Affidavit The undersigned affiant, being duly sworn, deposes and says:
1. That Affiant, Sergeant Steven Cifone #375 , being duly sworn , does depose and state that I am a member of the Southington Police Department and have been for approximately nine (9) years and five (5) months prior to this date. At all times mentioned herein , was acting as a member of said department. The following facts and circumstances are stated from personal knowledge and observations, as well as information received from other police officers acting in their official capacity, and from official police reports and statements made by prudent and credible witnesses.
2. That the name and address of the victim are not being disclosed in this report in accordance with C.G .S. 54-86d and 54-86e. The Victim will hereinafter be referred to as Victim within this report. Victim is described as a 36 year old white female. The accused is identified as Gregory Smith with a DOB of 12/21/1976. 3 . That on 1/21/2020, Victim came to the Southington Police Department to report that over the past several months, Gregory Smith, has violated a Protective Order on multiple occasions by visiting her residence in Southington, CT. 4 . That Victim provided photos of Smith's vehicle in the parking lot of her complex as well as a sworn written statement regarding her relationship and twelve (12) separate violations of the protective order. Affidavit - Continued Victi m's statement is as follows ... Gregory "On 7/7/2019 I was the victim of a domestic violence incident. Smith , was subsequently arrested and an Order of Protection was issued. The order had since been modified on 11/6/2019 allowing the two of us to meet for•••counseling . At no point was Gregory allowed to come to my place of residence. I currently reside in Southington and have been living here since the end of July 2019. Since September 2019 Gregory has been showing up at my residence randomly. I would notice his dark red Chevy Silverado or his silver Chevy Impala parked in the parking lot and would text him asking why he was outside and he'd reply asking me to open the door but I'd refuse . I was consistently taking photos of his vehicle parked in the lot but he has since gained access to my phone and deleted them from a secured

..application by deleting the application . This occurred some time in early December I have been taking notes of the dates that he shows up at my apartment for various reasons.
I have
recorded the following dates since I have been living in Southington; September 2nd, 11th, 14th, 22nd, 23rd, and the 29th as well as October 2nd, 10th, 11th, 14th, and the 27th. On December 24th 2019 Gregory showed up to my apartment and parked his silver Chevy impala bearing CT registration 362WHK under my window. Gregory then proceeding to call me saying he wanted to talk. I let him in and did not notify the police to prevent a scene in my new neighborhood. I took several photographs of his car and one photograph of him walking back to the car and I still have these photos. Gregory would even show up to my front door by utilizing the keyless entry main lobby door. I have since stopped communicating with Gregory. The last contact I received from Gregory was on January 11th when he sent me a text message stating "-■■■-•a:sad face emoji)." I did not reply to him at that time. I have been in communication with the victims advocate and the courts family relations in the Middletown court system and have been advising them of Gregory's behavior. --

He has not tried to contact me in any way since that time. Due to Gregory's violent history and his psychiatric issues I am concerned for my safety." -
5. That on 7/15/19 a full Order of Protection was put in place with Victim as the Protectee against Smith. (This is page 2 of a 3 page Affidavit

ftidavit - Continued The order was later modified on 8/29/19 to add "The parties may have in-person contact in a therapeutic counseling setting, and may have contact through electronic written communication for the purpose of coordination and counseling." This order was effective 8/29/19 through 11/5/19. This order is checked for (CT01 ), (CT03) and (CT05) which prohibits Smith from the home of the protectee or wherever she resides. On 11 /6/19, the order was again modified but maintained conditions of (CT01) and (CT03) which again prohibits Smith from the home of the protectee or wherever she resides. 6. That based on the above mentioned facts and circumstances, this Affiant requests that an Arrest Warrant be issued for Gregory Smith (DOB 12/21/76) on twelve (12) counts of vioJations of CGS 53a-223 Violation of a Protective Order between the dates of 09/02/2019 and 12/24/2019.
