1 Developing a Client Care Profile We gather information and ask questions to get a better understanding of what is needed, such as any plan of care that has been prescribed by you. Then, a basic needs and environmental assessment is conducted during an in-home visit, and we answer any questions that may exist.
2 Creating a Person-Centered Care Plan A person-centered care plan is tailored to the needs of your patient, incorporating their history, routines and preferences as well as your prescribed plan of care. This helps create support activities that will benefit them the most.

Identifying the Best Caregiver Your patient’s interests, personalities and service needs are reviewed and used to carefully pick a caregiver to match. Caregivers are screened, including background and reference checks, and bonded/insured.
