he picture of the Indian healthcare system consists of a variety of divergent sceneries.

On the one hand, we can see extra-ordinary hospitals with advanced medical facilities charging a very high cost that seems quite expensive for a middle-class man to afford.

While, on other hand, rural areas consist of small units bearing a few outdated medical amenities in the name of healthcare.

The majority of rural patients are referred for a big-city hospital nearby to get the desired treatment for any serious case. Though the Indian Healthcare system is gigantic, one can find several discrepancies in the rural and urban areas as well. Moreover, public and private sector healthcare systems also have numerous differences in their amenities.

India always has been a great attraction for medical tourists for a prolonged period. There are diverse health challenges found in India during the study in comparison to developed countries.

It is good from a learning point of view for medical students. Also, along with time, numerous efforts have been made for the advancement of India at the global level.

Since independence, the Indian Healthcare system has met with several enhancements. Many diseases like cholera, tuberculosis, and smallpox have been worked for downfall and have succeed. Issues like preventive healthcare, public sanitization, healthcare educational needs, and communicable disease control, etc. are also being resolved dynamically.

Currently, some non-communicable diseases like hypertension, diabetes, cardiovascular diseases, and cancer are great means of concern. Where rural areas lack advanced medical services, public sector hospitals are facing insufficient manpower. Moreover, hospitals also lack good infrastructures such as furniture, medicine, and equipment to handle the vast population of India. Overall, the GDP share on healthcare in India is not as per norms issued by WHO. There is a serious need to speed up the progress of Healthcare system in India.

While one part of this is because of the expertise of the doctors, another part is because of their continued investment in state-of-the-art technology. Six decades later, this facility has retained its reputation of providing world-class care with advanced medical equipment. Dr. Manisha Karmarkar who has shattered the glass ceiling to become Ruby Hall Clinic’s first woman COO speaks at length about her growth trajectory, the hospital’s success story and her changing role in times of a global healthcare crisis.

Could you tell us about the success story sub-specialities, there is virtually no part of the human of Ruby Hall Clinic and elaborate on its anatomy and mind that our doctors don’t care for.

Be it cancer treatment with advanced surgeries and services?

the world’s most advanced linear accelerator, or even housing the city’s most sophisticated one-stop breast clinic with the Senographe Pristina Mammography machine, a 3T MRI with a breast coil and even the provision of Vacuum Assisted Biopsy, we have it all.

From SPECT-CT to 3D laparoscopy, advanced IVF, robot assisted procedures with the daVinci Surgical robot and advanced urosurgery, vascular care, ENT, interventional cardiology and paediatric procedures — our experts are well versed with the nuances of challenging procedures to ensure you get the best results. We’re also a nationally accredited centre for organ and bone marrow transplantation and have some of the highest success rates in the country.

Ruby Hall Clinic traces its deep roots back to the year 1959 when it first began as a two-bedded nursing home.

Over the years, it has become a brand synonymous with technologically advanced quality healthcare and sprawls across a 600 bedded structure in the heart of Pune along with a 130 bedded unit in Wanowrie and a 70 bedded one in Hinjawadi. In addition to this, the hospital has 28 full-fledged diagnostic units across the breadth of Maharashtra.

We’re one of the only hospitals in the country with nine quality accreditations and certifications by the Quality Council of India in Healthcare. Housing a team of close to 4000 members, our doors are open to lakhs of patients each year. With over 40 specialities and

The healthcare industry is seeing a number of changes. What is your hospital doing to stay ahead of these changes?

for patients who tested positive. Our OTs are fully equipped with with HEPA filters and an independent AHU while maintaining a standard temperature. In fact, we maintain a high air exchange cycle rate of at least 35 air changes per hour to effectively reduce the spread of any virus. For all laparoscopic surgeries, we have a comprehensive smoke evaporating system to absorb aerosols.

Out of every crisis, a new opportunity arises. For telehealth, it may be an opportunity whose time has finally come. In the last two years, we had commenced virtual services for those requiring specialist attention whether they were patients living in Nigeria or those from Dhule, a remote town of Maharashtra. The spike in coronavirus cases in India made the availability of this service all the more important.

