Preparations underway for Stay Well Pharmacy campaign A ‘Stay Well’ campaign launching next month will focus on the advice and support available from community pharmacies.

NHS England is launching a new Stay Well campaign focused on encouraging parents with young children to use community pharmacies.

The Stay Well Pharmacy campaign will run from 12th February to 26th March with tactics including: • Television advertising; • Social media activity; • Posters and leaflets; and • A media partnership with NetMums.

The campaign will be targeting parents of children aged five years or under, aiming to increase their confidence and trust in the advice given by community pharmacy teams, and encourage them to use pharmacies rather than visiting their GP for minor health concerns.

Campaign materials will be available for community pharmacies ahead of the campaign, and these feature

real community pharmacy team members along with prominent green cross imaging. The campaign materials will explain to parents that they can speak to the community pharmacy team about a range of minor health concerns.

Local NHS England teams have been notified that the campaign will feature as one of community pharmacy’s six public health campaigns for the year. PSNC will share further details on the launch of the campaign and the materials available as soon as we have them.

The national pharmacy organisations are also helping NHS England to collate training resources that will help community pharmacy teams to manage the conditions that parents of young children are expected to talk to them about.

Flu cases continue to rise Widespread increases in flu cases and respiratory illness are putting considerable strain on healthcare services across the country such as GPs, hospitals and NHS 111. Community pharmacy teams are busy playing their part in helping patients and the NHS to cope with the challenges winter is bringing, but where possible, they can help by: • encouraging anyone who is eligible and not yet vaccinated to take up their offer of vaccination, explaining that it is not too late to get vaccinated; • making sure pharmacy team members have been offered a flu vaccination; and • promoting relevant PHE winter campaigns: ‘Stay well this Winter’, ‘Keep Antibiotics Working’ and ‘Catch it. Bin it. Kill it’.

Consultation on the prescribing of OTC products NHS England has published a consultation on proposals to “rein in prescriptions” for a number of products that are available ‘overthe-counter’ (OTC).

The proposals follow on from a commitment to hold a full consultation on restricting the prescribing of OTC products made by the NHS England Board in November 2017. The intention is to produce consistent national guidance for CCGs to refer prescribers to.

Views are now being sought on stopping the routine prescribing for 33 minor conditions, as well as prescribing of probiotics and vitamins and minerals. The

products have been chosen because they meet one of the following criteria: • They treat a condition which is self-limiting and therefore does not require treatment; • They treat a condition which could be managed by self-care, i.e. a person suffering does not normally need to seek medical care; or • They have low clinical effectiveness but high cost to the NHS, e.g. vitamins/ minerals and probiotics.

The proposals would not affect the prescribing of items for chronic conditions or minor illnesses that are symptomatic or a side effect of something more serious, NHS England said.

PSNC appoints new CEO Current CEO of fraud prevention service Cifas set to take over the helm of PSNC from Sue Sharpe in May 2018.

PSNC has appointed Simon Dukes as its new Chief Executive Officer (CEO).

Mr Dukes has been the CEO of Cifas, the UK’s Fraud Prevention Service, since 2013, and will take over from the current PSNC Chief Executive, Sue Sharpe, in May 2018.

Cifas is a not-for-profit membership organisation which provides a platform to enable organisations to share fraud data to help reduce their risk from fraud and to protect consumers. It has a membership of over 400 companies of all sizes, from FTSE100 organisations to small enterprises and charities, and works to ensure that all its members, large and small, are sufficiently catered for.

Prior to his role at Cifas, Mr Dukes held several senior roles in the UK public sector, with specialisms ranging from public affairs to cyber security. He has therefore had considerable experience in managing risk, negotiating in highly sensitive situations, working in complex legal environments, and dealing with the UK media.

Mr Dukes believes that with the current economic challenges facing the UK and public service, we need a more collaborative approach to healthcare – and that the role of the community pharmacy is an important factor in this mix.

Simon Dukes said: “Community pharmacies perform a vital role for the citizen, and I am very pleased indeed to have been selected as the new CEO for PSNC. I look forward to working with the Chairman, Members and PSNC team to help develop the NHS community pharmacy service for the future.”

