Every 70 seconds someone in America is diagnosed with Alzheimer’s. As the 7th leading cause of death, the prevalence of Alzheimer’s is higher than ever. With symptoms beginning at the higher risk senior citizens, 55-65, the disease symptoms and problems become worse as the patient ages.

Patients reaching these ages for Alzheimer’s complain about similar types and themes of symptoms. The patient may seem more irritable and uncomfortable in his or her surroundings. This may be a rare occasional onset, but rest assured that these are the main early symptoms of Alzheimer’s. Progression of this may lead to increased confusion and override rational decision making. Still at the early stages, patients may complain about problems formulating proper words. This will also involve math problems, decision making processes for abstract situations, as well as loss of stream of consciousness and thought.

Patients who are suffering from early stages of Alzheimer’s may also have noticeable differences in their attitudes about things they once enjoyed, as well as changes in personality. This becomes more evident with the progression of the disease. Patients may seem apathetic and depressed about activities or thoughts that once made them happy and content.

They may also begin to stumble during words and forget places and people that they had a vague recollection of. Again,

it is important to note that at the early stages, occurrences such as these are rare and may happen infrequently. Progression to medium stages will disable the patient further, where more and more faces, places, and events will be forgotten to a greater degree.

Alzheimer’s: Recognizing Symptoms

Finally towards the end of the early stages of Alzheimer’s, symptoms may become evident in the personality of the patient suffering from the disease.

The patient may be noticeably different in his personality, gaining new and weird behaviors. They may speak or act inappropriately, with this lack of judgment.

As the early stages of Alzheimer’s, it is important to make a checklist and record differences in activity and behavior. If one notices the patient in a more aloof and gloomy mood, reflective versus participatory in their activities (and over the age of 60) then it may be symptomatic of Alzheimer’s. It is key to accept the changes in behavior and understand that paranoia, hiding, and irrational behavior are part of the suffering for both family as well as patient.

Home Care Definitions: Cardiac Care refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac rehabilitation is a program designed to help a person with heart disease return to health. Cardiac rehabilitation may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

Diabetic Management refers to services including patient assessment, diet information, management of the disease process, medication management and education on all of the above. Services are provided by a Registered Nurse under the supervision of an M.D. Some agencies may employ certified diabetic nurse instructors or cardiac care specialty RN’s.

Home Health Aides provide support services such as personal care which do not require a licensed nurse. Medicare does not cover home health aides unless the person is also receiving skilled nursing care or other therapy. Certified Nursing Assistants (CNAs) provide personal care such as bathing, dressing, transporting and other essential activities.

Hospice is a comprehensive program of care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment.

The dying are comforted. Professional medical care is given, and sophisticated symptom relief provided. Very few Home Health Agencies are also licensed for Hospice care.

In-Home Care/Personal Care is care related to activities of daily living, including: mobility, eating, toileting, bathing and personal hygiene, dressing and grooming or cognition support. These services are usually provided on a private pay basis and do not require a physician’s order.

Intravenous Therapy is any medical treatments/medications that need to be introduced to the body via a patient’s veins. Numerous IV Therapies can be provided inhome under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

Psychiatric Services refers to the agencies ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications.

Registry Services refer, but do not employ, independent caregivers. Clients control all aspects of the arrangement, including compensation, directly with their caregivers.

Registries may include free non-profit listing services or companies that require training, bonding and insurance of their caregivers. Clients are charged a referral fee.

Skilled Nursing is a higher level of care where services are preformed by a licensed Registered Nurse or Licensed Practical Nurse. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver (if available) related to the disease process, treatment, health measures and medication management. Skilled Nurses provide medical care as prescribed by physician.

Care & Health

Respite Care is short-term or temporary. It is designed to give families a break from the stress of daily caregiving. Respite care can be for a few hours, overnight, or for days.

The amount of care changes with the needs of the family and the ill or disabled person.

Social Worker refers to a clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature.

24-hour care is “round the clock treatment� in a home environment.

The care may be shared by multiple caregivers or by a single caregiver.

Therapy Services (ST, PT, OT, RT) Occupational Therapy, (OT) refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. The goal is to help patients become as independent, productive, and satisfied as possible.

Physical Therapy (PT) refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills. Exercise techniques are only part of the special skills used by therapists to treat patients to help lessen disability, pain and improve overall function and quality of life.

Respiratory Therapy (RT) refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.

Speech Therapy (ST) refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.

A Therapist provides individually designed treatment programs to maximize communication and swallowing.

Choosing a Caregiver for Loved Ones Selecting a home care provider is a much more challenging process as there is a large range of experience, and professionalism. Here are a few insights to help guide your decision-making to make the best choice.  Is the home care agency licensed by the State of Oregon? The State Health Department requires that home care agencies follow a strict set of standards to ensure that clients are provided appropriate care in a manner that promotes their health, safety.  Is the agency bonded and insured?

