POINT OF CARE TESTING: WHAT’S NEW?

It has been one of the primary goals in the reformation of our healthcare system to have patients more involved in managing their own health. The Affordable Care Act (ACA) promotes the shift from curative to preventive medicine by focusing on early detection and management of chronic disease, along with a more patient-centered approach to health care. POCT promotes these goals with rapid test results that providers can use to immediately inform patients of their condition or progress, and modify their treatment on-site. This provides a face-to-face opportunity to ensure understanding and discussion of future goals, thus more directly involving the patient in their own care.

POCT is defined by the College of American Pathologists as “tests designed to be used at or near the site where the patient is located, that do not require permanent dedicated space, and that are performed outside the physical facilities of clinical laboratories”. POCT also streamlines the testing process by obviating the need to collect a specimen, transport 4

it to the lab, then perform testing, and transmit the results back to the provider. This speed and efficiency often improves both patient outcomes and patient satisfaction.

Increasing Demand for POCT These advantages are further enhanced by continuous technological innovation, improving the quality, convenience, connectivity, speed, and range of applications. Emerging technological innovations include smartphone apps, biosensors, lab-on-a chip, and wearable devices. The transmission of POC test results in Electronic Health Records (EHR) further enhances the advantages of POCT. These results can be shared instantaneously with all members of the medical team, enhancing immediate patient assessment and treatment as well as making POC data available for analysis.

automated laboratory testing include: • The push for patient centered care (rise of personalized medicine) • Chronic laboratory staff shortages • Rapidly increasing older population, leading to increased incidence of chronic diseases • Rising incidence of lifestyle diseases (e.g. cardiac, diabetes) • Increased demand for home-based POC usage (facilitated by technological innovations) • Increasing trend toward healthcare decentralization (linked to rise of mobile technology) • Long-term savings/cost benefit analyses (preventive vs. curative medicine) • Rural locations with limited laboratory services (linked to laboratory staff shortages).

POC Testing: What, Where and Who? In light of the many factors leading to the increased demand for POCT, manufacturing companies are pushing the envelope to make POCT devices faster, easier, and more reliable. The global POC diagnostics market is forecasted to grow at a compound annual growth rate (CAGR) of 9.3% from 2013 to 2018, and to reach $27.5 billion by the end of 2018.

Hundreds of tests once considered too complex for POC are now routinely performed outside the laboratory.

3 THINGS PATIENTS REALLY WANT FROM THEIR DOCTORS

The doctor-patient interaction is the absolute core of clinical medicine. Maybe I’ll go much further: it’s the core of health care in general. I always try to remember, whenever I’m ever feeling frustrated with the system, the crazy bureaucracy — and of course, the debacle of our clunky electronic medical records and their data entry requirements — to separate myself from all of that when I’m face-to-face with my patient and their family. This time is priceless, it’s why I went into this.

The interactions and honor of serving my patients at a low point in their lives, makes it all worth it. It’s where the magic of medicine happens, and is something that is untouchable by any external factor, if you choose to make it that way.

Here are three things that patients want from that doctorpatient interaction: 16

1. A compassionate doctor who communicates well.

A physician who understands that they are coming to them for help, shows empathy, and listens carefully to the problem. The doctor should exhibit both in their verbal and non-verbal communication, that they are fully engaged in being their doctor, who truly cares for them. There are an array of phrases and body language techniques that can be used to show empathy, all of them are very learnable, and deeply appreciated by your patients.

2. A doctor who is honest and transparent. I actually think almost all physicians are this way anyway — highly trustworthy professionals in the first place (anyone not that way, would unlikely have made it so far). Nothing but complete honesty with a sincere interest in getting the best possible www.PhysiciansOfficeResource.com

outcome. Explain everything slowly, clearly and in understandable terms — as if you are talking to a family member who knows nothing about health care.

3. A non-distracted physician. Nobody wants to pour their heart out to somebody who is not fully present in the conversation. No turning around, clicking, or looking elsewhere for extended periods of time. This is patient face time, not screen time. A technique that I use, that gets very positive feedback: I actually ditch the computer completely, especially when meeting a patient for the first time. I sit opposite them, lean in, and am fully present. I jot notes down on a piece of paper on my lap. Patients love it (actually anyone would like that kind of service when they are explaining something in a professional situation, even at Home Depot!). Writing things down when faceto-face with someone, subconsciously gives a better impression that you are concentrating and mentally processing, rather than typing on a keyboard and turning at a screen. It just does.

So these are just three things that any physician should strive to do in their interaction, to give their patients the experience they deserve. It may just be another “name on the list” for us — but for patients who may have waited hours or days to see us, it’s the part of their day that they will usually remember and appreciate the most.

Over my years of treating thousands of patients, seeing so many misunderstandings and poor interactions, and now being in a position where I am teaching many of these skills — I am really of the belief that what patients ask for is really not that much. We are already very good with our scientific knowledge and have stellar treatments at our disposal — but it’s the human side that is too often forgotten.

many of our administrators and technologists, who are now dominating health care, and you’ll soon see why the above is the last thing they want to hear.

To lots of them, health care is all about spreadsheets and numbers, expensive new technologies, and building factorylike processes. But it really isn’t, and never will be. Unfortunately too, the doctor-patient relationship is something that may even be intimidating to many administrators — because it’s something that they can’t “get to” or really control.

