PRESIDENT

Hello everyone. It’s time to put the sticks in the trunk and get ready for golf and preparing the yard for summer. The winter was mild here in Indiana and not very cold, but I know some of you had a real winter and I hope you’re coming out on the other side of it now unscathed.

We are still getting a fair amount of mail returned because of bad addresses. It is our agents’ responsibility to show up and facilitate the process for our members, but as a member you can help by logging into our website and sending us a change.

It is important to receive all communications regarding your coverages and the magazine, so please do so if you don’t get contacted by our agent when you change addresses.

Naming a beneficiary is an important right granted to the owner of a policy. If you want underage kids to receive policy proceeds, we will follow your wishes, but it is much better to have an adult. I note in this column at least twice a year to check your beneficiary. I sound like a broken record, but it is imperative that the proceeds go to the person the insured wishes to receive them, not someone who was named years before.

Circumstances change, and you need to contact your agent or our office. If it ends up in lawyers’ hands or the court system, it usually is not what the insured intended.

Our scholarship submission site is ready, so watch Facebook and our website for instructions to submit applications.

We are fulfilling our fraternal purpose in society, and our members are financially and fraternally better off than nonmembers. We will continue to live up to the standard set before us, so do your family a favor and become a member.

Deputies First Day

Two officers observed a house engulfed in flames and responded immediately to assist. Police Officers Robbie Goering-Jensen, a nine-year veteran, and Anthony Abboud, first day on the job officer, rescued five physically disabled adults from a house fire. Two occupants were immediately rescued as well as their dog, Diamond. Three other occupants were saved shortly after Goering-Jensen re-entered the residence, hearing cries from the home. All occupants were assisted to safety as the home engulfed in flames. Several officers arrived prior to two engine companies and three medical units to help. A true team effort for all involved exhibiting supreme professionalism and teamwork. The cause of the fire was determined accidental due to clothing being too close to a light bulb.

The Red Cross distributed blankets and provided the family with three nights at a local hotel.

ichael Stokes gave a donation to sponsor a golf tee flag for the Lt. Robert D. Westbury Scholarship Fund. The event raised over $50,000 to go towards college scholarships for active and retired police officers, children and grandchildren. He visited three injured officers and processed over 10 claims.

n October 2, 2015, 12-inches of rain came fast to Horry County in South Carolina the first six hours. When it was over they had received over 26-inches of rain and eleven people had lost their lives due to the floods.

While Lt. Richard Schupner (aka “Schup”/PFIA member) was volunteering for Horry County’s Fire-Rescue Station, his own home was slowly being invaded by the floodwaters. After a long day of rescuing 18 people, he finally made his way back to his own neighborhood. He waded through waist-deep floodwaters just to get to his home.

Schup and his wife, Claudia, lost almost everything in their home. Since there was no flood insurance, the work began immediately and continued for days. Workers were clearing out the water logged carpets and damaged furniture when three angels arrived to help. Members of the North Carolina Men and Women’s Relief Ministry disaster team descended upon the area to help those who had been flooded. These men worked tirelessly for three days helping the Schupner’s recover from the flood. Sheetrock was removed and disinfectant was sprayed throughout the house to stop the mold and mildew that had already set in. In that short time, these three men had the entire house ready for new sheetrock. They asked for nothing and were proud that they could lend a hand to someone in need.

On the second evening of their work, they were all treated to an Outback Steakhouse dinner courtesy of Account Rep. Rick Buddelmeyer for their generosity and genuine care for those in need.

One of the most critical needs the Hotline meets involves teens who have decided to hurt themselves or take their own lives, but who at the last minute reach out to one of our Crisis Counselors in a desperate cry for help. In thousands of situations, our Crisis Counselors connect with those young people, provide a caring ear for their problems, and save their lives by taking action to stop a potential suicide.

The Boys Town National Hotline® (1-800-448-3000) is a free resource and counseling service that assists callers 24/7, 365 days a year, in the United States, the U.S. territories, and Canada. Open to everyone, but with an emphasis on helping children and parents, the Hotline has trained professional counselors who provide emergency or direct assistance, or refer callers to community resources.

The need for the life-changing and live-saving assistance the Hotline and its services provide is critical, far-reaching, and more common than you might imagine. The calls and contacts from teens who are thinking about hurting themselves come from all over the United States, and they may even come from someone in your neighborhood or community.

Through our extensive computer database, counselors can refer callers to people and services in their own community and quickly provide up-to-date information on thousands of agencies and services nationwide.

The Hotline is accredited by the American Association of Suicidology and Contact USA, and is a member of Child Helpline International and the North American Alliance of Child Helplines.

