Improving provider care at Estes Park Health

A

fter the immense global and local challenges of 2020, and with the continuing challenges of COVID that is still heavily impacting Estes Park Health, through it all, EPH continues to move ahead to build, rebuild, and reinforce our core ser vice lines. In 2022, we’ve been ver y fortunate to begin to turn the corner on physician and other care provider recruitment and to bring back some of the services that were interrupted during the two years of pandemic.

Our chemotherapy program is just on the cusp of re-launching at EPH, after pandemic-caused staffing shortages forced us to take a hiatus of this critical in-town service. Infusion and chemotherapy now happen in the “hospital outpatient services center,” where rehabilitative services used to happen before those moved to our new Urgent Care Center in mid-2020. This is a beautiful, sunny, spacious area, and also has several private rooms for those who need that level of care. It’s a much improved patient experience than we were able to offer in the past.

Speaking of 2020, our Urgent Care started ver y slowly in 2020, launching in the most difficult time possible, as the pandemic exploded and tourism dwindled.

Now, in our second “real” summer since then, business is booming, and the positive margins we saw in 2021 from that ser vice line should be surpassed easily for all of

Ophthalmology ser vices were paused after long-time physician Dr. Michael Prochoda retired from his practice. Our search for a replacement has now reaped wonderful positive results: Dr. Sarah Dech will come on board this fall to revive the operation. We’ve also hired an ophthalmology assistant to support Dr. Dech as she begins to see patients and brings us to a new chapter of top-quality ophthalmologic medicine at EPH. Our physician clinic is busy and growing, and we’ve been able to solidify and expand our excellent internal medicine and family practices. Dr. Paige Dekker has come onboard to provide several days a week of family practice.

We’ve had the good fortune to have Dr. Hollis Burggraf fill in as interim for the

retired Amanda Luchsinger, and soon we’ll have Dr. Cynthia DiLauro here to permanently step into Dr. Luchsinger’s shoes. With the population of Estes Park having the oldest average age in Colorado, internal medicine is of high importance, and it’s a good thing to keep those positions filled.

We added a podiatry ser vice line (the first time EPH has had an employed podiatrist) with Dr. Paxon Riding.

This fills a care gap in podiatry that has not been filled in our community for many years.

At the start of the COVID pandemic, in early spring 2020, our radiologist retired from Estes Park Health, and for the subsequent two years we’ve used an external teleradiology agency to provide 100% of our diagnostic imaging reads. We are ver y happy

that radiologist Dr. John Knudtson has joined our staff of employed physicians. He is now providing full radiologic services four days a week, right at our EPH campus. We have hopes for expanding that ser vice line over the next months and years, stay tuned.

This has been a banner year for filling the open spots on our physician roster, and for adding to that list where appropriate, to provide the broad and high-quality ser vice you expect from Estes Park Health. As we continue to work to streamline EPH operations for a long-term sustainable future, it’s very important that we keep our core ser vices strong, and these recent additions and changes are moving EPH in the right direction and are improving and broadening the clinical care you need, right here in town.

New podiatrist joins the EPH team of physicians

stes Park Health recently added a podiatrist to their growing staff of physicians. Dr. Paxon Riding started June 1 and has been seeing patients at the specialty clinic at Estes Park Health since the last week of June.

Trained in everything from general foot care to trauma injuries of the lower leg, Riding looks for ward to working with people who thrive in an active community and at a hospital that emphasizes the importance of preventive care for their patients.

“Estes Park is a little bit of an anomaly,” said Riding.

“There is an epidemic of diabetes, but Estes Park is not like that. Estes Park is a retirement community with an influx of active people that use their feet a lot. At previous hospitals, I’ll see a 68year-old male and they are typically a non-retired, 300pound trucker with diabetes.

