As part of any good governance framework an organisation regularly reviews their risk register and adjusts policies and funding to ensure that, where possible, significant risks are mitigated.

Exemplified through strong leadership that is accountable to members, patient engagement at professional and organisational levels.

Like many other organisations we went into crisis mode. We stripped away all absolutely non-essential functions and focused on supporting our members through the initial frightening stages, as we saw increasing community transmission, a disproportionate infection rate among healthcare workers and decimation of community radiology services. All very alarming.

I wish to formally acknowledge the work of our Covid Taskforce. Established as a bi-Faculty working group covering public and private practices, these members worked tirelessly through the issues, as we could best define them, to deliver accurate high-quality advice to our members. They were an invaluable resource for the board and helped with many of the difficult decisions that had to be made.

Medicare, our advocacy team has been hard at work. It became clear that we needed advocacy capability to underscore our work in the standards of practice arena, where there are often competing interests with loud voices and deep pockets. It is a hard and sometimes disheartening environment, and I want to lodge my appreciation and thanks to those who continually work away promoting RANZCR’s role.

As we moved from the immediate concerns regarding keeping our members, their workmates and patients safe, we had to face up to the difficult issue of exams. Training and education, assessment and credentialing are our primary roles.

Churchill said, “Never waste a good crisis”. This seems a logical forward step from the opening quote.

This pandemic has forced us to think and act differently, and challenge ourselves to deliver on our purpose in innovative ways. Our Sydney and Wellington staff have had to learn to work from home, I have lost count of the number of Zoom meetings I have attended, and we have had to develop new ways of serving our members.

Our examination platform is the final hurdle to attaining FRANZCR, but how could we run our examinations in lockdown? We didn’t know how long this would last for, and that was before the second surge in Victoria.

Reluctantly we had to cancel the first series of examinations in 2020 which was a very difficult decision.

All RANZCR Fellows understand how stressful this time is for our trainees, but the physical welfare of our trainees, volunteers and staff had to come first.

Thanks to almost herculean efforts from our educational and assessment teams, office bearers and member volunteers we were able to schedule exams for the second half of the year. This was a technically demanding exercise and required many hours of work to configure—and I tip my hat to those responsible. At the time of writing we are just into the exam round. I wish all candidates the very best.

Of course, more has happened this last 12 months than Covid, although it is hard to imagine. This last year has seen fruit borne from our advocacy work, where we are increasingly recognised as the legitimate trusted source for medical imaging and radiation oncology advice. From the NZ Government’s decision to replace 12 linac machines and the development of a new cancer action plan, to indexation of CT, X-ray and ultrasound services under

I suspect we have only begun to scratch the surface of what can be done and the leverage we can get from developing technologies. Imagine, one day we may even have PACS systems for our radiology exams rather than old fashioned film, but let’s not get ahead of ourselves!

All that remains is for me to offer a heartfelt thanks to all of our member volunteers. Your time and dedication allow us to achieve our goals, even when faced with almost insurmountable problems such as we have seen this year. I look forward to working with you all in 2021—whatever the circumstances.

KEY ACHIEVEMENTS Face-to-Face Meetings

Indexation Advocacy

There have been 11 face-to-face meetings with Ministers and Shadow Ministers in both Australia and New Zealand discussing a variety of issues including AI, MRI access, regional and rural cancer centres, silicosis, digital health, the impact of COVID-19 on service delivery, Australian Government election commitments, specialist training positions and linac investment with the New Zealand health minister. The COVID-19 pandemic period has proved challenging with video meetings being utilised instead of the usual face-toface meetings. Health ministers have been preoccupied with the COVID-19 response, however there were many fruitful meetings with their chiefs of staff and senior advisors.

The College was instrumental in the reintroduction of Medicare indexation for all CT, diagnostic radiology and ultrasound services after a 20 year freeze

Ethical Principles for Artificial Intelligence RANZCR became the first professional peak body to develop Ethical Principles for Artificial Intelligence in Medicine, ensuring Australia and New Zealand are positioned to become leading players in the application of artificial intelligence (AI) in health care.

This covers some 90% of patient attendances to radiology departments and practices and is recognition of the value our members provide to the community.

SA Medical Imaging (SAMI) In a long overdue win for patients, the South Australian government announced that SA Medical Imaging (SAMI) will remain in public ownership as a single entity.

