Anthony Scribner received quadruple coronary artery bypass surgery at Providence Heart Institute and now volunteers as a Pathfinder, making in-room visits to other heart patients at Providence St. Vincent Medical Center. “It can definitely be intense,” he said. “Some younger patients can be angry, because they feel they don’t deserve what’s happened to them. Within each and every one of us is a remarkable engine. Awake, asleep. Resting, running.

In love or out, our beating heart keeps us going and growing. Yet, heart disease remains the No. 1 cause of death in the United States for both men and women.

In 2014, Providence Heart Institute launched a plan to accelerate our efforts to combat heart disease and advance heart health. To propel that plan, we began a campaign seeking $50 million in community contributions. This is our report on our progress and your investment.

Our plan called for us to better integrate our clinical services while bringing effective, compassionate and high-value care to more people. We’ve done that. Our clinics earned national recognition as models for how to collaborate and communicate with patients and their primary care providers. Our heart surgeons achieved the best results within Providence St. Joseph Health. The Ronald and Patricia Peterson Heart Valve Center earned national recognition for repair and replacement of heart valves without open heart surgery. And our Center for Advanced Heart Disease grew to serve thousands of people annually, including hundreds who needed reliable and reassuring care when Oregon’s only heart transplant program suspended operations.

We committed to help more people become their best, healthiest selves through heart disease prevention and cardiac wellness. We’ve done that too. We increased access to cardiac rehabilitation, tripling our capacity and growing from one to four sites.

Established programs such as Play Smart and Heart to Start grew dramatically, and fast-growing new ones joined them. We created “Basecamp” facilities with resources, guidance and companionship for people beginning their journey to greater heart health.

The community of people taking those journeys with us flourished, growing from 1,800 to more than 16,000 in two years.

Finally, the plan called on us to build on longstanding strengths in cardiovascular research.

In response, our physicians and surgeons participated in more clinical trials and brought our patients more new treatments and technologies. These trials powered growth and innovation in our clinical programs and helped us continue to recruit the very best medical talent from around the world. Meanwhile, our data-based research achieved new heights with the creation of the Center for Cardiovascular Analytics Research + Data Science, or CARDS.

Over the past five years, Providence Heart Institute has grown to serve more patients and has transformed the care it provides. With strong donor support, the Ronald and Patricia Peterson Heart Valve Center and the Center for Advanced Heart Disease emerged as leading national sites for treatment and research.

Institute heart rhythm specialists opened new clinics across the state and helped secure FDA approval for important new devices, while surgeons and interventional cardiologists developed new hybrid treatments combining traditional surgery and less invasive catheter-based procedures.

Perhaps nowhere has the transformation of care at Providence Heart Institute been more dramatic than in treatments for people with diseased heart valves.

Created in 2011, the Ronald and Patricia Peterson Heart Valve Center has emerged as one of the nation’s top sites for heart valve repairs and replacements using catheter-based techniques instead of open heart surgery.

Close relationships with industry innovators and a strong partnership with the institute’s clinical research program have meant that the center’s patients are frequently among the very first in the nation to benefit from new treatments and technologies.

Dave teaches sailing and volunteers with his Rotary Club to bring medical care, school supplies and clean-burning wood stoves to remote areas of Guatemala. He received a new transcatheter aortic valve in December 2018. “It has been an amazingly good outcome,” he said. “Before the procedure, if I was walking up a little hill I would have to stop three or four times in a block. Two months later, I’m walking a mile every 20 or 30 minutes, including the hills.”

Roger Toliver had a heart attack at age 47. He and heart institute cardiologists managed his badly damaged heart with medications and other treatments until 2012 when, as Roger said, “It was get a heart assist device or expire.” Jacob Abraham, M.D., oversaw the implantation of that device and cared for Roger until he received a heart transplant at OHSU in 2018, a procedure overseen by cardiologist Jill Gelow, M.D. “I went hunting and fishing with the mechanical device,” said Roger.

“But it’s really great to have a new heart and a team like this to care for me.”