But this is just one aspect. With telehealth comes the added benefit of remote monitoring, and while our intensivists have already been honing this skill, the pandemic has paved the path for other specialists to follow suit as well. Additionally, our hospital is also deploying the strengths of artificial intelligence (AI) to better patient outcomes — be it in radiology for quicker diagnoses or to improve patient satisfaction as a whole.

• Virtual services for asymptomatic patients: We offered telehealth services from our experts for those who have been quarantined at home, but still need care that’s equivalent or close to what they would get in a hospital.

• Emergency screening protocol: We ensure that every person entering the hospital premises is checked for possible COVID-19 exposure with “notouch” thermometers. If someone with a symptom is detected, they’re is escorted via a dedicated route to our Flu OPD for further assessment.

At a time when the country is grappling with COVID, what steps has Ruby Hall Clinic undertaken to manage the influx of • Robust Disinfection Practices: We have put into place enhanced disinfection measures while cleaning coronavirus patients?

We became persistent in finetuning our daily planning strategies and tactics based on the fluid nature of the outbreak while maintaining the highest level of integrity and transparency. To be specific, here’s what we have been doing:

How has your role as a leader changed in the midst of the pandemic?

As a leader, it has been essential to be both proactive and reactive when it came to dealing with this pandemic.

COVID-19 has affected the world and global healthcare systems on a multitude of levels and while it’s necessary to treat patients infected today, it’s also important to consider long-term implications on the continuum of healthcare as a whole. As an anaesthetist, I have led by example, performing procedures on COVID positive patients day-in and day-out. After all, my duty as a physician will always be towards my patients’ health.

Taking a cue from my example, my medical teams too stepped forth to perform their duties.

• Segregation of COVID positive patients: We vacated an entire building within our hospital premises and used the space solely for COVID-19 patients keeping the rest of the areas infection free.

• A COVID task force: We put together our team of experts in infectious disease, emergency medicine and emergency management who worked closely to keep our staff safe while caring for all of our patients that are impacted by COVID-19

• A dedicated Operation Theatre for COVID positive and non-COVID patients: As an added precaution, we also prepared a dedicated OT

For my staff, coronavirus presented a fear I haven’t seen before. I found that the most important thing is to listen to those fears, to acknowledge them, and to encourage

resolve by being present; by wearing the same mask. I emphasised learning in this unique situation. I emphasised transparency – we’re together, we’re learning, we’re constantly changing.

Could you highlight how the hospital supported employees at such a time?

We at Ruby Hall Clinic undertook a number of measures to ensure our staff remained healthy and safe in times of this pandemic. These included:

• Clarity of communication: Members of the COVID Task-force consisting of top management, doctors, infection control specialists and administrative staff met every single day to discuss updates, delegate activities and review protocols

• Provision of safety equipment: Having had the opportunity to watch how other countries had to struggle for PPEs, we ensured the availability of PPEs, right from N95 masks and full face shields to head covers, gloves, full sleeve waterproof gowns and shoes. All staff is trained to don and doff these in designated areas only.

•

Provision of quarantine spaces: We provided quarantine facilities for our frontline workers within the vicinity of the hospital. Herein, we’ve made arrangements for accommodation as well as food.

Psychological well-being: In order to alleviate burnout and anxiety, we actively scheduled duties and rotations to give our teams the rest they deserve.

• Continued education: We conducted a host of inhouse and national level webinars and individual sessions on infection control and ventilation techniques among others.

skills as an anaesthetist grew, I was eventually appointed as the Director - OT Services at the main branch of Ruby Hall Clinic while performing the same duties at both satellite centres of the hospital.

It was during this time period that my interest in the field of medical management increased tenfold and the hospital’s CEO, Mr. Bomi Bhote saw potential in me to be a leader. I was given the challenging task of holding the reigns of Ruby Hall Clinic Wanowrie as COO and immediately grabbed the opportunity. In the following years, we changed the healthcare landscape in Southern Pune with this high-tech boutique centre.