This followed extensive advocacy from RANZCR that privatisation may impact the trainee numbers, quality and safety of access to medical imaging services.

COVID-19 College Taskforce Established COVID-19 College Taskforce on 9 March to coordinate a whole-of-College response to the pandemic, which included proactive advocacy to health ministers, chief health officers and chief medical officers on access to PPE, appropriate use of CT, breast screening, provision of interventional radiology and interventional neuroradiology during COVID-19, radiology service provision, and continued patient access to radiology and radiation oncology. More than 655,000 people had the opportunity to see this advocacy in media stories and there were more than 20,000 visits to the College COVID-19 webpages.

New Zealand’s Health and Disability Review RANZCR provided comprehensive responses to New Zealand’s Health and Disability Review for both clinical radiology and radiation oncology.

Improved Access to Breast Imaging

Occupational Lung Disease

Recognition of the vital role diagnostic imaging plays in management of breast cancer with long-term Medicare advocacy resulting in indexation of mammography, removal of the multiple services restriction and the introduction of a new rebate for breast MRI.

Promotion of RANZCR’s Imaging of Occupational Lung Disease position statement resulted in more than 4 million opportunities to see our recommendation that CT scanning should be used for screening at-risk workers in mainstream and healthcare media.

New Zealand Government’s Cancer Action Plan RANZCR provided a comprehensive response to the New Zealand Government’s Cancer Action Plan.

Specialty Training Unit RANZCR worked through 2019 to prepare for the Australian Medical Council (AMC) re-accreditation visit in September 2019. A great deal of work has been undertaken since that date and the final report is imminent.

Played a significant role in securing multi-million-dollar investment in radiation oncology equipment in New Zealand. The Government committed to replace 12 linac machines across New Zealand.

ICT Transitioned 78 staff members to working from home arrangements in 8 days, leveraging the IT team’s preparedness for digitisation of the College with no loss in productivity. Withstood a 50 per cent increase in workload for the team over three months. The net result of the work of the IT team was a transition to a dispersed, paperless work environment with accompanying savings, which will stand the College in good stead for the future.

International Medical Graduates • IMG Assessor recruitment drive commenced in July 2019.

• Training program commenced October 2019.

• Pilot of IMG assessments via video conference commenced 2020.

• Consumer representative appointed to IMG Committee in November 2019.

Specialist Training Program • Increased engagement with the Commonwealth Department of Health, five state jurisdictions and Ministers’ Offices relating to: funding changes impacting public, metropolitan sites within the program; and workforce matters across both specialties.

• Transitioned the Director of Training Workshops to an interactive, online webinar format to ensure directors continue to be supported and upskilled in their roles.

Accreditation Increased engagement with all stakeholders associated with monitoring and implementing accreditation standards. This was demonstrated by: increased participation in accreditation visits; participation in key educational activities (both formal and informal teaching) and Network meetings; guidance of outcome of accreditation standards to support clinicians to interact in a more effective manner with executives of local health districts; and a formal education strategy developed for the NSW training networks, in consultation with the Health Education and Training Institute and Local Area Networks to be implemented in a Wide Area Network-based formal teaching program at the commencement of the 2021 clinical year.

• Successful management of the COVID-19 impact on the delivery of training programs and disruption to trainees’ experience, allowing flexibility in training through the Consideration of Special Circumstances Policy and principles and strategies set by Education and Training Committees.

• Dr Jash Agraval was appointed as the Inaugural Deputy Chief Censor for training and assessment.

• A new Trainee Liaison Officer (TLO) commenced in March 2020 with the focus on trainee wellbeing. The TLO has led an initiative to reach out to all trainees through phone calls and emails, particularly during the pandemic period. This initiative is part of the Trainee Engagement Plan to keep the dialogue open between the College and all trainees to more easily identify issues and provide support as required, facilitated by more frequent communication.

• The examiner Code of Conduct document was released.

• The 2019 S2 examinations were run successfully and proof of concept of new examination systems was completed.

• We worked with examinations panel and committees to develop options to run examiner clinical radiology and radiation oncology examinations during COVID-19 pandemic.

Both TAR Steering Committees completed the draft enhanced training program at the end of September 2019. The Committees then set in motion a series of implementation activities including stakeholder consultation, policy review, drafting of training handbooks, development of training and educational modules, and development of IT infrastructure.

