THE SECRET

to making AI work for physicians

If you are a physician or know a physician or have ever visited one, chances are you have probably heard them complain about technology in health care. More to the point, they are likely to be complaining about the one piece of technology that affects their lives minute-tominute: the electronic health record (EHR). To get a sense of how central EHRs are to our daily routines, consider that physicians now spend more time in the EHR than they do seeing patients (6 hours of an average 11-hour work day). And while it is easy to write them off as luddites unable to adapt to new technology, an important study by the RAND organization noted that physicians approve of EHRs in principle and see the potential for the technology to improve the delivery of clinical care.

So where is the disconnect between the vision that physicians see for the EHR and its implementation in clinical practice? Instead of being a tool to facilitate communication and coordination, the EHR has become an additional burden. Common critiques include that fact that EHR technology interferes with face-to-face patientphysician communication (this has become the aptly named “screen between”), has significantly increased the 4

amount of time physicians spend performing data entry and clerical duties (and in some cases, has created brand new administrative tasks), and is associated with poor user experience. It all feels as if the physician is working for the EHR rather than the other way around.

The changes physicians have had to make to clinical practice because of the EHR have had profound impacts.

For example, EHRs have been identified as the #1 reason for physician burnout. This is particularly problematic given that one study demonstrated over half of all physicians are suffering from professional burnout. What’s more, physicians who were dissatisfied with their EHR and clerical burden were also more likely to have plans of leaving clinical practice. This adds to ongoing concerns of an impending physician workforce shortage.

How have physicians responded? Medical scribes have emerged as one solution. Medical scribes follow the physician into the exam room where they observe the interaction and subsequently draft up the physician’s documentation. While scribes are a great way to alleviate some of the EHR burden, they are not without limitations: scribes can be unaffordable for some providers

(approximately $35,000 per year), require initial generalized training, need additional ramp-up time to learn the preferences of a specific physician, and are subject to turnover. Another approach has included retraining existing medical personnel such as clinic nurses or medical assistants to assist the physician with clerical duties. The challenge is both of these approaches involve mobilizing additional human resources to make up for the shortcoming of technology. Instead of creating more efficiencies and reducing human clerical burden, we continue to come up with creative workflow “hacks” for working around the EHR rather than improving it.

Technology is supposed to be an efficiency gainer. It is supposed to increase the productivity of the existing human workforce. Most sectors worry that technology will put humans out of work. With that said, skeptics may rightfully question the notion of improving technology with … more technology. However, the problem is not technology itself but rather the way it is envisioned, designed, and implemented into clinical practice.

Tools such as artificial intelligence (AI) and natural language processing (NLP) offer exciting opportunities to improve upon the existing healthcare IT environment.

And If three easy steps are followed in implementing these tools, we can make an important leap from our current status quo to a future where technology integrates seamlessly into the clinical workflow.

First, the creators of technology must take a problemoriented approach. Notice that above I mention “tools such as artificial intelligence (AI) and natural language processing (NLP).” That is because these should be seen as solutions to a specific problem rather than just another technology we should squeeze into the clinical workflow.

Therefore, by viewing AI and NLP as one of perhaps many tools we can mobilize to solve the problem of excessive data entry and clerical duties for physicians, we are more likely to succeed in creating value for physicians and building a product that they actually love (rather than the ones they currently lament).

Second, we must include physicians in the solution creation process. Good entrepreneurs refer to this as

customer discovery and would not dream of launching an initial product without getting extensive feedback from the end user. We must see the problem we are solving through the eyes of the physician. How do they understand it?

How does it impact their workflow? Where are the greatest opportunities for improvement? By attempting to see the problem through their lens, we are able to build solutions with the right combination of features that physicians will actually want.

The challenge with making AI work for clinicians is as much one of packaging it into an intuitive design and an enjoyable user interface (UI)/ user experience (UX) as it is about enhancing performance. The best performing model is not much good if the physician hates the way it looks and feels; or worse, if it adds additional time to their workflow.

