ince commencing my role as President of our College in December 2021, I have been humbled by the kind messages of support from Fellows and Members around the world. It is an honour and a privilege to be elected to this position, and please know that my intentions are to serve this College and its members with the values that make us who we are as a global community of caring healthcare professionals.

I take pride in the two mottos on our College coat of arms.

On the base, the words “Non vivere sed valere vita”, from Martial’s Epigrams which in rough translation mean ‘not just to live but to have a healthy life”. At the top, “conjurat amice”, from the Roman poet Horace, which means ‘joined in friendship’.

I extend that hand of friendship to each one of you. I believe there is great strength in our community of friendship and professionalism that spans different areas of healthcare and the globe – it is what sets us apart as a College.

Many of you will know that, in the UK, we are the only Royal College that brings physicians, surgeons, dental professionals, travel medicine specialists and podiatrists together to share learning. We achieve this while also working with the other Royal Colleges and organisations to develop and deliver more specialised education, assessment and professional opportunities within the different disciplines of healthcare that we represent.

30 YEARS OF COLLEGE LIFE When former President Jackie Taylor handed over the President’s medal to me at the AGM in December of last year, it not only marked the beginning of my term as President but also marked exactly 30 years to the day since I was first admitted to the College as a Fellow on 3 December 1991. I have been grateful to the College at many times throughout my career for the support I have received to enable me to progress through training and assessment, and have been able to give back through teaching, examining, and other professional duties including as Regional Councillor (2005-2013), Vice President (Surgical) (2013-2016) and Director of Global Health (2017-2020).

In 2005, Nelson Mandela said, “As long as poverty, injustice and gross inequality persist in our world, none of us can truly rest… overcoming poverty is not a gesture of charity. It is an act of justice.”

This statement still rings true, and I believe we have the ability and the power through our collective voice to take steps to improve equity in health and healthcare by embracing a culture of global citizenship that supports international partnerships in health with mutual benefits. The College’s leading role in this area, particularly working with Scottish Government colleagues, is something we can all be proud of.

The Covid pandemic has highlighted some of the greatest strengths and weaknesses of global health. The rapid mobilisation of services, introduction of treatment regimes, research collaboration and the development of vaccines have demonstrated the great strengths of the international scientific and medical communities. In contrast, the disparity in vaccination rates between higher and lower income countries has starkly illustrated the inequalities between nations. None of us will truly have protection from Covid until the whole world is vaccinated and so there is now an urgent need to consider new approaches which focus on equity.

CLIMATE CHANGE IMPACTING HEALTH INEQUALITY The climate crisis is another stark example of global inequality. It is those who have contributed least to the problem who are now bearing the worst of it, with consequences for health and mass migration. In September last year, our College joined with more than 400 health organisations around the world in support of a letter delivered to the COP26 Presidency which called on wealthier nations to do more to ensure a just transition to climate change mitigation and adaptation for the benefit of all our health. We will continue to work in this area and use our College voice to speak out on climate change and sustainability.

In my first month as President, I was able to travel to Egypt with College colleagues to strengthen our bonds and partnerships there. The visit was an excellent example of the benefits there are from working together in friendship to improve training and education and ultimately patient care.

Over the coming years, I look forward to reaching out, engaging and, I hope, meeting in person with many of you across our College community.

QUESTIONS FOR THE PRESIDENT TELL US ABOUT YOUR CAREER SO FAR After graduation from Glasgow University in 1985 I began training in surgery, mainly in the west of Scotland, with a year in London and six months in Manchester on what would nowadays be termed an oncoplastic fellowship, before taking up my consultant post at the Royal Alexandra Hospital in Paisley in 1997. Over the next few years, I was able to establish breast reconstruction surgery at Paisley and bring the breast services at the three Clyde hospitals into a single unit. In 2010 our growing specialist team was also able to repatriate breast screening referral and so we now diagnose and treat over 350 newly diagnosed breast cancer patients each year. I continued with a commitment to emergency general surgery for 20 years but now, in common with most breast surgeons across the UK, I am no longer on call, though I do miss the challenges of out-of-hours work. Over the years my focus has been on the development of high quality clinical services and so I was proud to be appointed National Clinical Lead for Scottish Breast Services in 2019. As I begin my time as President of the College I am particularly pleased to be able to step back a little from the Clyde breast service knowing that the team now in place really no longer needs me!

WHEN AND WHY DID YOU FIRST GET INVOLVED WITH THE COLLEGE? As a trainee I was interested in surgical education and so signed up for one of the first Training the Trainer courses at College in 1995. From there I became involved in teaching on the basic skills courses and as a young consultant I set up and led a weekly educational programme for the west of Scotland basic surgical training scheme based in College. As I became more involved, l joined College Council in 2005 and served two terms as a regional Councillor at which point Professor Frank Dunn asked me to consider standing to be one of his Vice Presidents. At first, I was a little doubtful about taking on that role as I was aware of just how much of a commitment it would be. I realised however that there was an opportunity to bring our College into the growing global health community and with Frank Dunn and then David Galloway’s support, College’s commitment to global health has grown. Our report on the value of volunteering, “Global Citizenship in the Scottish Health Service” which I wrote with Leadership Fellow, Stuart Fergusson, was accepted in full by the Scottish Government in 2017, who then invited me to work at St Andrew’s House in the newly created Global Citizenship Co-ordination Unit.

