PSNC Chief Executive Over the past five weeks, together with PSNC Committee members and the executive team, I have travelled the country talking about the five-year deal for community pharmacy. Thank you to the thousands of people who have attended our roadshows or logged on to our webinars.

Those of you who have done so will have heard me talk about leverage, and the fact that going into these negotiations we had none. This is one of the reasons why the Community Pharmacist Consultation Service (CPCS) is such a game-changer for the sector.

If we succeed and in time take millions of appointments annually away from already overwhelmed GP practices, we will find ourselves at the heart of primary care from the perspective of the NHS: demonstrating that we are taking pressure off our healthcare colleagues like never before.

Of course, for the CPCS to succeed, we need others to pull their weight too. Many of you have quite rightly pointed out that there are potential points of failure within NHS which could prevent us getting the referrals we need.

ensure NHS111 triages and refers patients to community pharmacy in the most effective way possible. Once we start getting data through from the service we will be able to monitor this process and identify where the problem areas are and where referrals are not coming through as they should: NHSE&I are committed to tackling that with 111 providers should it occur.

If we are successful, the CPCS will demonstrate to all patients the clinical effectiveness of community pharmacy. It will in the longer-term effect the way people interact with us and the other health services.

Over time, patients will choose to go to their pharmacy as the first port of call when they feel unwell or need general clinical advice. Over time we will therefore see referrals from NHS111 and from general practice increase and then plateau and subsequently reduce – in line with changing behaviours of patients who will realise that excellent clinical advice is available on the high street as and when required.

This is why I am so keen to start gathering data now about ‘walk-in’ patients as a baseline for future negotiations and annual reviews: if we can show how many people we are treating now, and how this changes as the service beds in, then PSNC will be able to start making the business case to Government so that pharmacies are fairly rewarded for all the clinical advice they are giving.

As with any change process, I know this will feel difficult and daunting. But I am convinced that in signing up to and delivering this service, community pharmacy can and will get to a better place: for pharmacists and their teams, patients, and for the rest of the local healthcare network.

Digital CPCF Roadshow announced PSNC’s roadshow events across England have been helping community pharmacy teams to understand the five-year deal on the Community Pharmacy Contractual Framework (CPCF), but now there will also be an online event.

Flu Vaccination Service: Your ‘at a glance’ guide • PSNC secures Flu Vaccination Service payment increase to £9.58 • Contractors are no longer required to ask patients to complete a questionnaire • Payment claims and notifications move to electronic systems PSNC has worked with NHS England and NHS Improvement (NHSE&I) to agree some changes to the community pharmacy Flu Vaccination Service for 2019/20, including negotiating an uplift to fees for the second consecutive year.

Documentation The service specification, Patient Group Direction and amended Secretary of State Directions for the 2019/20 Community Pharmacy Seasonal Influenza Vaccination Advanced Service have now been published. This means contractors that fully meet the requirements for provision of the service can offer it from September 2019.

Payment claims Claims for payments must now be made using the NHS Business Service Authority’s ‘Manage Your Service’ (MYS) platform – PSNC has already encouraged contractors to register for MYS as the first step for preparing for the 2019/20 Pharmacy Quality Scheme.

Funding increase The fee for the service will increase to £8.08 per administered dose plus an additional fee of £1.50 per vaccination, making the total payment £9.58.

Notification of the patient having received a vaccination can no longer be sent to the patient’s GP practice by fax – this supports the Health and Social Care Secretary’s tech vision to ban fax machines in the NHS by April 2020.

Key aspects of the 2019/20 service

Less paperwork Due to the proven success of the service, contractors are no longer required to ask patients to complete a patient questionnaire after they have received a flu vaccination.

Whilst it has not been possible to put a national IT solution in place to support provision of the service, in many areas LPCs and local NHSE&I teams have been able to arrange IT support for the service.

Contacting GPs

Training requirements The training requirements for the Flu Vaccination Service 2019/20 remain the same as for the 2018/19 service.

Pharmacists need to attend faceto-face training for both injection technique and basic life support (including administration of adrenaline for anaphylaxis) training at least every three years. They must also, within the last two years, have completed and signed the Vaccination Services Declaration of Competence.

