CAMP REGISTRATION INFORMATION

SMARTLINK INFORMATION You must have a SMARTlink account established prior to registering for any program. EXTENDED CARE INFORMATION • You must first be registered in the corresponding day camp before registering for extended care. (Not all camps offer extended care.) • Please complete extended care registration at least one week prior to camp start date. • You must pay in full at the time you register for extended care. • Extended care registration is NOT available via SMARTlink telephone.

PAYMENT & PAYMENT SCHEDULING OPTIONS You may pay in full or make a $50 deposit (per camper, per session) when you register, and schedule the remaining payments. The $50 deposit is non-refundable and non-transferable.

CANCELLATION POLICY Registrants are entitled to a 100% refund (or SMARTlink credit) in the event that M-NCPPC cancels a camp.

Scheduled payments must be paid in full by June 1 for sessions that begin on or before July 11; or by July 1 for sessions that begin after July 11. (June 1 is the last date personal checks will be accepted.) You may pay your balance before the due date; when paying on line or using the automated touch-tone telephone system, your payment will be shown as a credit to your account. On the due date, the system will automatically apply that credit to the balance. REGISTRATION DATES Prince George’s County residents: Camp registration begins: February 26, 9 am Extended care registration begins: February 28, 9 am Non-Prince George’s County residents: Camp and extended care registration begins: March 5, 9 am

WAYS TO REGISTER... To register via Internet or telephone, have your SMARTlink barcodes and family PIN, course number, and MasterCard/VISA credit/debit card number. Register on the web – visit www.pgparks.com and click on the SMARTlink logo before midnight June 12. Register via SMARTlink automated, touch-tone telephone – call 301-583-BOOK (2665) before midnight June 12. Register in person at any M-NCPPC staffed facility in Prince George’s County. All walk-in registrations during the week prior to camp start date must be processed at the offering facility, and are subject to supervisor approval.

CHECK POLICY Checks returned for insufficient funds will be assessed a $20 fee by M-NCPPC, in addition to your bank charges. NO starter checks accepted. FEE ASSISTANCE PROGRAM Fee assistance is available (subject to approval) for Prince George’s County residents only. For complete information and application forms, stop by your local community center or obtain the forms on line at www.pgparks.com>About Parks and Recreation>Fee Assistance. Applications must be processed no later than 3 weeks prior to start of program. LATE PAYMENT PENALTIES If you’ve made a deposit and full payment is not received by the due date, your space may be forfeited and you will lose your $50 deposit. If the camp accepts your late payment, you will be assessed an additional $20 late payment fee. WAIT LISTS If a camp session is full, you may put your name on a wait list and register for another camp during that session. If a spot opens at your first-choice camp, upon your request your registration will be transferred to the first-choice camp without penalty. WITHDRAWAL & REFUND POLICY Withdrawal and refund requests for fully paid or partially paid registrations are subject to a $50 non-transferable, non-refundable withdrawal fee per child/per session. Withdrawal and refund requests from extended care (with continued enrollment in associated camp) will be assessed a 20% fee. NO EXCEPTIONS. Withdrawals from sessions must be processed by the facility offering the program. WHAT (R), (NR) AND (IA) MEAN (R) means Residents of Prince George’s and Montgomery Counties; (NR) means Non-Residents of all other jurisdictions; and (IA) means residents living in the Impact Area of the Prince George’s Sports & Learning Complex. Impact Area residents receive a fee reduction for camp programs at the Complex. Call 301-583-2400, TTY 301-583-2483 for information about the Impact Area.

SUMMER CAMP INFORMATION AND POLICIES

Swimming Trips Policy – SWIMMER DESIGNATIONS Please read the following information carefully prior to selecting your child’s swimming designation on the Health Form. To help ensure the safety of your child while he or she is participating in swimming pool trips this summer, you will be asked to designate him or her as a “Non-Swimmer” or “Swimmer” on the Health Form. Use the following descriptions as a guide for selecting your child’s appropriate swimming designation. NON-SWIMMER: Non-swimmers have limited or no previous experience with swimming pools or other aquatic environments. They are unable to independently swim distances of 20 yards or more and/or cannot swim or play comfortably in water that is above their heads. If you designate your child as a “Non-Swimmer” on the Health Form, he or she will be restricted to chest-deep water on swim trips. SWIMMER: Swimmers have previous experience with swimming pools or other aquatic environments. They can swim a minimum of 20-yards and can swim or play comfortably in water that is above their heads. If you designate your child as a “Swimmer” on the Health Form, he or she will be required to pass a swim test each time he or she visits a pool. The swim test requires participants to enter and to exit the pool unassisted, swim a minimum of 20 yards in water more than five feet deep and continuously tread water for a minimum of 20 seconds. Upon successful completion of the required swim test, children designated as “Swimmers” will have full access to both shallow and deep-water areas of the pool during swimming trips. Designated “Swimmers” who do not pass the required swim test will be re-designated as “Non-Swimmers” on their Health Form and restricted to chest-deep water. If your child’s swimming abilities change between the time you register for a program and the start of the program, please contact staff, you will need to fill out and sign a new Health Form.