PSNC Chairman Sir Mike Pitt said: “I am very pleased Simon will be joining us as CEO. PSNC appointed recruitment consultants to advise and assist our Appointments Panel in a rigorous search and selection programme, to ensure it found the best possible candidate for this important and complex role. The Appointments Panel was unanimous in agreeing that Simon had all the qualities we were seeking, and would be the right person to lead the work of PSNC in the demanding environment community pharmacy faces. The Committee too approved the appointment unanimously, and we look forward to working with Simon.”

Current CEO Sue Sharpe said: “I am delighted that Simon will be our new CEO. He has the experience and expertise, from outside the health sector, and will bring fresh thinking and strong leadership of the organisation, supported by the excellent team here.”

PSNC elections update

LPC elections 2018

As previously announced in November’s edition of CPN magazine, the terms of office for some PSNC committee members are due to expire on 31st March 2018. We have therefore initiated election proceedings.

The four-year term of office for many Local Pharmaceutical Committees (LPCs) finishes at the end of March so the process to form the new committee has begun.

All 13 Regional Representatives and three non-CCA Multiple Chemist Representatives are up for election to PSNC, and invitations for formal nominations were opened on 15th January.

LPCs are local leadership bodies representing, promoting and supporting community pharmacy contractors. Your LPC’s website should provide you with a good insight into their work, details from past meetings, the latest news, contractor support and resources: http://lpc-online.org.uk. Many also have a guide for prospective LPC members.

Clinical governance deadlines fast approaching PSNC’s Regulations and Support Team reminds community pharmacy teams of the actions required by 31st March 2018 to meet their contractual obligations.

The end of the financial year (31st March) is fast approaching and community pharmacy contractors are reminded of four key deadlines relating to the clinical governance aspects of their terms of service.

The clinical governance requirements of the community pharmacy contractual framework (CPCF) cover a range of quality related issues and are set out in Schedule 4 of the NHS (Pharmaceutical and Local Pharmaceutical Services) Regulations 2013. Adherence with the clinical governance requirements is thus a part of the terms of service.

PSNC urges contractors to ensure that they have completed, or are working towards completing, the clinical governance requirements highlighted below to meet the end of year deadline.

Are you meeting NHS Identity guidelines?

Community pharmacy contractors are reminded that last year, NHS England updated its guidelines on the use of the NHS Identity, which included guidance for all primary care contractors commissioned under a primary care contract, including community pharmacies.

From 13th January 2018, in most cases community pharmacy contractors will no longer be able to charge customers a fee for the use of a specific payment method, such as a personal debit or credit card. This is due to the introduction of The Payment Services Regulations 2017.

When the revised guidelines were published, NHS England advised that implementation of the updated NHS Identity guidelines could be gradual; existing offline materials could continue to be used until they needed to be replenished or updated. Contractors, however, should aim to update existing digital channels, such as websites and social media accounts, by January 2018.

In practice, this means that pharmacy businesses will not be allowed to add a surcharge or fee when customers purchase goods using, for example, a personal credit card. This is despite the additional cost to the pharmacy when accepting such payment methods.

Contractors who choose to use the NHS Identity should ensure that the revised guidance is adhered to when promoting NHS services, for example, if developing marketing materials to promote the NHS Flu Vaccination Service; the NHS Identity should not be used when promoting non-NHS (private) services. National approach to NMS PPV to begin Transfer of Advanced Service monitoring responsibilities to NHS BSA continues with commencement of NMS post-payment verification.

The NHS Business Services Authority (NHS BSA) has begun postpayment verification (PPV) of payments to community pharmacy contractors for provision of the New Medicine Service (NMS).

This work follows a similar process to that implemented for the PPV of Medicines Use Review (MUR) payment claims, which commenced in October 2017.

resolution of those issues with the individual contractor. Where common issues are identified, advice will be provided to all contractors on how such issues could be avoided in the future.

Until the NHS BSA commenced their PPV work on MUR claims, this monitoring had been the sole responsibility of local NHS England teams; however, in many areas resource constraints had limited their ability to perform this work, which has resulted in a lack of consistency in how it was carried out across the country.

