Dear Sir/Madam,

You've been referred to Provide Physiotherapy for a possible shoulder issue. We're inviting you to a Community Appointment Event designed to address your shoulder pain.

This one-day event brings together a range of MSK services assessments, advice, rehab, and community support all in a non-clinical setting. It’s a new, collaborative approach to help you access everything you need in one place.

Date – Tuesday 16 September 2025

Time – Appointments available between 0900 and 11.00

Location – Corner Stone Church, 83 Witham Rd, Black Notley, Braintree CM77 8NB

Parking – parking onsite

What to bring – pen/pad, comfortable clothing

What to expect?

When you arrive, you will be greeted by a member of our team and checked in.

During this appointment one of our advanced physiotherapists will lead an educational talk regarding shoulder pain. Here they will help you understand why people get shoulder pain and what can help it. You will then discuss your specific musculoskeletal (MSK) problem one to one with our physiotherapy team. This will be followed by an assessment and the start of treatment for your shoulder. There will be no individual clinic rooms, but there will be the experienced physiotherapy team on hand, as well as a range of non-medical services from the community. The aim of the event is to focus on what matters to you as a whole person and as well as elements of your pain or conditions.

Your appointment may take up to 60 minutes depending on the services you choose to engage with. Please be aware that this will be an open environment and if you wish for a therapist to look at a specific

body part, please dress appropriately. You may also be seen after the time stated so please allow plenty of time, but we will do our best to run to time.

Before the conversation about what matters most to you, it can be helpful to think about:

• What do I feel I need from today?

• What do I think the problem is? Have I any ideas what it could be?

• Have I done any reading or talked to others about what it is?

• Have I tried anything that has helped?

You are encouraged to make notes about what is discussed, who you have spoken to and the outcome of the day. Your physiotherapist will make notes to go on your health records.