“Bonded” means the agency offers protection to the client if a caregiver steals. “Insured” means the agency carries liability coverage for personal injuries and property damage, as well as workers’ compensation insurance.

Ask to see documentation to verify the exact kind of coverage.

 Does the agency schedule around you?

Some agencies have set times that caregivers are available. Ask if they are flexible when it comes to your schedule.  Does the agency have a live person on-call 24 hours? Nothing is more frustrating than having a problem and calling your home care agency, only to find an answering machine. Insist that your home care agency is available night and day.  Does the agency have 24-hour RN supervision? If you have an emergency question at any time of the day, it’s important that a Registered Nurse is on call 24 hours a day to answer your questions and provide help if needed.

 Are the caregivers experienced? Make sure your home care agency employs only qualified, experienced caregivers.

 Will the home care agency bill supplemental insurance companies?

Find out if billing is handled by the home care agency or if it is entirely your responsibility to submit claims.

In addition, it’s important to contact your insurance company and ask what they require from the home care agency for payment. Make sure you know what your waiting period is and if there are any exclusions.

 How does the agency train their staff?

Your home care agency should provide on-going training to keep caregivers up to date and proficient on skills

Making Oregon Vital for Elders (M.O.V.E.) is a statewide coalition of dedicated organizations and individuals united in changing the way Oregonians feel about health and long-term care for older adults. M.O.V.E. is a partner of the Pioneer Network, a national grass-roots organization committed to changing the culture of aging in America.

The ultimate vision of the Pioneer Network is to create a culture of aging that is ‘life-affirming, satisfying, humane, and meaningful.’ Culture change can transform a ‘facility’ into a ‘home,’ a ‘resident’ into a ‘person,’ and a ‘schedule’ into a ‘choice.’”

The current national culture change movement, launched by the Pioneer Network to transform older adult services, is based on person-centered values and person-directed practices. The concept of “person first” is the heart of both the values and practices which start and end with putting the person before the task. The culture change transformation is focused on both long and shortterm living environments, as well as community-based settings, where those in supported living and their caregivers are able to express choice and practice self determination in meaningful ways.

M.O.V.E.’s members represent many aspects of long-term care including: long-term care providers, state surveyors and other government agencies, Oregon’s Medicare

Quality Improvement Organization, profit and not-for-profit provider organizations, private businesses and consultants, senior advocates, and institutions of higher education. This collaboration inspires many of our initiatives and represents partnership at its best.

ARDC is Now Available Online NorthWest Senior & Disability Services (NWSDS) recently held an open house celebration to demonstrate the opportunities available through a new web-based portal called the Aging and Disability Resource Connection (ADRC).

The concept of the ADRC is to provide a single source of access for information and assistance to seniors, people with disabilities, their family members and community partners, on issues affecting their health and well-being.

The centerpiece of the ADRC is the development of an extensive comprehensive resource database that is readily available to virtually anyone, at any time. Complementing the web-based resource connection is the availability of trained information and assistance specialists who can more fully assist individuals seeking more detailed information, or counseling to understand the variety of service options available. We can also help assess an individual’s level of need for available public services, as well as their eligibility for assistance.

Through the ADRC, anyone can get access to information and counseling to help sort through the options available for making decisions about long-term care, Medicare, Social Security and other available programs. We can provide information and resources for programs to help you stay healthy, prevent illness, enhance your social activities and educational opportunities. The resource data base will also include information on local

services and products that could be useful to seniors and people with disabilities, including such things as: in-home services; family and caregiver support; peer counseling; home-delivered meals; transportation; legal assistance; programs for healthy living, and more.

The idea is to create an interactive format where individuals can find useful information on how to access services and supports for themselves or a loved one, while community providers can make that kind of information available to those in need. The information could be easily accessible through the web site, or individuals can seek additional assistance by telephone or by walking into any NWSDS office.

NWSDS is a small participant in a much larger national effort to implement a section of the federal Older Americans Act calling for the establishment of Aging and Disability Resource Centers nation-wide.

Our ADRC is a collaborative effort with the State Unit on Aging, and other local Area Agencies on Aging.

To expand health care to a recordnumber of Veterans, reduce the number of homeless Veterans and process a dramatically increased number of new disability compensation claims, the White House has announced a proposed $125 billion budget next year for the Department of Veterans Affairs.

“Our budget proposal provides the resources necessary to continue our aggressive pursuit of President Obama’s two over-arching goals for Veterans,” said Secretary of Veterans Affairs Eric K. Shinseki. “First, the requested budget will help transform VA into a 21st century organization. And second, it will ensure that we approach Veterans’ care as a lifetime initiative, from the day they take their oaths until the day they are laid to rest.”