Getting ready for the CPCF Watch the CPCS webinar In September PSNC held a webinar to help pharmacy teams prepare for the new NHS Community Pharmacist Consultation Service (CPCS) in collaboration with NHS England and NHS Improvement (NHSE&I).

New MUR target requirements Seventy percent of MURs conducted from 1st October 2019 to 31st March 2020 must be within the following two target groups: • patients taking high-risk medicines; or • patients recently discharged from hospital who had changes made to their medicines while they were in hospital.

Contractors will need to update their standard operating procedure for the service to reflect these changes.

Review your shared NHSmail account Community pharmacies will be required to have a shared NHSmail account to deliver the Community Pharmacist Consultation Service (CPCS), but only accounts listed accurately in the NHSmail pharmacy directory will be recognised as meeting this requirement. Check your HLP status From 1st April 2020, community pharmacies will be required to be a Healthy Living Pharmacy (HLP) Level 1 as part of their Terms of Service. HLP accreditation is only valid for two years therefore contractors who are currently an HLP Level 1 are encouraged to check the date when they were accredited to see if they need to start the process of re-accreditation.

The launch date for the NHS Community Pharmacist Consultation Service (CPCS) has been set as 29th October 2019.

This date has been agreed by NHS England and NHS Improvement (NHSE&I), the Department of Health and Social Care (DHSC) and PSNC as part of the ongoing work to implement the first phase of the service, which will see NHS 111 call advisers referring suitable patients to community pharmacy.

To help pharmacy contractors and their teams to prepare to provide the service, which will connect patients who have a minor illness or need an urgent supply of a medicine with a community pharmacy, the draft service specification has been published on the NHS Business Services Authority (NHSBSA) website; the final version will be published by NHSE&I once it has completed its journey through their internal governance process.

A toolkit to support pharmacy teams to provide the service has also been published.

It contains two sections – the first is a short briefing on the service which is aimed at pharmacy team members. The second section contains more detail on the service, so it can be used as a reference guide.

Pharmacy contractors can now register to provide the service via the NHSBSA Manage Your Service (MYS) portal. This includes claiming the Transition payment supplement, which will be £900 for those who claim by 1st December 2019.

PSNC and NHSE&I held a joint webinar for pharmacy teams and others interested in learning more about the service and how to implement it. This webinar is now available to view on-demand (see opposite).

PSNC comment Commenting on the progress being made towards implementing the service, Alastair Buxton, Director of NHS Services at PSNC said: “The launch of the CPCS on 29th October will be a landmark moment for community pharmacy’s expanding role within the NHS. This service will not only demonstrate the value community pharmacy can bring to patients, but it will also enable us to help the NHS to meet key targets.

“I therefore encourage all contractors to register early to provide the service, both to show community pharmacy’s willingness to provide this type of clinical service and to take advantage of the supplement to the Transition payment.”

Following further discussions between PSNC and NHS England and NHS Improvement (NHSE&I) on the Pharmacy Quality Scheme (PQS), new information has now been made available about the Scheme.

A notable change is that there will be no review point as there has been in previous Quality Payments Schemes. Instead, community pharmacy contractors will simply declare that they meet the gateway criteria on the day of making their declaration. That declaration must be submitted between 9am on 3rd February 2020 and 11.59pm on 28th February 2020.

Other key points that contractors need to be aware of include the following elements.

Gateway criterion: NHS website Contractors will need to update their NHS website profile and their Directory of Services profile between 00.00am on 1st October 2019 and 11.59pm on 30th November 2019.

Please note, the time for making these updates is ahead of the declaration period, which is different to previous schemes. If contractors miss this deadline, they will not

meet the gateway criteria (updating the NHS website profile is a gateway criterion) and will therefore not be entitled to claim a PQS payment in February 2020.

Quality criterion: domains Contractors need to meet all the quality criteria listed in each domain to achieve payment for that domain. For example, in the Risk management and safety domain, there are five quality criteria that contractors must meet in order to claim the 30 points available for that domain.

Submitting claims Claiming for an Aspiration payment and making a PQS declaration must be made on the NHS Business Services Authority (NHSBSA) Manage Your Service (MYS) application. Contractors are required to register in advance to be able to access to the MYS application (see opposite).

Wording changes There have also been amendments to some of the wording of the gateway and quality criteria. Contractors are therefore advised to ensure they have read the latest PSNC Briefing for all the up-to-date information (see opposite).