During this time I also chaired the College’s charity the HOPE Foundation, which involved fundraising and distribution of financial support to projects at home and overseas.

HAVE YOUR PERCEPTIONS OF THE COLLEGE CHANGED OVER THE YEARS? Yes they have. As a trainee I did not find preparing for my Fellowship exams at all easy and in common with many of my contemporaries, I perhaps rather resented the time spent on study after long hours of clinical work. I now value College’s role in education and assessment and appreciate the importance of College in maintaining standards of professional practice. This was really brought home to me when I travelled to represent our College as an external examiner for the recently formed College of Surgeons of East, Central and Southern Africa (COSECSA).

I witnessed the impact that the recognition of study and training had on the young people gaining their Fellowship of the College and recognised afresh the importance of institutional guardianship of standards.

WHAT HAVE BEEN THE HIGHLIGHTS OF YOUR CAREER SO FAR? I have been very fortunate in my career to have worked with many excellent colleagues, and if there have been highlights or achievements, then they have been very much a result of team effort. As I look back across my clinical career, work in Scottish Government and College, a few things stand out. Firstly the clinical service for breast patients in Clyde. Secondly the Livingstone Fellowships which have allowed trainees to come from Malawi and Zambia to work in the Scottish NHS before returning home with new skills and ongoing support to help them develop their specialties. Thirdly the Global Citizenship Programme, which is now so well established within the Scottish NHS. Without doubt, however, the greatest highlight is having the opportunity, and privilege, to serve as College President.

WHAT WILL YOU FOCUS ON IN YOUR THREE-YEAR TERM? I am clear that we should continue the work established by my predecessor as President, Professor Jackie Taylor, on workforce, wellbeing and inclusivity. These were prescient priorities for College to be working on as the pandemic began and are now all the more essential as we recover from the impact of Covid on health services. In that regard I would like us to consider not just restoring, or recovering, the NHS in the UK, but to take this opportunity to adapt and renew our services, improving them for patients and our health workforce alike.

As we do so, we need to be mindful of sustainability and climate change issues, recognising the impact of climate change on health, addressing the NHS’s own carbon footprint and recognising the actions which we can all take to mitigate the rise in global temperature.

I will also make College’s global health work a priority. The pandemic has helped collective understanding that the health of people elsewhere is intrinsically linked to our own and I very much hope that we can continue to speak out on issues like equity of access to vaccines.

NEWSBITES COLLEGE CONGRATULATES JANE CHIODINI ON AWARD OF MBE Our College extends warmest congratulations to Mrs Jane Chiodini, former Dean of the Faculty of Travel Medicine, who has been awarded an MBE for services to training and development of travel medicine.

Mrs Chiodini demitted office as Dean of the Faculty in October 2021, but continues to be involved in the development and delivery of the Faculty’s educational programme.

During her time as Dean, Jane was instrumental in leading the publication of the Good Practice Guide for Providing a Travel Health Service, which sets the standards of care for practitioners in travel medicine, and the establishment of the RCPSG Postgraduate Diploma in Travel Medicine and the Professional Development Certificate in Travel Medicine.

Mr Mike McKirdy, College President, said: It is wonderful to see Jane’s work and contributions to travel medicine recognised in this way. On behalf of the College I offer our warmest congratulations.

Dr Sam Allen, Dean of the Faculty of Travel Medicine, said: We are immensely proud that Jane Chiodini has been awarded the MBE for ‘services to training and development of travel medicine’. Jane has been an inspirational contributor and leader in the mission of our Faculty to raise the standards and standing of travel medicine. This recognition is so very well deserved and we congratulate Jane on receiving this honour.

COLLEGE WELCOMES NEW COHORT TO LEADERSHIP DEVELOPMENT SCHOLARSHIP PROGRAMME The Leadership Development Scholarship Programme is a partnership between our College and the College of Medical, Veterinary and Life Sciences, University of Glasgow, and is designed to support those underrepresented in medical leadership positions, within either a clinical or academic setting.

The Programme’s second cohort recently gathered at the College for the first time and was welcomed by College President Mike McKirdy and Programme Facilitators David Wylie, Director of Professional Education at the College, and Karan Janman from partner organisation Silvermaple.

Our College extends a warm welcome to the eight participants, who will follow a bespoke programme of education, using this opportunity to network with, and learn from, the others on the programme.

