CLINICIAN’S CORNER Comparing Pain-Free v. Pain-Threshold Rehabilitation for Acute Hamstring Injury and Return-to-Play Clearance By: Cody Morrissey, PT, DPT, East Hampton PT

Hamstring injury is among the most prevalent sport-related injuries, typically resulting in a significant amount of time removed from activity.

Physical therapy intervention is recommended to improve strength (isometric, concentric, and eccentric) in order to return-to-play (RTP) with a reduced risk of re-injury.

Hickey et al., performed a randomized control trial comparing a standard rehabilitation protocol for hamstring injury with a pain-free group and pain-threshold group. Forty-three patients were randomly assigned to the pain-free group (22) or pain-threshold group (21). The primary measure assessed was days to RTP, with secondary measures assessing isometric knee flexor strength and re-injury occurrence at 6-month follow-up.

The rehab protocol included a variation of exercises targeting the hamstring muscle groups with varying angles. A 9-stage progressing running program

was also incorporated into the rehab protocol. The two groups worked at different levels of intensity solely based on pain limits, however, the intensity level was not specifically defined. The rehab protocol was completed when the patient met all of the RTP criteria.

Results of the study showed no statistical difference between groups in the primary outcome measure, days to RTP, as the pain-free group returned within a median time frame of 15 days and the pain-threshold group with a median time frame of 17 days. However, the pain-threshold group displayed increased isometric knee flexor strength by 15% at RTP and 2-month follow-up.

Therefore, pain-threshold rehab for acute hamstring injury did not accelerate RTP clearance. There were two reinjuries in both the pain-free and pain-threshold groups when assessed at 6-month follow-up.

Based on the results of this study, clinicians should feel safe when having patients with acute hamstring injury exercise within limitations of pain during treatment sessions without the risk of further injury. However, if a patient is highly irritable and exhibits a high-level of pain it may be more beneficial to perform exercises in a pain-free range. Clinicians should educate their patients on the efficacy of physical therapy intervention in the acute phase of hamstring injury in order to return-to-play.

Time Off Refresher

Vacation Time - Use it or lose it!

Employees will lose any unused accrued vacation hours by his/her reset date (employee’s anniversary date). Unused vacation hours will be deleted on that date.

Full time employees are allowed to use up to 40 hours vacation before the hours are actually accrued. Part time employees are allowed to use up to 20 hours vacation before the hours are actually accrued. The balance will be displayed as a negative. The system will not allow entry of vacation requests if hours exceed -40 or -20.

Sick Time - Roll it over!

Employee accrued sick hours will rollover each year up to a maximum of 160 hours for FT, 120 hours for PT 30 and 80 hours for PT20 employees.

Holiday Time off Policy The accrual will remain the same for all employment categories however the total hours will include hours to use as personal hours.

The updated holiday time off accrual schedule is listed below: Full Time (FT) Part Time 30 (PT 30) Part Time 20 (PT 20)

Spot light Dominique Riley Shelton Patient Services Coordinator

History

Dominique has been with PTSMC for five years. She currently lives in New Haven, which is also where she grew up. Dominique started at Eastern Connecticut State University, but once there, knew it was not the right place for her. She transferred to the University of North Carolina at Charlotte, and she says it was one of the best decisions she’s made in her life. In Charlotte, far from home, Dominique enjoyed her newfound independence, learned life skills and resourcefulness, and met some of her best friends who are still in her life today.

With a degree in Exercise Science, Dominique envisioned going “the PT route.” In March 2017, Dominique joined PTSMC Orange to gain experience in the field. During her time as a PT Aide, she was also working as a Rehab Aide at St Vincent’s and at QU supporting medical personnel. Definitely a busy start to her career! Within the first few months in Orange, Dominique was doing some light front desk support, and that role grew into jumping in to help whenever the front desk needed it.

In June of 2021, when the new PTSMC Shelton clinic opened, Dominique joined former Orange colleague Nick Almonte. Dominique says she was ready and willing to jump into the role because she believes you’ve got to continue to challenge yourself to see what skills you can learn. She approaches every opportunity with this mindset and always takes the approach of “tell me how to do it and I’ll do it!” One area of being a PSC that she loves is the intake process, especially meeting new patients.

Dominique also acknowledges that, being part of a PTSMC clinic startup, she and Nick went through the “grind” together. She says, while it was a humbling experience, she was able to learn what’s expected from the business end, such as understanding clinic targets and goals.

