New funding deal agreed for NHS Cautious welcome for news that the Government plans to give the NHS a £20 billion funding increase for its 70th birthday.

Prime Minister Theresa May has confirmed that the NHS will receive an extra £20 billion a year by 2023, but NHS leaders must continue to reduce waste to ensure that this extra funding is spent wisely.

The announcement, made last month, is being labelled a ‘birthday present’ as the NHS turns 70. The figure is equivalent to a 3.4% rise to NHS England’s budget for frontline services.

Whilst the Prime Minister has suggested that the money will be funded by a combination of a Brexit dividend and a tax increase, former Health and Social Care Secretary Jeremy Hunt said that full details will be revealed by the Chancellor in the Autumn Budget.

Public think tanks such as The King’s Fund welcomed the cash boost, but warned that funding will still fall short of that required to sufficiently meet the health service’s needs.

PSNC Chief Executive Simon Dukes said: “We are pleased to see HM Government backing the NHS with this historic funding deal, and we look forward to working with the NHS to understand how community pharmacy can help to meet the challenges ahead as the NHS develops its ten-year plan. The first step in that journey will be for us to begin substantive negotiations on community pharmacy’s immediate future, which we hope to be able to do soon.

Community pharmacies are the most accessible healthcare locations with much to offer to support both patients and the health service.”

New Health and Social Care Secretary Matt Hancock, the MP for West Suffolk, has been appointed as the new Secretary of State for Health and Social Care.

Mr Hancock has been the Culture Secretary since the beginning of the year and has previously held various ministerial roles; most interestingly he worked in the Department for Business, Innovation and Skills.

Prior to entering politics, Mr Hancock has worked in a software business, as an economist for the Bank of England, and later as both an advisor and Chief of Staff to George Osborne when he was Shadow Chancellor.

PSNC congratulates the new Health Secretary on his new role and looks forward to working with him to develop community pharmacy services for the benefit of patients, the NHS and pharmacies.

Pharmacy CEOs offer support for NHS Assembly from commissioning bodies as well as those who work in the healthcare sector.

The leaders of five national pharmacy organisations have sought involvement in a new stakeholder forum being created to oversee progress on developing long-term plans for the NHS. At a recent board meeting between NHS England and NHS Improvement, it was announced that an ‘NHS Assembly’ would be created, to include representatives

The national pharmacy bodies have written a joint letter offering their services to the Assembly and highlighting what their members can offer to help the NHS to achieve its ambitions.

As well as describing the vital role that community pharmacies and pharmacists working in other settings play in ensuring that patients have access to medicines and can use them effectively, the letter sets out how the sector could help to improve patient care and the wellbeing of local communities.

The signatories to the letter were the

Chief Executives of the Association of Independent Multiple Pharmacies (AIM), the Company Chemists’ Association (CCA), the National Pharmacy Association (NPA), the Pharmaceutical Services Negotiating Committee (PSNC) and the Royal Pharmaceutical Society (RPS).

The letter signifies the shared ambition of the national pharmacy bodies to ensure that the NHS uses pharmacy to its maximum potential for the benefit of patients and health services, and the commitment of the Chief Executives to work together to achieve that.

As the flu season approaches, PSNC will be holding a webinar on the 2018/19 Flu Vaccination Advanced Service on Wednesday 15th August at 7.30pm.

On the night Alastair Buxton, Director of NHS Services at PSNC, will explain how the Flu Vaccination Advanced Service will differ for 2018/19, highlighting some important changes community pharmacy teams need to be aware of.

We will also hear from Ben Tindale, Digitisation Lead for NHS Prescription Services (part of the NHS Business Services Authority), who will describe the new digital submission process for flu vaccination claims.

PSNC submits evidence to MP inquiries PSNC has provided written evidence to two inquiries in the past month: one on the primary care support services contract held by Capita, and the other on the generic medicines market.

