Over the past few years there have been some tremendous technical advances for patients with diabetes. In particular, insulin using patients have some of the best tools at their disposal. Insulin pumps have gotten smarter and less of a hassle, the same goes for insulin pens which now offer connectivity.

Whether it’s diagnosing vitamin D deficiencies, managing thyroid disorders or monitoring prostate cancer, one clear motivation for performing diagnostic testing in-house is the ability to minimize return patient visits for medical decision-making, thereby enhancing patient care.

DIABETES PATIENT MANAGEMENT: Understanding Insulin Dosing Algorithms and Apps

BY DAVID KLIFF OF THE DIABETIC INVESTOR Over the past few years there have been some tremendous technical advances for patients with diabetes. In particular, insulin using patients have some of the best tools at their disposal. Insulin pumps have gotten smarter and less of a hassle, the same goes for insulin pens which now offer connectivity. Continuous glucose monitoring systems (CGMs) are becoming the standard for glucose measurement. Perhaps the most important tool is not a device, but a software program more commonly known as an algorithm, which processes all the patient data and helps them more effectively dose their insulin.

These sophisticated algorithms not only have the ability to process 6 | PHYSICIANS OFFICE RESOURCE

information, they also are capable of learning and adjusting. Simply put the more data fed into the algorithm the better it gets.

What may come as somewhat of a surprise is that there are newer algorithms designed specifically for patients that only use long-acting insulin, either alone or in combination with oral medications. Even better these algorithms don’t require that a patient use a CGM and can be very effective using a conventional blood glucose monitor. And no they don’t require the patient consistently check their glucose levels and can be used with just one glucose test each day. The goal here is simple: help the patient more effectively dose their insulin.

To help sort through the complex maze of all the various programs available here is overview of some of the more popular programs: ISAGE RX: This program supports all of the leading long acting insulin’s and requires just one glucose test per day.

The key here of course is getting the patient to use the programs. Here are some tips on how to make this happen: Proper positioning – as effective as these programs can be, they can and do add another step in the insulin dosing process. Therefore we’d recommend positioning these tools as an enhancement to insulin therapy helping the patient more effectively manage their diabetes.

Enhancing the doctor patient relationship – since all the programs share data with the physician it’s important the patient understand that these programs do not replace the physician rather helps the physician better advise the patient.

Baby steps – while it would be great if these tools were used each day, we would recommend phasing them in. Get the patient to try the program first but don’t make them mandatory. It’s ok if the patient doesn’t use the program every day.

Setting the proper expectations - data can sometimes be a doubled edge sword creating more questions than answers. This is to be expected as for the majority of patients this will be their first experience seeing this data. Some patients may be slightly overwhelmed which could cause them to stop using the program. We’d recommend explaining that just as insulin is a tool to help the patient more effectively manage their diabetes these programs are just one more tool to help in that regard.

Manage Expectations – while there is an extensive portfolio of clinical evidence that patients utilizing these programs achieve better outcomes these results do not happen overnight. It’s critical that the patient understand this and not expect immediate improvements in outcomes.

There is no question that advanced technology is changing the nature of the patient physician relationship. As noted previously, as effective as this technology can be it can at times seem overwhelming. Hence the reason we recommend taking a slow methodical approach when adopting new technology. Positioning the technology as an enhancement to diabetes management will help the patient feel more at ease.

BRIEF SUMMARY OF PRESCRIBING INFORMATION FOR AJOVY® (fremanezumab-vfrm) injection, for subcutaneous use SEE PACKAGE INSERT FOR FULL PRESCRIBING INFORMATION 1 INDICATIONS AND USAGE AJOVY is indicated for the preventive treatment of migraine in adults.

2 DOSAGE AND ADMINISTRATION 2.1 Recommended Dosage Two subcutaneous dosing options of AJOVY are available to administer the recommended dosage: • 225 mg monthly, or • 675 mg every 3 months (quarterly), which is administered as three consecutive subcutaneous injections of 225 mg each.

When switching dosage options, administer the first dose of the new regimen on the next scheduled date of administration. If a dose of AJOVY is missed, administer as soon as possible. Thereafter, AJOVY can be scheduled from the date of the last dose.

2.2 Important Administration Instructions AJOVY is for subcutaneous use only.

AJOVY may be administered by healthcare professionals, patients, and/or caregivers. Prior to use, provide proper training to patients and/or caregivers on the preparation and administration of AJOVY prefilled syringe, including aseptic technique [see Instructions for Use in full Prescribing Information]: • Remove AJOVY from the refrigerator. Prior to use, allow AJOVY to sit at room temperature for 30 minutes protected from direct sunlight. Do not warm by using a heat source such as hot water or a microwave. Do not use AJOVY if it has been at room temperature for 24 hours or longer.

