At PruittHealth, we have spent more than 55 years caring for our neighbors in hundreds of communities throughout the Southeast. We value the trust they put in us each day, and remain committed to providing high-quality services for all our patients, residents, and clients.
I am pleased to present our 2025 Quality Report. As the post-acute care market grows, we are expanding our services into new communities and striving for excellence in those we currently serve. Readers will learn about our growth and our quality success stories.
Additionally, we have made considerable progress on The Oaks at Whitaker Glen, our $385 million active senior living and continuing care retirement community, with new amenities scheduled to open in the fall of 2025.

PruittHealth Premier offers Medicare Advantage plans in some markets. These plans offer benefits that can include a Nurse Practitioner or Nurse Care Guide to manage care and medications, as well as coverage for medications, glasses, hearing aids, flexible spending card, dental care, transportation to doctor appointments, and more.
PruittHealth @ Home allows patients to return home and continue to heal with nurses and support teams monitoring their well-being. Our expert teams also teach patients how to manage their health after an illness or surgery, helping them heal, remain independent, and avoid unnecessary hospital visits.
Home Health patients must be 18 years or older and meet homebound requirements. They can be referred to PruittHealth by various provider sources, including hospitals, skilled nursing centers, primary care physicians, same-day surgical centers, oncology treatment centers, and other healthcare facilities, for as long as the care is medically necessary for the patient’s recovery.

Our hospice and palliative care programs utilize a patient-centered approach focused on quality of life and the important role of family and loved ones.
Our family-focused approach to end-of-life ensures that patients spend their final months well cared for, comfortable, and surrounded by their loved ones. Typically, hospice care serves individuals who have decided to stop pursuing treatments that could cure or control their condition and who have a life expectancy of six months or less.
With a team of nurses, social workers, certified nursing assistants, chaplains, and volunteers, PruittHealth delivers individualized hospice care 24 hours a day, seven days a week in the setting of the patient’s choice.
Our hospice care is always family-centered, involving the patient and family in the decisionmaking process.
PruittHealth Palliative Care provides comfort, ongoing treatment, and spiritual support to ensure the best possible quality of life.
Palliative care is specialized care for those with an acute or chronic illness and focuses on symptom management and stress relief. Patients may have palliative care at any stage of their illness and may receive curative therapy simultaneously. The goal is to reduce a patient’s suffering, provide ongoing treatment, and promote the best quality of life for those with serious illnesses.

Both palliative care and hospice care provide comfort. However, palliative care can begin at diagnosis, and continues at the same time as treatment.

PruittHealth Home First offers in-home services to eligible elderly and disabled people receiving Supplemental Security Income (SSI) and active full Medicaid or pending Medicaid coverage.
Our program ensures the safety of our members and enhances their overall health and wellbeing. We offer adult day health, emergency response, meal delivery, support services, and more to meet and exceed individual health care needs and expectations.

• Medical conditions such as diabetes, high blood pressure, lung disease, arthritis, Alzheimer’s disease, dementia, heart disease, or stroke that require monitoring and regular doctor’s care

PruittHealth Family First is a concierge care program that offers personalized, in-home services to help loved ones maintain their independence while providing peace of mind for the entire family. From assistance with daily activities to meal preparation and light housekeeping, our Family First caregivers offer expert care and compassionate support.
• Personal care, such as bathing, dressing, feeding, grooming, walking assistance
• Respite care for primary caregivers in need of some relief or planning a well-deserved break
• Memory care and support
Based on the client’s condition and requests, a Registered Nurse (RN) makes an initial assessment and creates a Service Plan for specific tasks that need to be performed by a Certified Nursing Assistant (CNA) or qualified Personal Care Assistant (PCA). The tailored Service Plan can be revised whenever changes are warranted.


With more than 55 years of expertise and a family-owned approach, our Pharmacy Services team is committed to supporting better patient outcomes, reducing costs, and increasing staff efficiency in delivering patient medications.
We provide pharmacy solutions in a wide range of settings, from skilled nursing to senior living and more, allowing our customers to serve their patients with safe and convenient medication delivery systems.
As patients transition home from a health care center, we can provide up to 14 days of their prescription medications through Rx@Home. This program allows patients to return home with their medications, offering convenient user-friendly packaging without having to stop by a pharmacy or wait on prescriptions to be filled.
PruittHealth provides patients with everything they need to receive quality infusion services in the comfort of their home or in our pharmacy offices. Our team of experts verifies benefits, determines out-of-pocket costs, and presents any available options to minimize costs for our patients.

