to a pre-designated apartment that has standard furniture supplied. Linens and personal hygiene supplies are provided.
Once a resident becomes permanent in the Health Center, they have the option to choose a different unoccupied apartment based on their preferences and needs. Residents are responsible for supplying their own furniture, TV, linens, and personal items once they become permanent.

lPlanning ahead for short stays: If a resident is having a procedure or surgery that might warrant time in Assisted Living, he/she should contact Jen Skelton, the Director of Nursing (603-371-0408). She will make a home visit to learn more about your upcoming needs. Even if you are unsure that you will need a stay in Assisted Living, it is helpful for the Director of Nursing to be aware of your condition in case your needs change. Emergency admissions are handled as they arise. Short stays may also be scheduled to provide respite care for a resident or to support the caregiver’s needs.
In general, short stays range from a night or two to six weeks. Residents come to the Health Center for their planned stay to a pre-designated furnished apartment in the Mead. Linens and personal hygiene supplies are provided. Nursing coordinates with the referring physician, assuring that orders and services are in place. As discharge nears, the resident confers with the care team to determine a discharge plan. A home visit may be done to assure that the resident can return home safely. Short stays are not available in the Village.
Care Coordination: The RiverMead Care Coordination team consists of RiverMead professionals who strive to keep all residents in the setting that best promotes maximum independence and safety. The team consults with residents and/or families when a change in a resident’s health status or self- sufficiency indicates a move to a different level of care. The move may be temporary or permanent, depending if the agreed upon goals have been met.
If a resident wants to explore a permanent transition, Dan Moore, the Assisted Living Transition Counselor (603-371-0453) is available to assist in this process. A resident has the option to choose any of the unoccupied apartments based on their preferences and needs. Once deciding to move, the resident is responsible for supplying their own furniture, TV, linens, and personal items. Arrangements need to be made to vacate the Independent Living apartment within 30 days. For permanent moves to the Health Center, the Facilities staff will move the large pieces of furniture and resident belongings. The resident is responsible for arranging the packing, moving and unpacking of smaller items. Monthly service fees continue until the independent residence is vacated.
Resources: Residents may access additional information in either library or in the Health Center section of the APP where a copy of the Health Center Welcome Book is maintained. Members of the Health Center team are available to give tours of the Health Center.