The EHR we have today does not have to be the one we have tomorrow. Efficient and enjoyable clinical practice is possible. A future where doctors spend more time with their patients than they do with the EHR is achievable. It just requires a problem-oriented approach, end-user engagement, attention to the clinical workflow, and beautiful design.

INTERPERSONAL MEDICINE ALREADY EXISTS:

IT’S CALLED FAMILY MEDICINE

A commentary in the New England Journal of Medicine titled “Beyond Evidence-Based Medicine” received much well-deserved criticism for not only mischaracterizing EBM, but advocating for a novel approach, “interpersonal medicine,” that was explicitly codified in the recognition of the U.S. specialty of family practice nearly 50 years ago. Here’s what the authors wrote about this practice of medicine that is, apparently, new to them but well-known to the rest of us: Interpersonal medicine would recognize clinicians’ influence on patients and informal caregivers and the relationships among them. It would be anchored in longitudinal, multidirectional communication; broach social and behavioral factors; require coordination of the care team; and, constantly evaluate and iterate its own approach.

After reading these sentences via a tweet from Dr. Eric Topol, I quickly added my perspective on interpersonal medicine, which, judged by the volume of likes and retweets, was greeted enthusiastically by other primary care clinicians and their allies: It was one of the few times in recent memory that other physicians were eager to jump on the #FMRevolution bandwagon, as geriatricians, general 14

internists, and general pediatricians tweeted that their generalist fields provide interpersonal medicine too. I agree. But I draw the line there. No matter how excellent one’s beside manner, a subspecialist whose job description revolves around treating a specific a body part, organ system, or disease state, or intermittent contacts during specific periods of illness (e.g., emergency medicine, hospitalists) is not using the generalist approach described by longtime Annals of Family Medicine editor Kurt Stange in a 2009 editorial: A generalist approach involves working on the parts while paying attention to the whole; being connected by sustaining relationships; having a broad base of knowledge while being grounded in specific information; scanning and prioritizing, then focusing on what is most meaningful; moving back and forth between the universal and the particular. The generalist approach is rooted in recognizing connection to person, community, and cosmos.

The skills of generalist physicians — and family physicians in particular — have long been devalued by our health system’s mechanisms of measuring and paying for clinical work in discrete tasks, rather than for caring for the whole person. The movement toward “paying for

performance” has not helped. As Dr. Dhruv Khullar and colleagues observed in a recent JAMA Viewpoint, “because these programs are disconnected from the needs of patients and physicians within organizations, they often result in erroneous metrics, gaming of the system, and unidirectional assessments that emphasize meeting thresholds over open dialogue.”

In other words, never mind that I carefully reviewed with my 65-year-old patient of the past 5 years with a recent blood pressure of 145/92 the pros and cons of intensifying his medication regimen, the limitations of the evidence, and his personal values and preferences; 140/90 is my practice’s non-negotiable cutoff for poor quality. This is hardly surprising, since quality management has rarely accounted for what makes a difference to patients in primary care – particularly, as Dr. Justin Mutter and colleagues suggested in “Core Principles to Improve Primary Care Quality Management,” prioritizing therapeutic relationships over time. Reflecting on changes in the role of the personal physician since the dawn of the specialty, a group of senior leaders in academic family medicine has observed:

We have watched our patients age with us. They beg us not to retire. For our patients, we are caregivers, healers, advisors, friends, and navigators through a complex system. Our patients are admirable human beings who taught us our craft, offering clinical challenges and providing us with the gratification that makes

practicing medicine worthwhile. A principal challenge for the present and future … is to be able to establish and maintain the long-term trusting relationships that have characterized family doctors and our role in health systems and society.

At the same time, one of my colleagues notes that insurers are paying five times as much for patients to have a video or telephone visit with a “teladoc” than with their family physician.

