MOLECULAR LATERAL FLOW DIAGNOSTICS: A NEW ERA FOR POINT-OF-CARE TESTING

If you’re like many physicians and Point of Care (POC) laboratories, you may be dealing with suboptimal performance from some of your current diagnostic methods. For example, conventional Rapid Influenza Diagnostics Tests (RIDTs) vary in terms of sensitivity and specificity when compared with viral culture or reverse transcription-polymerase chain reaction (RT-PCR).

But the technology of point-of-care diagnostics is changing.

One particular evolving technology is molecular diagnostics, which detects specific sequences in DNA or RNA that may (or may not) be associated with disease.

At present, there are molecular applications in infectious 4

diseases, oncology, pharmacogenomics, genetic disease screening and coagulation, among others.

PCR: The Gold Standard, but Not Feasible in POC Polymerase chain reaction (PCR) has long been considered a diagnostic gold standard due to the high accuracy of the technology, which accounts for its use in core or reference labs. It can detect disease with as few as several copies of a gene. DID YOU PRESCRIBE A WRENCH TO TURN A SCREW? THE ROLE OF INHALER TECHNIQUE IN RESPIRATORY DISEASE MANAGEMENT

Adherence to medication regimens is a well-known, welldocumented issue in chronic lung disease care. The reasons for this are many; from an inability to afford prescribed medications, to formulary-related barriers, to ineffective patient education on devices and dose schedules. For example, a recent cross-sectional study of nearly 3,000 European patients with COPD found over half were unable to use their devices correctly, despite having been previously educated on proper usage1. The issue appears at a similar rate in the asthma population2, highlighting the need for better education. However, even when patients are able to perfectly execute the steps involved with priming, loading, and actuating their inhalers, clinicians often fail to consider their patients’ inspiratory flow characteristics when assessing technique. This omission can mask the true cause of treatment plan non-adherence, and prevents clinicians 14

from providing effective alternatives.

Fortunately, there are multiple, easy-to-implement strategies available to evaluate virtually all aspects of inhaler device use.

The InCheck DIAL (Clement Clarke International) is a reusable inspiratory flow meter that can simulate the internal resistance of various dry-powder devices on the market. The clinician sets the resistance range appropriate to the patient’s device and instructs the patient to inhale maximally on a disposable mouthpiece. The resulting measurement can be used to determine if the patient is physically able to generate sufficient inspiratory flow to disaggregate the powder bolus into respirable particles. Use of the InCheck DIAL provides clinicians with customized, patient-specific data that allows for more informed clinical decision-making without adding significant time to encounters, and empowers clinicians to spend their patient education time focusing on the particular www.PhysiciansOfficeResource.com

mechanics of each device, decreasing the likelihood of preparation and actuation errors.

The InCheck DIAL is also capable of measuring inspiratory flow without resistance, simulating usage of metered-dose and soft-mist inhalers (MDIs and SMIs). However, as these devices are not breath-actuated, evaluation of inspiratory characteristics alone is not sufficient to assure competency. In the case of the SMI (Respimat) device, demonstration with teach-back remains the best available solution.

Clinicians should be careful to observe the patient through the entire process beginning with loading the medication cartridge, in order to determine whether the patient has the dexterity to prepare and prime the inhaler. Patients should also be observed during the inhalation phase to detect coughing or other inadvertent exhalation that reduces airway deposition.

MDIs are arguably the most difficult inhaler devices to use appropriately. Research data has consistently demonstrated the majority of MDI users experience at least one error while using their devices, with as many as 76% experiencing a critical error that affects the delivery of their

CONTINUEDFROMPAGE15 At Western Michigan University Homer Stryker M.D. School of Medicine, both the AIM and InCheck DIAL are routinely utilized to ensure patients are not only knowledgeable about their medications, but able to use them properly. As the majority of our patients use at least one non-powder device, the AIM in particular has led to significant quality improvements in our practice.

The integration of a dedicated respiratory therapist serving as patient educator and inhaler education tools in our practice has allowed us to markedly improve our education practices and customize our patient disease management regimens to more accurately reflect our patients’ respiratory status. This has led to significant improvements in our COPD population’s quality of life, as measured by the COPD Assessment Test (CAT), as presented in 2017 at COPD10usa4.

Unlike medications administered via other routes, the effectiveness of inhaled

medications depends heavily on an individual’s technique and abilities.

Unfortunately, despite expert opinion and evidence-based practice recommendations, evaluation of inhaler technique and inspiratory flow characteristics remains overlooked and incomplete in most patients. This leads directly to increased healthcare costs, increased symptom severity and instability, and lower quality of life.

This is unfortunate, considering the availability and comparatively low cost of devices such as the Aerosol Inhalation Monitor, which provide rapid, comprehensive feedback that empowers clinicians to improve personalized disease management strategies. These devices should be considered just as indispensable as otoscopes and blood pressure cuffs, and should be come just as ubiquitous in primary care and pulmonology practices alike. Patients with respiratory compromise deserve nothing less.

Medical Buzzwords? it’s a term with resonance, first officially appearing in a 1971 book entitled Basic Endocrine Pathology and referring to the cell nucleus as “relatively empty (like Orphan Annie Eyes.).” Practicing american physicians of the era would have likely understood the reference because the Little Orphan Annie comic strip was a daily comic strip that ran in syndication from 1894 to 1968. In fact, many physicians currently practicing would understand

the reference as Little Orphan Annie is a bona fide cultural icon, but this kind of ubiquity won’t last forever and decreases with each passing year. The term is still taught to young medical students mostly unfamiliar with the character, many of whom were born and raised outside of the united states and have absolutely no context of Annie, her eyes or the adventure she shares with her rich benefactor, Daddy Warbucks.

I’m no stooge. I know that of buzzwords lend a gleeful air of contemporaneity to the language we use, and can even clarify its meaning, with “laymen” cultural definitions. Still, for our language to remain truly effective in a globalizing industry, we must assure that all who engage it are acutely familiar with the backstory and in an age with such rapid informational turnover, we simply can’t count on our buzzwords to stay in vogue infinitely.

Do They Really Matter Though?

We likely use buzzwords, phrases and sayings every day that betray their original intention. I won’t claim to know the etymological backstory of “OK” for example, and I can admit that I just barely learned that when we say “pull out all the stops” we’re

actually referring to an organ player increasing the volume of the instrument. Does my understanding of

Maybe these buzzwords offer a quick workaround explanation for an improvising professor. That's all fine and good.

Where buzzwords become problematic is when they require clarification and explanation in order to be truly useful to those who hear them. If you think about it in a The Tibetan Book of Living and Dying kind of way, all the time you’d save inventing new term, even those which sidestep medical jargon, you’ll lose explaining to the next few generations what that buzzword even means.