• Follow aseptic injection technique every time AJOVY is administered.

• Inspect AJOVY for particles or discoloration prior to administration. Do not use if the solution is cloudy, discolored, or contains particles.

• Administer AJOVY by subcutaneous injection into areas of the abdomen, thigh, or upper arm that are not tender, bruised, red, or indurated. For multiple injections, you may use the same body site, but not the exact location of the previous injection.

• Do not co-administer AJOVY with other injectable drugs at the same injection site.

4 CONTRAINDICATIONS AJOVY is contraindicated in patients with serious hypersensitivity to fremanezumabvfrm or to any of the excipients [see Warnings and Precautions (5.1)].

5 WARNINGS AND PRECAUTIONS 5.1 Hypersensitivity Reactions Hypersensitivity reactions, including rash, pruritus, drug hypersensitivity, and urticaria, were reported with AJOVY in clinical trials. Most reactions were mild to moderate, but some led to discontinuation or required corticosteroid treatment. Most reactions were reported from within hours to one month after administration.

6 ADVERSE REACTIONS The following clinically significant adverse reactions are discussed in greater detail in other sections of the labeling: • Hypersensitivity Reactions [see Warnings and Precautions (5.1)] 6.1 Clinical Trials Experience Because clinical trials are conducted under widely varying conditions, adverse reaction rates observed in the clinical trials of a drug cannot be directly compared to rates in the clinical trials of another drug, and may not reflect the rates observed in clinical practice.

The safety of AJOVY was evaluated in 2512 patients with migraine who received at least 1 dose of AJOVY, representing 1279 patient-years of exposure. Of these, 1730 patients were exposed to AJOVY 225 mg monthly or AJOVY 675 mg quarterly for at least 6 months, 775 patients for at least 12 months, and 138 patients for at least 15 months. In placebo-controlled clinical trials (Studies 1 and 2), 662 patients received AJOVY 225 mg monthly for 12 weeks (with or without a loading dose of 675 mg), and 663 patients received AJOVY 675 mg quarterly for 12 weeks. In the controlled trials, 87% of patients were female, 80% were White, and the mean age was 41 years.

The most common adverse reactions in the clinical trials for the preventive treatment of migraine (incidence at least 5% and greater than placebo) were injection site reactions. The adverse reactions that most commonly led to discontinuations were injection site reactions (1%). Table 1 summarizes adverse reactions reported in the 3-month placebo-controlled studies (Study 1 and Study 2), and the 1-month follow-up period after those studies.

Adverse Reaction

AJOVY® (fremanezumab-vfrm) injection 6.2 Immunogenicity As with all therapeutic proteins, there is a potential for immunogenicity. The detection of antibody formation is highly dependent on sensitivity and specificity of the assay. Additionally, the observed incidence of antibody (including neutralizing antibody) positivity in an assay may be influenced by several factors, including assay methodology, sample handling, timing of sample collection, concomitant medications, and underlying disease. For these reasons, comparison of the incidence of antibodies to fremanezumab-vfrm in the studies described below with the incidence of antibodies in other studies to other products may be misleading. Clinical immunogenicity of AJOVY was monitored by analyzing anti-drug antibodies (ADA) and neutralizing antibodies in drug-treated patients.

The data reflect the percentage of patients whose test results were positive for antibodies to AJOVY in specific assays.

In 3-month placebo-controlled studies, treatment-emergent ADA responses were observed in 6 out of 1701 (0.4%) AJOVY-treated patients. One of the 6 patients developed anti-AJOVY neutralizing antibodies at Day 84. In the ongoing long-term open-label study, ADA were detected in 1.6% of patients (30 out of 1888). Out of 30 ADA-positive patients, 17 had a neutralizing activity in their post-dose samples. Although these data do not demonstrate an impact of antifremanezumab-vfrm antibody development on the efficacy or safety of AJOVY in these patients, the available data are too limited to make definitive conclusions.

8 USE IN SPECIFIC POPULATIONS 8.1 Pregnancy Risk Summary There are no adequate data on the developmental risk associated with the use of AJOVY in pregnant women. AJOVY has a long half-life. This should be taken into consideration for women who are pregnant or plan to become pregnant while using AJOVY. Administration of fremanezumab-vfrm to rats and rabbits during the period of organogenesis or to rats throughout pregnancy and lactation at doses resulting in plasma levels greater than those expected clinically did not result in adverse effects on development [see Animal Data].