PQS information hub As has been the case for previous Quality Schemes, the PSNC Services Team has pulled together all the information on the new PQS so it can be accessed from one location. The PQS information hub includes guidance and resources on varying aspects of the Scheme from how to achieve the individual criteria to where and when to claim payment. Asthma criterion resources Support is now available from PSNC and on PharmOutcomes to assist contractors with meeting the asthma quality criterion of the PQS. Five-year CPCF: support for contractors PSNC supports community pharmacy teams to understand the details of the new CPCF agreement as they are finalised in the ongoing negotiations.

The team at PSNC is working hard to provide as much information and as many resources as possible to help community pharmacy contractors get ready for the five-year Community Pharmacy Contractual Framework (CPCF). There is a lot to do and details will be coming out in stages, but here we have outlined the latest news and guidance.

CPCF Roadshow continues

FAQs briefing updated

PSNC has been running a series of Sunday roadshow events for community pharmacists across England.

At the time of the announcement we published a briefing answering more than 45 key questions about the future of the CPCF and negotiations for contractors.

The events, hosted by PSNC Chief Executive Simon Dukes and his team, aim to help community pharmacy teams to learn about the deal, share their opinions and ask questions. By hearing directly from members of PSNC’s Negotiating Team, attendees gain insights into how this agreement was reached and what it means for pharmacies.

The Roadshow is travelling to all the English regions, finishing at the Pharmacy Show on 6th October. Whilst events are free to attend, community pharmacists and their teams are asked to register in advance. Most events have now taken place, but a digital event has been added to the schedule.

Drug reimbursement reform consultation PSNC has published its response to the Department of Health and Social Care’s (DHSC’s) public consultation on community pharmacy drug reimbursement reforms (see ow.ly/eyXF50w711j).

The proposals are far reaching, complex and interlinked. They do not impact the agreed funding envelope and should help to smooth delivery of retained margin, but could have an impact on individual pharmacies depending on their dispensing mix.

PSNC looks forward to discussing the detail and mechanics of how DHSC and NHS England and Improvement (NHSE&I) intend to reimburse pharmacy contractors as part of the second stage of the consultation process.

Brexit: Update for community pharmacies • PSNC Brexit Forum meets to review no-deal Brexit planning • DHSC makes improvements to preparations following ‘dress rehearsal’ in March • Medicines supply contingency arrangements strengthened by express freight service

HM Government and representatives of the medicines supply chain, including pharmacy, are continuing to work together to prepare for the UK’s exit from the European Union.

Earlier this month, PSNC’s Community Pharmacy Brexit Forum reconvened to discuss the current issues around Brexit for the first time since the change of Government.

Organisations at the meeting included the national pharmacy organisations, as well as the Dispensing Doctors Association, Healthcare Distribution Association and Proprietary Association of Great Britain.

During the meeting, representatives from the Department of Health and Social Care (DHSC) and NHS England and NHS Improvement (NHSE&I) updated the group on the Government’s planning work. Contingency arrangements include: • Improving the readiness of manufacturers and importers for new border arrangements; • Securing additional roll-on, roll-off freight capacity away from Dover and Folkestone (seen as key pressure points) for medicines to continue to come into the UK from 31st October; • Building up buffer stocks of prescription-only and pharmacy medicines (by DHSC and industry) in the UK before 31st October; • Buying extra warehouse space to hold additional stock; • Booking space on aeroplanes for products that require an immediate shipment due to short shelf-life or specific storage conditions; • Making changes to regulatory requirements so companies can continue to sell their products in the UK even if we have no deal; and • Strengthening the processes and resources used to deal with shortages in the event that they do occur despite everyone’s efforts.

On 12th September, PSNC Director of Operations and Support, Gordon Hockey, was interviewed for BBC Radio 5 Live’s Drive programme. Whilst Gordon said it was right to take this issue seriously, he explained that DHSC has been meeting regularly with representatives from across the supply chain (including national pharmacy organisations) to plan, and additional work has been possible using extra money made available by the Government.

Recent Government updates Additional funding: The Treasury has allocated £434 million to support preparations for the continuity of medicines supply in the event of a no-deal Brexit.