Riding, a native of Idaho, completed his undergraduate degree in ecology at Brigham Young University – Idaho in Rexburg. He originally planned to follow a path as a biologist but after learning more about the challenges of frequent moves, low starting pay and seasonal work — all difficult factors for him and his quickly growing family — decided to look at other career options.

with his brother-in-law, also a dentist. He shadowed a few dentists and decided that path had good opportunities, even though looking at mouths upside down struck him as an odd perspective.

Riding started the process of attending dental school, with Temple University in Philadelphia being a top choice. While researching Temple, however, he noticed the university also had a podiatr y school.

“I grew up in the small town of Rexburg, which only had about 10,000 to 15,000 people and only one podiatrist,” explained Riding. “He let me shadow him for a few days and I confirmed this was a better option than dentistr y because he seemed happy, seemed to have a See Paxon, pg. 21

Old Fall River Road is a great route to the top

old Fall river road is an unpaved road which travels from endovalley Picnic area to above treeline at Fall river Pass, following the steep slope of mount chapin’s south face.

We were treated to many appearances by marmots along the route. The road itself is lined with tiny perfect “Christmas trees.” When you pass over water, roll down your windows to enjoy the sound of the rushing streams.

Fall River creates some spectacular rapids that are a joy to look at.

here is something extra sweet about savoring a seasonal delight. Old Fall River Road in Rocky Mountain National Park is one of summer’s highlights for many people who love to explore Rocky Mountain National Park. It follows the steep slope of Mt. Chapin’s south face, taking drivers through forests and by waterfalls.

The first automobile road to penetrate into the interior of the park, Old Fall River Road was constructed by convicts between 1913 and 1920. Search online to see of the amazing photos of this engineering feat.

As a one-way gravel road, cars are not allowed to travel at more than 15 miles per hour. Not that you would want to! The winding road takes you 9.4 miles up to the Alpine Visitor Center.

The scenic pullouts allow you incredible vistas you cannot find on any other road in Rocky Mountain National Park.

Stanley Caretaker’s Cottage pays homage to the past

uring the past year, owners of The Stanley invested time, research and money into a new visitor site on campus.

The Caretaker’s Cottage has undergone a $2 renovation and is the newest tour attraction at the historic hotel in Estes Park.

“This is such an exciting project,” said John Cullen, President & CEO of Grand Heritage Management LLC. “This is a fabulous space and we expect at least 30,000 to 50,000 visitors to see it this year.”

Original hotel owner F.O. Stanley used what Cullen called “the Amazon of his time” which was the Sears catalog to furnish much of the Caretaker’s Cottage. The renovation team acquired a 1904 Sears catalog and found or made replicas of many of the furnishings.

The original caretaker, Alfred Lamborn, and his family lived in the two-story cottage adjacent to the hotel. It was one of the only buildings on site to have insulation and running water in the winter.

He lived there year round.

Lamborn also ran the golf course and the hydroplant on Fall River.

When author Stephen King published “The Shining” in 1977, he was inspired by a stay at The Stanley. The caretaker plays a central role in the horror film based on the novel. The movie was directed by Stanley Kubrick and released in 1980.

Just as in the movie, the Lamborns had a young child.

What to know when it comes to sun protection

ith summer here, you may be rushing outside to enjoy the sunshine and warm weather.

You may also be thinking about how to best protect your skin.

More people in the U.S. are diagnosed with skin cancer each year than all other cancers combined, and most skin cancer is associated with sun exposure. Knowing how to help protect your skin from the sun may have a lasting impact.

To help set the record straight, here is some useful

True: When walking down the sunscreen aisle, the choices can be overwhelming. Start by looking for a sunscreen that provides broad spectrum protection.

That means it filters both types of ultraviolet radiation.

The Skin Cancer Foundation says that ultraviolet A rays are present all day and can cause skin-aging and wrinkling while ultraviolet B rays are strongest during peak hours (10 a.m. to 4 p.m.) and can cause sunburns. Both types have been linked to skin cancer.

