Nominations must be submitted in writing, along with this form, on a single, 8.5” x 11” sheet of paper. Forms submitted without an additional, written testimonial will not be considered. Please describe the scope and nature of the nominee’s community service, the agencies/organizations for which the nominee volunteers and the positive impact the nominee’s service has made upon the community.