EXTENDED CARE OPTIONS • Supervised, non-structured playtime before and after camp session hours are available for many camp sessions. Extended care sessions are listed with each camp session in this brochure. Not all camps offer extended care. Please check activity descriptions. • All disability accommodation requests (inclusion services) for extended care must be indicated on the Participant Profile Form (p. 57) and requested at least two weeks prior to the start date of the camp. LATE PICKUP FEE POLICY A late fee of $5 per camper for every 15 minutes late (or portion thereof) will be assessed for campers who are not picked up by the program’s scheduled closing time. Payment is due when you pick up your camper. We recognize that emergencies arise and request that parents call the campsite if they are delayed for any reason; however, the late fee still will be assessed. REQUIRED FORMS A complete list and explanations of forms is available at ww.pgparks.com, and in the Parent Handbooks (available in English & Spanish), or obtained from staff. All forms and handbooks can be downloaded from www.pgparks.com. NOTE: Maryland state law requires that each child have a complete set of health forms at each program. Completed health forms must be turned in prior to the first day of the program session. It is the parent/guardian’s responsibility to deliver all necessary, completed health forms to each program location. Children may not attend any session without completed health forms on site. NO EXCEPTIONS. • Participants must provide date of last tetanus or DPT vaccination. • All participants must complete and return a Participant Profile Form (p. 57) and Health Form (p. 58). • All participants who take prescription or over-the-counter medication or use medical devices (i.e. inhalers, EpiPens®) must complete and return a Day Camp Medication Form. (A physician’s signature may be required.) • All participants must complete and return a Day Camp Authorization Form. • All forms are due by June 1. Art Camp: Air Brush Painting

Art Camp: Creative Expressions, Teens

Learn how to create and design your own high fashion clothing using various airbrushing techniques. This camp will introduce you to airbrushing on many different surfaces including t-shirts, jeans, shoes, canvas and more!

This camp is designed for children to have fun while they expand their imagination, express their creativity and sharpen their learning skills. Each camp will pursue a variety or artistic media, along with complementary reading and writing activities around such themes as animals, nature and adventure. Art Camp: Fashion Design

Art Camp: Shakespeare

With a little imagination, learn to create fashionable designs with fabric paints, tie dye, and more! Also includes arts and crafts, games, swimming and a camp-wide event.

Campers who are eager to express themselves through performance will learn about Shakespeare. Theatre professionals and area artists will lead campers through an exploration of stage combat, clowning, acting, voice and movement. Campers will perform scenes from a Shakespeare play for family and friends on the final evening at 7 pm.

Art Camp: Performing Arts Workshop Teens who have an interest in theatre production—drama, dancing, singing, costumes and sets—will work from a written script, with classes in all aspects of the performing arts needed to produce a play/musical. Basket Weaving Camp: Teens

Computer Camp: Digital Media, Teens

Campers will learn about the tools and techniques used to make artistic and functional baskets. Each camper will develop skills used to complete at least 3 baskets to take home at the end of the week. Camp includes a museum field trip to see handmade baskets.

This camp is designed for campers who already have a basic understanding of computers, exposing them to a variety of computer programs and uses for technology (e.g. PowerPoint presentations, digital video editing, digital photography, animation, and web design). Campers will receive copies of everything they create in camp. Also includes arts and crafts, games, swimming and a camp-wide event

SUMMER XTREME TEEN CENTERS

CANCELLATION POLICY Registrants are entitled to a 100% refund (or SMARTlink credit) in the event that M-NCPPC cancels a program. GENERAL INFORMATION • Registration begins April 1, 9 am. • Playgrounds and teen center sites will be closed July 4. • Participants must provide their own transportation. • Participants must bring their own lunches; at sites without refrigeration, please bring non-perishable lunches. • Optional field trips and special events may be offered for additional fees on a first-come, first-served basis. • Closed-toed shoes are required; sandals are not permitted. • Sites are subject to change based on availability and sufficient registration. • All activities and programs are subject to change and/or cancellation on code orange or higher weather days. • Federal Tax ID #526001550 REQUIRED FORMS Maryland state law requires that each participant have a complete set of health forms at each site where he/she is registered. Completed health forms must be turned in prior to the first day of participation. It is the parent’s/guardian’s responsibility to deliver all necessary health forms to each site. Participants may not attend without completed health forms on site. NO EXCEPTIONS. • Participants must provide date of last tetanus or DPT vaccination. • All forms (in English & Spanish) may be downloaded from www.pgparks.com • All participants must complete and return a Participant Profile Form (p. 57) and Health Form (p. 58). • All participants who take medication (prescription, over-thecounter) or use medical devices (i.e. inhalers, EpiPens®) must complete and return Playground/Teen Center Medication Form. NO EXCEPTIONS. SMARTLINK INFORMATION Establishing a SMARTlink Account is the name of the system used for all registration Internet, telephone, and in-person registration. You must establish a FREE SMARTlink account before you can register. Open your SMARTlink account as soon as you can. Stop by any staffed M-NCPPC facility in Prince George’s County and bring appropriate proof of residency (driver’s license, MVA card, utility bill, lease agreement or property settlement sheet), identification (driver’s license, MVA card, passport, resident/alien card, school ID card, military ID card, or other government-issued ID), and age (driver’s license, MVA card, passport, resident/alien card, birth certificate with raised seal, baptismal certificate with birth date and raised seal, or an original letter from school on official stationery) for each member of your immediate family. You’ll receive a family Personal Identification Number (PIN) and Client Barcode that will allow you to register by phone, Internet, or in person. PATIOS DE RECREO Y CENTROS PARA JÓVENES DURANTE EL VERANO