A second major transformation has been the emergence of Providence Heart Institute as a national center for the care and research of advanced heart disease.

Cardiologists in the Center for Advanced Heart Disease care for thousands of patients annually and collaborate with other physician-researchers at Providence Heart Institute and the country’s other leading medical centers. The institute’s heart failure specialists are national leaders in the use of temporary and permanent mechanical circulatory support, transcatheter mitral valve repair, and remote monitors that prevent hospitalizations by catching early signs of a weakening heart.

According to the center’s medical director, Jacob Abraham, M.D., “The population of people who need our care will continue to increase, and we will grow and innovate to advance care for all patient populations. And growth will make us an even more attractive partner for industry and academic innovators testing new treatments and technology.”

Cardiac Rhythm Management

Since 2013, Providence Heart Institute’s electrophysiology program has significantly advanced its care of heart rhythm disorders and grown to serve more than 10,000 patients annually.

Heart rhythm specialists brought their patients novel treatments for atrial fibrillation—the most common heart rhythm disorder—as well as new treatments for other disorders. Patients benefited from procedures that reduce X-ray exposure, technology that eliminates the need for blood thinners, and a pill-sized pacemaker. While making heart rhythm services more accessible by opening clinics in Medford, Astoria, Willamette Falls, Gresham, Hood River and Newberg, the institute grew its device-monitoring clinic to support more than 6,000 patients with pacemakers, implanted defibrillators and other technology. Meanwhile, Providence Heart Institute electrophysiologists were frequent participants in National Institutes of Health and industry-supported clinical trials.

According to Scott Brancato, M.D., the institute’s heart rhythm specialists plan to continue this progress. “Our team will continue to grow, increase access and treatment options, participate in research, and optimize the delivery of care,” he said.

Smart Federico Ardanaz had a heart rhythm irregularity thatPlaywas difficult to diagnose and that made exercise difficult. He and his electrophysiologist, Scott Brancato, M.D., used a smart phone device, then an implanted heart monitor, to identify the issue, which Dr. Brancato treated with a catheter-based intervention. Today, Federico is training for the Seattle to Portland bicycle marathon. “I haven’t had a single event since the procedure,” he said. “I’m very happy with where we are and our whole approach.”

Interventional Cardiology

Interventional cardiologists at Providence Heart Institute built the busiest center within Providence St. Joseph Health for using catheters and stents to treat blocked arteries in the heart and elsewhere in the body. Interventionists treated patients with critical life-threatening blockages and advanced treatments of complex coronary artery diseases such as chronic total occlusion, or CTO, a longstanding and hard-to-treat blockage that slowly starves the heart muscle. The heart institute is now a leading center for CTO treatment, frequently using temporary heart pumps and advanced intracardiac imaging to make more complete and accurate repairs than were possible before.

Suzanne O’Connell had three coronary arteries that had slowly developed nearly complete blockages.

Interventional cardiologist Jason Wollmuth, M.D., used a temporary heart pump and highly specialized catheter techniques to re-open them. “I love doing this,” he said. “I just can’t get enough.”

Suzanne knew she was in good hands. “Everywhere I went people told me, ‘He is the best. You’ve got the best doctor.’” A realtor on the island of Molokai in Hawaii, she added, “I feel great.

I just closed a transaction this morning.”

Cardiovascular Surgery

The consolidation of all open heart surgery to Providence St. Vincent Medical Center in 2017 allowed heart institute surgeons to achieve the highest volumes, best outcomes and lowest costs for cardiovascular surgery in the 50 hospitals within Providence St. Joseph Health.

Surgeons also advanced highly technical treatments for large aneurisms and dissections of the aorta and other life-threatening conditions.

Donor support helped create new heart surgery facilities, a clinic at Providence St. Vincent Medical Center brought patients shorter wait times and increased access, and a new hybrid operating suite at Providence Portland Medical Center supports procedures combining conventional vascular surgery and catheter-based interventions.

Empower people to be their best, healthiest selves.