YourLifeYourVoice.org is the Hotline’s website

created especially for teens. In a safe, online environment, young people can share information with other teens, ask questions, get answers to the challenges they are facing, and access assistance from trained counselors in emergency or crisis situations.

The site enables and encourages teens to share their problems, concerns, and challenges in positive, creative ways.

Boys Town launched YourLifeYourVoice.org in 2009 to give teens a place where they can safely share their problems, concerns, and challenges with other teens in a positive way. Teens also can ask questions by email or chat online with a counselor to get answers to the challenges they are facing.

Today, about 25,000 teens visit the website each month. This service reaches young people from all socio-economic, ethnic, racial, religious, and cultural backgrounds.

The Hotline’s Texting Service is the newest innovation, developed to connect with teens through their favorite way of communicating. For teens who are more comfortable sharing their problems and asking for help YEAR 2015 through texting Calls & Messages rather than talking

So far, the new Boys Town app has more than 10,000 downloads and is getting positive reviews from young people. A 14-year-old girl recently emailed the Hotline, saying: “I really love your app... I have it on my iPod… it makes me feel so much better if I’m upset about things!”

Noah Leotta - December 10, 2015 Montgomery County Police Department (MD) 3-year veteran Police Officer Noah Leotta succumbed to injuries sustained on December 3, 2015, when he was struck by a drunk driver.

He was conducting a traffic stop while working a special assignment on the agency’s Holiday Alcohol Task Force. He had contacted the driver and was getting back into his patrol car when a second vehicle struck his patrol car and then struck him.

Officer Leotta was transported to Suburban Hospital, where he remained on life support until passing away on December 10, 2015.

Patrick R. Wolterman - December 28, 2015 Hamilton Fire Department (OH) 6-year veteran Firefighter Patrick Wolterman died from injuries sustained when he became trapped in a residential structure fire. According to initial reports from the media and fire department, firefighters responded and upon arrival observed heavy smoke coming from the home. Firefighters had been advised that the house might be occupied by two elderly residents. They entered the home to

search for any residents and began an interior attack on the fire.

Firefighter Wolterman fell from the first floor into the basement, and firefighters transmitted a “Mayday” alert calling for help.

Firefighters quickly located and rescued Firefighter Wolterman and performed life-saving measures while en route to Fort Hamilton Hospital, where he succumbed to his injuries.

Daniel V. Capuano - December 14, 2015 Chicago Fire Department (IL) 14-year veteran Firefighter/Paramedic Daniel Capuano died from injuries sustained in a fall down an elevator shaft while operating with fire crews in heavy smoke conditions inside a burning warehouse. Capuano was quickly removed from the burning building into a waiting ambulance, then taken to Christ Hospital, where he succumbed to his injuries.

Jeffrey Radford - October 28, 2015 Bell County Constable’s Office (TX) Deputy Constable Jeffrey Radford was returning to Temple after serving papers in Troy, Texas, when his department vehicle collided with another car. He was transported to a local hospital, where he succumbed to his injuries. The two occupants of the other vehicle were injured.

Jeffrey proudly enlisted into the United States Army in May 1967 as a tactical transport helicopter repairer. After serving 10-years, he reenlisted as a military police officer for an additional 9 years, which brought him to his retirement in May 1987. During his military career he earned many decorations including the National Defense Service Medal, Vietnam Service Medal, Vietnam Campaign Medal, Overseas Service Bar (3), Republic of Vietnam Counteroffensive, Republic of Vietnam Counteroffensive phase II, IV, V, and VI. American Campaign Medal, Bronze Star, Combat Infantry Badge, NCO Professional Development Ribbon, Army Service Ribbon, Overseas Service Ribbon (3), Republic of Vietnam Cross of Gallantry w/Palm, and Aircraft Crewman Badge.

After retirement from the military he began his career in law enforcement. He worked for Israel Police Department, Bartlett Police Department, and Nolanville Police Department as a police officer. In April of 2012, he became a Bell County deputy constable, where he proudly served in his position until his death.

Nate Carrigan - February 24, 2016 Park County Sheriff’s Office (CO) 12-year veteran Corporal Nate Carrigan was shot and killed as he and seven other deputies served a high-risk eviction order at a home in Bailey, Colorado.

The subject named on the eviction order was an activist in the Occupy Denver movement. The subject exited the home and then reentered it, with Corporal Carrigan and other deputies following back inside to stop him. The man was able to grab a rifle and opened fire, killing Corporal Carrigan and wounding the other two deputies. The subject was shot and killed by return gunfire.

The Police Helpline is a peer-driven program for working and retired law enforcement officers, dispatchers, retirees, and family members and provides assistance for any issue that may impact home and work life. Specially trained Police Peers representing a broad base of law enforcement disciplines are available 24/7 for anyone needing to contact the Helpline.