Express freight service: DHSC is making a new freight service available to transport medicines into the UK quickly in the event that manufacturers’ contingency arrangements are not sufficient.

Planning document released: The Government has released details of its planning for a no-deal Brexit (known as ‘Operation Yellowhammer’).

New Pharmacy Minister announced Jo Churchill, MP for Bury St Edmunds, is the new minister with responsibility for pharmacy. She has been appointed as Parliamentary Under Secretary of State for Prevention, Public Health and Primary Care, which includes pharmacy matters.

In 2017 Ms Churchill was Parliamentary Private Secretary to Jeremy Hunt, when he was Secretary of State for Health, and from 2018 she was Assistant Government Whip.

She has been broadly supportive of the pharmacy sector in parliamentary debates, and has also made many local pharmacy visits.

Heart checks service pilot NHS England and NHS Improvement (NHSE&I) have issued a press release to announce the trialling of heart checks in pharmacies to the general public, but please note that the cardiovascular disease (CVD) pilot is still in the planning stages.

Funded by the Pharmacy Integration Fund (PhIF), the CVD pilot is expected to involve community pharmacies offering on-the-spot checks of patients’ blood pressure and where appropriate, referral to treatment within Primary Care Networks (PCNs), complementing the CVD service specification in the new GP PCN contract. Subject to the pilot being successful, it is expected that this will lead to a fully funded national service to start in 2021/22.

Statins review to consider pharmacy provision NHS Chief Executive Simon Stevens has announced a review into the availability of statins from community pharmacies without a prescription.

It is understood the review will consider whether a recommendation should be made to manufacturers and the Medicines and Healthcare products Regulatory Agency (MHRA) that high dose statins should be moved from prescription-only status to being pharmacy (P) medicines.

LPC Focus: CPSS and Avon LPC case studies In this series shares examples of best practice from across the network of Local Pharmaceutical Committees (LPCs).

Two new ‘LPC Focus’ case studies have been published on the PSNC website.

Local Integration Discovery Tool

As part of PSNC’s work to identify and share best practice across the LPC network, Head of LPC Engagement and Collaboration Luvjit Kandula is developing a series of articles which provide an opportunity for LPCs to share their success, tips and good practice with others.

The first two are as follows.

A federated administrative structure Three LPCs across the South East region have set up an overarching structure to deliver the management and administration of East Sussex, West Sussex and Surrey LPCs. This case study describes the rationale and key considerations for setting up the structure.

NHS Long Term Plan Contractor Roadshows Avon LPC has held a number of contractor roadshows to raise awareness of the current situation for community pharmacy.

This case study has also been written up in collaboration with the LPC to help understand the initiative, benefits and outcomes achieved.

PSNC has launched a Local Integration Discovery Tool to develop plans to better support LPCs with their integration progress locally.

It is hoped that the tool will provide a baseline level to identify support needs and measure progress on integrated working with Primary Care Networks (PCNs).

We would like to thank all the LPCs that took part in our recent survey to assess the current status of community pharmacy engagement with local health systems across the country. The results are now being analysed and a report will be made available shortly.

New Clinical calculations e-learning A new e-learning programme working through clinical calculation case studies is now available from the Centre for Pharmacy Postgraduate Education (CPPE).

The programme aims to support pharmacy professionals with calculations in clinical settings when preparing, prescribing or checking medicines or when advising other healthcare professionals. The principles can be applied to any sector of pharmacy practice.

PSNC discusses drug shortages on the BBC Following reports of shortages of HRT products, medicine supply issues once again garner attention from the national media.

Over the summer, PSNC Chief Executive Simon Dukes appeared on multiple BBC programmes to discuss medicine supply issues.

Following reports of shortages of Hormone Replacement Therapy (HRT) products, Simon was interviewed by both BBC News and BBC Radio 5 Live about this and other supply problems that community pharmacy teams are dealing with.

Simon highlighted the work that community pharmacy teams are doing on a daily basis to make sure that patients get their medicines when they need them.

He explained that “pharmacists will do everything they can to protect the patient from what’s going on,” whether that is by spending hours phoning suppliers to track down a medicine or by finding alternatives that GPs can consider prescribing.

However, Simon also pointed out that pharmacies efficiently dispense over a billion prescription items a year, keeping the NHS spend on medicines low, and that the vast majority of these medicines are supplied without issue.