Following the National Audit Office investigations into NHS England’s management of the primary care support services contract with Capita and NHS spending on generic medicines in primary care, the Public Accounts Committee decided to conduct inquiries into both. The committee of MPs, responsible for overseeing government expenditures, began by inviting stakeholders to provide information on their experiences of these issues.

by PSNC in July 2016. It also outlines how PSNC has worked, and continues to work,

New requirement to disclose pricing information to DHSC The Department of Health and Social Care (DHSC) gains new powers to obtain pricing information from those involved in the medicines supply chain for the NHS. As of 1st July 2018, the DHSC now has greater powers to request information about the prices and costs associated with products purchased for the NHS. The Health Service Products (Provision and Disclosure of Information) Regulations 2018 is being introduced largely to help DHSC control the price of medicines and its primary focus will be to require manufacturers and wholesalers to provide information on costs. Any failure to provide the information requested could lead to a financial penalty.

It is expected that the Regulations will help improve the information available to DHSC when reviewing shortages and setting price concessions.

The Regulations will also apply to community pharmacy contractors, who will be affected in two main ways: 1. Keep, record and provide information on price and volume This requires contractors to record and keep relevant information on the price and volume of medicines, appliances and borderline substances dispensed for the NHS and provide this to the DHSC in response to a written request.

Contractors will be used to keeping records for products they dispense, and many will have previously submitted this type of information as part of the Margin Survey. The DHSC has indicated that its intention is to continue with the current Margin Survey.

Smaller contractors will only be required to provide invoices and existing documents.

2. Provide information on costs This requires contractors to provide any relevant information on the costs of distributing or supplying a medicine or appliance associated with NHS dispensing, in response to an information notice from the DHSC. Guidance from PSNC PSNC has produced a briefing which is directed at NHS community pharmacy contractors in England but also includes brief details on the requirements relating to wholesalers, as some community pharmacies have wholesale dealer’s licences.

July 2018 Category M prices The Category M reimbursement prices for July 2018 show a net overall reduction of around £15 million per month for medicines compared to April 2018. This equates to a reduction in Average Item Value of approximately 17 pence per item on all items.

The movement in July is driven by an overall reduction in manufacturers’ selling out prices in the reference period (January to March 2018), mostly associated with price concession lines, and not due to planned management of retained margin. Since we were made aware of the new prices we have been in constant contact with the Department of Health and Social Care (DHSC) to validate their analysis.

PSNC is working with DHSC on the 2017/18 margins survey to ensure that the full effect of price concessions is accurately captured. However, at this point it is unclear what will happen to the August 2018 Drug Tariff. We will inform contractors via our usual channels once more information is known.

Patient materials for data opt-out system Community pharmacy teams are being sent a pack of materials to help inform patients of the national data opt-out system, as reported in May’s CPN. The pack for pharmacies, due to arrive by the end of July, will include a number of posters and handouts to give information to patients. Pharmacy teams are advised to use the materials provided and signpost patients that ask about their data choices to the patient-facing website nhs.uk/yournhsdatamatters or NHS Digital on 0300 303 5678.

Frequently Asked Questions 1. Can I close my pharmacy on these days?

Yes. Community pharmacy contractors can close their pharmacies on days which are specifically mentioned in the Regulations (namely Christmas Day, Good Friday and Easter Sunday) or a day which has been designated as a ‘bank holiday’, unless directed to open by NHS England.

2. What about my core contractual hours?

A pharmacy will be treated as having been open for its usual hours on that day for the purpose of counting the core contractual hours that week. Therefore, the contractor can close their pharmacy on the days indicated without giving notice or making an application to change their core hours.

3. Are there exceptional circumstances when I must open on these days?

Yes, if a contractor has been directed to open their pharmacy by NHS England.

4. Can I close my pharmacy early any other time?

Sometimes contractors may wish to close an hour or so earlier than usual on a particular day for any reason. If the hours that the contractor wishes to close early are supplementary hours, then notice of that intention should be given at least three months in advance. If the hours are core hours, then an application is needed, and the contractor would have to be able to satisfy NHS England that there have been changes to the needs of people in the area, otherwise the application would likely to be refused.

Unless you have notified NHS England of changes to your supplementary hours at least three months before the date, or applied successfully for amendment of core hours, the contractor will not be able to close early.

5. Do I need to notify NHS England of opening intentions on bank holidays / public holidays?

Contractors are encouraged to inform NHS England whether their premises will be open on bank holidays. This information is of critical importance to NHS England in order that it is able to plan pharmacy provision during holiday periods.