The Police Peers represent 15 local, county, and state police departments and have generously volunteered their time to this program.

There are no paid members involved in this project and the Helpline is sustained solely by contributions.

Given today’s intense scrutiny of law enforcement and the high rate of police suicide, the level of stress facing police officers may be at an all-time high.

In the Spirit of Brotherhood (continued) On February 27, 2016, members of the Pawtucket, Rhode Island Fire Department donned their turnout gear and SCBA at the Omni Hotel in Providence. They ran up 22 floors, 44 flights, and 348 stairs all while on air to raise funds for the American Lung Association. Team Captain Lieutenant Dave Marshall stated that this is the 7th year the PFD has had a team in the event, and the team, on average, raises about $3,000 for the American Lung Association.

Expanded Access to Naloxone: Options for Critical Response to the Epidemic of Opioid Overdose Mortality Abstract The United States is in the midst of a prolonged and growing epidemic of accidental and preventable deaths associated with overdoses of licit and illicit opioids. For more than three decades, naloxone has been used by emergency medical personnel to pharmacologically reverse overdoses. The peers or family members of overdose victims, however, are most often the actual first responders and are best positioned to intervene within an hour of the onset of overdose symptoms.

administering naloxone and reversing opioid overdose. Current evidence supports the extensive scale up of access to naloxone.

We present advantages and limitations associated with a range of possible policy and program responses.

RAPID INCREASES IN DEATHS from heroin-related overdose began in the 1990s, as average mortality per 100,000s population in 25 U.S. cities increased from 8.7 in 1988 to 13.8 in 1997. By 2004, poisoning was the second leading cause of death from unintentional injury in the United States. Nearly all such deaths were attributed to illicit and prescription drugs, fueled by a dramatic rise in the incidence

of opioid-involved overdose, which paralleled similar increases in Denmark, Finland, Iceland, Norway, Sweden, Spain, Italy, Austria, Australia, England, and Wales.

Fatal overdose is the leading cause of death among those who misuse illicit drugs, exceeding mortality from AIDS, hepatitis, or homicide. In a 33-year longitudinal study in California, 581 opiate-dependent participants had experienced an average of 18.3 years of potential life lost before age 65, with heroin overdose accounting for the largest proportionate mortality (22.3%). The years of potential life lost for this group was six times greater than in the general U.S. population.

Although heroin-related overdose deaths have continued to rise, recently there has been an alarming increase in mortality from drug overdose associated with the misuse of prescription opioid analgesics. Data from

the National Vital Statistics System indicate that the recent 62.5% increase in deaths from unintentional poisoning—from 12,186 in 1999 to 20,950 in 2004—was primarily attributable to increased misuse of prescription opioid analgesics. According to mortality data on multiple causes of death from the National Center for Health Statistics, the number of opioid analgesic poisonings listed on death certificates increased 91.2% between 1999 and 2002; in the latter year, it accounted for 5,528 deaths, more than those associated with either heroin or cocaine.

The current U.S. epidemic of opioid-related overdoses is spreading geographically and demographically. Mortality from such overdoses is expanding from urban areas to suburban and rural regions, where overdoses are usually prescription related and general awareness and treatment services are relatively lacking.

Likewise, overdose mortality is on the rise among non-hispanic whites, women, adolescents and young adults, and those with a history of chronic pain and depression. Methadone, oxycodone, hydrocodone, and fentanyl account for the vast majority of misused prescription opioids. Common sources include not only illicit dealers but friends, relatives, physicians, and emergency departments. For instance, in a study in rural southwestern Virginia, about half of the women who died of opioid-related overdose had prescriptions for the substance as well as a history of chronic pain, depression, or anxiety.

FATAL OVERDOSE Overdose occurs when the opiate

binds to the μ2 receptors in the brain stem, desensitizing it to the carbon dioxide levels in the blood so that breathing mechanisms are not triggered, leading to respiratory failure.

This process, however, can be interrupted by introducing naloxone, a safe and effective opiate antagonist that can reverse the effects of a wide range of natural, semisynthetic, and synthetic opioids. Naloxone can be safely administered by intravenous or intramuscular injection, a procedure that requires little skill. More recently, naloxone has also been formulated for intranasal administration, which has been shown to be a safe and generally effective “first line pre-hospital intervention.”

Once administered, naloxone displaces the opiate at the μ2 receptors, effectively reversing potentially fatal opiate effects within a few minutes. Naloxone, which is an uncontrolled substance, has no potential for abuse or overdose nor does it have any pharmacological activity in the absence of opioids or other opioid antagonists.

Moreover, emergency room staff and paramedics have routinely used naloxone to reverse opioid overdoses for 3.5 decades without any widely reported safety problems.