Recently, I was trusted with the role of COO at Ruby Hall Clinic’s main centre at Sassoon Road. The rest, as they say, is history! I have the incredible chance to lead an organisation which combines my three lifelong passions: my love for medicine, the ability to empower all those around me and create a healthcare environment that focuses on clinical excellence, quality care and patient satisfaction.

Along with pursuing my duties as COO, I’m still a practising anaesthetist as well as head robotic surgery and a state-of-the-art breast centre at the hospital.

Along with this, I play a key role in the financial management of the organisation and am responsible for the profit centres.

Recently we have noticed that healthcare organisations have started relying on technology to a large extent; what is your opinion regarding the role of technology in transforming healthcare?

The fact that technology and healthcare are interdependent is not new — healthcare needs spur the development of new technology, and in turn, technology improves healthcare delivery. Moreover, concepts such as wearables, artificial intelligence, IoT and telemedicine are not new terms in the healthcare sphere. Although their reception has previously been lukewarm, as the world grapples with a new way of life and the COVID-19 crisis peaks, we’re increasingly looking at healthcare systems and people turn towards technology for solutions.

Remote monitoring is here to stay, and we expect it to become the standard of care for our most vulnerable patients in the face of this pandemic. The impact of AI, IoT, and other technologies through natural language processing (NLP) and machine learning (ML), is also transforming care delivery. From diagnosing diseases faster, to continually monitoring patients, and even remaining virtually connected to patients and their healthcare records, we’re inevitably turning towards technology to transform healthcare for the better.

What advice would you give to other female leaders?

There’s a favourite quote of mine from Eleanor Roosevelt which reads, ‘One’s philosophy is not best expressed in words; it is expressed in the choices one makes, and the choices we make are ultimately our responsibility.’ While many of us have believed passionately in women leaders for many years, now is a time for action. My advice to female leaders would be: Believe in yourself and your potential.

For all of the women reading this, let’s choose to be a force to reckon with. This means connecting the dots to preserve our legacy, promoting our colleagues, taking on a mentee and making their success a necessary part of our own, tackling the systems that are inadvertently holding us back, and be really good at what we do. We are not weak, we are not whiners, we are trailblazers.

Could you share with us your vast experience in the healthcare domain, both as a clinician as well as a management professional?

A journey that started 25 years ago when I was still finding my feet as an anaesthetist has culminated into me leading the same organisation in the capacity of the COO. It was in the year 1995 that I started my career as a Senior Registrar in Ruby Hall Clinic’s Intensive Care Unit (ICU). Two years later, I was recruited as a consultant in the Department of Anaesthesiology. It was from there that I slowly began the climb up the ladder. While my

NCs around the world have now had time to adapt to a new way of life. Although, with uncertain prospects for the future, CEOs must avoid growing complacent.

Most business leaders are used to dealing with uncertainty but little could have prepared them for the upheaval caused by COVID-19. All of a sudden, CEOs were forced to adapt, setting up their teams to work remotely and, where possible, moving their services and operations online.

But now, six months on and with this exceptional situation set to last, how can business leaders continue to operate and meet the often-contradictory demands of employees and the organization? Also, what lessons have they learnt from the early stages of the pandemic that can help?

Contradictory Challenges

Crisis Mode

When the pandemic started, CEOs’ first priority was to protect their people, their company and their customers.

Besides this, ensuring business continuity was their main focus, whilst very quickly managing their cost base.

Appreciating the scale of the challenge was vital. Although realizing that COVID-19 isn’t going away, CEOs are moving out of crisis mode and considering how best to do business in a new ‘normal’.

Most often, the demands being placed on CEOs are diametrically opposed, which is pulling them in different directions.

The economic environment is quite tough at present, requiring equally tough decisions. It has been forecasted by OECD that the global economy will shrink by 6-7.6% this year, representing the deepest recession since the Second World War. Several jobs are being axed every day in every sector, with the OECD predicting that “even in the more optimistic scenario for the evolution of the pandemic” the unemployment rate in OECD countries shall exceed to a new high ever since the Great Depression.

