IMPORTANCE OF A HEALTHY LIVER

The liver is one of the most important organs in the human body, and one of the hardest working. Helping our liver to function well is of huge importance to our physical and mental wellbeing.

By Cathriona Murphy Tralee woman, Elaine Burrows Dillane, has fast become one of Irelands most decorated sea swimmers. From Fenit to the Maharees to the exceptionally dangerous English Channel, it seems nothing is beyond her capabilities. A fact recognised later this month with her induction into the Irish Marathon Swimming Hall of Fame. I sat with Elaine to chat about how, in 6 years, she turned from stay at home mom to a world class athlete.

Elaine, it’s a pleasure to get to chat with you today given your busy schedule, I’m sure our readers will be very excited to hear all about your journey! Can you tell us where your love of swimming came from and how it transpired to where you are today?

My love for swimming came from my father, he wasn’t a major swimmer himself but he always had us in the water from a very young age. When we would go on holidays he would have us straight in the water, jumping off his shoulders into the sea from 4 or 5 years of age. From there I progressed into swimming lessons in the Tralee Sports Complex with Michael O Regan and Michael Moss. Michael O Regan saw something in me and went to my mother and asked would she sign me up to the newly started Kingdom Swimming Club which I did. I continued to swim competitively up until the age of 17 but my heart was never really in competitive swimming, I worked myself up too much with it.

I eventually gave up swimming around the time of my Leaving Cert and didn’t return to it until around 6 years ago, it was more a case of I was enticed back! Two of my good friends, John Brick and Cathriona Chester, they were at me to go out into the sea with them and after having 2 kids and away from swimming for so long I was nervous! It’s not that easy to get into a swimming togs and decide that you’re going to get in and start swimming again so I bought a wetsuit. I think I wore it maybe 3 or 4 times and then just decided it wasn’t for me, I wore my swimming togs from then on in and absolutely loved it, I knew this was where I was meant to be.

Where did the idea come from to swim The English Channel?

Just like anybody else, I took it slowly because I hadn’t swam in so long. I started by swimming out to the first buoy and back, gradually increasing my distance over time. I then knew I had to set goals and that’s where my fundraiser came in. When I was getting married, my page boy was diagnosed with an inoperable brain tumour and he was only 3 years old, it was a heart-breaking time for us all. I promised that if he got well, one day I would attempt the English Channel to raise money for the charity, Brain Tumour Ireland. He is actually 30 years of age now and doing brilliantly. So, in 2019 I swam the English Channel (33km) and I raised over €21,000 for Brain Tumour Ireland by the time I was finished the swim.

How did you cope with the mentally challenging aspect to your training and swims?

Mentally, it really is all a learning curve? Everyone has different challenges and I am no different to anyone else. When I decided I was going to swim the English Channel it was 2 years of preparation, training to build up to something as big as that.

I wanted to set goals for myself every year, in 2018 I swam Blennerville to the Maherees and back and finally in 2019 I swam the English Channel. So reflecting back, it was all progression but it was a big challenge mentally to build up to.

and I didn’t hear my name mentioned, when they got the 5th, 4th I just thought they had forgotten me or something. To hear my name in second place, I was in absolute shock, I never imagined I would achieve something like that.

Do you find it difficult managing family time with training and competing?

During the winter it’s okay because its more maintenance so I can get that done when the kids are in school but in the summer of course it probably is harder on them because they are off on their summer holidays and that is when I am in peak training season for the winter swims. I’m in and out of Fenit 24/7 doing 6 hour swims, I’m tired when I come home so it does be hard on them but in fairness to them they have always been great about it and extremely supportive.

With the Channel swim you have to be extremely focused, very selfish in that you are putting all your time and effort into it to try and get everything correct. But really I am so lucky to have such a good team around me, they will tell me when I need to turn around from Fenit carpark and go home to have a rest! Of course, like with anything in life, you will find it hard, you will have a meltdown but they are all there for me which is just fantastic, I am honestly so lucky with them all. It's better though in the long run for me to swim alone in that I have to prepare myself mentally for the challenges I do and a huge part of that is preparing for that time spent on my own in the ocean. I have to manage my feeds which are liquid carbs, I tie them on to the buoy out in Fenit when I’m training so I don’t have to go back out on to land during the 6 hour training session. Each feed is 175mls which I have to drink in 20-30seconds to keep swimming consistently and learning to space them out too because your stomach has to get used to the feeds too!

