Decompressive Osteotomy Introduction
This leaflet will explain what will happen when you come to the hospital for an operation to relieve pressure, pain and stiffness in the first metatarsal (big toe) joint. Why do I need this surgery? This operation is used when there is a painful and slightly stiff big toe joint. We will have discussed that the reason behind the pain and stiffness of your joint is that there is some wear and tear of the joint and damage to the cartilage overlying the bones. This cartilage is designed to keep the bones apart and allow them to move pain free. Unfortunately the cartilage can become damaged and the result is pain and stiffness.
What are the treatment options? There are a number of ways to try and help resolve this pain and that depends on the level of damage to the joint. Treatment options include manipulation of the joint and steroid injection, joint replacement, joint fusion, cheilectomy (smoothing down the joint surfaces) and decompressive osteotomy surgery. The decompressive osteotomy to the first metatarsal is designed to shorten the toe bone. Because the bone is relatively long it jams the joint and increases the pressure within the joint. Shortening this bone will reduce the joint pressure and allow it to move more freely. This operation is normally recommended when the first metatarsal (big toe) bone is relatively long, and is not recommended when the bone is short.

What does surgery involve? On the day of surgery you will be admitted to the ward and one of the nursing staff will check you in, take your blood pressure and perform any other tests that may be required. Your surgeon will remind you of the surgical process and possible complications and will ask you to sign a consent form. The anaesthetist will also meet you and discuss any queries that you might have about the anaesthetic and any issues related to your general health. As the surgery is performed under local anaesthetic only, you can have a light meal before the procedure. At some point during the morning/afternoon you will be escorted to theatre. You may bring a book or personal music player into the theatre. The operation takes about 30-40 minutes.