COLLEGE ILLUMINATED TO MARK ANNIVERSARY OF DEADLY COUP IN MYANMAR

COLLEGE SKILLS LAB RECEIVES NEW LAPAROSCOPIC EQUIPMENT William & Elizabeth Davies Charitable Trust

On Tuesday 1 February, one year since the deadly coup in Myanmar took place, we joined Royal Colleges across the UK in lighting up our building in red in a moment of solidarity and remembrance for health workers who have risked, and lost, their lives to provide care over the past year. The joint action was led by the Tropical Health and Education Trust (THET).

Mr Mike McKirdy, College President, said: We stand in solidarity with health professionals in Myanmar who risk their lives every day to deliver essential care to those in need, under hugely challenging circumstances. We have deep rooted connections with the medical community in Myanmar. Our thoughts and prayers remain with them and all those caught up in the conflict.

We steadfastly stand together with our members, fellows, doctors in training and wider healthcare professional communities inside Myanmar. We have hence developed a free online learning platform “RCPSG Learning Hub for Myanmar”, to enable healthcare professionals to continue their own learning and developments in this difficult time. This digital learning hub has included fit for purpose online learning resources for physicians, surgeons, doctors working in rural and conflict zones, including those preparing for MRCP Clinical Examinations.

The Macewen Skills Lab, located in our College building in Glasgow, has recently taken delivery of new state-of- the-art equipment providing the latest technology in image definition and clarity. The 4K Stryker Laparoscopic Stacks have been obtained with funding support from the William and Elizabeth Davies Trust.

This equipment will greatly enhance the experience of delegates on our Surgical courses. The laparoscopic stacks, combined with new laparoscopic body manikins, will allow delegates to practise practise surgical techniques with greater realism.

BREAKING DOWN BARRIERS TO HEALTHCARE FOR MIGRANTS, ASYLUM SEEKERS AND REFUGEES Our College has hosted a webinar to highlight the barriers faced by migrants, asylum seekers and refugees in accessing health care across the United Kingdom. In the United Kingdom, all individuals have the right to register with a GP and access free primary care - regardless of immigration status. Although everyone has equal entitlement to primary care, there remains inequity in access to healthcare for these groups, resulting in poor health outcomes and an exacerbation of health inequalities.

Dr Peter Gough, a GP volunteer and Trustee of the charity Doctors of the World, described the barriers to healthcare that exacerbate health inequalities for these groups include language barriers and a lack of understanding about the right to care and how the NHS works. Importantly, Dr Gough also highlighted the fact that many healthcare staff are poorly educated on the rights and entitlements to healthcare of the most vulnerable people in our society. As a result, it is common for GP practices to ask new patients about their immigration status and require photo ID and proof of address. Doctors of the World has found as many of 19% of GP practices wrongfully refuse registration of new patients on this basis.

In response, Doctors of the World has launched a network of Safe Surgeries, a growing community pf GP practices committed to tackling barriers to healthcare and creating more inclusive GP practices. There are now 656 Safe Surgeries across the UK, where both clinical and non-clinical staff are proactively welcoming patients who have arrived in the UK by way of migration.

College President Mr Mike McKirdy also contributed to the webinar by reflecting on the responsibility of healthcare professionals to understand the barriers to effective delivery of healthcare to those who have joined our communities by moving here from all parts of the world. He acknowledged that much is to be done to address the health inequalities faced by migrants, refugees, and asylum seekers – both in terms of ensuring true equity of access to healthcare, and to ensure parity of service provision.

“CARE IS MORE IMPORTANT THAN TREATMENT” GRANT MCINTYRE ON LESSONS LEARNED AS SCOTLAND’S SICKEST SURVIVOR OF COVID-19

Professor Grant McIntyre, Fellow of our Faculty of Dental Surgery, is said to be Scotland’s sickest survivor of Covid-19. After contracting the virus at the very start of the pandemic, Grant spent four months in hospital. Together with his wife Amanda, Grant has written a book about his experience, in which he pays tribute to the many who cared for him.

I

n March 2020 Professor Grant McIntyre started to feel unwell. As a consultant and Professor of Orthodontics at Dundee Dental Research Hospital, Grant was busy making preparations to adapt their service delivery to the impending pandemic. While at work he started to feel feverish and breathless. Over the course of a week, his condition deteriorated and he was eventually admitted to hospital after testing positive for Covid-19.

Perhaps due to his own clinical training, Grant was fully aware of the severity of the situation as his illness progressed.

Grant describes being ‘terrified’ and bluntly asked his medical team to put him on a ventilator and ‘save his life’. Grant went on to suffer a cytokine storm, sepsis and multi-organ failure. His doctors did not expect him to live but he survived 50 days on ECMO life support before embarking on a long and difficult journey of recovery and rehabilitation.

DYING TO LIVE - ILLNESS AND CARE Grant is credited with being Scotland’s sickest survivor of Covid-19. Together with his wife, Amanda, he has written ‘Dying to Live’, a book about his illness and recovery. It is a story in two parts. The first is an account of Grant fighting for his life as an acutely unwell intensive care patient. The second part is about Grant rebuilding his life through months of physical and occupational therapy in ICU, on the medical ward and, eventually, in his own home.

