AT-HOME CARE Program Overview The MI Choice Waiver provides long -term support and services within the comfort of your own home. Our staff work with you and your family or friends to give you more control over the care you receive. At no cost, the MI Choice Waiver Program gives qualifying aging adults and adults with disabilities a choice over their care.

Eligibility
Adults 18 and older must meet financial and medical eligibility in order to enroll in the program.
Services Provided
Participants must need Supports Coordination and one other MI Choice Waiver service. Eligible services include: Adult Day Care Chore Services Community Living Support Counseling Home Modifications Home Delivered Meals
Medical Equipment Nursing Services Personal Emergency Response System Respite Care and much more.
