Note: 100% of the moneyreceived for the above physical examinations go directlyback into the SCHS Athletic Department to payfor our on-campus athletic trainers and athletic trainer supplies.
A PARENT MUST ACCOMPANY STUDENT-ATHLETE TO FILL OUT MEDICAL HISTORY AND SIGN CONSENT FORM BEFORE YOUR SON/DAUGHTER CAN BE EXAMINED.
ATHLETIC CLEARANCE PHYSICAL FORM AND INSTRUCTIONS WILL BE AVAILABLE ONLINE @ www.sctritons.com after May 15th, click on Athletics - click on Athletic Clearance)
Bring PHYSICAL FORM ONLY to your assigned time on this day and then you will upload the form along with a copy of your medical insurance card and submit online @ www.athleticclearance.comyour son/daughter will then be cleared for the entire 2023-24 school year!!