What does a typical week of training look like for you?

In a week I would be looking at roughly 50-60 km. It’s more about time in the water, so the way that I work it is that I might spend 4 hard days in the water and then have some rest days too. In a typical 4 hour swim I would cover maybe 16km, a 5 hour swim would be 20km and it all just depends how much training you have done in a particular week really. Some weeks will be heavy, other weeks it will ease off but it all essentially comes down to the hours spent in the water. I’m currently starting in the pool training because the temperature in the sea is slowly dipping, but I’ll get back into the sea from time to time to stay climatised. It is very important that you do really climatise yourself for example, when I swam the English Channel it was 18 degrees and beautiful, but for my North Channel swim it was between 11-13 degrees which is the equivalent to winter swimming. I was 14 hours 54 mins in that for the North Channel You recently came in 2nd place in the Women’s World Swimmer of the Year, a mighty achievement by anyone’s standard! How has that felt for you, to be recognised on a global scale?

I was absolutely blown away by my nomination, it came out of nowhere really? I had just completed the Irish triple crown and it was during Covid, I will always say Covid was good to me in the sense that it allowed me all this time to train in the sea because there was no pools open at the time. This suited me great because I was training for the North Channel swim.

So for the open water nominations I had done the North Channel, Fastnet and Galway Bay so that gave me the Irish Triple Crown all in 11 months and that was probably why I was nominated for it. The award itself was announced live on YouTube so I tuned in to watch, it started with a countdown 8

Have there been moments out there in the water where you thought things might not go to plan or could perhaps become dangerous?

That’s a good question, you know I’ve never really felt afraid out there in the water or had thoughts that something might go wrong. I’ve had dolphins, seals everything you name it and I’ve seen it out on my swims. There was actually a seal that swam the whole way to France with me, when I did Fastnet I had Dolphins all day and never felt worried by their presence. It would always be in the back of your head that something can go wrong but you just have to think very positively because if you have any doubt at all in your head, you just won’t finish the swim. As long as you don’t have a doubt and you believe in yourself and most of all, believe in your team, you can accomplish anything. For example for the North Channel I started off from Scotland in the dark so I had to rely on my own team and the Northern Ireland team (Infinity Channel Swimming) with torches for the first 4 hours because of just how dark it was leading to poor visibility, the North Channel is infested with Lions mane Jellyfish so I had to have a trust there between myself and the team to watch out and guide me.

Finally Elaine, we here at Connect and on behalf of our readers would love to congratulate you on your upcoming induction into the Irish Marathon Swimming Hall of Fame, how has that felt for you since it was announced?

Thank you! I am absolutely thrilled and so honoured at my induction to the Ireland Marathon Swimming Hall of Fame at the ILDSA Awards in Mullingar this month. To come so far in 6 short years is just beyond my wildest dreams and I am forever thankful to everyone that helped and supported me along the way from my team to my family and everyone in between, thank you. It will be an honour and a privilege to be up there in the Hall of Fame in such good company, but most particularly Kevin Williams. I am so proud to represent Kerry along with Kevin as the only two Kerry people in it and I am very excited to see who joins us in the hall of fame over the years to come.

Marie Carroll O’Sullivan on going beneath the mask of a pandemic to raise over €21,000

When Covid hit and we were placed under our first lockdown, it gave us all a moment of clarity through the fog of fear we found ourselves under. Learning how to reconnect with loved ones through the most creative ways, it gave Killarney photographer, Marie Carroll-O’ Sullivan an idea that would set her on a path to becoming a published author. I sat with Marie to talk about “Killarney- Behind the Mask” Marie, to take on such a huge commitment and project would take fierce determination. Where did your love of photography begin?

I am originally from Roscrea in North Tipperary. I moved to Killarney and at the time my job was hospitality based, working in local hotels for a number of years. I started to develop a keen interest in photography and as I went on to have my children it just really suited my life more. I got a bit of press work and got my name out there and that is how my business, The Little Memory Gallery came into fruition, kick starting my love and passion for photography.