Advocacy Financial Performance in 2019–2020 The Financial Statements for the year ending 30 June 2020, along with the auditor’s report are presented in this document. Detailed versions are available from the College website: www.ranzcr.com The final months of our FY2020 were of course overshadowed by the COVID-19 pandemic. The social and economic stresses as a result of COVID-19 and the necessary restrictions have been profound. Cancellation of major activities including but not limited to the Series II examinations followed by refunds had strongly impacted the College’s cashflow. The College acted quickly to bolster RANZCR’s financial position and ensure there was sufficient liquidity to operate in the uncertain economic environment.

This included an immediate focus on cash preservation, ceasing or deferring all non-essential expenditure particularly travel, and obtaining timely support from the Australian and New Zealand governments. Consequently, total FY2020 net income is a surplus of $658K for 2019–2020. The College’s retained surpluses increased to $8.7M which act as an important financial buffer during uncertain times, as well as enabling us to support a freeze on fees for members in 2020/2021 FY.

infrastructure to increase the reliability, cyber security and other capabilities like video conferencing. The Training and Assessment Reform (TAR) project was postponed to create scope to implement E-examinations. IT costs will continue to be a major expense to maintain the security, reliability of our infrastructure and delivery of rigorous E-examinations and the TAR.

The COVID-19 pandemic has been a catalyst to digital transformation, while rapidly introducing an array of new and elevated risks. While the College was already well-equipped for remote working, it prioritised the upscaling of

Our focus on Advocacy remained with persistent level of activities in lobbying governments for increased funding to support the services our members deliver. We are seeing a good return on this investment for both our members and patients alike. However, ongoing interaction with governments and stakeholders is necessary to ensure timely implementation of these changes and to bring new policies to fruition.

A great intangible aspect of our positive approach throughout the COVID-19 pandemic to Advocacy has strengthened the College’s prominent standing both with the general public and key decision-makers in government.

Program Funding The Specialist Training Program (STP) continues, however funded by excess funds from prior period. The timing of changes to STP funding exacerbated the cashflow stress due to COVID-19 and the situation is being carefully managed. The program covers the College’s overhead costs as well as produce the desired outcomes which benefit the community and our profession. The funding is expected to resume in early 2021 until the end of the current contract in December 2021.

Membership Subscriptions The Board resolved to freeze subscription fees due to the impact of COVID-19 on our membership, hence the 2020–2021 annual subscription remains at $2,795 for Fellows and Educational Affiliates. The fee will assist in covering our ongoing expenses and

will contribute to the long-term financial viability of the College.

Donations and Other Support The College is grateful for the support it received from its Fellows, members, corporate supporters and associated organisations throughout the year: detailed acknowledgement is available on page 10 of this report.

The College is also very grateful to Fellows and members who have donated to the Education and Research Fund annual appeal.

Without the generous pro bono assistance from so many members and Fellows, the cost of maintaining and growing the scope of the College would require serious fee increases.

We also thank those Fellows who have contributed their time through the voluntary work and help contain our costs in training, examinations and many other activities.

Outlook under COVID-19 The COVID-19 situation continues to evolve both locally and globally.

The impact of restrictions on regional and international travel to many of the College’s activities is severe.

Major activities like examinations, ASM and ACR courses have been cancelled resulting in inevitable financial stress.

While a solution on E-examinations is being implemented as a priority, other activities such as the AGM and elections will have to be conducted virtually which will also impact the 2020/2021 financial year.

Furthermore, investment returns are likely to be depressed with record low interest rates and volatile financial markets. The Board and Management continue to actively monitor the situation, adapt our responses and control costs as required to ensure the continuation of core activities like exams and other strategic priorities as well as safeguarding the College’s financial assets.

Responsibilities of the Responsible Entities for the financial report

Independent Auditor’s Report

The Responsible Entities of the Registered Entity are responsible for the preparation and fair presentation of the financial report in accordance with Australian Accounting Standards and the Australian Charities and Not-for-profits Commission Act, and for such internal control as the Responsible Entities determine is necessary to enable the preparation of the financial report that is free from material misstatement, whether due to fraud or error.