The efficacy of naloxone is fundamentally time dependent.

Death from overdose typically occurs within 1 to 3 hours, although earlier in some cases, leaving a brief window of opportunity for intervention.

Between 64.6% and 97.4% of those who misuse drugs have

reported witnessing an overdose, with respondents in one study recounting an average of 6 instances. Other surveys have reported that 58% to 86% of heroin-related overdoses occur in the company of others.

Despite the presence of others, timely transportation of victims to emergency departments or contact with first-responder services is inhibited by numerous structural barriers. For those in attendance, calling 911 is often a last resort, occurring only an estimated 10% to 56% of the time, because, in the United States, the police are usually notified of a 911 call reporting an overdose and frequently appear at the scene. Because many of those who misuse opioids are on parole, have outstanding arrest warrants, or otherwise don’t want to be identified, they are understandably disinclined to invite police involvement—a problem reported in numerous countries. In one study, 42% of those who had called 911 because of a heroin overdose reported seeing police accompany the paramedics. Some even reported being searched and interrogated by the police, checked for parole violations and warrants, or treated disrespectfully. Even murder or manslaughter charges are possible if the caller is suspected of supplying the drugs used in a fatal overdose.

Instead, peers (at the scene) often attempt to revive victims themselves, sometimes employing such dubious strategies as injecting them with salt, milk, or stimulants like cocaine; immersing them in a cold bath; massaging their hearts; or deliberately Spring 2016 • The PFIA Protector

inflicting pain. Such interventions are generally ineffective and potentially dangerous to the victim, but peers, even those who call 911, almost always first attempt these methods. Others may initiate cardiopulmonary resuscitation but, because few are adequately trained, their attempts are often unsuccessful. Thus, although peers are often on the scene and willing to help, time can rapidly run out for those in need of urgent intervention.

Compared with heroin users, misusers of prescription opioid analgesics misuse smaller amounts of opioids and report less current and lifetime intravenous drug use, fewer family and social problems, and less income from illegal sources. However, their tendency to keep opioid misuse concealed leaves friends and family members ill prepared to identify and respond to an overdose. Such secrecy predisposes adolescents, who most frequently misuse opioid analgesics, to accidental death.

In all cases of opioid overdose, it makes intuitive sense to reduce the time it takes to administer naloxone by getting it into the hands of those best positioned to respond rapidly. Indeed, support for this idea among opioid misusers is overwhelming. In a survey of 82 street-recruited drug users in the San Francisco Bay Area, where 89% had witnessed one or more heroin overdoses, 87% expressed a strong desire to participate in an overdose management training program in which they would receive takehome naloxone and training in resuscitation techniques; 91% said they would want peers to

administer naloxone to them if they overdosed. A quality-of-life survey in Rhode Island similarly found that 88.5% of drug users were willing to administer naloxone to prevent an overdose fatality. A study in London also found that 70% of 142 opiate users in methadone treatment were in favor of naloxone distribution, and 89% said that they would have administered naloxone at the last overdose they witnessed had they been able.

The logic and support for placing time-critical medications in the hands of nonmedical persons is not new. Epinephrine injections are made available as a lifesaving measure for people at risk for suffering anaphylaxis, and glucagon injections are provided to diabetes patients in case of severe insulin reactions.

SAFETY, EFFECTIVENESS, AND RISK–BENEFIT RATIO OF NALOXONE Opposition to proposals that would enable high-risk persons and their peers to access and administer naloxone typically questions the safety of its pharmacological properties and administration procedures, as well as the potential of higher-risk drug use practices. However, the available data suggest that these concerns are not valid, nor, if they were, would they outweigh the potential benefits of increasing access to this emergency intervention.

The first set of objections relates to concerns about the unsafe administration of naloxone, lack of follow-up care, and the possibility that overdose victims

may be subjected to unsafe practices by their peers. One commonly cited safety concern is that drug users will prematurely administer additional opioids to counter the withdrawal effects precipitated by naloxone. In at least one study, however, premature reinjection of heroin was not found to be a problem among overdose patients who had signed out of a hospital against medical advice after being revived with naloxone.

Furthermore, the country’s largest naloxone distribution program, in Chicago, reports that since its inception in 2001, drug users who were trained in naloxone administration experienced no difficulties in persuading victims not to re-inject more heroin.

There are no documented cases of practices or problems associated with the re-administration of opioids following naloxone administration.