Despite the fact that CEOs may want to hold on to as many people as possible, they have a cost base to manage and they have to deliver to investors. Hence, their dilemma is how to provide employees with the support they need while optimizing business performance. Also, how to focus on current priorities – grappling with coronavirus-driven shocks to the business – while assessing new opportunities created by the dislocation.

There is another challenge to look at which is achieving the right balance between virtual and human interactions.

However, CEOs also recognize that connections across their business are starting to fray and that some people’s workfrom-home arrangements are less than ideal. So, the leaders need to find out the ways to maintain cohesion when people are physically apart. Also, how can they be understanding of individual circumstances while remaining focused on business performance, is another aspect to look at.

Ability to unify teams

Working from home has allowed organizations to continue to operate, and with companies from Amazon to Zillow extending their remote working policies, many will be doing it for a lot longer.

Although reflecting on their early pandemic experience, many CEOs feel that they need to do more to bring their virtual teams together. Going ahead, CEOs should conduct more one-to-one check-ins, establish better methods of linkage – across senior people, functions and projects – to replicate the collaboration facilitated by office life, and enforce black-out times to avoid employee burnout.

Leadership Skills

CEOs need to be utmost flexible with a multi-faceted skill set while managing such dilemmas. Driving performance is simply not enough.

Flexibility

At times CEOs have to deliver complex, yet opposing messages. There are many instances wherein CEOs need to maintain staff morale and keep people informed and then, the next day, having to issue a note about the pandemic’s ruinous effects on the business and the urgent need to improve performance. It is a delicate balancing act which the CEOs should learn.

Long-term thinking

It is difficult to plan ahead amidst so much uncertainty, with some organizations putting detailed five-year plans on the back burner. While there are other companies as well that are continuing with their usual plans, even if they aren’t sure how much faith to have in them.

For many CEOs, long-term alignment is a relatively low priority right now and it is hardly surprising, but some level of forward planning is useful. It is recommended to find small amounts of time to focus on it and acknowledge that the uncertainty can be unsettling.

Decisiveness and Innovation

Business leaders were forced to make quick decisions with incomplete information during the initial stages of the pandemic. Likewise, CEOs will need to continue to lead without the visibility they are used to. During this inherently unknowable situation, no amount of analysis can tell them what proportion of their retail outlets will be open by next March or how efficient transport links in their supply chain will be. Meanwhile, innovation is vital.

According to André Helfenstein, CEO of Swiss Universal Bank at Credit Suisse, the pandemic brings opportunities that swift decision-making, and the courage to change, can unlock. André further adds that this pandemic situation forces us to more radically question certain businesses, set-ups, processes and investments that previously seemed a given or were changing, but too slowly. Stricter cost management and more efficient ways of operating ultimately free up resources that can be invested in new

It is expected that for CEOs, the COVID-19 crisis turns out to be the ultimate leadership challenge. Those leaders that make it through will be seeing meaningful jump to their businesses, personal development and in societal terms.

It is a galvanizing experience knowing that you are robust enough to survive, and possibly even thrive, in a crisis situation of this magnitude. This shall provide CEOs with the confidence and multi-faceted skillset to weather most future black swan events.

Mr. Prathap Selvan’s Inspiration behind Frutunes Food Products

The driving force behind establishing Frutunes Food Products is the plight of Indian farmers who work in tough conditions but end up getting less price for their produce.

Indian Farmers need to deal with numerous factors before they can sell their produce in the market.

Factors such as limited connectivity, inadequate cold storage, infrastructure, supply-demand mismatches, exorbitant payments to middlemen, and agents lead to grave losses.

This has impacted farmland income as well as the morale of the farmers who then have to rely on land-linked loans to survive. This situation has pushed many Indian farmers to the brink of bankruptcy and some to suicide.

ccording to estimates by the United Nations’ (UN) Food and Agriculture Organization, 194 million Indians go hungry every day. About 40% of fresh produce is wasted annually before reaching consumers due to a lack of supply chain infrastructure. The UN organization attributes this problem too massive food wastage — about US$14 billion.