Thanks to extraordinary donor support, Providence Heart Institute’s disease prevention and cardiac wellness initiative has grown into a flourishing suite of programs serving more than 16,000 participants annually from five locations across Oregon.

The institute’s medical director for prevention and wellness is James Beckerman, M.D. “Someone’s first steps toward greater heart health might be small,” he said. “Our goal is to help them find their path and connect with others who are working to be their best, healthiest selves. We want to support their commitment everywhere they go — the jogging path, the kitchen table, the exercise studio and out in the community. No matter how small their first step, if they start today we know their progress can be remarkable.”

PREVENTION + WELLNESS

Cardiac Rehabilitation The foundation of Providence Heart Institute’s prevention and wellness programs has been the longstanding success of its cardiac rehabilitation program, which helps people improve their weight, fitness and nutrition while reducing stress and heart disease risk factors.

To bring these benefits to more patients, donors over the past five years have helped Providence Heart Institute double its cardiac rehab capacity at Providence St.Vincent, open new centers at Providence Portland and Providence Willamette Falls medical centers, and complete plans for a center at Providence Newberg Medical Center that will open in 2019.

Pathfinders, who are volunteers with past experience of cardiac events, began making hospital room visits to heart patients to offer support and encourage them to participate in cardiac rehab. This peer support and easier access to care helped increase participation nearly three-fold since 2013, to more than 1,300 people. Now, Providence Heart Institute is testing mobile technology to encourage even more involvement. According to Charlotte Douglas, the institute’s manager of cardiac rehabilitation, “Cardiac rehab works.

We’re excited to bring more people into our centers and to use technology to engage people outside the centers.”

In 2018 the institute opened the Hal Lesser Cardiac Rehabilitation Center at Providence Portland Medical Center, easing access for patients on Portland’s east side. According to the center’s medical director, David Schroeder, M.D.,“Too many patients wouldn’t do cardiac rehab because of the travel. So it’s really great to have it here.”

Ginger Park used the center after surgery to replace a failing mitral valve. “The staff in cardiac rehab, they’re my guardian angels, eternal cheerleaders. Before the heart, they’re the soul.”

Lowering Cardiac Risk

The great news about heart disease is that each of us can control 80% of its risk factors.

With this 80 percent opportunity in mind, over the past five years the institute’s Basecamp | Prevention + Wellness has developed free and low-cost programs that empower people with information and tools to lower their risk.

Basecamp participants received free heart screenings, quit smoking and learned CPR. They studied the heart and took home free advice about how to keep it healthy.

Basecamp even provided automated external defibrillators (AEDs) to 34 community organizations, giving the people they serve a much greater chance of surviving cardiac arrest at their site.

Play Smart Play Smart provides free heart screenings to help keep kids safe, active and healthy. It has been Providence Health Institute’s flagship risk-reduction program since its launch in 2012. Founded by James Beckerman, M.D., and funded entirely by donors, Play Smart over the past five years has provided free screenings to more than 30,000 youth from 212 communities in Oregon and southwest Washington.

“Play Smart’s growth has been amazingly gratifying,” said Dr. Beckerman.

Ida York and her husband Brian Beiriger spoke at a women’s heart health fair with Lori Tam, M.D., director of the women’s heart program. Ida had a heart attack when a tear formed inside a coronary artery. Alone when it happened, her quick call to 911 brought medics who took her to Providence St. Vincent for life-saving treatment. “I am so grateful for my care and all the classes—the yoga, the meditation,” she said. “I know this is where I’m going to heal myself.” Brian attends classes with Ida. “It’s been amazing having something like this,” he said. “I wouldn’t know what to do on my own.”

Heart disease is the No. 1 killer of American women, accounting for 1 in 4 deaths.

To help women better understand their risk of heart disease and give them tools to reduce it, Basecamp created monthly Women’s Tea and Talks. Participants received information specific to women and had a chance to try a fitness exercise, yoga or a stress reduction technique. Thanks to donor support and leadership from cardiologist Lori Tam, M.D., in 2018 the talks grew to serve more than 50 women a month. In the coming year, the program will offer women’s heart wellness exams to assess risk factors, as well as treatment from providers with a special focus on women’s heart health.