Naloxone has a shorter half-life than heroin, leading to concerns about the potential recurrence of respiratory depression after the effects of naloxone wear off. On one hand, such situations would simply warrant a second dose of naloxone. On the other hand, of 319 reports of peer administration of naloxone in the Chicago pilot study, not one case required a second dose of naloxone to counter a recurrence of overdose symptoms. This is consistent with the reported efficacy of single-dose administration in formal medical settings. There is also apprehension that risk would increase if the companions of someone overdosing were themselves intoxicated; however, peers already attempt various interventions when naloxone

is unavailable. As indicated by the extremely low rates of adverse events in the evaluation of numerous pilot studies of naloxone distribution, drug-using peers are capable of saving lives with naloxone.

Complications such as seizures and arrhythmia have been reported after naloxone administration on very rare occasions. However, their links to naloxone have been questioned in the medical literature, and, even if there is a connection, it constitutes a risk only for patients with preexisting heart disease. A 1996 study linking naloxone to asystole, fits, pulmonary edema, and violence is often cited to suggest its pharmacological dangers, but these events occurred in just 1.3% of the 453 administrations in that study. Similarly, in a study of 1,192 episodes in Norway in which paramedics administered naloxone out of hospital, just three adverse events—or 0.25% of cases—were considered serious enough to require hospitalization.

The same study observed such non-serious side effects as confusion, headache, nausea or vomiting, and aggressiveness in about 45% of cases. These experiences are not adverse events per se, but acute withdrawal symptoms that fade within one to two hours and do not require hospitalization.

Another objection is that naloxone availability may encourage more frequent or higher-volume drug use by acting as a safety net. However, naloxone precipitates the same unpleasant symptoms that opioiddependent people are trying to stave off with their opioid use

in the first place, except that, with naloxone, the symptoms are more intense. People who use opioids and who have experienced the acute withdrawal effects that accompany naloxone consistently deny that they are more comfortable using heroin frequently or in higher doses because of naloxone availability.

Rather, studies suggest that increasing health awareness through training programs that accompany naloxone distribution actually reduces the use of opioids and increases users’ desire to seek addiction treatment.

Despite the availability of this safe and effective treatment, by and large, U.S. public health institutions have not adequately concerned themselves with the issue of accidental opioid overdose. Only a handful of individual state health departments, researchers, and other organizations that serve people who misuse opioids have begun to take some initiative. On June 25, 2008, the United States Conference of Mayors unanimously called for increased city-coordinated drug prevention efforts, which included measures to expand access to opioid antagonists.

However, given the size and growth of overdose fatalities in cities and rural communities in the United States, further action to dramatically increase the availability of naloxone for these often hard-to-reach populations is urgently needed. In order to promote discussion about this critical research, policy, and implementation agenda, we describe three potential policies for expanding access to naloxone in the following sections.

Each option would incorporate appropriate standardized educational instructions just as existing naloxone distribution programs already do.

PRESCRIPTION NALOXONE In 1971, the Food and Drug Administration (FDA) determined that access to naloxone required a prescription from an authorized health care provider. Although some physicians may be reluctant to prescribe naloxone to suspected opioid-misusing patients, at least one legal analysis, by Burris et al., concluded that health care providers do not act outside state and federal regulations in prescribing naloxone to their at-risk patients and the risks of liability are low and commensurate with those generally associated with providing health care. Recent Good Samaritan laws in states such as Illinois and New York have specifically provided legal protection to physicians for prescribing naloxone and to laypersons for carrying and administering the drug. The U.S. Conference of Mayors has also expressed strong support for the increased adoption of Good Samaritan immunity policies.

Encouraging clinicians to prescribe naloxone, as standard practice not only to suspected or reported opioid abusers but also to all patients who receive moderate to high doses of prescription opioids, seems warranted. Project Lazarus, a pilot program for naloxone distribution in North Carolina, for example, describes 13 indications for which health care providers should consider prescribing naloxone.

Very few studies have examined health care providers’ naloxoneprescribing attitudes or practices, but at least two suggest that significant efforts to increase awareness about naloxone and overcome negative stereotypes about those who use opioids are desperately needed. In a survey of 327 emergency medical service providers, 56% of respondents did not feel that training drug users to administer naloxone would be effective. Respondents who had worked in emergency medical service longer, however, were more likely to believe that the intervention would be effective.

In another study, with a sample (n = 571) that was skewed toward physicians with greater awareness of drug use issues, only 23% indicated that they had heard of prescribing naloxone to drug users as a way of preventing fatalities in the event of overdose.

A slight majority (54%) also indicated that they would never “consider prescribing naloxone and explaining its use to an [intravenous drug-using] patient.”

Although pursuing additional legislation, education, and in-service training to expand physicians’ willingness to prescribe naloxone is essential, access to naloxone for those who are either unable or afraid to see physicians or to talk about their drug misuse will remain limited.