If NHS England is not able to determine the opening hours of pharmacies with a high degree of certainty after consulting the LPC, its only option may be to issue directions to one or more pharmacies requiring them to open.

This clearly is not in the best interests of pharmacies if there are other suitable pharmacies that would have been open; so PSNC recommends that all pharmacies notify their intentions and then open as they have notified.

NHS England has published template forms which can be used to provide notification of a pharmacy’s opening intentions.

NPA report backs new integrated care model Utilising the whole pharmacy team could free up 18 million GP appointments, says the National Pharmacy Association (NPA).

A new care model could free up 18 million GP appointments a year if it were replicated across the whole of England, a report from the NPA has estimated.

at the GP practice, with IT integration, including read/write access to the full GP Clinical Record, enabling information to be shared between the pharmacy and the practice in real time.

The Integrated Care Model saw staff at Jaunty Springs Medical Practice in Sheffield referring patients to nearby Basgreen Pharmacy as part of a ‘Shared Care’ agreement. The aim was to improve the patient experience by encouraging people to be more active in their own care management, whilst at the same time reducing GP workload. The report concludes that through this model the community pharmacy team can provide costeffective care at scale.

The model closely followed a larger scheme in Sheffield, funded by the Prime Minister’s Challenge Fund programme.

Tasks undertaken by the community pharmacy team included support for patients with long-term conditions, delivering structured medication reviews, co-ordination of repeat prescriptions, and ensuring evidence-based prescribing in the GP practice. The vast majority of these interventions took place in the community pharmacy’s consultation room rather than

The report goes on to make a number of recommendations to work towards implementing this model across England.

Endorsing Webinar: Your Questions Answered Our Dispensing and Supply Team answers some of the most common endorsing queries that were asked by webinar viewers.

In June almost 400 people tuned in to PSNC’s Endorsing Webinar when our in-house Drug Tariff experts outlined the dos and don’ts of endorsing. Over the next few months we will feature some of the questions that we ran out of time to answer during the Q&A session.

Q. I have an EPS prescription which states Ensure Plus milkshake style liquid 200ml (Flavour Not Specified) in the prescribing line.

Can I dispense multiple flavours and claim for multiple dispensing fees?

A. Where a prescriber selects the ‘Flavour Not Specified’ option on EPS, but intends for multiple flavours to be supplied, the prescriber needs to select the ‘AF’ (assorted flavours) function on their prescribing system. The ‘AF’ endorsement would appear in the prescriber endorsement field and allows pharmacy teams to dispense and receive a dispensing fee for each flavour supplied and endorsed.

The pharmacy will need to select the actual medicinal product pack (AMPP) for each specific flavour dispensed along with the quantity dispensed for that flavour.

Q. Could you confirm that ticks are allowed in the description area but not in endorsement area of a prescription?

If the EPS prescriptions does not specify ‘AF’, you can still dispense multiple flavours but will only receive one dispensing fee.

For the following EPS Release 2 prescription: Ensure Plus milkshake style liquid 200ml (Flavour Not Specified) ACBS, AF 2000ml Take as directed If the pharmacy dispenses three different flavours, they would select and return: • Ensure Plus milkshake style liquid fruits of the forest (Abbott Laboratories Ltd) x 200 ml. Quantity dispensed: 800ml • Ensure Plus milkshake style liquid chocolate (Abbott Laboratories Ltd) x 200ml. Quantity dispensed: 600ml • Ensure Plus milkshake style liquid peach (Abbott Laboratories Ltd) x 200ml. Quantity dispensed: 600ml Please note: NHS Prescription Services only extracts prescriber endorsements from the prescriber endorsement field of the EPS prescription message. If the prescriber has added an ‘AF’ endorsement in the dosage line or in the ‘additional instructions’ field of the EPS message, this will not be recognised by Prescription Services for reimbursement purposes.

A. Endorsements, ticks, dispensing stamps or other marks encroaching onto the prescribing area of the prescription can make it difficult for the Intelligent Character Recognition (ICR) software used by Prescription Services to price correctly, which can lead to inaccurate reimbursement for your pharmacy.

Other than the usual endorsements required on the left-hand margin, please avoid making any additional marking to the left-hand margin or the prescribing area of the prescription using ticks and/or other marks during the dispensing/checking process as these could affect how Prescription Services interprets prescriptions.