The book is a first person account of Grant’s illness, and he does not shy away from describing the despair and the joy of his time in hospital. He describes hallucinations of wild animals and a period of paranoia when he thought the nurses were trying to kill him. But he is also generous in describing the jubilation of seeing his wife Amanda for the first time after only communicating via an ipad for weeks and taking his first steps after months in a hospital bed.

‘Dying to Live’ tells the story about one patient and the many members of staff that cared for him along the way. It is a tribute to the NHS and the many professional groups within it. Grant credits the doctors and nurses involved in his care with saving his life, but he also wishes to pay special tribute to his rehabilitation team for rebuilding his life. He shows great appreciation to the Physiotherapists and Occupational Therapists that help him take small steps towards regaining hope, strength, and independence.

Grant says his experience has changed his view of the NHS. “I met so many different people along the journey that I had never encountered and I now have a much deeper understanding of many jobs that exist within the NHS. My respect for Allied Health Professionals has increased manifestly.

We wanted to celebrate their input to the overall journey”.

WRITING AS PART OF HEALING It was the rehabilitation team that suggested Grant and Amanda write a book to help them process what happened.

They say that “writing about this difficult time in our lives was a cathartic experience and helped us to process and make sense of the journey. We hope it helps others too”. Since the book has been published, Grant and Amanda have been contacted by patients and families who tell them the book has helped them understand the experience of being critically unwell and life in intensive care.

In October 2021, Grant gave a talk at our President’s Conference: Time To Heal, where he spoke about the lessons he learned as a patient. “Care is more important than treatment”, he said. The experience has had an impact on his own clinical practice: “My approach to my patients has now changed completely from treatment for their condition to caring for them. Our patients need both treatment and care, but the latter is the key to success. Caring for patients, and their families, is crucial and absolutely fundamental to a positive outcome”.

SUSTAINABLE SURGERY FOR SOUTH SUDAN’S KIDS CHANGING LIVES Consider this fact.

1,750,000,000 children lack access to safe surgery globally.

One billion, seven hundred and fifty million children.

T

hat means 10 out of 11 children live in a country lacking the dedicated surgical infrastructure to get the care they need. Without those facilities, common, easily treatable illnesses become deadly diseases or cause life-long disability.

We’re at a point where, despite significant progress elsewhere in global health, mortality and morbidity from common conditions needing surgery have grown in the world's poorest regions, both in real terms and relative to other health gains.

The Lancet Commission on Global Health recently estimated that 143 million additional surgical procedures are needed annually in low-and-middle- incomes countries (LMICs), 54 million for children alone.

Put in the simplest terms, there are hundreds of thousands of children needlessly dying every year, with more children, dying from surgically curable conditions than HIV, Malaria and TB combined.

How do we even begin to solve a problem on that scale?

How can the College be part of a sustainable solution?

Through our HOPE Foundation, our College has chosen to support Scottish charity Kids Operating Room (KidsOR) and in so doing are beginning to play our part in ensuring that every child has access to safe surgery. At the end of 2021, the HOPE’s Trustees decided to support the training of South Sudan’s first-ever paediatric surgeon.

For context, despite being a country of 11 million, over half of whom are children, South Sudan lacks a single paediatric surgeon.

To further emphasise that point, a country where 10% of children die before their 5th birthday, lacks both the infrastructure (with no dedicated paediatric operating theatre) and the expertise (without a single paediatric surgeon) needed to care for its children.

That’s why HOPE’s Trustees supported KidsOR’s project to catalyse change. We’re not acting alone: HOPE and our College have teamed up with our friends at the Royal College of Surgeons of Edinburgh – and their Global Surgery Foundation – to jointly support KidsOR’s Paediatric Surgical Scholarship Programme.

KidsOR is a global health charity focused on the provision of high quality, safe surgical services for children in low- and middle-income countries. From their bases in Edinburgh, Dundee and Nairobi, they provide surgeons and their teams with the infrastructure and training needed to transform the care available for their nation’s children.

SUSTAINABLE LOCAL EXPERTISE Importantly, KidsOR put sustainability at the heart of their model and, unlike some other participants in international development, only invest in local people, building real capacity and promoting self-reliance in the long-term. Over the past four years, the KidsOR team have moved quickly to put this model into practice, installing 50 operating rooms (ORs) across 22 countries; creating additional capacity for at least 30,000 operations each year.

Many of these ORs sit as part of KidsOR’s Africa 30 strategy, an ambitious but achievable, plan to deliver 120 ORs, train 100 paediatric surgeons and 100 paediatric anaesthesia providers across the continent by the end of the decade. The immediate outcome will be 10 million years of disability prevented, as some 635,000 children access emergency and essential care.

The economic benefit to partner nations will go on to be an estimated $5.6billion.