Where did the idea for the book come from?

I never really thought about or considered doing a book, it was just something that gradually happened. When the first lockdown hit and we all had to stay within our immediate area, well I’m just not a person who is very good at sitting still! I decided that I would use my platform to visit gardens, luckily I am living in Killarney town centre so I was very central to a lot of different densely populated areas. Some people had parents living in the town while they themselves where living out in Rathmore or beyond and couldn’t travel to see them, but I could. I got this idea that I could go visit people in their gardens and photograph them to post on my social media so their loved ones could see them. So from that, it more or less grew legs of its own! I was visiting 3 or 4 gardens a day photographing individuals and posting them online on my social media. I was doing nothing else so I thought it was a good way for me to be able to do something worthwhile and help others. The more I did it, the more it turned into so much more. People had stories to tell about how they were coping and I listened to them, documenting everything until I realised just how much I had stored on my computer and it understood the historical importance of the information and that is where the idea of the book started.

How did you decide what you would do with the funds and why?

Initially I had thought about just doing the one for myself but then I thought if I could get the financing to print more, I could do so much good with the funds. I put it out there on social media and the response was just phenomenal. Garda Diane Collins and Garda Paudie Twohig both got involved in their own way straight away which helped massively. Paudie was diagnosed with cancer at the time of becoming involved and had chosen to do so given that the first charity I wanted to get involved with was the Irish cancer society. To give his permission for his wife Diane to tell his story on platforms such as the Ryan Tubridy Show, all whilst he was fighting a courageous battle just so that he could help bring awareness to the book through his wife, it was nothing short of amazing. Unfortunately we were all extremely devastated when Paudie lost his battle and will forever be grateful to himself and Diane for lending their story to bring light to my book. When it came to choosing the two other charities I wanted to honour charities that represented prominent issues in the pandemic. The two I chose were Pieta house who helped so many people who struggled with mental health and the Covid ICU staff of UHK. I met with the clinical nurse manager, Margaret Griffin who gave me the list from the top down to the tea lady and that’s what I wanted, to acknowledge everyone I could that were on the front line, not managers or top tier staff.

Can you talk to us about the launch of the book and what was involved?

When I gathered everything I put it on a timeline from January to December so that each part would reflect the restrictions that were relevant to that time. I took it all in baby steps, it was such a huge learning curve for me because I hadn’t a clue about publishing so I took the whole process one step at a time and I really was so lucky it was received and supported so well by the local community.

I launched it in December 2021 at The Killarney Avenue Hotel who so kindly provided the room for the event. I also chose the Brícín Restaurant as Johnny McGuire was the first to dig deep when Behind the Mask was merely a notion. O'Donoghue Ring Hotels, specifically Gemma Ring and Manager Denis McCarthy at the Killarney Avenue Hotel who provided the venue and all the trimmings free of charge on launch day. And the International

Hotel as their sister business Eason Killarney, owned

by Tanya Coyne O'Shea, facilitated the sale of Behind the Mask commission free, most substantially. Initially backed with thousands of euros of generous donations in the making, I was in turn backed by the very talented Sinead Quinn of Design by Sinead who carefully displayed my work so beautifully.

The equally talented John O’Mahony, O’Mahony Media assisted me night, noon and morning through the tireless process of editing together with sound advice throughout. I had ordered 2000 copies of the book hoping that it would sell and as of now I have approximately ¾ of them sold at a cost of €35 per book. My brother has been selling copies in his shop in Roscrea, Co Tipperary, Eason’s in Killarney have it in stock, Supervalu in Tralee, Carry Out and Kerry Catering supplies stock it also. I also have copies available on my website www.thelittlememorygallery.com and after some consideration I have decided not to do a reprint so any remaining copies are all that will be available to purchase. I feel it is better to keep it as a collector’s item, a piece of history that documents one of the most challenging times our generation has faced. So with that in mind, I sent a copy up to the library in Trinity College which they were thrilled with. It is also in the National Library of Ireland in addition to also being available in the libraries throughout Kerry.

Finally, Marie, as we go to print you have presented the 3 charities with a large sum of money in the form of vouchers, can you tell us about that?