Improving Weight and Fitness Staying active and eating healthy are two of the most important ways to help prevent heart disease and improve personal well-being. But it’s easy to be intimidated by exercise classes and confused by a thousand competing claims about what to eat. Enter fitness and nutrition

by Basecamp.

Heart to Start, a 5K training program that makes exercise enjoyable, achievable and inclusive was launched by James Beckerman, M.D., in 2014. With the opening of Basecamp in 2017, nearly 7,000 people have had the opportunity to “start small and finish remarkable.” Participants shared movement, nutrition, and cooking classes and toured farmers markets.

In 2018, families joined the FIT Project, a donor-supported program that helps them become their healthiest selves through a sustainable approach to wellness, fitness and nutrition. Participation in the program

helped Layla Resendiz Rangel control her diabetes.

“At first I didn’t know what to expect from it,” she said.

“Before we participated in the FIT project, my son wasn’t very active.

Now he always wants to play outside with his friends and is always moving. And he’s eating more vegetables.

My kids really think about what they eat.”

Today she has lost more than 100 pounds and has completed three 5K charity walks and a Portland to Coast relay. She serves as volunteer liaison between Basecamp and the Providence Portland Medical Center campus. She also volunteers in the Providence Infusion Center and cancer support areas. Reducing Stress Stress may affect behaviors and factors that increase our risk of heart disease: high blood pressure, smoking, physical inactivity, overeating, even cholesterol levels. To help people counter stress, Basecamp offered classes in tai chi, meditation and mindfulness. It also has spread research-backed recommendations for stress management—things as simple as engaging with others in volunteer work and counting our blessings, which are proven to make us happier, more confident and more resilient when facing tough situations.

Tai chi’s slow, fluid movements are low impact, require no equipment or special clothing, and can be performed anywhere.

Its consistent practice is an easy way for people of every age, body type, and athletic ability to bring meditation, awareness, and exercise into their everyday life.

Advance discovery in heart health.

Donors propelled Providence Heart Institute’s longstanding strengths in clinical trials and data-based outcomes research to new heights in a Center for Research + Analytics.

The institute’s growing participation in clinical research continued to bring our patients more of the latest treatments and technologies, doing so years before they became generally available elsewhere. “Five years ago we participated in 27 trials,” said Olivia Derridinger, clinical trials manager. “In 2018, that number grew to 40.”

At the same time, strong donor support launched a transformation of the institute’s data-based research, creating the Center for Cardiovascular Analytics, Research + Data Science, or CARDS.

Clinical Research As it continued to demonstrate high-level clinical expertise and high patient volumes, Providence Heart Institute became an even more desirable partner for medical innovators seeking to test new treatments and technologies.

The institute’s clinical trial program helped secure FDA approval for technologies such as the world’s smallest pacemakers and stents that open arteries and are then absorbed. And through a series of trials, the institute became one of the nation’s leading sites for transcatheter aortic valve replacements, or TAVRs, which allow heart valve replacement without open heart surgery.

John Sargent traveled with his wife, Joan, from Wrangell, Alaska, to have open heart surgery PROVIDENCE HEAR Prevention and Wellne Play Smart for aortic valve replacement. They were thrilled to discover that he was eligible to participate in a clinical trial testing transcatheter replacement of the valve in low-risk patients. A week after his procedure, John was looking forward to getting back to water aerobics, splitting firewood and fishing in the waters in front of his home. “I feel great,” he said. “Let me tell you, coming here expecting open heart surgery and then avoiding it, well that just feels super.” Joan added, “It’s also great to have him feeling better—and so quickly!”

Steve Pokropowicz participated in a trial testing an advanced stent for severe blockages of arteries in the arms and legs. Clinical research coordinator Ellen Muir managed the trial. “I’ve seen Ellen juggle a dozen things at once and still have a pleasant demeanor,” he said. “The whole study was very smooth. And my legs don’t cramp anymore when I go grocery shopping.” The two debated the best technique for making golumpki, or Polish stuffed cabbage rolls. He makes his with meat. Hers are vegetarian. “If he tasted mine, he’d never tell the difference,” she insisted.