The Colleges’ joint investment into South Sudan will form an important part of KidsOR’s plans, providing the funding for the first year of a five-year scholarship. That scholar, Dr Betty Arkangalo Yuggu Phillimona, will begin training at Baylor College of Medicine in Lilongwe, Malawi under Dr Bip Nandi, before returning to work at South Sudan’s first dedicated OR for children, within Al Saba Children’s Hospital, Juba.

Once qualified, it is estimated Dr Phillimona will go on to perform approximately 6,000 procedures over the course of 10-years (the average life-span of an OR).

That’s 6,000 children just like Jibril, who had waited his whole ten-year life for a simple surgical procedure before KidsOR installed an OR in his town.

BLANTYRE BLANTYRE RESEARCH FACILITY:

AN EXEMPLAR OF BEST PRACTICE IN PARTNERING WITH MALAWI

P

erhaps you have heard the African proverb above, it very much typifies the ethos of the team that established the Blantyre Blantyre Research Facility based at the Kamuzu University of Health Sciences (KUHeS), formerly the College of Medicine, University of Malawi.

The team is headed up by Professor Paul Garside, Dean for Global Engagement (Africa & Middle East) and Dr Mwapatsa Mipando, immediate past principal of the former College of Medicine, both University of Glasgow. Over the last five years, the team has successfully established a state-of-the-art research facility out of an old lecture theatre on the KUHeS campus.

Today the facility is complete and houses cutting-edge research and diagnostic equipment, including advanced microscopy and a clinical sample repository and management system. The project came about after Professor Garside spent a

three-month sabbatical in Malawi during which he met Dr Mipando. It soon became evident that by working together to build the new laboratory facility they could develop research programmes that would have mutual benefit for healthcare in both their home countries by comparing the causes of poor health and low life expectancy in Blantyre, Scotland and Blantyre, Malawi. Hence the lab facility name – Blantyre Blantyre.

The facility is funded from an award of £1.2 million from the Scottish Government International Development Fund (with match funding from the World Bank and the Wellcome Trust). The laboratory has already attracted interest from across the globe, with academic researchers, pharmaceutical and industry partners seeking to use the services and facilities available to develop a wide range of projects.

In the early years, it rapidly became clear that creating the correct training environment to support future staff and further strengthen capacity in Malawi would be key to the facility’s long-term success. By developing a pipeline of local research leaders and a critical mass of biomedically trained Masters’ students, doctoral fellows and senior researchers, essentially by designing a road map for local talent to compete on an international basis, the project has ensured a ready supply of trained expertise. To this end the University of Glasgow linked up with The Beit Trust to jointly fund Masters’ training at University of Glasgow for Malawian students. A perfect example of capacity building in action.

Similarly, the importance of professional services staff in areas such as administration and finance was also recognised with two-way training visits proving very important as the facility and its services develop and expand. For example, the appointment of a commercial manager who implemented a business plan to ensure that the facility will be self-sustaining secures its longevity and is a valuable asset for KUHeS. POWERED BY SUSTAINABLE GREEN ENERGY As it has gathered momentum, the Blantyre-Blantyre project has also attracted additional components. A recent highlight has been the installation of solar power to the laboratory.

Investment secured from the Scottish Funding Council Global Challenges Research Fund allowed the management team to install 80 solar panels on the roof. All the lab’s energy needs are now provided by this clean green energy solution with excess energy being distributed across the KUHeS campus.

It’s envisaged that using this exemplar model, surplus energy will be sold back to the Malawi energy supply companies.

Critically, the solar power installation was by a company from Botswana (African Sun Energy Ltd) established by a UofG graduate and providing a great example of south-south cooperation. The knowledge and experience gained from the solar installation project also benefitted the wider Malawian community: it trained solar engineers in installation methods and created full-time employment for a female trainee. The solar power project has been so successful, that there are now plans for it to be rolled out to other rural healthcare settings.

Thus, some rural clinics should soon be powered by Malawi’s 3,000 plus hours of sunshine each year.

But why has the Blantyre Blantyre Research Facility project been so successful? The project was recently mentioned in a European Commission, Director General for Research report with its management team being congratulated as an exemplar of best practice in working with African partners. Perhaps it’s the emphasis on mutual trust and the realisation that one-way benefit for partners is not sustainable in the longer term. By taking the time to establish a respectful working environment and listening to African partner’s needs, the management team continually puts an emphasis on experiencing and seeing face to face the in-country challenges faced by African colleagues.

The management team prioritised resources in support of better financial accountability and lab quality assurance.

Professor Garside explains “Dr Mipando and I pulled together a cosmopolitan team from different backgrounds with a variety of skills: research scientists, grants administrators, technicians, marketing, PR and IT specialists. These professionals from both Malawi and Scotland came together to develop a mutually beneficial partnership that has had tremendous impact and resulted in valuable knowledge transfer. With the Blantyre Blantyre Research Facility, we have established a lab open to all researchers, which will have international accreditation and provide the necessary infrastructure for a conducive research and academic environment. Malawian scientists told us what they needed and we listened, worked with them resulting in a quality research environment in which to study an array of multimorbidities and priority research areas that will ultimately benefit a global audience.