In the end the total funds to date have amounted to €21,300 which I am so proud of and so thankful to everyone that purchased or helped in any way. To be able to give UHK over €7,000 in gift vouchers and the other two charities over €7000 in a cheque donation each, was just an incredible feeling. I purchased vouchers from three Killarney establishments who supported me from day one - Brícín Restaurant Killarney, O’Donoghue Ring Hotels & the International Hotel Killarney.

Each voucher was to the value of €80 unless they decide to use at the Killarney Plaza Hotel where an O'Donoghue Ring Hotel voucher will almost double in value, at the hotels Spa for a one hour treatment of their choice. A fabulous gesture from O’Donoghue Ring Hotels. I was in awe of everyone’s willingness to help me put this plan into action and so proud of the end result.

Ask the Pharmacist

In Novembers edition of Ask the Pharmacist, Mounthawk Pharmacy's Pat discusses the prevalence and management of pain in community pharmacy and expands on the options available for self treatment and when it is time to see your doctor.

Pain is defined as "an unpleasant sensory and emotional experience associated with actual or potential tissue damage". Pain may be a symptom of a condition, for example a twisted ankle or an infected toe. In these instances the pain is acute and can be significant. It serves a protective function, in that you

will seek to remedy the source of the pain so as to avoid feeling more pain down the road. In other cases, people may suffer from chronic pain and this is a more complex issue and will necessitate a trip to your GP for further investigations. Chronic pain should be treated more as a disease than as a symptom in its own right. In the following paragraphs, I will detail the various treatments available over the counter to treat pain, explain how they work and also I hope to highlight those areas when it is no longer clinically appropriate to treat your pain over the counter.

Paracetamol is the most popular pain reliever available and is generally very well tolerated. It is present in many well known brands and as it can be dangerous for the liver to take too much, it is very important to check the ingredients of any medicines before taking them in case there is a risk of "doubling up".

Paracetamol is found in the following products-Panadol, Calpol, Uniflu, Benylin 4 Flu/ Day & Night, Sinutab,, Lemsip among many others. As you can see it is extremely prevalent in numerous cold preparations and unfortunately paracetamol poisoning is a very real danger if people are not careful in dosing. No more than 4gm of paracetamol can be taken by an adult in 24 hours and a single Lemsip Max sachet contains 1gm so only 4 of these may be used in 24 hours. Always read the dosage schedule and ingredient list of any medication you are taking to make sure that you are taking the recommended dose and if in doubt ask your pharmacy team.

Paracetamol comes in a range of dosage forms, tablets are most common but it also comes in liquid form (Calpol for example) . A drug in liquid form will always be absorbed quicker than in tablet form so this is why products such as Solpa-Extra are so effective and appear to act quicker than other preparations.

For those who may not be able to ingest an oral medicine, whether that be a result of constant vomiting or somebody who is nil by mouth receiving end of life care, suppositories are available and are a quick and effective way of providing analgesia. These are available in a range of strengths and are suitable for children and adults when the oral route is not an option. Ask in store which product is most suitable for your specific needs.

The other major painkiller available over the counter is Ibuprofen and this is found in such products as Nurofen, Advil, Brupro, Sudapro

and others. This is a powerful anti-inflammatory medication and is particularly good for sports related injuries, reducing temperature and for period pain. Ibuprofen belongs to a class of medication called NSAIDS and when used for a short period of time is a safe and highly effective painkiller. Unfortunately Ibuprofen is not as well tolerated as paracetamol and can affect the stomach lining.

For this reason we recommend that people take it with food. It can also aggrevate asthma in certain individuals so it should he used with caution in those with breathing problems.

Over the counter there is also Voltarol gel available and this contains a strong NSAID called diclofenac and can be really effective for sore joints. All NSAIDs increase the risk of a bleed and this is very important to remember if you are taking blood thinning medication.

There are several other options available over the counter to manage pain and some of them are drug-free. Pain and heat/cold often travel along the same nerve endings as pain so sometimes applying heat or cold to the area can help in pain relief. This is the principle behind heat patches applied to sore backs and cooling forehead patches applied to migraine sufferers. These can often be as effective if not more so than taking painkillers and are another option.

Of course paracetamol and Ibuprofen are only the tip of the iceberg when it comes to painkillers.