Patients will be assigned their own clinical expert who will communicate weekly to ensure the patient is responding to IV therapy and to answer any questions.

The recent expansion of our in-house, state-ofthe-art laboratory and microbiology department allows us to provide a wide range of services for our patients. From comprehensive metabolic panels, complete blood counts, and renal panels to urinalysis, cultures, and respiratory testing, we can provide quick results to caregivers and patients, helping deliver high-quality care to those we serve.
PruittHealth Medical Supply offers medical and nutritional products at competitive prices, shipped directly to our customers. We provide convenient and reliable distribution processes, product quality, and cost containment, with the support of PruittHealth experts.
Through our Biomedical Services division, customers can rent supplies such as C-PAP and BiPAP systems, oxygen concentrators, medical gas delivery, nebulizers, and more.

PruittHealth Therapy Services partners with health care centers and home health agencies throughout the Southeast to provide rehabilitative solutions, including physical, occupational, and speech therapies. With both inpatient and outpatient services, we help patients get back to their full potential – whether in our skilled nursing centers, assisted living communities, or in their own homes.
The goal for our therapy patients is to reach the highest level of function possible. We provide techniques to get patients back to their full potential and make needed changes to their environment to allow for optimal function.
PruittHealth Physicians Services is a medical practice division within our family of providers, consisting of physicians and nurse practitioners who provide services to enhance patient care. We collaborate with hospitals, referral sources, and payers, which are critical to care transitions and reducing rehospitalizations.
PruittHealth Consulting Services provides contracted health care centers and medical providers with clinical guidance and support in nursing, nutrition, social services, rehabilitation, medication therapy management, and more. We provide teams with tools to improve compliance, develop best practices, and create policies and procedures.
Aquatic therapy helps patients recover and rehabilitate. Our HydroWorx® therapy pools feature adjustable depth, resistance jets with massage features, underwater cameras and viewing monitors, and computer documentation.
Cardiac care works with a patient’s team of health care providers, including primary care physicians, cardiologists, clinical nurse specialists, dietitians, mental health professionals, case managers, and pharmacists to help improve overall health and quality of life.
Memory support is designed to assist Alzheimer’s and dementia patients by attempting to slow memory loss and other cognitive changes. It provides life enrichment activities that stimulate the mind and promote overall health. The program also provides family support and education to help caregivers keep loved ones engaged.
Pain management helps patients control chronic pain, enhancing the enjoyment of life and limiting lost productivity and wages. Our pain management team addresses pain and its underlying causes with therapy and treatment plans that include physical or occupational therapy, medication assessment and management, education, injections, or surgery.
Respiratory therapy offers high-quality care and independence to patients who have had a tracheostomy, experience an exacerbation of respiratory symptoms, or require BiPAP/CPAP or ventilator support. Our team of clinical experts develops comprehensive care plans designed to increase long-term health and overall wellbeing.
Stroke rehabilitation helps patients improve physical, cognitive, and lifestyle functioning after minor and major strokes. Each plan is personalized to meet the patient’s unique needs, priorities, and preferences.
Wound care offers comprehensive, advanced treatment options to alleviate suffering and mitigate infection, hospitalization, and limb loss. Our policies and procedures are grounded in research, consultation, and the expertise of certified Wound, Ostomy, and Continence Nurses (WOCNs).

The CNA Career Ladder Program offers skilled nursing and hospice caregivers an opportunity to gain experience and become leaders amongst their peers.
PruittHealth is dedicated to creating opportunities for individuals who are interested in pursuing a career as a health care administrator. Through our multi-year Administrator-in-Training program, an individual receives classroom instruction, hands-on training, and mentorship from experienced administrators. The goal of this program is to develop strong leaders throughout our health care centers, resulting in increased patient care and partner retention.
The PruittHealth Nurse Advancement Program pays the tuition (up to $15,000) of 20 caregivers, including CNAs, Senior CNAs, and LPNs, who wish to become registered nurses.
We recognize that quality people thrive in an environment that promotes lifelong learning. That is why we established PruittHealth University, an online learning resource that is continually updated with new and innovative training programs.