Over the last few years, the world has experienced a global pandemic and only by collaborating to share data, techniques, vaccines and newly discovered knowledge have research scientists made headway in how best to tackle COVID19.

GLOBAL PARTNERSHIPS BUILT ON MUTUAL UNDERSTANDING AND RESPECT Successful global health partnerships generally have positive impacts in achieving objectives: they raise the profile of the diseases/areas studied, they mobilise commitment, accelerate funding and are often able to leverage additional funding. A truly successful global health partnership will fit within country national priorities and deliver wider benefits beyond original objective and tackle broader issues.

The Blantyre Blantyre project achieved all the above and the added value achieved from partnerships, strong collaborations and further research and ongoing funding applications that have emerged are immense and will have long-term benefits for both Scotland and Malawi.

In Malawi a common first greeting is Muli bwanji (How are you?) Start your conversation with African partners with something similar, ask what they need, don’t tell them what you think they need, listen to them, get an in-country view of what would help, and you are on the right track for success.

In a recent address at The African Research Universities Alliance (ARUA) conference held in South Africa, Dr Mipando said “Africa is ready and made a call for respect and for scientists to work together and listen to each other for mutual global success.”

Develop your career in Travel Medicine RCPSG Postgraduate Diploma in Travel Medicine This course has been designed in line with the Faculty of Travel Medicine of the Royal College of Physicians and Surgeons of Glasgow’s publications Good Practice Guidance for Providing a Travel Health Service and Recommendations for the Practice of Travel Medicine.

The aim of this course is to develop and support healthcare professionals in their role as travel medicine practitioners. It offers an academic base that assists individuals to become autonomous and reflective practitioners in the delivery of travel medicine services.

About the Course This course consists of eight postgraduate modules delivered over a period of 18 months. The course will be delivered by specialists in the travel medicine field using a blended model of two residential elements and online interactive learning. Students will be expected to attend and contribute to online educational and peer support platforms. There will be two residentials in Glasgow in January and October, specific dates for 2023 will be released later in the year.

Those who complete the Postgraduate Diploma in Travel Medicine will be eligible to be admitted to the College as Members of the Faculty of Travel Medicine and able to use the post nominal MFTM RCPS(Glasg)

A STRATEGY FOR NHS SCOTLAND FOCUSED ON

REDUCING EMISSIONS AND ENHANCING WELLBEING Dr Elaine Mulcahy, Director UK Health Alliance on Climate Change

I

n 2019, the Scottish Government announced its ambition for NHS Scotland to become net-zero by 2045. After much anticipation in the lead up to COP26 in Glasgow, the NHS Scotland Climate Emergency and Sustainability Strategy was published on 10 November and opened for consultation until February 2022.

The strategy raises the bar on the initial target by bringing forward the date for achieving net-zero emissions to 2040.

Fundamental to the goal of reducing emissions is an ambition to do this is a way that enhances wellbeing and reduces health inequalities: “A health service that improves the opportunities, life chances, health and wellbeing of every citizen in our country and fully contributes to a more cohesive, resilient and net-zero society in a just way that contributes to population wellbeing and a reduction in health inequalities.”

Achieving such an ambitious aim will require commitment across all elements of the health service, including the people it serves. Significant progress in reducing emissions will be made through a shift from fossil fuels to renewables, and improving infrastructure and facilities to make them more energy efficient.

The expectations of the healthcare workforce, patients and visitors of the way in which the service operates will also need to adjust to enable the transformation to more sustainable practices, new modes of consultation, and in how they travel to and from their hospitals.

Delivery of the strategy is focused on improving sustainability across the five priority areas of: buildings and land, travel, goods and services, care, and communities.

DECREASING THE CARBON FOOTPRINT NHS Scotland currently has 1,250 buildings as part of its estate. A commitment to invest £10 billion over ten years, to renew, reshape and replace buildings is proposed through a programme focused on creating more resilient and sustainable healthcare facilities that use climate-considerate construction practices, have lower dependence on fossil fuels, and deliver healthier accessible internal and external environments. The impact of this work is intended to enable NHS Scotland to reduce greenhouse gas emissions from its buildings by 90% by 2040, compared to a 1990 baseline. Significant progress on this front has already been made – greenhouse gas emissions associated with building energy use has reportedly already been reduced by 63.9%, primarily as a consequence of switching from oil and coal to gas for heating, the decarbonisation of the electricity grid, and energy efficiency actions that have been taken at individual health boards.

Domestic transport is a huge contributor to Scotland’s total carbon footprint, contributing an estimated 25% of the total emissions. A high dependence on motor vehicles is the primary reason for this, with data suggesting that more than half of all car journeys taken in 2019 were under 5km – 17% of them were for journeys under 1km. A societal and cultural shift will be needed across multiple sectors to reduce our reliance on cars and move towards cleaner modes of transport – but the systems and accessibility to do this will be vital to making any progress.