False: Sunscreen should

be reapplied throughout the day and SPF, or sun protection factor, can help you know how often. If you typically burn after 10 minutes in the sun, multiply that number by the SPF. For SPF 30, this would translate to 300 minutes, but the sunscreen will start to rub off before then. So, reapply often, at least every two hours.

Sunscreens should also be reapplied immediately after swimming, toweling off or excessive sweating.

Report: Connections are important for health of seniors

In the last 10 years, nationally, we saw a 13% increase in the suicide rate, a 9% increase in the prevalence of depression, and an 8% increase in the prevalence of frequent mental distress among adults 65 and older.

W

e are all wired for emotional connection and that doesn’t stop when we retire.

In my experience as a geriatrician, seniors who have family or a strong support network by their side are more likely to remain living in their homes longer, get the care they need, and have better health outcomes.

Unfortunately, seniors who are more isolated with less of a social support system, experience more challenges and often don’t receive important care.

Through my practice, I learned the path to better health for our seniors isn’t always through the doctor’s office. When seniors are a part of an active community, their health outcomes improve. But in many instances, these communities have been less active due to COVID-19. The lack of community support throughout the pandemic has exacerbated the impact of social isolation and mental health challenges, which affect quality of life and a sense of purpose and connection as our seniors age.

The United Health Foundation’s 10th annual “America’s Health Rankings Senior Report” – which highlights a 10-year analysis of improvements, challenges and disparities in health and well-being across all 50 states – reflects this reality for older adults in America who felt the impact of the pandemic in many ways.

Unfortunately, the Senior Report revealed a decade of worsening mental and behav-

ioral health challenges among older adults. In the last 10 years, nationally, we saw a 13% increase in the suicide rate, a 9% increase in the prevalence of depression, and an 8% increase in the prevalence of frequent mental distress among adults 65 and older. And drug deaths increased 147% among seniors, ages 65-74.

The behavioral health measures in Colorado were most concerning in this year’s report, with the state ranking 43rd for suicide among those 65 and older. Additionally, Colorado ranked 37th in excessive drinking.

The correlation between connection, community and independent living for seniors is so important to achieving healthy outcomes and a higher quality of life, including those behavioral health measures. But when someone doesn’t have a relational or chosen family, it is up to society to fill that need.

Colorado ranked 5th overall healthiest state in this year’s Senior Report, which is ver y strong. If the state is looking to continue to invest

in the health of its seniors, there is a strong case for supporting programs that center on connection and community, especially for seniors who don’t have a built-in familial network of their own.

A study published in Health Ser vices Research in 2013 found that increases in home- and community-based ser vices for older adults, such as home-delivered meals, congregate meals or community centers, were associated with a decrease in the rate of low-care residents in nursing homes. And data from the 2018 Older Americans Act Report to Congress also suggested community support ser vices are effective — 65% of congregate meal clients and 92% of home-delivered meal clients reported that these ser vices helped them continue to live independently at home.

Increasing the number of licensed home health care workers per capita is another area of opportunity.

The AARP Foundation offers a program, Connect2Affect, that helps seniors who are experiencing isolation or loneliness. This program offers solutions in collaboration with the Gerontological Society of America, Give an Hour, USAging and UnitedHealth Group.

Connect2Affect uses research to create a deeper

understanding of loneliness and isolation, draw crucial attention to the issue, and catalyze action to end social isolation among older adults.

It’s clear that programs that support behavioral health and social services for older adults can help them maintain independence and hopefully, improve behavioral health measures.

We need to help the seniors in our lives reconnect with their communities and activities they have enjoyed in the past but have not yet returned to on a regular basis. And we need to bolster programs that offer evidencebased community support.

Equipped with this data, I hope we can work together to address emerging health disparities among older Americans so they can remain independent, in their homes, and connected to their communities.