PSNC continues to brief national and trade press on supply issues to support their reporting of the ongoing situation.

HRT supply issues The Department of Health and Social Care (DHSC) has stated that patients may need to be switched to alternative HRT products that contain the same active ingredients for a temporary period.

Clinicians are advised to work closely with community pharmacies to understand local availability of HRT products and to help make decisions about appropriative alternatives for patients who are affected by the supply issues.

Helping patients understand charge exemptions Support your patients to understand their entitlement to free prescriptions with the NHS ‘Check before you tick’ campaign.

Following the success of last year’s campaign to improve patients’ understanding of prescription charge exemption criteria, NHS England and NHS Improvement will be running its ‘Check before you tick’ campaign again this autumn and sending pharmacies updated materials.

The campaign focuses on those patients making benefit-related claims who felt they needed more support to understand they are not automatically exempt. Research with the audience found that people find the benefits system difficult to understand and can mistakenly assume they’re automatically entitled to free prescriptions – which could result in a penalty charge of up to £100.

Asking patients who claim exemption from the NHS prescription charge for evidence of their entitlement is standard practice in community pharmacies, and the ongoing ‘Check before you tick’ complements this work. ‘Check before you tick’ helps people to understand their entitlement and how to claim correctly to avoid a penalty charge, by using an online eligibility checker which is easy to use and takes less than five minutes.

The campaign will also encourage people who pay for their prescriptions to take advantage of other means of reducing costs, such as prepayment certificates and the NHS Low Income Scheme.

The public campaign is due to launch from the beginning of October and pharmacies can continue to use promotional materials they received last year. Community pharmacy teams will receive additional materials as part of an ‘NHS Winter Pressures’ pack to be delivered in early November. Alongside materials for display, the pack will also contain a pharmacy team booklet to help aid conversations with patients.

Further resources including alternative formats of the booklets in audio, large print, easy read, BSL and braille will be available from Public Health England’s Campaign Resource Centre. Patient-facing resources - including access to the eligibility checker - are available via nhs.uk/checkbeforeyoutick The main campaign activity (through radio, video, billboard posters and social media) will run for ten weeks, but the ‘Check before you tick’ message remains relevant year-round.

How you can help • Displaying the campaign resources somewhere visible; • Making sure all staff are familiar with the prescription charge exemption criteria; • Encouraging patients to check their entitlement prior to making a claim/completing the declaration on the prescription; • Asking to see evidence of patients’ entitlement and checking the expiry date on any certificates presented. (REMEMBER: If a patient can’t provide proof, mark the ‘Evidence not seen’ box on the prescription); • Checking that any patient exemption status held on the pharmacy’s PMR system is regularly reviewed and kept up to date; • Informing the person claiming exemption from payment of NHS prescription charges (where evidence is required but not provided) that NHS checks are routinely undertaken to verify their entitlement, as part of arrangements for preventing or detecting fraud or error. Patients who claim free prescriptions that they’re not entitled to, may have to pay a penalty charge of up to £100; • Advising patients on Universal Credit who meet the eligibility criteria to tick the income-based Jobseeker’s Allowance box (K) on the back of the prescription; • Advising patients who are unsure of their entitlement to pay for their prescription(s) and provide them with an FP57 receipt with information on how to claim a refund (within three months) if they can later confirm that they are entitled. If you are unable to find a particular product, you can use the prescription item report to reconcile the payment.

Q. Where on our item report can I view my ‘payments for containers’? How do the NHSBSA know how many containers we have used?

A. The item report is a document that provides a line by line reconciliation of what a contractor has been paid for their prescription bundle for the relevant month. It is available to contractors from the NHS Business Services Authority (NHSBSA).

Column T of the prescription item report shows each individual payment.

Serious Shortage Protocols (SSPs) Endorsing Factsheet This factsheet explains what an SSP is, how pharmacy teams should endorse an SSP and how pharmacy contractors will be reimbursed and remunerated for supplies against SSPs. What is an SSP? An SSP is a potential way to help pharmacies manage any serious shortages of medicines or appliances that may occur, without needing to refer patients back to prescribers. In England, the Secretary of State may issue an SSP if, in his opinion, there is, or may be, a serious shortage of a medicine/appliance in England or any part of England.