NHS Scotland plans to play its part in this by changing how people access the health services they need. The strategy outlines an ambition to make it easier to walk, cycle and access public transport to NHS services, along with plans to deliver more localised community-based facilities and reducing the number of appointments individual patients need by better integration of care services. Decarbonising the NHS fleet will also play a role in reducing the carbon footprint linked to travel. The Greener NHS England programme revealed its first electric ambulance during COP26 and there is hope NHS Scotland will soon be able to do the same.

The production, supply chain, consumption, and waste linked to products and resources place a significant burden on global emissions. A shift to embracing a circular economy with a greater focus on reducing demand, reuse and repair is outlined in the strategy as the direction in which NHS Scotland aims to move forward. About £2.5 billion is currently spent annually on goods and services purchased by the Scottish health service from as many as 8,000 different suppliers. It is impossible to measure the scale of the carbon footprint created through the manufacture, transport, delivery, use and disposal of these products, but the opportunity to be selective in the suppliers NHS Scotland works with, the durability and recyclability of products, and their life span can all be considered in their aims to reduce emissions and waste. Another announcement made by the Greener NHS England programme in November was a commitment from its top eight suppliers to deliver their own net zero programmes over the next two decades. Many of the suppliers used by NHS England will also work across NHS Scotland, and there is an opportunity for Scotland to benefit from delivery of these commitments.

GREEN HEALTH AND NATURE-BASED SOLUTIONS Central to the ambition of sustainable care is a focus on re-orienting the health service to move away from one that acts as a “national illness management service” to one that is a “national health providing service”, which realises the benefits of ‘green health’ prescribing. Four Green Health Partnerships are being piloted across Scotland to develop this approach to using nature-based solutions to deliver health outcomes. A renewed focus on Realistic Medicine that delivers the combined benefit of potentially easing the pressures on the health working while also moving towards more sustainable practices is also outlined in the strategy. Minimising prescribing, use of medicines, and the establishment of a national green theatre programme are other examples of ways in which the delivery of sustainable care will be achieved.

The final priority outlined in the strategy focus on building sustainable communities by delivering initiatives that support health and wellbeing, develop resilience to climate change impacts, and engage with people in decision making.

There is clearly great scope across the service to adapt the way care is delivered to bring down emissions. Putting in place a strategic plan for delivery with realistic targets and annual reporting will be key to achieving its success. Also key will be ensuring the strategy becomes embedded across the service through its governance, structure and systems. Developing and delivering new approaches for sustainable ways of working is no longer an add-on – it needs to be core to the way in which the service operates at every level.

PHYSICIAN TRAINING AND THE PANDEMIC:

WHERE DO WE GO FROM HERE? Dr Jennifer Cathcart, Trainees’ Committee Co-Chair (Medical)

T

he Covid-19 pandemic was declared on 11 March 2020.

The global death toll stood at just over 4000 people. Six people had died in the UK of the disease. I had just finished a set of nightshifts. My speciality interview was cancelled. All restaurants were shut. All my study leave and annual leave was cancelled along with clinics and procedures.

Medical courses were cancelled. I had seen my first set of patients with Covid-19 and I was paranoid of getting it myself.

At the time I had never heard of Teams or Zoom. My story is typical of what was happening to many doctors in training across the country with slight variations.

Two years later, over five million people have died. Many of us have had coronavirus and many have had loved ones and colleagues die. We have witnessed deaths, heartbreak, survival and burnout. We have got to grips with Teams, Zoom and telemedicine. We have put on the various face masks and eyewear and been fit tested multiple times. We have read and followed ever changing new guidance on treatments, cardiac arrest management and aerosol generating procedures. We have adapted, innovated and pushed through. We now have vaccines. We now have treatments. We are now trying to carry on with our training and we keep up hope that it will return to RCPSG.AC.UK

the standard it was before the pandemic. However, increasing pressures, social distancing, waves of Covid-19 and staff absences continue to thwart training opportunities.

URGENT INVESTMENT IN TRAINING NEEDED I want to remind ourselves of the hurdles we have come through and are still facing as Trainees. The Covid-19 pandemic has had a massive impact on our training. The GMC national training survey found that burnout was high amongst trainees. No wonder: there are huge geographical variations in training within the context of Covid-19 pandemic, with some areas being harder hit than others, though no area has been unaffected. We have highlighted these findings to the various bodies that regulate and oversee medical training. There are no easy solutions to bridge gaps in training, and investment of time and money is needed. Derogations were made previously along with Covid-19 outcomes for ARCP which are to remain for now. ARCPs need to acknowledge the realities of the current state of training and should not be distant from the pressures on the ground. ARCP panels are being advised to be pragmatic to compensate for this. For higher speciality training, we are embarking on a new curriculum for general internal medicine, please find guidance for this on the JRCPTB website.

As a Trainees’ Committee we will continue to advocate for better training, more recruitment as needed, more access to LTFT (less than full time) training and better training environments.