There are far more potent ones available on prescription and you will need to be assessed by your doctor before these can be issued. This highlights the importance of distinguishing between acute and chronic pain and the need to seek medical help if need be. There is absolutely no justification for taking a strong painkiller for a niggle and conversely there is no need to suffer from the agony of a slipped disk in your back if powerful painkillers are available. Always discuss your problems with your pharmacist or doctor who will be happy to guide you along the right path.

A final point of note that needs to be borne in mind when using strong opioid painkillers is their potential for misuse. Codeine is merely a weak version of morphine but taken in high doses can cause a range of unpleasant side effects ranging from constipation to dependence. It is for this reason that products that contain codeine are restricted and that people are warned that they should only be taken for as long as is necessary and never for longer than 3 days. These are powerful medications and can have real long term effects on body systems if taken for long periods.

So always try to stick to the recommended dose and do not take products like Nurofen Plus or Solpadeine OTC for any period longer than 3 days. The risk of overdose, stomach ulceration, acute liver failure etc are real. If you find yourself in a situation where you feel you may have a dependence problem on these or any other medication, please do not hesitate to come in to us where we will gladly instruct you in strictest confidence of help and supports you can access to help you along.

Nora's Kitchen Recipes Keeping you Slim for Autumn

Ham and Mushroom Toasties

Featuring rich herby tomatoes and melted mozzarella, our Italian-inspired toast topper is a slim-safe way to grab a pizza the action!

Ingredients: •Low-calorie cooking spray •1 small onion, finely chopped •2 garlic cloves, finely chopped •400g can of chopped tomatoes •1 tsp dried mixed herbs •8 slices wholemeal bread from a small 400g loaf •8 slices lean wafer-thin ham, visible fat removed •100g button mushrooms, thinly sliced •200g mozzarella, grated •A handful of fresh basil, to serve (optional)

Method Place a small saucepan sprayed with low-calorie cooking spray over medium heat. Add the onion and cook for 3-4 minutes, stirring, until well softened and beginning to brown.

Add the garlic, tomatoes and half the dried herbs. Bring to a gentle simmer and cook for 15-18 minutes, stirring occasionally, until the sauce is very thick (take care that it doesn’t burn, though).

Italian Chicken and Tomato Soup

Preheat your grill to hot. Grill the bread slices on one side, then turn them over and spread them with the tomato sauce. Top with the ham and mushrooms and spray with lowcalorie cooking spray.

Sprinkle over the mozzarella and the remaining dried herbs. Grill for 3-4 minutes, until the mushrooms have softened and the cheese has melted.

Season to taste and serve with the basil scattered over, if using.

This lovely soup makes a filling lunch thatʼs ready in just over half an hour Ingredients: •1 onion, finely chopped •2 carrots, grated •2 lean back bacon rashers, visible fat removed, finely chopped •2 large skinless and boneless chicken breasts, cut into thin strips •600g canned chopped tomatoes •400ml hot chicken stock •1 tbsp chopped rosemary •Finely grated zest and juice of 1 unwaxed lemon Chopped fresh parsley, to serve.

Method Place the onion, carrots and bacon in a large non-stick saucepan and stir-fry for 2-3 minutes.

Add the chicken, tomatoes, stock, rosemary and lemon zest and juice. Bring to a boil, stir, then cover and simmer on medium heat for 15 minutes, stirring occasionally. Season to taste and serve sprinkled with chopped parsley.

Implications for Dental Treatment from Cancer and Osteoporosis Drugs Medication - related osteornecrosis of the jaws (MRONJ) is a serious condition that can affect patients on certain medications. This is a potentially severe adverse drug reaction, resulting in progressive bone destruction of the jaws. Patients on certain medications for osteoporosis or cancer treatments are most at risk.

This condition can occur if patients on certain medications have a tooth removed or some other surgical dental procedure in the mouth. The bone fails to heal which results in a very sore open wound in which the jaw bone is exposed and it is very difficult to get the area healing again.

It is very important that patients who are about to start treatments for cancer or osteoporosis should always attend a dentist for a complete dental examination. This will allow any dental treatments to be carried out prior to starting their medication for cancer or osteoporosis. Patients should always consult with their doctor whether certain medications may put them at risk of MRONJ in the future, so that they can bring this to the attention of their dentist prior to treatment.