Remember, it’s normal to have some worry about a change in your routine — and you may not be the only one at your company who’s feeling it. In a survey by the American Psychological Association, roughly half of adults reported being uneasy about returning to in-person interactions.

A major life change — like where you work — may be one of your stress triggers.

The important thing is to recognize the anxiety and come up with healthy ways to help manage it.

The renovation of the Caretakers Cottage includes a guest room which has been turned into a replica of “The Shining” suite – Room 237.

Visitors will see the axe used in the film and they can crawl into the bathroom tub.

“Ever y husband or boyfriend is going to want to put his partner in the tub and take a photo,” Cullen joked.

No one has slept in the Caretaker’s Cottage in a hundred years. The Stanley is auctioning off the first stay in the renovated building during an auction the first week of September. The money

raised will be donated in its entirety to a women’s cancer charity in California.

“The auctioneer is suggesting the opening bid be $100,000,” Cullen noted.

The Caretakers Cottage renovation was completed on Monday, June 13. Visitors are already streaming through.

“The tours last about an hour,” Cullen explained.

“They are greeted by Betty, the grandmother who did all of the cooking back in the old days. She will serve the visitors a complimentary drink and a cookie from a 1904 recipe.”

When asked if he had ever been to Estes Park, Riding answered that he had not, but he had visited the Front Range where, until recently, family lived in the LovelandFort Collins area.

According to Riding, podiatrists work on anything from the knee and below, including high-level trauma, bunions, orthotics and anything on the lower leg. They also are an important part of the diabetes care team, helping diabetic patients prevent the serious and life-changing outcomes, like amputations, that can result from poor care and prevention.

“Just before starting this job, I was working at a reservation in Oklahoma,” stated Riding. “Diabetes is rampant down there. It wreaks havoc.

In the hospital, I was doing amputations three or four times a week as a result of uncontrolled diabetes. Helping people prevent that result is a big part of podiatric practice.”

Now that he is in Estes Park, Riding anticipates continuing preventive care as part of his practice. He also anticipates being able to apply his experience in trauma work because of his

Riding went back to school at the University of Idaho – Idaho Falls to complete his prerequisite courses and was accepted into Temple University School of Podiatric Medicine. Riding completed his Doctor of Podiatric Medicine degree in 2015 and went on to complete his residency in podiatry at Main Line Health in West Philadelphia.

The Oxford Dictionary defines podiatry as “the treatment of the feet and their ailments” but, as Riding explains, there is much more to the specialty.

“During my first year of residency, I took the majority of the emergency cases because Main Line Health relies on podiatry for lowerlevel trauma cases,” said Riding. “In my first year, I got a call from a trauma surgeon about a woman who had been run over by a bus. At first glance, the x-ray looked good with just one fracture.

You can stop at the parking area at Chasm Falls. It’s worth it to get out here and hike the short trail down to the falls which have a dramatic 25-foot drop.

The road continues up into the subalpine zone with more spruce and fir trees. The view opens up from here and if you stop at one of the overlook pullouts, you will be able to drink in the dramatic views of the Fall River Valley.

Near the top of the treelined part of Old Fall River Road, you will notice the trailhead for Mt. Chapin on the right hand side.

When you reach the tundra, the trees disappear and

you travel through a distinctive landscape. To the left, you will see dramatic small waterfalls created by snowmelt that fall hundreds of feet down sheer rock faces.

If you want to stop for a tundra hike, there is a great pull off just before you read the Alpine Center.

Of course, to get back to Estes Park, go through the parking lot of the Alpine Center, take a left, and return on the paved, two-way Trail Ridge Road.

Old Fall River Road usually opens 4th of July weekend and closes in early October.

Enter the park at the Fall River entrance for easiest access.

Old Fall River Road was constructed by convicts between 1913 and 1920.

While sometimes back pain and other orthopedic problems can’t be avoided due to previous injuries or other factors, it’s important for people to focus on their CORE, which stands for correct posture, overweight (avoid it), relax, and exercise. To build on that concept, here are three strategies and evidence-based care methods to consider to help prevent or treat this common issue.