An SSP may give NHS community pharmacists the ability to supply lesser quantity or supply an alternative strength, pharmaceutical form, generic equivalent or therapeutic equivalent of a product. if considered reasonable and appropriate An SSP may provide for one or more of these options and will be issued from a specific date and have an end date, which may be revised. Where supply in accordance with an SSP is considered reasonable and appropriate by a supervising pharmacist, the alternative product or quantity (as specified in the protocol) may be supplied subject to patient consent. How will I know if there is an SSP? Any SSPs will be published on a dedicated section of the NHS Business Services Authority’s (NHSBSA) website and PSNC, DHSC and NHS England and NHS Improvement (NHSE&I) will use their various communications channels to alert pharmacy contractors to its publication.

Patient charges Generally, prescription charges are payable for the supply of alternative products in accordance with SSPs, if they were payable for supply under the original prescription. However, no prescription charge is payable by a patient for any supplies made against an SSP that requires a pharmacist to supply a smaller quantity of drug or fewer appliances than was originally prescribed.

Labelling The dispensing label must include information to the effect that the product is supplied in accordance with an SSP and identify the SSP (usually by its number). This generally will be achieved by free typing in the directions field of the label, e.g. ‘Supplied under Serious Shortage Protocol number 002’.

Endorsing an SSP To ensure correct payments (fees and reimbursement) for any supplies made against an SSP, a contractor is required to endorse the following information on the paper prescription or if issued using EPS, using either the original electronic prescription or its associated dispensing token (as outlined in Part II of the Drug Tariff):

• NCSO – to indicate that the supply was made in accordance with an SSP; • Details of product supplied in accordance with the SSP (drug name, quantity, strength, formulation, supplier name or brand) *; • Quantity supplied*; • Pack size – best practice where multiple pack sizes are available; and • Invoice price – only if NHSBSA has no listprice available on dm+d. * The drug details and quantity supplied and endorsed will depend on the specific SSP. Endorsement Examples For illustration purposes we have used the example of an SSP that provides the authority to supply an alternative strength of a drug for Atenolol 50mg tablets where the SSP specifies supply of double the quantity of the lower strength Atenolol 25mg tablets.

Note: The dose remains as the prescribed 50mg daily, therefore, the dosage instructions to the patient change from ‘Take one daily’ (1 x 50mg) to ‘Take two daily’ (2 x 25mg).

Electronic prescriptions Contractors have the option to endorse either electronically or using the dispensing token. Where a dispensing token is used it is regarded as the prescription for payment purposes and will also be used as a record of supply for prescription charge/ exemption purposes. If the product or quantity supplied in accordance with an SSP is endorsed on a dispensing token, the Electronic Reimbursement Endorsement Message (EREM) – also referred to as claim message – should be marked as ‘ND’ or ‘Not Dispensed’ before the claim is submitted to the NHSBSA. Reimbursement and remuneration of an SSP The Schedule of Payment will outline the total reimbursement, VAT and remuneration paid to a pharmacy for any supplies made against SSPs in a given dispensing month.

Reimbursement: Reimbursement is for the medicine or appliance supplied in accordance with an SSP, not the originally prescribed medicine/appliance. Payment is normally calculated on the basis that the exact quantity ordered by the prescriber has been supplied, apart from any supply made against an SSP for an alternative quantity. Additionally, where a supply is made against an SSP, the reimbursement price will account for VAT payment Remuneration: Aside of the usual dispensing fees and allowances for any items supplied in accordance with an SSP, pharmacy contractors will receive an additional SSP fee of £5.35 per item.

PSNC top tips • Patient consent/agreement is required for supply of an alternative product or quantity in accordance with an SSP. • Paper FP10 prescriptions and NCSO-endorsed tokens must be placed in the front of the red separator.

• If using a dispensing token to endorse NCSO, remember to mark the item affected by the SSP as ‘ND’ or ‘Not Dispensed’ on the corresponding EREM. • The endorsement ‘PC’ (Prescriber Contacted) or ‘PNC’ (Prescribed Not Contacted) cannot be used for supply in accordance with SSPs. • No prescription charge is payable if the patient is supplied a lesser quantity of drug or fewer appliances than originally prescribed.