POLÍTICA DE CANCELACIÓN En caso de que M-NCPPC cancele un programa, el registrado recibirá un reembolso completo, es decir de 100% (o crédito SMARTlink) INFORMACIÓN GENERAL • Registración empieza el jueves, 1 de abril a las 9 am. • Los patios de recreo y sitios de centros para jóvenes estarán cerrados el lunes, 4 de julio • Cada participante debe proveer su propio transporte (M-NCPPC no provee transporte) • Cada participante debe traer su propio almuerzo. En sitios sin refrigeración, deben traer alimentos no perecederos. • Cada participante debe cumplir los requisitos. • Paseos y eventos especiales son opcionales y requieren pago adicional. El cupo es limitado. • Se permiten solo zapatos cerrados. No se permiten sandalias. • Los sitios están sujetos a cambios basados en la disponibilidad y número de niños/jóvenes registrados. • Todas las actividades y programas están sujetos a cambios y/o cancelaciones los días calurosos cuando el tiempo sea designado como código naranja o rojo. • Identificación Federal de Impuestos #526001550 FORMULARIOS DE SALUD La ley del Estado de Maryland requiere que cada participante tenga formularios de salud completos en cada sitio donde este registrado. Visite www.pgparks.com. • Cada participante debe llenar y entregar un formulario “Perfil de Participante” (página 57). • El formulario titulado “Formulario de Dispensa de Aparatos Médicos Patios de Recreo/Centros para Jóvenes” debe estar completo en caso que el participante esté tomando medicina en casa (una firma del médico puede ser requerida). ¡OJO!: Los únicos medicamentos (dispositivos médicos) que se permiten en los patios de recreo y centros para jóvenes son los inhaladores y los lápices anafilácticos. POLÍTICA DE CHEQUE SIN FONDOS El M-NCPPC cobrará $20 por cheque que rebote por falta de fondos, este cobro es por separado de lo que le cobre el banco. No se aceptan cheques que no hayan sido imprimidos con su nombre y dirección.

POLÍTICA DE REEMBOLSO Y CANCELACIÓN • El pago de $40 por patios de recreo y centros para jóvenes durante el verano no es reembolsable ni transferible. NO HAY EXCEPCIONES. INFORMACIÓN DE SMARTLINK Estableciendo una cuenta de SMARTlink Para servirle mejor, M-NCPPC en el Condado de Prince George’s, utiliza un programa llamado SMARTlink. Para establecer su cuenta, preséntese en cualquier centro comunitario del Departamento de Parques y Recreación M-NCPPC con documentación que compruebe la residencia (por ejemplo: licencia de conducir o tarjeta de identidad MVA, cuenta de agua, teléfono, luz; contrato de alquiler, certificado de propiedad), edad e identidad (por ejemplo: licencia de conducir u tarjeta de identidad MVA, pasaporte, visa de residente, certificado de bautismo y certificado de nacimiento con sello autorizado, carta del colegio u otra identificación emitida por el gobierno) de cada miembro de su familia que quiera incluir en su cuenta SMARTlink. Una vez establecida su cuenta, se le asignará un código y un número de identificación personal o “pin”. Una vez que tenga esa información podrá matricularse telefónicamente (301-583-2665), en persona, por correo o a través del internet (www.pgparks.com). Debe establecer una cuenta de SMARTlink para poder registrarse. Seguridad M-NCPPC usa los métodos mas avanzados en la seguridad de pago. La colección y retención de información del cliente es confidencial. No damos, compartimos, vendemos, transferimos o alquilamos ninguna información personal a nadie. SUMMER XTREME TEEN CENTERS Summer Xtreme Teen Centers offer Prince George’s County teens, ages 13-17, age-appropriate activities on a drop-in basis. Centers operate Mondays-Fridays from June 27 until August 5. The fee is $40 per teen, whether a teen attends six hours a day for all six weeks, or select days and times only. Optional field trips and special events may be offered for additional fees. Teens must be the minimum age of 13 on or before December 31, 2011. All sites are closed July 4.