Each month, NHS BSA will request signed patient consent forms as evidence from a sample of contractors to support the payment claims they have submitted for MUR and / or NMS over a specified three-month period.

A new standardised approach has been agreed by PSNC, NHS England and NHS BSA. It aims to identify any issues related to the claiming of payment for Advanced Services and to facilitate

If a contractor is selected to take part, they will receive a letter which includes full instructions on how to submit the MUR and/or NMS consent forms to NHS BSA.

In Public Health England’s (PHE) latest Healthy Living Pharmacy (HLP) newsletter, it has announced that over 8,300 pharmacies have progressed through the profession-led selfassessment process and registered with the Royal Society for Public Health (RSPH) to attain Level 1 HLP status.

Due to increasing flu levels in the community, the Department of Health and Social Care (DHSC) has issued a letter advising that the prescribing of antiviral medicines for the prophylaxis and treatment of flu at NHS expense is now permitted. Read the letter at: ow.ly/jZBn30hjVB7

CPPE Corner: Mental health e-course A new e-course is now available from the Centre for Pharmacy Postgraduate Education (CPPE).

Mental health e-course. This eight-week course begins on Monday 8th January*, and is designed to equip you with the knowledge and skills to support people with mental health conditions that you encounter in your practice.

Through the Mental health e-course you will be set a series of activities to improve your knowledge of mental health conditions, and use this knowledge to support people in your practice. As this is a live course, there will also be opportunities to take part in discussion forums with your colleagues.

Following an introduction to mental health, the course will concentrate on depression, anxiety disorders and insomnia, bipolar disorder, schizophrenia, older people with dementia and children and adolescents. The e-course will then conclude by looking at supporting people with mental health conditions across all care settings.

Parents and carers advised to inspect Buccolam oral syringes before use The European Medicines Agency has advised that pharmacists who prescribe and dispense Buccolam (midazolam) oromucosal solution pre-filled plastic syringes speak to parents/carers about the need to inspect the syringes before use.

In a small number of cases, the translucent (white) tip-cap can sometimes remain attached to the syringe after the red cap has been pulled off, therefore presenting a choking hazard. The manufacturer Shire has distributed information explaining the defect and giving practical steps to follow. Information on medicines supply for patients When community pharmacy teams struggle to source medicines, this can lead to queries from patients as to why there are delays. PSNC has therefore created a factsheet which provides some lines that could be used to explain the situation to patients.

Ask PSNC The PSNC Dispensing and Supply Team can provide pharmacy teams with support and advice on a range of topics related to the Drug Tariff and reimbursement. Questions asked in recent months have included: Q. I have been presented with a prescription for Norethisterone 350microgram tablets which has not been endorsed “CC” by the prescriber. This item is not listed in Part XVI, Clause 10 of the Drug Tariff however the brand is – is this item exempt from the prescription charge as the brand is listed?

A. No. Only items listed by name in Part XVI, Clause 10 are exempt from prescription charges. The only exceptions being the following items which are automatically exempt from prescription charges: • spermicidal gels, creams, films and aerosols; • systemic contraceptive preparations listed in Part XVI; and • contraceptive devices listed in Part IXA of the Drug Tariff.

Where an item is not listed in Part XVI, Clause 10 or does not meet the above criteria for automatic exemption the prescription must be annotated with “CC”, “OC” or the female symbol (♀) to make it clear that the item is for contraceptive use.

A charge must be levied for any item not meeting the abovementioned criteria where the patient does not have a valid exemption.

Q. How should I endorse a prescription for a licensed medicine not listed in the Drug Tariff?

A. The following information should be included in the endorsement: • Pack size — where there are multiple pack sizes available; and • Brand name or manufacturer/supplier.

If the product does not have a price listed in the NHS database the Dictionary of Medicines and Devices (dm+d)*, you should also endorse net price before discount and excluding VAT. Please note the Pricing Authority reimburses using the NHSlisted reimbursement price, but if this is not known by them (i.e. the supplier does not list the product/price on dm+d) then they will need to use your endorsed price.