In the U.S. general population, the estimated background risk of major birth defects and miscarriage in clinically recognized pregnancies is 2-4% and 15-20%, respectively. The estimated rate of major birth defects (2.2-2.9%) and miscarriage (17%) among deliveries to women with migraine are similar to rates reported in women without migraine.

Clinical Considerations Disease-Associated Maternal and/or Embryo/Fetal Risk Published data have suggested that women with migraine may be at increased risk of preeclampsia during pregnancy.

Data Animal Data When fremanezumab-vfrm (0, 50, 100, or 200 mg/kg) was administered to male and female rats by weekly subcutaneous injection prior to and during mating and continuing in females throughout organogenesis, no adverse embryofetal effects were observed. The highest dose tested was associated with plasma exposures (AUC) approximately 2 times that in humans at a dose of 675 mg.

Administration of fremanezumab-vfrm (0, 10, 50, or 100 mg/kg) weekly by subcutaneous injection to pregnant rabbits throughout the period of organogenesis produced no adverse effects on embryofetal development. The highest dose tested was associated with plasma AUC approximately 3 times that in humans (675 mg).

Administration of fremanezumab-vfrm (0, 50, 100, or 200 mg/kg) weekly by subcutaneous injection to female rats throughout pregnancy and lactation resulted in no adverse effects on pre- and postnatal development. The highest dose tested was associated with plasma AUC approximately 2 times that in humans (675 mg).

8.2 Lactation Risk Summary There are no data on the presence of fremanezumab-vfrm in human milk, the effects on the breastfed infant, or the effects on milk production. The developmental and health benefits of breastfeeding should be considered along with the mother’s clinical need for AJOVY and any potential adverse effects on the breastfed infant from AJOVY or from the underlying maternal condition.

8.4 Pediatric Use Safety and effectiveness in pediatric patients have not been established.

8.5 Geriatric Use Clinical studies of AJOVY did not include sufficient numbers of subjects aged 65 and over to determine whether they respond differently from younger subjects.

Once a comparative value of advantages and disadvantages has been explored, your office can make a determination of which tests would be good candidates for your in-house Physician Office Lab. A Review From a Healthcare Provider Miramont Family Medicine is a mid-size physician’s office in Colorado that was seeking a way to provide the highest-quality care while maintaining the lowest-cost services to their patients. When Miramont Family Medicine adopted their Patient-Centered Medical Home (PCMH) model - implementing a Physician Office Laboratory (POL) was key part of the model. How a Point-of-Care POL Works While there are key differences in each medical practice, there are some commonalities among the requirements for a POL: startup funds, appropriate space, staff to train and allocate to testing, and a sufficient testing load to make it feasible and profitable. Among these requirements the top three considerations are:

SPACE While a physician’s office may not require a large space for a lab, it will require a dedicated space.

In some cases, a large closet may suffice. The lab space must have electrical outlets and running water - and be located away from food prep or eating areas. You’ll need to ensure there is enough room for a person to maneuver with tabletop instruments, a sink, and a refrigerator in easy reach to optimize efficiency.

STAFF Your practice will need to have staff that can learn to operate the equipment and perform the test protocols. This may or may not require additional staff, since bringing technology in-house to perform diagnostic testing will likely lead to an overall increase in labor efficiency.

TESTS Determining which tests to offer in-house will be driven by the particulars of your practice. Standard test menus which are ordered routinely in a general medical practice may include: General chemistry (CMP, BMP, lipid profile, CPK, lipase, iron panel, uric acid, HbA1C, urine microalbumin and others) Hematology (CBS, PT/INR, D-dimer, ESR) Rapid tests (Flu, Strep, IFOB, Trichomonas, BV, hCG) Immunochemistry (TSH, free T4, PSA, testosterone, vitamin D, troponin, myoglobin, BNP, RPR, HIV, Hep-C and others)

Geographically challenged? I was before visiting the Bahamas, then I discovered that the Caribbean country is made up of around 700 individual islands and islets over more than 100,000 square miles of ocean, 30 of which are inhabited and more than 2000 cays (coral reefs). The Cat 5 Hurricane Dorian, which slowly churned for 24 hours straight, over the area just over a month ago, the record-breaking 185 mph sustained winds contributed to 61 dead, hundreds still missing, an estimated 70,000 needing shelter, and colossal damage left in her wake.

fter hearing news reports about the destruction and death toll rising, I questioned whether I should travel to the country at all. It wasn’t until I arrived that I realized the hardest-hit islands of Great Abaco and Grand Bahama were the only communities effected. The hurricane eye was relatively small at 25 nautical miles.