Dominique wanted to give credit to Nicole Skirkanich, Orange PSA, Stef Cagno, Verification Representative, and Vicki Palmieri, Authorization Representative, for being so helpful and supportive in her transition and growth as PSC for Shelton!

More About Dominique

Dominique LOVES music. It keeps her going every day, and with her eclectic taste, she listens to a bit of everything...including country music just at the "surface level."

Dominique is an outgoing person who loves being out with friends and is very close with her family. She also loves to watch sports, especially in person, where you can feel the energy and passion of the home team fans! She watches NFL the most. When asked who her favorite team is, she laughed and explained, “I’m a complicated soul.... I cheer for the Giants [mom’s favorite], the Vikings [dad and brothers’ favorite] and the Panthers [North Carolina connection]. I also like Tom Brady now that he’s not a Patriot. Basically, anyone who’s not the Cowboys!” A highlight of her year was when she and her family went to Baltimore last year to watch the Vikings play the Ravens.

What is next for Dominique? She is researching going back to school for a master's degree in public health.

Ultimately, her experience over the last few years in healthcare has taught her that she loves to help people get better. While the PT route is no longer her calling, helping people understand their options and the programs available to them in healthcare speaks to her. Through her experience with PTSMC, she believes people across all demographics can use that type of support. She explains, “whether they realize it or not, most people need help,” and she wants to be the one making sure people are being taken care of!

Congratulations to Carol Corbo!

Carol has been promoted to Patient Services Administrator in our Waterbury clinic. A long-time employee, Carol is often called “the glue” in the Waterbury clinic. Carol will be responsible for PT Aide and front desk schedules and coverage, training all new PT Aides, overseeing inventory and sales, managing building and equipment maintenance, and maintaining the Waterbury clinic’s community presence and social media presence.

Partner Erin Walsh says, “Carol has been more than a PT Aide for our clinic, and I am thrilled she accepted this opportunity. Carol is amazing with patients. So many patients have returned over the years and inevitably, they all remember Carol and she remembers them! In addition to her customer-centric ways, Carol also brings amazing organization, institutional knowledge and a ‘can do’ attitude to everyone on staff. On the personal side, Carol recently learned she has a biological brother. Her brother was recently told by his aunt that their mom gave up a baby when she was 18 (Carol). He tracked Carol down with the help of Catholic Family Charities. Carol and her newfound brother have met once. In her free time, Carol enjoys oil painting. Carol is excited to take on more responsibility in her new role, assisting Erin with additional duties on the administrative side of the clinic. She looks forward to learning more about the front desk operations in order to help the office run more efficiently.

2022 401(k)Plan Changes

Fund Addition: New World Fund Added the option to invest in the Emerging Markets asset class.

The passive target date fund option is changing from Vanguard to State Street.

The funds are very similar and have performed about the same. They are both low-cost passive target date funds.

If you are currently in the Vanguard option, it will automatically be transferred to the State Street option with the same target retirement date.

Primary reason for the change: LOWER COST Expense Ratio Vanguard State Street

Target Date Fund Options: A target date fund is an investment option that chooses an appropriate allocation to stocks and bonds based on the year of your “target” retirement date.

PTSMC has 2 target date options: Active option ~ John Hancock • Fund manager makes more tactical decisions based on market conditions • Higher fund expense ratio of 0.43% Passive option ~ State Street • Index approach with less changes to the allocations • Lower expense ratio of 0.07% Both are great options. Choice comes down to if you prefer a lower fee or a more active investment approach.

Historically the active fund has performed slightly better, but that is no indication of future performance.

Bear Mountain – Salisbury, CT: Bear Mountain is the highest mountain that lies wholly within the state of CT. A portion of the Appalachian trail runs through the mountain, offering a moderately changeling 6.1 mile loop.

Farmington Canal Heritage Greenway – New Haven, CT to Northampton, MA: Grab your bike or running shoes and hit the trail. An ongoing project throughout CT much of this trail has been developed as a paved trail for non-motorized recreation and commuting. the history of the #GetMoving challenge?! This time, 104 employees committed to reaching personal goals - making up 26 different teams competing to be the ultimate champions.

So far the group is trying to get outside and get active. Also bonus points are now up for grabs when teammates #getmoving together!

Brag Recipient: Alex Gauthier, PT Essex Bragger: Anonymous Alex became part of the Essex team approximately 1 year ago. He has managed to seamlessly integrate into a group of well established, experienced therapists. Alex is always positive, extremely efficient, consistently and quietly getting his job done. His demeanor is mature and beyond his years, an "old soul". Essex is fortunate to include him as part of the team.