It is important to only include the necessary information in line with the endorsement requirement as any additional information could risk misinterpretation which could lead to an incorrect payment being made. *For more information on dm+d, please visit psnc.org.uk/dmd

Q. A patient has presented a non-repeatable prescription for ‘28 x 5mg tablets’. The 5mg strength is unavailable as it is in short supply; however, I do have ‘56 x 2.5mg tablets’ in stock. As this is the same amount of drug (i.e. 140mg) which can be administered at the prescribed dose by doubling the number of tablets; can I dispense 56 x 2.5mg tablets against this prescription instead?

A. No. It is part of the Terms of Service that pharmacy contractors must, with reasonable promptness, “provide drugs so ordered” once a prescription has been presented. “Drugs” includes medicines e.g. the tablets. In this scenario, the patient has presented a non-repeatable prescription for 28 x 5mg tablets and therefore supplying 56 x 2.5mg tablets would not be providing the drugs “so ordered”.

If you intend to dispense 56 x 2.5mg tablets then the prescription would need to be amended or a new prescription would need to be issued by the prescriber at the prescriber’s discretion. It would also be essential to ensure the patient understands the different dosage to be taken if there were an amendment to their prescription.

Dispensing and Supply Factsheet: Information on medicines supply for patients Your local pharmacy team works hard to make sure that you get the medicines that you need, but we – like many other pharmacies across the country – sometimes experience difficulties in sourcing medicines.

We know how important getting your medicines is and I’d be happy to tell you more about why getting your medicine may be difficult for us at the moment.

Why is my medicine out of stock?

If manufacturers suffer problems in making medicines, or if more patients in an area are put onto a certain medicine, then shortages can occur. Also, recent changes in the exchange rates means that stock sold to pharmacies in the UK may now be being sold in other countries meaning there is less stock available.

I can’t get my medicine from you (my usual pharmacy) but another pharmacy has it – why is that?

There are two main reasons for this: 1. Different pharmacies use different suppliers and it will depend on whether they have stock or not.

2. Some manufacturers restrict the amount of a specific medicine that a pharmacy can order. This is known as a quota. Once a pharmacy has used their quota for the month it can be really hard to get any more until the start of the next month.

What is the difference between brands and generics?

Medicines will often have more than one name: • A generic name which is the ingredient of the medicine (for example ibuprofen).

Often generic medicines are made by a number of manufacturers.

• A brand is the name the manufacturer or pharmaceutical company gives to the medicine (for example Nurofen). Only that manufacturer can make that brand.

If your doctor prescribes by brand name, the law says we have to supply that brand, which can cause delays. If your doctor prescribes using a generic name, we can supply any available product which matches that drug description.

Why are my new tablets a different size, shape or colour?

Because some medicines are in short supply, your usual tablets may not be available.

In this case, to make sure you don’t go without medication we may supply you with the same medicine but from a different manufacturer, so your tablets may change their appearance. If you have any queries, you can ask to speak to the pharmacist.

What are you doing to help?

The scale of the medicines shortages means that lots of suppliers don’t have stock available, but we are trying really hard to find stock for you and all our patients. This includes “staged” dispensing, liaising with GPs to find alternatives, many phone calls to suppliers, seeing if stock may be available in other pharmacies, and more. This is a national problem and pharmacy representatives are working with the Government to help resolve these problems.

What can I (the patient) do to help?

There are a few things you could do: • Order your medication in plenty of time (but no more than seven days before it is due).

• Only order what you require. If you have unused medicines in your cupboard use these first (remember to check the expiry date).

We are trying really hard to source these items so please bear with us if we are having difficulty getting medicines for you.

Changes to the monthly NUMSAS claims process As part of the preparations to extend the NHS Urgent Medicines Supply Advanced Service (NUMSAS) pilot to the end of September 2018, NHS England has worked with the NHS Business Services Authority (NHS BSA) to review how NUMSAS claims are submitted at the end of each month.

NUMSAS claims by completing the existing NUMSAS claim form, placing it along with the FP10DT EPS dispensing tokens (completed as part of the NUMSAS) in an envelope clearly marked ‘NUMSAS’, and sending the envelope to the NHS BSA with the normal monthly submission bundle.