Still, because it stayed in place for an entire day, crawled at just one mile per hour, it brought colossal damage and devastation to the people living on the two hardest-hit areas.

“More than ever, we need you to come on vacation. That’s the only way we can help our brothers and sisters in the north.”

Out of the total Bahamian population, 70% can be found in Nassau and suburbs in New Providence Island, on just 79 square miles of land. Compare that to the hardest hit landmasses of Great Abaco with a land area of 442 square miles and just shy of that at 423 square miles is Grand Bahama. Freeport and other suburbs of Grand Bahama have about 30% of the country’s population, and Hurricane Dorian’s wrath left 80% of this large island covered in water.

As I sat on the edge of the infinity pool of the gorgeous resort, soaking in the sun, I couldn’t help but feel pangs of guilt. The kind team members of the resort would greet me with “Welcome Home” and apologized that the color of the ocean water wasn’t as crystal blue as it usually is. A residual consequence of the hurricane that passed through. “The color will be back to its breathtaking color in a couple of days,” a friendly employee chimed. How could I be enjoying myself while rescue efforts were in full swing just 140 miles away? Thankfully I was offered an opportunity to volunteer and contribute some of my time and energy toward the relief efforts. There were many of us from several hotels all conjugated in the back hall of a large resort’s convention center, all donning our hairnets, plastic gloves, and World Central Kitchen black t-shirts. We were ready to work. Five hours later, we had made 686 sandwiches for lunch and over 600 dinners, which were wrapped up, put into large coolers, and driven in vans to awaiting helicopters to feed the hungry on the neighboring islands. It felt good.

Even if it was just a grain of sand in the bucket of effort needed in this arduous task, I was so happy I was able to work side by side with others that cared.

A storm of this magnitude hasn’t been in this area since 1935. But after seeing the devastation first hand via helicopter, United Nations Secretary-General Antonio Guterres was “horrified” by the aerial views.

“I’ve never seen such a level of systematic devastation,” exclaiming, “Hurricane Dorian has been classified as a Category 5, I think it is Category Hell, but it was not powered by the devil.” Ellison “Tommy” Thompson, deputy director-general of the ministry of tourism and aviation, pointed out that the ravaged areas “were our second and third most important destinations.” Thompson contends, “When people say the Bahamas have been devastated, it gives the wrong impression.” He continues, “It’s going to take years to get those islands back up, so we need the tourism revenue in order for us to sustain the reconstruction that’s going to be necessary. We definitely need the visitors to come.”

The Bahamian government is profoundly dependent upon the tourism sector. Tourism accounts for over 50% of the GDP and employs about half of the Bahamian workforce. A small decline in visitors consequentially will impact the economy negatively. “Between January and

July of this year, more than 4.5 million visitors went to the Bahamas by air and sea,” Thompson said. Of that number, some 700,000 visited the now ruined communities. The tourism ministry informs that 14 of the country’s most-visited islands are unaffected. Thompson stresses, “One of the things we want to get across is the fact that the Bahamas is not one island.”

The Bahamian people are as resilient as they are welcoming. From my experience, they are warm, loving, and caring, but they’re also proud of their country and culture. A country that is mourning is also a country that needs its tourists back. As much as a person may feel it is inappropriate in a time of tragedy and calamity to vacation there.

Minister of Tourism and Aviation, Dionisio J. D’Aguilar counters that feeling, “More than ever, we need you to come on vacation,” he said, “That’s the only way we can help our brothers and sisters in the north.”

He offers an analogy, “If a hurricane would hit Jacksonville in Florida, it wouldn’t mean that you wouldn’t go on a vacation to Miami or Fort Lauderdale.”

Many resorts in Nassau and Paradise Island have found ways to spearhead relief efforts through in-kind sponsorship and volunteering.

Hotels like The Ocean Club Four Seasons Resort, Atlantis Paradise Island, and Baha Mar Nassau have set up separate donation pages, contributed millions of dollars to help their island neighbors as well as food and medical supplies. These luxury resorts are creating special packages to entice visitors and fundraising events to draw tourists to the Bahamas. The #PackWithLove initiative was generated to encourage visitors to “Carry-on your love for the Bahamas. All you have to do is pack an item or two to be shared with the victims of Hurricane Dorian, every little bit helps.”

Bahamas Tourism Minister Dionisio D’Aguilar invites us all, “We are grateful for the outpouring of support and love for our islands, and we would like everyone to know that the best thing they can do for us right now is visit Nassau, Paradise Island, and the Out Islands. Our beautiful island nation is ready to welcome you.”

Bahamians live in one of the most breathtaking places in the world.