NALOXONE DISTRIBUTION PROGRAMS Measures to expand the number and reach of current naloxone distribution programs may help to improve access to the users of illicit opioids that they typically

serve. Such programs legally distribute naloxone via physician prescriptions to those who are at risk for overdose and are most likely to be available as a first responder in emergency overdose situations. Naloxone has been distributed to those at high risk of overdose in Germany and Britain.

In the United States, 52 such programs are operating legally in 17 states as of December 2008 (L. Enteen, project manager at DOPE, Harm Reduction Coalition, written communication, December 2008. Evaluations of pilot distribution programs in Chicago, IL; San Francisco, CA; New York, NY; and Baltimore, MD, have consistently demonstrated positive outcomes. For instance, since 2001, the Chicago program has distributed over 10,000 vials of naloxone and received over 972 verified reports of successful overdose reversals without any adverse events from naloxone administration (D. Bigg, Director, Chicago Recovery Alliance, written communication, December 2008). Indeed, a recent evaluation of six naloxone training and distribution programs showed that trained laypersons were as adept as are medical experts in overdose recognition and treatment at recognizing an opioid overdose and knowing when naloxone use was necessary.

The current reach of these distribution programs is quite limited, because they are usually part of needle exchange programs that predominantly serve persons misusing illicit drugs in urban areas. Unless these programs are dramatically expanded, the vast majority of those misusing opioids will not have access to naloxone. With political goodwill

and sufficient funds, distribution capacity could be significantly increased. Although the U.S. Conference of Mayors has shown support for these programs, mustering the resources to benefit people at risk of misusing opioids, a socially unpopular population, will require aggressive political advocacy based on empirical evidence.

FDA RELABELING OF NALOXONE Given the fundamental limitations of access to physician prescriptions and distribution programs, relabeling naloxone as an over-the-counter drug is an option that merits consideration.

Naloxone has been available over-the-counter in Italy since the 1980s without any reported negative consequences. It has no potential for abuse and therefore is not a controlled substance, even in the United States. Its FDA label as a prescription drug, however, triggers state health care provider licensing laws as well as laws against practicing medicine without a license or possession of a prescription drug without a prescription (S. Burris, JD professor, Temple Law School, Philadelphia, PA; written communication, January 2008).

FDA relabeling of the drug would clear up any legal uncertainties that are currently associated with prescribing to third-party lay nonusers, which is still prohibited except in California, New York, New Mexico, and Connecticut (S. Burris, written communication, January 2008). Greater legal certainty would allow wider availability and distribution of naloxone and help organizations

that serve opioid users to expand overdose prevention services without the burden of obtaining physician prescriptions. Those whose contact with physicians or naloxone distribution programs is limited would have additional access through pharmacies and peer-to-peer networking and distribution. Other potential first responders, such as law enforcement officers or the staff of establishments frequented by those at risk, could also be trained and equipped to provide emergency life-saving care.

Evidence also indicates that friends and families of those who use opioids are an overlooked carer population that is in need of training in overdose prevention and reversal.

There are two basic forms of naloxone delivery that should be considered by the FDA for overthe-counter labeling. Naloxone is commonly injected—either intravenously, intramuscularly, or subcutaneously—and added risks associated with needles and syringes may pose barriers to relabeling. An alternative is intranasal naloxone. A quick review in 2005 of eight studies suggested that intranasal naloxone is a safe and effective “first line pre-hospital intervention.”

A recent randomized study of 166 participants confirmed no significant difference between intramuscular and intranasal routes of delivery in terms of mean response time, adequate response, hospitalizations, and minor adverse events. Officials in Massachusetts and New Mexico, for example, are already distributing intranasal naloxone kits to heroin-dependent persons to treat potential overdose, and

More noncontrolled studies are possible and may be necessary.

The FDA requires that overthe-counter drugs be generally recognized as safe and effective and not be misbranded. Evidence showing a low incidence of adverse reactions or of significant side effects, a low potential for harm from abuse, and a reasonable expectation of a clinically significant pharmacological effect would need to be provided.

The drug would require proper labeling with written instructions for appropriate use as well. The benefit–risk ratio demonstrated by the existing evidence supports a reasonable case for relabeling naloxone as a nonprescription drug on the basis of these criteria.

Within the competitive pharmaceutical industry, there would be little expectation of a return on investment for any single company from a successful relabeling effort.

TIMELINE A History of Anderson, Indiana and Its Fire Department

I

t was 1989 when I stepped into a fire service office and heard, “We are a modern department now and we don’t need these anymore.” I caught a glimpse of a photograph being thrown into the trash can. During lunch, I dug out that photo. It was an excellent photo of one of our horse-drawn fire wagons from Station 2.

I’ve always liked history and George Santayana’s quote came flashing back to memory; “Those who do not remember the past are condemned to repeat it.” I wanted to save that photo. In the next few days I decided to try and preserve our history the best I could, and I began collecting all the old photographs I could find.