The $125 billion budget request, which has to be approved by Congress, includes $60.3 billion for discretionary spending (mostly health care) and $64.7 billion in mandatory funding (mostly for disability compensation and pensions). “VA’s 2011 budget request covers many areas but focuses on three central issues that are of critical importance to our Veterans – easier access to benefits and services, faster disability claims decisions, and ending the downward spiral that results in Veterans’ homelessness,” Shinseki said.

Reducing Claims Backlog The president’s budget proposal includes an increase of $460 million and more than 4,000 additional claims processors for Veterans benefits. This is a 27 percent increase over the 2010 level. The 1,014,000 claims received in 2009 were a 75 percent increase over 579,000 received in 2000.

Shinseki said the Department expects a 30 percent increase in claims – to 1,319,000 – in 2011 from 2009 levels.

One reason for the increase is VA’s expansion of the number of Agent Orangerelated illnesses that automatically qualify for disability benefits. Veterans exposed to the Agent Orange herbicides during the Vietnam War are likely to file additional claims that will have a substantial impact upon the processing system for benefits, the secretary said.

“We project significantly increased claims inventories in the near term while we make fundamental improvements to the way we process disability compensation claims,” Shinseki said. Long-term reduction of the inventory will come from additional manpower, improved business practices, plus an infusion of $145 million in the proposed budget for development of a paperless claims processing system, which plays a significant role in the transformation of VA. Housing Definitions: Adult Day Care Programs offer a protective setting during the day, for those who cannot be left alone due to health care and social needs. The structured program within a community setting offers: socialization, planned activities, care services, companionship and meals. Adult Day Care provides daytime respite for the caregiver who typically works, needs time for other responsibilities, to simply to take a break. Some programs are designed for older adults with dementia and/or physical limitations. Programs through senior housing communities that are listed in this guide must be able accept non-residents to be included.

Adult Care Home or Family Care Home are private residences that provides a home-like setting, and serves 5 or less residents. The caregiver may perform several functions, such as personal care, housekeeping and activities and group meals. There are three levels of Adult Foster Care Home licenses. The classification system is based on level of care the Adult Foster Home may provide to residents who live in the home as well as the experience and training of the providers and their ability to assist residents with: personal hygiene, mobility, eating, dressing, toileting, and behavior management. Residents may require very light assistance or may be dependent with several care needs; it is best to ask individual locations for their specific policies and license.

Assisted Living Communities are State-regulated rental properties where six or more residents may reside. Care assistance is available, including: medication management, bathing, dressing, toileting and eating. Apartments must be fully self-contained private living units with a lockable door, private bathroom, and kitchenette facilities. The fee schedule is regular monthly rent along with additional fees for specific services and amenities. Assisted living communities are best suited for individuals who want to remain as independent as possible and who are able to direct their own care.

CCRCs (Continuing Care Retirement Communities) are communities that include a continuum of care from independent living, assisted living, residential care and skilled nursing services all on one campus, allowing individuals to live within the same community as their needs progress through the spectrum of care. They typically offer the full selection of amenities associated with retirement living. An endowment fee in addition to a monthly maintenance fee can be expected.

Independent Living Communities provide residents an independent living setting without the burden of home ownership. Typically, residency is established on a monthly rental basis. Residents live in fully equipped private apartments or cottages from studios to large two-bedroom units that may be rental-assisted or market-rate depending on the community. Amenities and hospitality services such as housekeeping, linen service, transportation and social and recreational activities may be included, provided for an additional charge, or may not be available at all. These communities do not provide assistance with activities of daily living or personal care.

Housing

Memory Care or Dementia Communities offer or provide care for residents with Alzheimer’s disease or other dementia in a home like environment. These communities, must receive an endorsement and is governed by additional regulations that are specifically intended to support individuals with dementia, including: a secure building that alerts staff if a resident has exited, a secure outdoor area that provides outdoor freedom safely, interior finishes that are non-glare and well lit, and visual contrasts between floors, walls and doorways. Alzheimer’s units must also have programs, which include: gross motor, self care, social, craft, sensory enhancement and outdoor activities.

Rehabilitation & Therapy is treatment for an injury, illness, or pain with the goal of restoring function, including nursing and therapy services. Rehab is ordered by a physician and services are provided by nurses and physical, occupational, and speech therapists. Examples include working with a physical therapist to help you walk and with an occupational therapist to help you get dressed.

Residential Care Facilities provide housing and supportive services for six or more people who do not require 24 hour nursing care. Pricing can vary greatly depending on the level of care provided and the size and amenities of each apartment.