It shouldn’t be that way.

We are currently in a health care swamp of epic proportions, where we lose the forest for the trees. We keep moving further away from the frontlines, among a tidal wave of bureaucracy. Of course, a sector as big as health care does need some oversight, rules, and regulations — but we’ve gone too far.

That moment when a physician sits down with their patient, is what the practice of medicine is all about, and should take front and center stage in any health care system. So Doctor, this is your time. Make the most of it and let’s give patients what they deserve when we are face-to-face. Even if the rest of the world around us feels like its tumbling down.

FIXING PRIMARY CARE’S BROKEN BUSINESS MODEL

Primary care visits are never quick; we don’t give much advice over the phone or online; and we prioritize the government’s and insurance companies’ public health agenda over our own patients’ concerns.

Imagine health care as a retail customer experience for a few minutes: Imagine you’re going to Walmart to buy a bag of dog food, a new coffee maker or to equip a small kitchen in your newly built mother-in-law apartment.

1. You’ve bought dog food there before, so you know exactly where it is. You just want to quickly grab a bag and get out of there.

2. You have a rough idea of where the coffee makers are; you know some brands you trust, but you might have one or two questions before you select one, and if they don’t have one you like, you might get it somewhere else. Or, you might even check their website to see which models they carry.

3. For the new kitchen, you have a list, but know you probably haven’t thought of everything, so you plan to walk 34

down the aisles in the kitchen and home departments. You plan to spend a fair amount of money, so you might be on the lookout for special sales or promotions. But, you definitely don’t want someone else to choose all the items for you.

Can Walmart meet your needs in all these situations?

Probably yes.

Now, think about how your doctor’s office works 1. Can you quickly get in and out if you have a simple problem like conjunctivitis?

2. Are they able to give you information on what your options are for a recurring shoulder dislocation; could they refer you to a shoulder specialist without first waiting weeks to see your primary care doctor?

3. You have lots of issues and try to get an appointment to deal with them all at once; you think of it as a physical, but last time you had a physical, your doctor brought up all kinds of things you don’t particularly see as priorities for yourself. www.PhysiciansOfficeResource.com

Here are the existing realities of primary care We can’t afford to just see you for something quick. Our quality indicators, which more and more will determine how we get paid, will go down if we don’t screen you at every visit and offer interventions for depression, smoking, alcohol misuse, hypertension, weight management, immunization needs and much more.

We won’t refer you without seeing you, and we often hesitate giving you medical advice over the phone. Our providers are not scheduled for anything else besides seeing patients, because the rules of how we are paid still emphasize face-to-face visits over “population management.” So our providers are busy all day long seeing patients for visits that could have been simple but are loaded up with mandatory screenings and interventions and our medical assistants, besides being busy with all our screening questionnaires, are discouraged from giving medical advice they aren’t formally trained to provide.

Is there a doctor shortage?

We are said to have a doctor shortage.

We have an aging population with more and more chronic diseases, like diabetes and heart disease. The need for skilled and experienced medical providers is continually increasing.

We have no public health system to speak of in this country, so the government, through Medicare and Medicaid, has mandated that health care providers do the things the public health system does in other countries.

This is, plain and simple, what is clogging up the works in health care today: Too much non-doctor work is crammed into each patient visit, and we can’t charge for giving advice or directing care except in a face-to-face visit.

You don’t need to go to medical school to give immunizations, tell people smoking is bad for you, explain that “low fat” foods cause obesity, or promote regular exercise. You don’t even need to be a doctor, PA or NP to screen for high blood pressure – only to

treat it. (Some pundits, in utter desperation, have suggested we send pharmacists to school to learn how to treat hypertension, but there are of course plenty of licensed medical providers who are able and willing to do that if we get freed up from the lessskilled tasks I just listed above.)

And all the mandated screenings are there because Medicare, in particular, has the right to micromanage doctors’ work because they are paying for health care visits, which could be quicker and less costly if patients had control over their health care spending.

Patients and doctors have no control Now, why are we doing all those things we do if they are so inefficient? Quite simply, whoever pays us has the power to define our work. We call that “health insurance,” but that is not exactly what we are dealing with. Insurance, for home, auto or employer liability, has nothing to do with predictable events or minor issues. Your car insurance doesn’t pay for oil changes or tire wear, not even for a minor paint scratch. But somehow that is what we expect health insurance to cover for our bodies. In terms of auto insurance, most people probably figure an insurance job carries an inflated price tag and lots of paperwork. The same is true for health care, which should not be a surprise to anyone.

For example, years ago the overhead cost of insurance billing for each primary care doctor was reported to be $80,000. That, put very plainly, is money that patients and employers are ultimately paying through

How could we do better?

We do three things in primary care, each with its own workflow and, really, each with its own economics.

1. We could do our part of public health more effectively. Allow us to promote immunizations and other primary preventions outside our already crammed fifteen-minute visits. Pay us a per patient per year stipend to reach out to target populations through mail, phone, web or, when appropriate, in person about general health issues. Stop imagining we can do all of it and still treat diseases, acute and chronic, in our measly fifteen minutes. Right now, that is just clicking boxes with little actual substance. Use some of the government money that should have been spent on a working public health system...