As the volume of trials increased, so did the variety of patients who benefitted from Providence Heart Institute’s program. Trials of permanent and temporary implanted heart pumps and remote heart monitors opened new horizons for patients with heart failure and patients whose hearts needed temporary support to heal. Trials testing technology to repair failing mitral valves dramatically reduced hospitalizations and the two-year mortality rate for high-risk heart failure patients. Another trial tested optical imaging from inside arteries instead of X-rays to revolutionize treatment of complex coronary artery disease. Providence Heart Institute’s clinical trial program will continue to accelerate innovation as it tests new treatments for heart rhythm irregularities, vascular blockages and aneurysms, ailing valves, and heart failure.

CARDS: Mining Big Data Hidden inside millions of electronic medical records lies a treasure trove of innovations for better health. Since 2017, more than $7 million in donor support has accelerated Providence Heart Institute’s plans to discover them. Building on the institute’s years of experience with cardiovascular data, the new Center for Cardiovascular Analytics, Research + Data Science (CARDS) develops tools to identify patients at risk for recurrent cardiac events. CARDS also measures and incorporates treatment outcomes into care, identifies treatments that add little value, and identifies treatment disparities among diverse populations.

In one of his studies, Ty Gluckman, M.D., medical director of CARDS, is applying PROVIDENCE HEAR Research and Innovati a novel formula to estimate LDL (bad) cholesterol levels and identify patients St. Vincent’s Medical Center who might benefit from a more intense cholesterol-lowering regimen. First PROVIDENCE HEAR Prevention and Wellne developed at Johns Hopkins Hospital in a study of fewer than 1,000 patients, Play Smart the formula will be tested at CARDS against the records of more than a million patients. The results could change how Jeff VanHoy, one of Dr. Gluckman’s patients, reduces his own cholesterol. “There’s a lot of different information out there about what works for different people,” he said. “So if this research can deliver a more definitive answer, that would be great.”

CARDS: Leveraging Real World Experience In addition to unlocking insights in electronic medical records, CARDS is leveraging the experience of Providence Heart Institute’s physicians, advanced practice professionals and caregivers by expanding support for investigator-initiated research. CARDS provides help in research design and data analysis, with the goal of sharing findings through publication. With more than 26 currently active projects, Providence Heart Institute’s clinicians are researching complications of transcatheter artificial heart valves and novel ways of delivering those valves, the long-term results from implantable blood pressure sensors, and new approaches to streamline patients’ journeys through primary care and cardiology.

“Our goal is to allow busy clinicians to ask and answer questions that will transform the practice of medicine,” said Ty Gluckman, M.D., CARDS medical director.

Xiaoyan Huang, M.D., collaborated with the heart institute’s clinics to help them each become a Patient-Centered Specialty Practice, the first in the nation to be so recognized. In a patient-centered specialty practice, all primary care providers and specialists serving a patient collaborate closely and include patients and families in planning and managing care.

Dr. Huang and her colleagues have published their work in the New England Journal of Medicine and the Harvard Business Review.

The work doesn’t stop. “Our team is still working hard to increase access, efficiency and patient satisfaction,” said Dr. Huang. Her patient, Bob Moore, appreciates this work. “That’s big stuff!” he said.

New Facilities A key objective of the plan we launched in 2014 was to create facilities that make it easier for people to access our care; that support the latest treatments and technologies; and provide wellness-focused experiences for our patients, visitors and staff.

Donors have been vital partners in meeting this goal. With their support, we soon will have created nearly a dozen new or significantly upgraded facilities. Gifts from donors helped create new centers for clinical care and research, our flagship Basecamp prevention and wellness facility, a new cardiovascular surgery clinic, three operating rooms for catheter-based treatment, and four cardiac rehabilitation centers that triple our capacity to provide this life-changing care.