In preparing the financial report, the Responsible Entities are responsible for assessing the Registered Entity’s ability to continue as a going concern, disclosing, as applicable, matters related to going concern and using the going concern basis of accounting unless the Responsible Entities either intend to liquidate the Registered Entity or to cease operations, or have no realistic alternative but to do so.

Opinion We have audited the financial report of The Royal Australian and New Zealand College of Radiologists (the “Registered Entity”), which comprises the statement of financial position as at 30 June 2020, the statement of profit or loss and other comprehensive income, statement of changes in funds and statement of cash flows for the year then ended, and notes to the financial statements, including a summary of significant accounting policies and the Responsible Entities’ declaration.

Those charged with governance are responsible for overseeing the Registered Entity’s financial reporting process.

In our opinion, the financial report of The Royal Australian and New Zealand College of Radiologists has been prepared in accordance with Division 60 of the Australian Charities and Not-for-profits Commission Act 2012, including: 1.

giving a true and fair view of the Registered Entity’s financial position as at 30 June 2020 and of its financial performance for the year period then ended; and

2.

complying with Australian Accounting Standards and Division 60 of the Australian Charities and Not-for-profits Commission Regulation 2013.

Auditor’s responsibilities for the audit of the financial report Our objectives are to obtain reasonable assurance about whether the financial report as a whole is free from material misstatement, whether due to fraud or error, and to issue an auditor’s report that includes our opinion. Reasonable assurance is a high level of assurance, but is not a guarantee that an audit conducted in accordance with the Australian Auditing Standards will always detect a material misstatement when it exists. Misstatements can arise from fraud or error and are considered material if, individually or in the aggregate, they could reasonably be expected to influence the economic decisions of users taken on the basis of this financial report.

As part of an audit in accordance with the Australian Auditing Standards, we exercise professional judgement and maintain professional scepticism throughout the audit. We also:

Basis for opinion We conducted our audit in accordance with Australian Auditing Standards. Our responsibilities under those standards are further described in the Auditor’s Responsibilities for the Audit of the Financial Report section of our report. We are independent of the Registered Entity in accordance with the ethical requirements of the Accounting Professional and Ethical Standards Board’s APES 110 Code of Ethics for Professional Accountants (the Code) that are relevant to our audit of the financial report in Australia. We have also fulfilled our other ethical responsibilities in accordance with the Code.

Identify and assess the risks of material misstatement of the financial report, whether due to fraud or error, design and perform audit procedures responsive to those risks, and obtain audit evidence that is sufficient and appropriate to provide a basis for our opinion. The risk of not detecting a material misstatement resulting from fraud is higher than for one resulting from error, as fraud may involve collusion, forgery, intentional omissions, misrepresentations, or the override of internal control.

We believe that the audit evidence we have obtained is sufficient and appropriate to provide a basis for our opinion.

Obtain an understanding of internal control relevant to the audit in order to design audit procedures that are appropriate in the circumstances, but not for the purpose of expressing an opinion on the effectiveness of the Registered Entity’s internal control.

Evaluate the appropriateness of accounting policies used and the reasonableness of accounting estimates and related disclosures made by the Responsible Entities.

Conclude on the appropriateness of the Responsible Entities use of the going concern basis of accounting and, based on the audit evidence obtained, whether a material uncertainty exists related to events or conditions that may cast significant doubt on the Registered Entity’s ability to continue as a going concern. If we conclude that a material uncertainty exists, we are required to draw attention in our auditor’s report to the related disclosures in the financial report or, if such disclosures are inadequate, to modify our opinion. Our conclusions are based on the audit evidence obtained up to the date of our auditor’s report. However, future events or conditions may cause the Registered Entity to cease to continue as a going concern.

Evaluate the overall presentation, structure and content of the financial report, including the disclosures, and whether the financial report represents the underlying transactions and events in a manner that achieves fair presentation.

Emphasis of matter – COVID-19 We draw attention to Note 21 of the financial report, which describes the circumstances relating to the material subsequent events regarding COVID-19 and the uncertainty surrounding any future potential financial impact. Our opinion is not modified in relation to this matter.

In the financial year reported on, 2019 - 2020, the College had one Appeal that went to the Appeals Committee for a hearing on 15 August 2019. An IMG candidate appealed the result of one of their radiology viva exams. The ‘fail’ examination result was upheld by the Appeals Committee.
