To Obtain or Renew a Tourist Passport:
Completed passport application (DS 11 or DS 82) with 2D barcode printed on letter size paper and one sided (Go to https://pptform.state.gov to complete online see instructions on right side).
“Original” or “Certified Copy” citizenship and name change documents (if applicable): i.e. birth certificate, consular report of birth abroad, naturalization certificate, certificate of citizenship, marriage certificate, divorce decree, adoption decree, legal name change document, court order, etc.
All current or expiring passports (if applicable). A no fee passport may be used to prove citizenship if applicant never had a tourist passport.
Military CAC/ID (for parents of children under 16, include copies of both parent ’s ID cards).
Two 2x2” or 5x5 cm identical passport photos (not older than 6 months; no eye glasses permitted).