Here comes a visionary who aims to revolutionize the Agritech space in India. Mr. Prathap Selvan established Frutunes Food Products as an Agrotech startup company which has redefined the agri-market and helping farmers in scaling up their agricultural business. According to industry estimates, the global agrotech market is growing at a rate of CAGR 18 percent and is expected to reach the worth of $ 24 billion by 2022.

Selvan shared his experience that during the initial years, there were many challenges in front of Frutunes Food Products. According to him, the primary challenge which the company faced was the big exporters in the market who knew the gaps in the system and often outbid them.

The other challenge was to maintain the quality of Organic Products. Due to large volumes, it was tough to procure and grade the farmers’ produce but gradually, as the company gained experience, things improved.

What Can We Expect From Frutunes in Future

Selvan shares his plans for Frutunes. They are planning to focus on packed fruits and pulp. There is tremendous opportunity in this segment for Frutunes Food. Consumer preferences are changing towards healthier options.

People now tend to prefer food products that are naturally prepared from organic raw materials. Fruit pulps have a longer shelf life as compared to the natural form leading to lesser transportation and storage costs for manufacturers.

Selvan told that the best thing about Frutunes is that they procure directly from farmers. As an agrotech startup, they have eliminated any scope of middleman or agents. 116 FPOs have registered with them and they have 5 preferred food manufacturers also. Frutunes have a locking period of 90 days, where it will give orders to the FPO/Manufacturer at the agreed rate. They aim to fulfill requirements along with the shipment of the products within 10 hours.

Mr. Prathap Selvan’s Journey as an Entrepreneur

For Frutunes, clients are supreme and hence they pay a great amount of attention to understand the needs and requirements of the customers. On the domestic front, they are getting amazing support from FPOs for large orders.

They have a good set of clients at GCC and also work on GCC government tenders as their product quality is organic and quite good. The company has grown tremendously in the last one year and has branches at Jordan, Utah, USA, GCC, and Asia-pacific regions.

Why Customers Should Trust Frutunes Food Products

Selvan describes his entrepreneurial journey as an adventurous one which has taught him great lessons. He was in the final year of engineering when this idea of entrepreneurship struck him. According to him, it was the best decision that he had taken. Since then, it has been 6 long years and a Failed Business that has brought him where he is today.

Presently, he is also working as a Brand Strategist and Consultant for Businesses and helping them to grow. He believes that he still has numerous things in his bucket list that needs to be ticked before he ends his entrepreneurial journey.

With Frutunes, one thing is sure that there is no question of mistrust. The company has developed a mechanism to transfer 100 percent money without commission directly into the farmers’ bank accounts within 24 hours of purchase. Otherwise, in the open market, it takes more than 15 days at least for farmers to get their money, and sometimes they don’t get 100 percent also.

What Do You Hope To Accomplish Within Next Few Years

In the words of Prathap Selvan, “Agriculture is 20% of the Indian economy and 49% of the workforce. So, why do I need to do more than agritech?” He also shares that 13 agritech startups have raised total funding (disclosed amounts) of around $65.6 million in 2018, up over 21 percent from total investments of around $54 million for 18 agritech startups in 2017.

Frutunes claims to be transparent in their dealings with farmers which is seldom to be seen in the Indian markets.

They communicate the actual demand and estimated price before the harvest. It assures the farmers and safeguards their earnings.

Although the size of the funds raised by agritech startups is small when compared with the overall size of investments raised by startups in the country, there is an overall improvement from the investments seen in the space in previous years. Investors firmly believe in Agri Tech startups who are doing social ventures to farmers and also to society globally. They are investing in Agri tech because of the massive value it is generating.

Frutunes’ model is based on removing inefficiencies and intermediate middlemen from the supply chain. Keeping this in mind, the company has developed “Quality Basket” comprising of quality measures that are adopted in GCC. It has entire details of the Farmers and FPOs.