Q. Could you clarify if anything that the ICR software may have a problem with, should go in red separator?

A. Using the red separator when submitting your prescription forms for payment ensures that forms including certain items will be subject to an additional check before final payment is confirmed. Prescription Services has specified the items that should be submitted in the red separator to include: • Expensive items i.e. items with a Net Ingredient Cost (NIC) of £100 or more; • Unlicensed Specials/Imports; • Broken Bulk (BB); • Items where out of pocket expenses have been claimed (OOP or XP); • Prescribers signature encroaches into the prescribing area (i.e. goes over an item at the bottom of the prescription); • Items where prescribers have added supplementary product information; and • Items with prescriber handwritten amendments.

The following should not be included within the red separator: • Forms where items have not been dispensed; • Forms where the total of all items on the form is £100 or more, but none of the items have a NIC of over £100; • Hand written prescriptions; • Forms where the patient signature goes over or outside the box in part 3 on the reverse of a prescription; and • EPS dispensing tokens.

Q. If a doctor prescribes 500g Aveeno cream but patient requests 100g tubes, will we be paid for dispensing 5x100g?

A. Aveeno cream is available in three pack sizes – 100ml, 300ml and 500ml. As Aveeno cream is not listed in Part VIIIA of the Drug Tariff, you will be reimbursed based on your endorsement. If the pharmacy only has stock of 5x100ml or patient prefers 5x100ml tubes rather than 1x500ml, you will be reimbursed for the 5x100ml if this was dispensed and endorsed accordingly.

If an item is not in the red separator it will still be processed.

The reason Prescription Services ask for it to be placed in the red separator is so that it is double checked. The unsorted items will be processed through the normal manner but will bypass the additional checking process. Prescription Services state that including any other items in the red separator may lead to a delay in payment.

Supporting valproate medicines use New materials for pharmacies who come into contact with women and girls prescribed valproate medicines.

Women and girls using valproate medicines must now be protected by a Pregnancy Prevention Programme which brings valproate in line with other medicines which are dangerous during pregnancy, such as isotretinoin and thalidomide.

A new clinical audit has been created by the Company Chemists’ Association (CCA), host of the Community Pharmacy Patient Safety Group, to explore pharmacy’s contribution in supporting girls and women of childbearing potential in taking valproate medicines.

The first phase of the audit started in July,

with the second phase in November. The audit is split into four sections and should be completed in stages. CCA member companies will be disseminating the audit resources directly to their pharmacy teams over the next few weeks so that they can participate, however, all community pharmacy teams can participate in the audit, which is available via: ow.ly/smOw30kCc0p

Diamorphine 5mg and 10mg injection shortage Accord, who supply Diamorphine 5mg and 10mg injections, is currently experiencing manufacturing issues and the other UK supplier, Wockhardt, is unable to support the entire market.

The Department of Health and Social Care (DHSC), NHS England and the

manufacturers have agreed the following distribution arrangements to manage the supply issue during July and August 2018: • Diamorphine 5mg and 10mg injections will only be available to order from Alliance. No minimum surcharges will be levied.

• Morphine 10mg injection (Martindale) will be available from AAH only.

• 30mg, 100mg and 500mg injections will be available through the usual wholesalers.

FMD system suppliers list

The UK Community Pharmacy FMD Working Group has published a list of Falsified Medicines Directive (FMD) IT system suppliers who will be offering an FMD IT solution.

PSNC recommends that community pharmacy contractors use this list to start considering the offerings from their PMR supplier and other FMD IT suppliers.

QPS evaluation – please share your views

NHS England is evaluating the implementation of the Qua lity Payments Scheme (QPS) and how this has been received by community pharmacy contractors.

The feedback will involve a 15-20 minute telephone interview. If you would pre fer to complete the questions electronically the evaluation team may be able to facilitate this.

Over 9,000 new HLPs registered In Public Health England’s latest Healthy Living Pharmacy (HLP) newsletter, it has announced that over 9,400 pharmacies have progressed through the profession-led self-assessment process and registered with the Royal Society for Public Health (RSPH) to attain Level 1 HLP status.