Brag Recipient: Christina Mogelnicki, Newington Director Bragger: Janet Tarasuk, Practice Liaison Christina Mogelnicki is such a team player, willing to contribute to projects that are companywide! She created a guide for clinicians to utilize when connecting with providerS for the first time. I appreciate Christina’s unsolicited feedback on the interactions she has had with providers in the field, which is where the idea for a guide came from. I am impressed with Christina’s ability to rise to the occasion and step into a leadership role at Newington. Brag Recipient: Alison Pearce Bragger: Kristen Forster I am so excited to be able to BRAG about Alison! I am lucky to call her a colleague and friend. She is always there to answer questions, jump in and help, give great advice, and she TRULY cares so much about the people she works with. Brag Recipient: Carole Pimenta, PSA Guilford Bragger: Anonymous Carole goes above and beyond for her clinic and staff members. She is always positive with a smile or a laugh and somehow manages her office with grace.

She is always a pleasure to interact with and cares very much about the people she works with, staff members, admin members and patients. She is a true team player! We are very lucky to have her at PTSMC.

Brag Recipients: Christina Mogelnicki, Newington Director and Roy Colter, Newington PT Bragger: Anonymous With changes at the Newington clinic, I'm proud to see how much positive impact both Christina and Roy have demonstrated; truly leading by example.

Christina took on the role of interim-director and, was left with additional responsibilities, management, as well as various other administrative tasks and continues to manage extremely well. In addition to her exceptional organizational and leadership skills, you'll rarely find a space open on her schedule as her patients truly appreciate the unmatched experiences she provides. Roy, although not even one year out of school, has also stepped up as a silent leader in the clinic ensuring all of our people are taken care of.

NPS survey responses reflect both of their quality work. It is great to have the opportunity to work with great leaders and "people" as this is hard to find in any organization. Thank you!

Brag Recipient: Christine Raffone, Senior Authorization Coordinator Bragger: Vicki Palmieri, Authorization Representative I want to give a shout out to Christine who is always there to lend a helping hand. She is quick to respond when something is needed. She constantly checks in to see if help is needed and how I am doing on most days. She is always so positive and makes my day brighter knowing I can go to her with anything whether it is help, a vent session or something personal. I know she will always go to bat for myself and our team. She is truly a team player who cares.

Brag Recipient: Christine Raffone, Senior Authorization Coordinator Bragger: Christine Hill, Referral Coordinator YHP Christine has proven to be a leader for her growing Auth. Team Department.

No matter what is going on in her life, she gives 110% to her job. She is supportive and always encourages others! Christine is a mentor and I know I can count on her whenever I need her help.

Brag Recipient: Cindi Theriault, PSC Southington Bragger: Anonymous Such a great friend and coworker, with everything she has going on. Cindi manages to still come to work all bright eyed and bushy tailed. Eager to assist patients with their needs and going above and beyond to make sure that they are getting what they need. Whether it be calling a doctor’s office to get a script that the patient forgot, or reading the outcome measure to the patient that can't see it without reading glasses. We are truly blessed here in Southington to have such a compassionate, charismatic, and beautiful individual.

She is very helpful when things get hectic at the front desk and is eager to jump in and assist. Danielle is always ready to jump in and "save the day". Patients love her and can’t say enough good things about her. We're lucky to have her!!

She brings amazing energy and expertise to our PL trio! Truly, Heather brings her own positive personal touch to the position. I have been in a few provider meetings with Heather and she does a great job representing PTSMC’s mission and sharing the uniqueness and quality of care our clinicians offer. When I took Heather out on the road with me to visit MD offices, she immediately took to the role without missing a beat. In the process, I felt like a learned a few things from her. I am so glad to be able to work alongside Heather. Thank you for joining our team!

She is always positive and willing to do whatever is needed, she is a total team player. We are all enjoying working with her. Brag Recipient: Jake Wall Bragger: West Hartford Staff Jake consistently goes above and beyond to make sure patients feel listened to and cared for. He often walks older patients to their car, stays to talk with a patient after their session, and forms bonds and relationships with patients we haven't seen other aides do. He will make a great PT someday!

Not to mention her infectious smile and charming attitude. Patients love her, and we love her more!!