Kilmoyley woman, Maeve Barry brings a piece of the Kingdom to The National Art Gallery Hailing from the heart of Kilmoyley, Maeve Barry has forged a career as one of Irelands top PR and Communications expert. With no end to her talents, Maeve has recently delved into the world of producing hand crafted jewellery. What began as a hobby in the evenings has now less than 2 years later landed her a prime spot in the prestigious National Art Gallery of Ireland. I recently sat with Maeve and caught up with her during this whirlwind time!

Maeve, with such a busy and successful career in PR, what prompted you to take the step into jewellery design and production?

It had been an idea of mine for years and like everyone else, I procrastinated over it for a very long time! So I suppose it really started with making pieces for myself and people would ask me about them when I wore them out. I decided to make a small collection and that launched 2 years ago in a shop called Om Diva on Drury Street in Dublin. They have an Irish design section upstairs called Atelier 27 so I went into the Christmas Market there just to give it a road test and see how the pieces worked. I got a feel for what worked and what didn’t and went on to launch my line, my day job is in PR so that really helped in terms of knowing how to promote and market my products. It grew slowly which suited me in terms of maintaining that and my full time job, I planned to go into the shops for this coming Christmas season. I got a call a few months ago from The National Gallery they contacted me and offered me an opportunity to stock my products in the shop. They were on my wish list of places to approach with the hopes of securing a product placement there but to have them come to me was just amazing. Myself and 2 other designers, Margaret O Connor from Clare and Kikinaart, the 3 of us launched on the same night because the gallery wanted to showcase more female Irish designers. The fact that I had built my brand slowly worked in my favour because the National Gallery didn’t want someone who had products everywhere, they wanted someone new fresh and exclusive.

Where do you go from such a successful start?! Have you plans on what direction you would like to take it in?

I have a lot of plans and ideas to do some collaborations with other people with products besides jewellery. The good thing about being Dublin based is that I have so much access to so many amazing female designers and artists and people to collaborate with which I am hoping will be on the cards for next year. I also want to launch new collections in the next year and also to go into more stores before Christmas too. The response I have received from the public and the media, especially local media has just been fantastic, I’m blown away by it all.

Your pieces are very colourful and contemporary, where do you draw your inspiration from?

When I was a child I used to make my own jewellery and sometimes would use clay in the process, this has come full circle for me now as some of my

current collection contains items made of clay. I take inspiration from a lot of things really but particularly my Perspex pieces are influenced and inspired by a Spanish artist called Joan Miró.

His work is very abstract with a lot of primary colours which is what I based my current collection on. But even in my college years I was always fascinated with Art and design, leading into my PR career I have a passion for all things fashion. Anything to do with fashion from clothes, accessories, jewellery, I’m a bit of a magpie really! So it’s fair to say I really do draw my inspiration from fashion magazines, people on the street and art.

Are you conscious of what materials you use and remaining as environmentally friendly as you can?

Sustainability is hugely important to me. The mirrored Perspex that I use is recycled plastic, it’s a material that can get such a bad rap but that is single use plastic. The plastic I use can be worn over and over again for decades even so that brings in the sustainability element. I think the message is really that not all plastics are bad, single use obviously is but if it’s something that can be recycled or used over and over again well then I am happy to use it in my products and design.

When people see the words hand crafted they can sometimes assume this implies a high cost, what are your price points currently?

They start at €45-€75 so its not crazy expensive! Obviously though I am conscious of the cost of living crisis and everyone is cutting back, unfortunately I can’t source mirrored Perspex here in Ireland, I can only get it in the UK plus the taxes I have to pay also so it all adds up! Even down to presenting the piece to your customer, whether it’s in a shop or an online order you want it look as pretty as possible.

It’s important to me that when someone goes online and orders a product that they receive it in a pretty box with a nice card inside it thanking them for their purchase and those extras cost money too so I do try to keep the cost as low as I can so as not to price anyone out of my products. It has to be fair for the customer but fair for me also too.

I feel its important that anyone who wants to own a beautiful piece of Irish handmade jewellery should be able to do so without too much expense.