This was the beginning of a 23year endeavor that ended with a book that I still am in awe of its coming to fruition.

It started with a simple plan.

I was in charge of the arson division and had access to the large, fancy printer in the chief’s office. My plan was to get in after hours and copy all the photos I could find to provide a stapled collection of Anderson, Indiana’s Fire Department history for each member of the department. Little did I know what I had set myself up for.

It wasn’t long into the task when I was introduced to Jim Jackson.

Jim was a retired Delco Remy supervisor who throughout his life

had been enamored with the fire service. When he was a young boy, his father would stop a block away from the hoses, trucks and smoke filled air of a firefight in progress and watch the action.

His now grown-children said he would never let a chance go by to take a side route that would pass by a fire station. He would slow down to get a glimpse of the trucks. He actually had a great collection of fire scenes and trucks from his own camera, which was kept handy. Jim became like a second father to me and for the next 23 years we researched every newspaper microfilm, old handwritten run books, city council records, and conducted many interviews looking for tidbits to

fashion a story.

It was hard to explain to my wife that I had to spend $1,000 on a computer program that would make it easier to put this book together. I purchased a publishing program called Quark.

The learning curve on how to use it was monumental by itself.

But with a little advice from a few people, we were combining written stories and photos. We contacted retired firefighters and widows across the country pleading for scrapbooks that were copied and returned as quickly as we could to get additional photos and personal mementos.

We scoured the city for people who had copies of old photos and reproduced and scanned them.

throughout the city. A factory was built, followed by surrounding neighborhoods and another station. So the book included not only a history of the fire department but snippets of the city’s history, which made it appeal to a dual audience.

Some of the things chronicled about the growth of Anderson were the gas boom, Amelia Earhart visiting during the Welsh Airport grand opening, and the development of the Remy Brothers building, a local empire that would combine with General Motors. The Interurban train system and the National Basketball Association started in Anderson.

After completing the early volunteer days and the building of a combination fire station and city hall, we moved to the need for a Station 2. That’s when we realized we had to tell the reader why another station was built. It was because the huge plant of American Steel & Wire just west of the city, resulted in new homes surrounding the plant by the hundreds of employees.

City fathers annexed that section, requiring new sidewalks, a water system, a school, and of course a fire station. As time passed the same thing happened

abundance of everyday living became vital parts of this history of Anderson Fire Department and the City of Anderson, Indiana.

It’s an authentic chronological review from before the Civil War through to the year 2000, reporting the fires of record in the town as it grew and the struggles and sacrifices of those who contributed to making the present day fire department and the city it serves.

Containing early 1,000 pages and 2,500 photos and maps, this book was published by M. T. Publishing Company Inc., Evansville, Indiana. It was truly a labor of love.

Our local history was chronicled through all the changes that the fire service throughout the country was facing, starting with hand pulled pump carts, to horse drawn and eventually motorized hose wagons. City wide fire alarms went from the court house bell to steam whistles at the water plant to radio communication systems.

Fire gear went from raincoats and hats with no gloves to helmets, Nomex, and self-contained breathing apparatus.

What Firefighters Should Eat for Breakfast What you eat for breakfast is really important because it can set you up for success or failure for the rest of the day. I’ve shared in the past that a good weight loss strategy is to try and eat more of your calories in the beginning of the day and taper down throughout the rest of the day.

There are two benefits to this strategy.

First, it gives you more waking hours to burn off those calories, and more importantly, it keeps your hunger in check. If you do not eat filling foods in the morning, you’ll end up eating more throughout the day because you’re more hungry.

The perfect breakfast for firefighters Firefighters especially need a breakfast that’s filling since you may not be able to eat lunch until 5 p.m. The most filling nutrients are fiber, fat and protein so it must include those (good fats of course). Fiber and healthy fats also ward off heart disease by lowering your cholesterol and reducing inflammation in your body.

A breakfast that meets all of these needs, and is super quick and easy to prepare, is oatmeal with fruit/berries, chopped nuts, milk (or a milk alternative), and some cinnamon. Here’s more: It can be any kind of oatmeal – instant has just as much fiber as traditional oats. Steel cut oats have more fiber but take a long time to cook. The best kind of nuts to add are chopped walnuts because they are the only kind that actually have omega-3 fats, the most heart healthy fat we can eat. Both the nuts and the milk add protein that makes it more filling. If you choose a milk alternative, know that those usually have less protein.

If you need to add some sugar or honey, I say go for it. You can choose a fruit or berry that’s sweet so you don’t have to add much. Berries like blueberries, blackberries and raspberries have the most fiber per volume, they also have a ton of antioxidants, which fight off cancer.