Brag Recipient: Jennifer Powers, PT Fairfield Bragger: Jennifer Rosenberg, PSC Fairfield Jen always goes above and beyond with her patients and her co-workers. She stays late, comes in early, will help the aides, the front desk staff, basically anyone who needs help. She cares about each patient’s success and it is evident when you are working with her or witnessing her work. As a PSC she is always willing to be flexible to make it work. Fairfield is lucky to have Jen here as a PT. I personally recommended several people to her and the best compliments that I hear are how she "fixes" them and then they send more people to her! PTSMC is lucky to have her!

Brag Recipient: Katy Sullivan, PT Orange Bragger: Anonymous Katy is loved by all of her patients and always steps up when someone is out of the office. She is a great leader among our physical therapists and connects with patients at a level that is unique to her personality and skillset. I really enjoy interacting with her inside and outside the office.

Brag Recipient: Kirsten Reaves, Authorization Representative Bragger: Renee Corsillo, PSC Danbury Kirsten has been an incredible asset to me in my new role as PSC at Danbury. She has gone above and beyond to make the transition seamless for everybody in the office, even as she takes on her new role with remote authorizations.

Brag Recipient: Kristen Forster, Director of Operations Bragger: Christine Raffone, Senior Authorization Coordinator What can I say about Kristen that everyone does not already know? She is the support system to EVERYONE! She is exactly what PTSMC is all about. Team player with a positive word and a smile. Kristen supports all of my crazy ambitions and I know will pick me up if I stumble even though her own schedule is quite chaotic. 6 am or 9 pm, I know she will be there.

She keeps me sane, guides me and has just made me a better person. There are no words that credit all that she does. I am grateful to work by her side every single day!

Brag Recipient: Lauren Zimmerman, PT Aide Fairfield Bragger: Anonymous Lauren is always putting in hard work, extra effort before and after her scheduled hours, and she always goes above and beyond for patient care. She is truly an amazing aide, and I don’t know where we would be without her as our clinic has been so busy! She’s going to make one heck of a physical therapist in the future!

Brag Recipient: Liza Peressini, PT Danbury Bragger: Mary Pacini, PT Aide Danbury Liza took over for Josh while he was out for 2 weeks with his newborn. Not only did she treat her own patients, she took over many of Josh's who have continued to rave about her care. She's the kind of PT that I want to be one day!

Brag Recipient: Mackenzie Anderson, Essex PT Aide Bragger: Lauren MacDonald, Essex PSA Mackenzie is a full-time under grad student at Central Connecticut and is on the path to be a phenomenal Physical Therapist one day. She currently works in Essex and Southington as a PT Aide who also helps as a PSC. She works well with all staff, helps to create a positive environment for patients and always gives 100%. We are so lucky to have Mackenzie and look forward to having her back full-time this summer.

Brag Recipient: McKenzie Daniels, Newington PSC Bragger: Anonymous Since McKenzie began working for PTSMC Newington she has continuously presented herself in a professional manner with all of our patients, their families, and our staff.

On many occasions, McKenzie has stayed at the clinic beyond her "scheduled time" to ensure all important work is complete for the day. Between her daily positive attitude, level of professionalism, and quality work, McKenzie is well deserving of this brag!

Brag Recipient: Madi Sides, PT Aide Simsbury Bragger: Shelby Howe, Health Fitness Specialist & PT Aide Simsbury Madi is the definition of Simsbury’s “flow”; when there is a task or something of need to get done, Madi is right on it! Always willing to run an errand or two in times of need, especially when we’ve run out of laundry detergent or caffeine! In Madi’s world, there’s always time for a quick Starbucks run to keep us all running- espresso shots for the win.

Madi, has been an excellent addition to our staff and we wish her the best of luck as she heads off to UConn for grad school this summer. Madi will be missed dearly as her talents fulfill so many gaps on a daily basis. We’re so proud of you Madi, best of luck with Athletic Training. You go girl!

Brag Recipient: Mario Paredes, PT Orange Bragger: Stephanie Weyrauch, PT Orange Mario and I are collaborating for clinical education experiences. He is assisting me in taking 2 students this year. As a new mom, his willingness to work with me and help me provide an excellent learning environment and experience for the future of our profession allows me to take on the role as a clinical instructor without sacrificing the quality of experience for the student and enables me to maintain my responsibilities at home. I am impressed by his ambition and ability to connect with providers. I have helped connect him with referral sources and he not only meets with the individual, but he then makes sure to thank them with a personal note and continues to foster those relationships through periodic follow up. Within that interaction, I can see nothing but a heart of gold who cares about her job, her clinic, and her role in PTSMC.