Focus on Posture. Whether you are now working at the kitchen table or on the couch, focusing on proper posture may help. Make sure you are sitting up straight

with your knees at a 90-degree angle, with your shoulders in a straight line over your hips and your ears directly over your shoulders.

If you’re working at a computer, adjust the screen height to eye level and consider elevating the keyboard to help keep your hands, wrists and forearms in line and parallel to the floor.

Stay Active. While some people with back pain or other muscular issues may be tempted to consider rest, staying active in many cases may be the best option.

Low impact activities to consider include walking and swimming, while research indicates that strengthening

leg muscles may also prove helpful. You might also try yoga and tai chi, as they’ve been shown to ease moderate to severe back pain. If time is a factor, a brief walk at lunch or going up and down the stairs a few times can help you stay active.

Examine Your Options. The American College of Physicians recommends exercise-based therapies first, including nonsurgical options such as physical therapy, chiropractic care, acupuncture and overthe-counter anti-inflammatory drugs. To make access even more convenient, new virtual physical therapy

options have emerged, including some that provide users with on-demand, 24/7 exercise feedback powered by artificial intelligence.

Even for people with chronic back pain, only a small percentage may need imaging or surgery. Taking these preventive steps – and selecting evidence-based care approaches when issues arise – may help reduce the risks and complications associated with back pain and other orthopedic issues.

Poppy’s Cafeteria was a play on words as a reflection of Mama’s and as another flower opposite of a rose. By the time the Piepers bought Poppy’s on Aug. 1, 1994 it had already been changed to Poppy’s Pizza & Grill, then they purchased Mama Rose’s in the spring of 2002.

The Piepers attention to quality is only matched by their willingness to care.

Mama Rose’s has become famous for fine dining and as host to fundraiser dinners.

The Piepers have been dedicated to a number of nonprofit organizations that support vital causes in the community.

Chief among those is the Estes Valley Restorative Justice Partnership (EVRJP) which partners three organizations (Police / Nonprofit/ Town of EP) to remediate crime instead of merely pursuing punitive punishment.

Rob joined the Board of Directors of EVRJP after he had a personal experience with the program. Unfortunately, he and Julie were victims of a burglary, but in the

“The judiciary process was settled with appraisals and deflation calculations, whereas with restorative justice we found peace and satisfaction with the result,” said Rob. The next fundraiser for the EVRJP will be on Nov. 30 just a week after Thanksgiving.

Two other fundraisers hosted by the Pieper’s have already occured this year, for The Learning Place and Partners Mentoring Youth. These were in Apr. and Mar. respectively.

Partners Mentoring Youth is synonymous with “Partners” which is a group that helps children be involved with positive people that are compelled to mentor as

opposed to mere babysitting.

Most of the children in this program are referred through an organization like Restorative Justice or The Learning Place, however any guardian can choose to enroll their child in the pool of partners if they feel like a trusted person with an outside perspective could reflect positive behavior for their child.

As it happens, many of these mentors become great friends over time, such stories are shared at the fundraiser dinners and new enrollees get to see that the Partners are truly just trying their best to be good, friendly people.

The Estes Park Learning Place is a local nonprofit operation directed by Sue Yowell; her team helps all ages, but the office at 600 S.

Saint Vrain is primarily for the children. Their services include extra tutoring, test preparation, and adult learning classes.

Both Julie and Rob have served on the Board and as President for The Learning Place. One of their employees who immigrated to the United States worked with The Learning Place to earn a GED, not because he needed the certification but to stress the importance of education to his children. Last year at the annual fundraiser dinner they announced that his daughter had just completed her first year of college as a first generation college student.

In the scheme of things creating a good community requires well-educated, wellrounded and thoughtful people, which is what any Mama and Poppy would hope for.