We ask for training doctors to be seen as an essential part of the recovery plan with a balance of quantity of trainees and quality of training as equally important. The tremendous hard work of trainees must be congratulated and commended: there are many people alive today because of you and the sacrifices you have made, so thank you.

Wellbeing resources: please check them out. Our College, the BMA and the Knowledge Hub all have good wellbeing resources. Looking after your mental health comes first.

2. Educational Supervisor meetings: an individualised approach to bridging training gaps is needed. Please ensure you have had a detailed meeting with your supervisor and have a plan on how best to the approach these. This is also an opportunity to discuss problems and flag up issues with achieving curriculum outcomes which can be fed back to the TPD for solutions to be made.

3. Courses/Webinars: many are running live or online.

Please make use of them to tick off curriculum outcomes.

4. Complain: If things aren’t right then please let us know or let your supervisor know. We can only fix things if we know they are broken.

MEMBERS’AREA THE INTERNATIONAL DIPLOMA IN EXPEDITION AND WILDERNESS MEDICINE

COLLEGE STATEMENT ON THE WAR IN UKRAINE

The International Diploma in Expedition and Wilderness Medicine, led by course directors Jon Dallimore and James Moore is well underway for this academic year. The course is made up of four modules, each with a week-long residential.

Pre-pandemic, this included a week in the Atlas Mountains in Morocco for the ‘High altitude and Desert’ module. Thanks to the hard work of the course directors, faculty and our education team the residential week was able to go ahead in the stunning Picos de Europa mountain range in Spain.

The Royal College of Physicians and Surgeons of Glasgow unequivocally condemns the war in Ukraine and the targeting of civilian infrastructure which has now displaced over two million people.

As a global community of medical professionals, we abhor the loss of life and humanitarian crises in all conflict areas across the world and particularly at this time in Ukraine.

Our College is joining the UK Academy of Medical Royal Colleges in encouraging our members and the wider medical community to consider supporting the DEC Ukraine Humanitarian Appeal if they are able to do so. Funds raised through this appeal will, amongst other things, support hospitals and healthcare facilities in Ukraine.

We welcome and fully support the announcements from UK and Scottish Governments to provide humanitarian aid to Ukraine, including the urgent provision of medical equipment, supplies and drugs. We also call on all governments to do whatever they can to assist the refugees fleeing the conflict and to work to bring this unnecessary war to an end.

Mr Mike McKirdy, College President, said: My thoughts are with all people affected in Ukraine and in particular our fellow healthcare professionals working there treating casualties and civilians, in desperate circumstances, often putting themselves at risk. It is appalling to learn that the WHO have confirmed 18 attacks on healthcare facilities since the Russian invasion of Ukraine began.

10TH ANNIVERSARY DINNER I am delighted to invite you to our Faculty of Podiatric Medicine 10th Anniversary Dinner on Thursday 19 May. The dinner will be held at our College buildings in Glasgow and will be a fantastic evening celebrating 10 years of the Faculty of Podiatric Medicine. The evening will include a drinks reception followed by a three-course meal and live music.

The celebrations will continue with our Faculty of Podiatric Medicine 10th Anniversary Conference, ‘Prepared for the Future’ on 20 May. At this event we will explore the future of orthopaedics, biomechanics, vascular health and wound management. This will be a hybrid conference, with delegates able to join in either in person or online.

I have been involved in our Faculty from the beginning and have seen it grow into a strong community centred around education and support. The anniversary is a fantastic opportunity to celebrate our achievements over the past decade and look ahead to what’s next. It will also be the first chance for members of our Faculty to get together for over two years. I look forward to seeing old friends and meeting many new colleagues for the first time!

Glasgow Oral Surgery Symposium Benign Odontogenic Tumours of the Jaws Friday 22 April 2022, 9am – 5pm BST We are delighted to invite you to attend the second Glasgow Oral Surgery Symposium. This event will be delivered as a hybrid Conference, welcoming face to face and online delegates, with the programme offering up to six CPDA credits.

This year’s event will focus on the management of benign odontogenic tumours of the jaws. We have gathered five specialists nationwide who will present strategies on current management including diagnosis, investigations, surgical advances, bone preservation and minimising complications.

Lectures will cover: • Radiological interpretation including the use of advanced imaging such as CBCT/CT/MRI

• The value of a pathologist’s input in surgical decision making

• The role of conservative and radical treatment options

Focus on Neurology for PACES Candidates Wednesday 18 May 2022 Delivered by neurology registrars and consultants, this half-day course will help candidates prepare for the neurology station and potential neurology cases encountered in the MRCP(UK) PACES examination.

The course includes a mixture of short lectures and practical exam stations, mirroring stations two, three and five of the updated PACES exam. Delegates will have the opportunity to examine neurology inpatients and outpatients at each station under exam conditions, with feedback from the examiners.

Who should attend?

The course is open to IMT and ACCS doctors preparing to sit the MRCP(UK) PACES exam.