Enough with interpersonal medicine! EBM is not the problem, and it never has been. With nearly 80 percent of internet users searching online for health-related information, it’s no wonder the catchphrase “Dr. Google” has caught on, to the delight of many searchers and the dismay of many real doctors.

What’s received little attention from physicians or the public is the company’s quiet metamorphosis into a powerhouse focused on the actual practice of medicine.

If “data is the new oil,” as the internet meme has it, Google and its Big Tech brethren could become the new OPEC. Search is only the start for Google and its parent company, Alphabet. Their involvement in health care can continue through a doctor’s diagnosis and even into monitoring a patient’s chronic condition for, essentially, forever. (From here on, I’ll use the term Google to include the confusing intertwining of Google and Alphabet units.)

Suppose you’re worried that you might have diabetes.

Googling “diabetes” brings up not just links but also a boxed summary of relevant information curated by the Mayo Clinic and other Google partners. Google recently deployed an app enabled with artificial intelligence for remote professionals to use that can all but confirm diabetes-related retinopathy, a leading cause of blindness.

Diabetes is also a diagnosis your doctor might have predicted using more Google AI applied to the electronic 32

health record.

Meanwhile, a Google joint venture called Onduo recently announced a partnership to allow a major pharmacy chain to use its “virtual diabetes clinic” to coach patients on managing their disease. And, of course, at home you can get daily diabetes reminders from your Google Assistant.

Or your doctor could actually be Dr. Google. The brick-and-mortar Cityblock clinic, whose first site opened in Brooklyn, N.Y., earlier this year, is an Alphabet spinoff.

It promises a “personalized health system” experience for low-income patients.

Other tech companies are also making forays into fields previously reserved for physicians as they compete for a slice of the $3.5 trillion health care pie. Renowned surgeon and author Dr. Atul Gawande was hired to head the still-nascent health care joint venture between Amazon, Berkshire Hathaway, and JPMorgan.

Apple recently hired more than 50 physicians to tend its growing health care portfolio. Those efforts include Apple Watch apps to detect irregular heart rhythms and falls, a medical record repository on your iPhone, a genetic risk score for heart disease, and a partnership with medical equipment manufacturer Zimmer Biomet aimed at

improving knee and hip surgery.

Google is hiring physicians, too. Its high-profile hires include the former chief executives of the Geisinger Clinic and the Cleveland Clinic. The company’s ambitious health care expansion plans reportedly encompass everything from the management of Parkinson’s disease to selling hardware to providers and insurers.

To be clear, I’ve connected the dots among separate Google companies in a way Google might dispute.

However, there are some concerns about how and whether any separation of information will be maintained.

In November, Bloomberg reported that plans in the United Kingdom to combine an Alphabet subsidiary using artificial intelligence on medical records with the Google search engine were “tripping alarm bells about privacy.”

In other words, what’s true about the way in which Google and its tech brethren handle your information today may not remain the way they use that information in the future.

Still, even if Google Assistant (or Alexa or Siri or …) tattles to your doctor about your eating takeout tacos in front of the TV, isn’t that OK as long as the information was conveyed with your consent? Shouldn’t we welcome

technologies that could lead to breakthroughs in diagnosing and managing disease?

There’s much to be said for that argument. I first wrote about the extraordinary opportunities arising from medicine in the information age more than two decades ago. When feminists of the 1970s launched the patient empowerment movement, their rallying cry was, “Our bodies, ourselves.” As patients, consumers and, simply, citizens, we should have the right to full transparency about the digitized data collected about our bodies and ourselves. We should be able to choose with whom our information is shared and with whom we wish to engage.

Finally, but no less important, we deserve clear standards of accountability that forthrightly address the unprecedented medical and quasi-medical relationships now emerging.

It’s time for policymakers to establish new rules that address the new roles being assumed by Dr. Google, Dr. Apple, Dr. Amazon, and others as health care and digital health blend together as one and the same.
