Affidavit The undersigned affiant, being duly sworn, deposes and says:

1. That, the undersigned Affiant, Trooper First Class Timothy H. Van Deventer #953, is a regular member of the Connecticut State Police Department and has been since January 20, 2006. That this Affiant is currently assigned as the Prospect Resident State Trooper under the Connecticut State Police Troop-I, Bethany, under the command of Lieutenant Michael Perry# 047, and at all pertinent times mentioned herein was acting in official capacity as such member of said department. The following facts and circumstances are stated from personal knowledge and observations as well as information received from brother or sister officers acting in their official capacity. 2. That on Saturday, 01 /12/19, at approximately 0100 hours, this affiant was dispatched to a roll-over motor vehicle accident located on I 91 Southbound between Exit 16-15 in the town of Meriden. On 01/12/19, at approximately 0105 hours, this affiant arrived at the scene and was greeted by a witness named Robert Modzelewski (DOB 11 /28/56) who stated that he witnessed a Jeep Wrangler rear-end a car in the center lane. Modzelewski stated that the car went into the woods in the right shoulder and he saw the Jeep flip over in the right shoulder. Modzelewski stated that he didn't see who was driving but he saw people laying on the ground in the area of the Jeep Wrangler.

3. That on 01/12/19, at approximately 0105 hours, this affiant spoke with the operator of vehicle# 1, Ashley (Tlr/s Is page 1 of a 1 page Affidavit) IJato