Now for the secret ingredient: a spoonful of cinnamon. Research shows that cinnamon helps control your blood sugar, helping you avoid type II diabetes and making this breakfast even better at dominating your hunger and your eating habits for the rest of the day!

Fire Hose Flags and Allegience Art Colton Blanchard and Jacob Taylor have been raising the patriotism bar in Evansville, Indiana. Colton has been a McCutchanville firefighter for three years, and is currently a new recruit for the Evansville Fire Department. Jacob Taylor is a 14year veteran of the Evansville Police Department. Flags are their second jobs.

Colton makes flags from fire hoses. He originally saw the idea online. He noticed that they were not very consistent nor done to scale. That’s when he and fellow McCutchanville Firefighter Todd Veek got to work. Their first creation took many hours to complete. After a while the process has been streamlined to a few hours. The unique flags come in a large size, 34x70 or a smaller 12x18, and can be fabricated with or without a frame.

Jacob’s idea came from a Pinterest board that his wife saw. It was some artwork made from pallets. He did one for his boys’ room, and shared it on social media. All of a sudden people were calling wanting similar designs. Within a couple months he had sold 20.

His designs range from a rectangular flag to a challenge coin flag to a board flag cut out in the shape of the USA, and furniture.

Giving back to the community is a priority for these guys as well.

They have both donated their talents to local charities and events.

West Warwick, Rhode Island firefighters recently participated in the #DStrong movement for Dorian Murray, a boy from Westerly, Rhode Island. Dorian had terminal cancer (rhabdomyosarcoma) and stated all he wanted in life is to be famous. Dorian said, “I’m just thinking before I go to heaven to try to be famous as much as I can.”

Thousands of photos and messages from across the globe with # DStrong have been sent to Dorian through various social media outlets to grant him his wish. Meritorious Service Award

Successful Resuscitation

Quick actions came as second nature for TJ Jacik, an 18-year veteran and Steve Walter, a 24-year veteran, of the La Vista Police Department, Nebraska last New Year’s Eve. While en route to the call, the officers found out they were on their way to a child in distress. This wasn’t just a child; this was a 5-week-old baby, new to life. The officers arrived at the apartment and headed to the back bedroom where this 5-pound little angel lay with blood coming from her nose and no pulse. Both officers training kicked in and they started chest compressions and breathing techniques, just as they were trained.

Each rescue is different. This was a difficult situation because when it had to do with a child, one that was just brand new to the world.

Officers Walter and Jacik were true professionals and stayed calm throughout this emergency situation as if it were just natural to them.

An amazing team effort allowed the officers to successfully resuscitate La Vista’s newborn.

Semi Driver Saved

It was a call too close to home for an off-duty firefighter, Paul Blanford. He heard a noise while he was giving his kids a bath.

“It was pretty loud so I got up to look and see what it was; and that’s when the whole front of the house was kind of orange-looking,” said Paul Blanford, a Decatur Township firefighter in Indiana.

A semi truck had crashed right down the road from Blanford’s Johnson County home. Cell phone video captured the fire. The flames and smoke were too

powerful to even make out what exactly was burning.

Blanford ran right to it.

“As soon as I came around the corner I started yelling at the driver to get away from the truck and that’s when I realized he wasn’t all there because he was trying to go towards the truck. He was really close to it already,” said Blanford.

Blanford saw the man on the ground right next to the truck. He knew there wasn’t a lot of time.

“I just grabbed ahold of him and dragged him so he wouldn’t get burned up,” said Blanford.

Within minutes, firefighters arrived and took care of the fire while Blanford took off his coat.

“It was kind of cold out, and I didn’t want him getting hypothermia. I just gave him my coat,” said Blanford.

Medics on scene told Blanford the driver had a diabetic issue and was in shock from the crash.

“He was a little coherent, but I wouldn’t say he was 100 percent with it,” explained Blanford.

The off-duty firefighter wasn’t sure of the driver’s name, where he was from, or where he was heading.

“Well, I tried to talk to him but he was kind of disoriented. He couldn’t really communicate very well,” said Blanford.

Even though the words “thank you” were never part of the conversation, they didn’t need to be.

“If you can help people, you help them,” said Blanford.

The driver was taken to the hospital as a precaution.

While Blanford didn’t feel like what he had done was that big of deal, he admitted his wife and three kids gave him plenty of praise.

Blanford has been with the Decatur Township Fire Department for nine years.

On September 27, 2015, Officer Matthew Laliberte

of St. Petersburg, Florida, responded to a call of a stab wound victim. Officer Laliberte found the victim with lacerations to the chest and without a breath of life. He administered chest compressions and resuscitated the victim.

Without immediate action and the effort of Officer Laliberte, this person would have not survived.