Accommodations are typically in a home like setting and may range from a shared bedroom with common bathroom, to private apartments with kitchenette and private bathroom. Monthly fees vary greatly based on unit amenities and care services. Many residential care facilities specialize in caring for individuals with Alzheimer’s or dementia.

Respite Care is short term relief for family caregivers to take a break from the caregiving responsibilities and allows for a healthier and better quality of life for both the caregiver and care receiver.. Respite services include: companionship, personal care, recreational activities and security. Respite care can be provided as short stay or for a longer period that allows the family caregiver to go on vacation or a business trip. Longer duration respite care is generally found at select senior housing communities although some Companion services may provide this service in the home.

Skilled Nursing Facility (SNF) or Nursing Homes are Statelicensed facilities that provide a safe, therapeutic environment for individuals who require rehabilitative care or can no longer live independently due to functional or cognitive impairment. Nursing Homes offer 24-hour skilled nursing care and medical services by registered nurses and support professionals. This is the highest level of care that can be provided that is not hospitalization. Additionally, nursing facilities offer residents planned social, recreational and spiritual activities. Nursing facilities are most appropriate for people who need 24-hour medical oversight and a protective/structured setting. Most residents must share their room, but residents are allowed to bring personal items to encourage a more home-like atmosphere.

Choosing A Community A good match between a community anda resident’s needs depends as much on the philosophy and services as it does on the quality of care.

The following suggestions can help you get started in your search for a safe, comfortable and appropriate assisted living community:  What are your future needs and how will the community meet those needs?  Is the community close to family and friends? Are there any shopping centers or other businesses nearby (within walking distance)?  Visit each community more than once, sometimes unannounced.  Visit at meal times, sample the food, and observe the quality.  Observe interactions among residents and staff and ask questions.  Ask about social, recreational, and spiritual activities.  Ask about the type and frequency of staff trainings.  Review state licensing reports.

These steps should also be considered:  Contact your state’s long-term care ombudsman to see if any complaints have recently been filed against the assisted living community you are interested in. In many states, the ombudsman checks on conditions at assisted living units and nursing homes.  If the assisted living community is connected to a nursing home, ask for information about it, too. (Information on nursing homes can be found on the Medicare website at http://www. medicare.gov/nhcompare/home.asp).

When is the Right time to Move?

What makes you start to look for Senior Living? Many different things can play into the decision to start the adventure of the next stage in your life. Is it the burden of your home, your finding it more difficult each day to get the day to day tasks done?

Are you worried about a friend or family member being home alone. Many things can play into the decision to start looking.

But many are just looking, I ask why wait?

Many move into assisted living, even though they do need assistance. They look at this as being proactive towards their future care. You can live in a community; take part in everything the community has to offer, while not needing to worry about a move later on. If you wait until something happens, than you may be putting your loved one unknowingly at risk, which can dramatically change their quality of life.

When you start the beginning stages of looking for retirement, assisted living and memory care. You may start looking on the internet, call a community or just stop in to see what’s out there. While just looking is a starting point what you do next may be the most important.

Memory care moves should take place sooner rather than later. As conditions change, it is easier for someone, who is in a familiar environment in their early stages, to acclimate, as their disease changes. Memory care communities are designed to provide a structured environment with activities and care specialized for those living with Alzheimer’s and Dementia, facilitated by trained staff. Having someone stay at home as long as possible, isn’t always the best decision.

So many great things can be missed by waiting even just a few months. Many wait for a crisis to happen before they take the leap to move into a community. Many times when that happens, you may have waited too long to really enjoy some of the great things that your community has to offer.

Retirement communities offer you socialization, activities, dining, and most of all, freedom. You’re no longer tied down to your home and the care and maintenance that it may require. You can now truly enjoy your retirement the way it was meant to be. The possibilities are endless as to what you can now do, and you will find that the little things around your home really did keep you from truly experiencing your retirement years to their fullest.

Check out the person offering the investment by contacting the Oregon Department of Consumer and Business Services. Its Division of Finance and Corporate Securities – the state’s securities, or investments – regulator – can tell you if the firm or salesperson is licensed to sell investments in Oregon and if the investment is registered for sale in Oregon.

Whether the opportunity comes from a broker, investment adviser or insurance agent, the Division can provide important licensing information including possible disciplinary action.

In addition, check a person’s professional credentials. In Oregon, those who sell financial and insurance products are prohibited from marketing themselves by claiming they are a “specialist,” “adviser,” or similar title when they have no substantive credentials based on legitimate professional training. Senior citizens are vulnerable to this type of deceptive marketing as many are looking for ways to invest and may targeted by those claiming to have specialized expertise.

You should also be wary of the following:  The promise of a rate of return that is better than what similar investments are paying.  A guarantee against failure.  Insistence that the opportunity exists today only—tomorrow will be too late.  Pressure to write a check immediately or for a bank account number “to make it easier.”
